You may also need pre-1968 military service records and last year’s W-2 forms or self-employment tax return. The exact list depends on your circumstances, and the online application will identify documents SSA needs for your case. Do not postpone filing simply because a record is missing; SSA says you can submit it later and may help obtain or verify it.
The core documents to prepare
Social Security Administration (SSA) requirements are conditional rather than identical for every applicant. Use this table as a preparation guide, then rely on the document list shown during your application or provided by an SSA representative.
As an Amazon Associate I earn from qualifying purchases.
| Document or record | When it applies | What SSA says about the copy |
|---|---|---|
| Social Security card or another record showing your Social Security number | Every applicant should be ready to provide the number. | Have the card or record available; SSA may already be able to verify the number in its records. |
| Birth certificate or other proof of age | Generally required to establish your date of birth. | Submit an original, a copy certified by the issuing agency, or another form of proof SSA accepts. Photocopies and notarized copies are not accepted for this purpose. |
| Proof of U.S. citizenship or lawful alien status | Usually applies if you were not born in the United States. | SSA requires original documents or copies certified by the issuing agency. Expired documents, ordinary photocopies, and notarized copies are not accepted. |
| Military service papers | If you served in the U.S. military before 1968. | A photocopy is acceptable. SSA-1 gives Form DD-214 as an example. |
| Last year’s W-2 forms and/or self-employment tax return | If you had wages or self-employment income that needs to be documented. | Photocopies are acceptable. |
If SSA already received proof of your age or citizenship or lawful status for an earlier Social Security or Medicare claim, it may not ask you to submit the same evidence again. SSA says it returns documents and photocopies unless you specifically tell the agency otherwise.
Proof of age: do you need your original birth certificate?
Often, yes—but “original” does not necessarily mean the only copy issued at birth. SSA generally accepts the original birth certificate or a copy certified by the agency that issued it. A regular photocopy and a notarized photocopy do not meet the stated proof-of-age rule.

If you do not have a birth certificate, SSA’s guidance allows other proof of age in appropriate cases. The agency decides whether the alternative is sufficient, so contact SSA rather than assuming a particular substitute will be accepted. Do not write on an original document. If you must send a document, follow the submission instructions in your application and keep a record of what you sent.
Citizenship and immigration-status evidence
Applicants born outside the United States may need proof of U.S. citizenship or lawful alien status. SSA’s rule is stricter for these records than for W-2s or military papers: provide an original document or a copy certified by the issuing agency. The document must be current enough to be valid; SSA says it will not accept an expired document, a standard photocopy, or a notarized copy for this purpose.
Do not mail a foreign birth record or a Department of Homeland Security document that you are required to keep. SSA instructs applicants to take those records to an SSA office for examination and return. The online application or an SSA representative will tell you where a particular document must go.

What to do when a document is missing
Missing paperwork is not a reason to delay filing. SSA’s official guidance says, “Even if you don’t have everything, don’t delay applying for Social Security retirement benefits.” You can provide missing items later, and SSA may help obtain or verify information.
For example, a local SSA office may be able to contact a state Bureau of Vital Statistics and verify information online at no cost. The available help depends on the record and your circumstances, so tell SSA exactly what is missing and what steps you have already taken to obtain it.

How to submit records safely
The application itself will display any documents SSA needs and explain where to submit them. Follow those instructions instead of sending every record you possess.
Originals and certified copies
Use originals or issuer-certified copies for proof of age and citizenship or lawful status when SSA requests them. Do not substitute a notarized copy unless SSA specifically tells you that it is acceptable for the particular record; the general rules for these categories say notarized copies are not accepted.
Photocopies that are allowed
Photocopies are acceptable for W-2 forms, self-employment tax returns, and qualifying pre-1968 military service papers. Keep the originals in your files unless SSA gives you different instructions.

Mailed documents
If SSA tells you to mail records, include your Social Security number separately as instructed so staff can match the papers to your application. Do not write on original documents. Use the address and delivery method SSA provides for your case, and retain proof of mailing if the documents are important or difficult to replace.

Documents that should be examined in person
Take foreign birth records and DHS documents you are required to keep to an SSA office rather than mailing them. An employee can examine the document and return it. Call ahead for an appointment before visiting an office.
Ways to apply and when to file
You can apply through SSA’s online retirement application, by calling the national toll-free number, or by contacting a local Social Security office. If you plan to visit an office, SSA advises calling ahead to make an appointment. Applicants outside the United States should follow the instructions for the appropriate SSA overseas office.
SSA’s FAQ dated April 24, 2024, says you may apply up to four months before the month you want retirement benefits to begin. Choose the start month carefully: it determines when SSA schedules the claim and may affect the amount of your monthly benefit. If you are unsure which month to select, you can still begin the application and ask SSA about the consequences of different start dates.

Step-by-step document workflow
- Choose an intended start month. Confirm that it is no more than four months in the future if you are filing under the timing rule described by SSA.
- Collect your identification and account details. Set aside your Social Security card or number, birth information, bank details, and citizenship or immigration-status records if applicable.
- Check for special records. Add pre-1968 military papers, last year’s W-2s or self-employment tax return, spouse and child information, and earlier claim details when they apply.
- Start the online application or contact SSA. Read the case-specific document instructions rather than relying only on a general checklist.
- Separate originals from photocopies. Reserve originals or certified copies for age and status evidence; prepare copies for tax and military records.
- Submit what you have. If an item is missing, file anyway and tell SSA which record is outstanding.
- Complete follow-up requests. Send additional documents through the channel and address SSA specifies, or take protected documents to an office.
- Keep a complete file. Save the application confirmation, copies of mailed records, delivery evidence, and all SSA notices.
Situations that often change the document list
You previously filed for Medicare or Social Security
SSA may already have proof of your age or citizenship or lawful status from the earlier claim. The agency will tell you if it needs the evidence again.

You worked recently
Have last year’s W-2 forms and, if you were self-employed, the relevant tax return. SSA may also ask about current-year earnings and an estimate for the following year for certain filing months.
You served before 1968
Locate your military service papers, such as a DD-214. A photocopy is acceptable under the SSA-1 guidance.
You were born outside the United States
Expect questions about citizenship or lawful status and prepare original or issuer-certified evidence. Do not mail a foreign birth record or a DHS document you must keep; arrange an office examination instead when instructed.
Your earnings record looks wrong
Apply rather than waiting for a perfect record. Tell SSA what appears incorrect and provide available wage or tax evidence so the agency can investigate.
Common mistakes that cause avoidable delays
- Waiting for a complete folder: SSA expressly says to apply even when documents are missing.
- Sending the wrong type of copy: A photocopy may work for a W-2 but not for proof of age or lawful status.
- Mailing protected immigration records: Take foreign birth records and documents you must retain to an office when SSA requires examination.
- Using expired status documents: SSA says expired documents are not accepted for citizenship or lawful-status proof.
- Failing to identify mailed papers: Include your Social Security number separately as directed so records can be matched to the claim.
- Assuming every applicant needs every item: The final list is case-specific and appears in the online application or comes from SSA.
- Ignoring an earnings concern: File and ask SSA to review the record instead of postponing the claim.
- Showing up without an appointment: Call the local office first, particularly when you must present an original document.
Final preparation checklist
- Social Security number or card.
- Original or issuer-certified proof of age, if requested.
- Original or issuer-certified citizenship or lawful-status evidence, if applicable.
- Photocopy of pre-1968 military service papers, if applicable.
- Photocopies of last year’s W-2 forms and/or self-employment tax return, if applicable.
- Date and place of birth, spouse and child details, prior claim information, and any other personal details requested.
- Bank routing and account numbers.
- Current, prior-year, and—when requested—next-year earnings information.
- A plan for obtaining missing documents without delaying the application.
The safest approach is to begin as soon as you are within the four-month filing window, use SSA’s case-specific instructions, and supply additional records when requested. That balances the agency’s strict rules for certain originals with its instruction not to lose time waiting for paperwork.
