What Documents to Bring When Applying for Social Security Survivor Benefits

By TheFinanceBase Team 7 min read

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Bring evidence of the worker’s death, your identity and birth, and the relationship that may qualify you for benefits. Depending on the claim, the Social Security Administration (SSA) may also need citizenship or immigration-status records, children’s records, work and tax documents, military discharge papers, and bank information for direct deposit. The exact packet varies by claimant and circumstances, so use SSA’s instructions for your application rather than waiting until every document is assembled.

SSA’s official SSA-10 checklist and survivors publication are the controlling guides. They also say you should not delay filing because a record is missing.

Core documents SSA may request

The following table covers the records most commonly relevant to a survivor-benefit claim. Some items apply only to particular applicants or situations.

Document or information Who should prepare it Purpose and qualifications
Proof of the worker’s death Every survivor claimant A death certificate or other proof accepted by SSA. SSA’s publication says proof may come from a funeral home or a death certificate.
Your Social Security number and proof of birth The person filing the claim Bring your Social Security number and a birth certificate or another proof of birth that SSA accepts.
Marriage record A surviving spouse A marriage certificate documents the relationship. SSA may also ask about other marriages or the history of the relationship.
Final divorce decree A surviving divorced spouse The decree establishes that the marriage ended. Other relationship details may be requested depending on the claim.
Citizenship or lawful-status evidence A claimant who was not born in the United States SSA may require proof of U.S. citizenship or lawful alien status.
W-2 forms or self-employment tax returns for the last year When the application or circumstances make them relevant These records can help SSA verify recent earnings information. The specific application determines whether they are needed.
Military discharge papers A claimant with military service before 1968, when relevant Bring the discharge documentation if SSA asks for it.
Children’s names, Social Security numbers and birth records A person claiming for an eligible child Have each child’s identifying information if known, plus a birth certificate or other proof of birth when relevant to the claim.
Bank account information Anyone choosing direct deposit A checkbook or bank document showing the account number; SSA’s checklist also lists the bank name. This information arranges payment and does not prove eligibility.

Survivor benefits can involve a surviving spouse, surviving divorced spouse, unmarried child or dependent parent. Because eligibility rules differ, SSA can ask for evidence not listed in a general checklist.

Documents by claimant relationship

Your relationship to the deceased worker determines which evidence deserves priority. Use this comparison to assemble the first packet, then follow any additional instructions SSA gives you.

Claim type Relationship evidence to prioritize Other likely information
Surviving spouse Marriage certificate and proof of your identity and birth SSA may ask about prior marriages or other facts affecting eligibility, along with proof of death and any status, work or military records requested for your case.
Surviving divorced spouse Final divorce decree and proof of your identity and birth Keep other marriage-history details available. The worker’s death proof and any additional status or earnings records depend on the application.
Unmarried child The child’s birth certificate or other proof of birth, name and Social Security number if known The adult filing for the child should also have the worker’s death evidence and the adult’s own identifying information when SSA requests it.
Dependent parent Evidence establishing the relationship and the circumstances of dependency The cited general checklists do not specify a single document set for every dependent-parent claim, so ask SSA which records apply to your facts.

Originals, certified copies and photocopies

Document format matters. The survivors publication says SSA needs original documents or copies certified by the agency that issued them. The SSA-10 checklist says photocopies are accepted for W-2 forms, self-employment tax returns and medical documents, but SSA generally needs to see originals of most other documents, such as a birth certificate, and returns them.

Do not assume an ordinary photocopy of a marriage certificate, divorce decree or birth certificate will be enough. Bring the original or an issuing-agency-certified copy when SSA’s instructions call for it. Keep a personal copy for your records, but follow the evidence directions SSA gives for your specific claim if they differ from this general guidance.

How to prepare your application packet

  1. Identify the claimant and claim type. Decide whether the application is for you as a spouse, surviving divorced spouse, parent or for an unmarried child. That decision determines which relationship records belong at the front of the packet.
  2. Secure proof of death. Obtain the death certificate or other evidence accepted by SSA. If a funeral home supplied the record, retain the document and any certified version you receive.
  3. Collect identity and birth records. Set aside your Social Security number and proof of birth. If you are filing for a child, gather the child’s identifying details and birth evidence as well.
  4. Add relationship evidence. Include a marriage certificate for a surviving-spouse claim or the final divorce decree for a surviving-divorced-spouse claim. Note any other marriage history so you can answer SSA’s questions accurately.
  5. Check conditional records. Add citizenship or lawful-status documents if you were not born in the United States. Add recent W-2s or self-employment tax returns if the application calls for earnings information, and military discharge papers if you served before 1968 and SSA requests them.
  6. Prepare payment details. If you want direct deposit, bring bank papers showing the account number and bank name. Verify the numbers before submitting them; this step concerns delivery of benefits, not whether you qualify.
  7. Separate originals from copies. Label which items are originals, issuing-agency-certified copies and photocopies. This makes it easier to hand over the correct version and retrieve documents SSA returns.

If a document is missing

Do not wait to contact SSA until your file is perfect. The agency’s SSA-10 checklist states: “Do not delay filing your claim just because you do not have all the documents. We will help you get them.” The survivors publication similarly says to apply promptly because, for some claims, benefits are paid from the application date rather than the worker’s date of death.

The separate $255 lump-sum death payment

Monthly survivor benefits and the lump-sum death payment are separate. SSA’s FAQ, dated March 13, 2024, describes a $255 one-time payment that may be payable to a qualifying spouse or child, subject to the program’s requirements. It is not automatic and is not a replacement for monthly survivor benefits.

Survivors must apply for this payment within two years of the worker’s death. Ask SSA to evaluate it when you file or contact the agency about an existing survivor claim.

Common preparation mistakes and fixes

  • Waiting for every document: Contact SSA and file as soon as you can; the agency says it will help obtain missing evidence.
  • Bringing only ordinary photocopies: For most records, use originals or copies certified by the issuing agency. Photocopies are specifically noted as acceptable for W-2s, self-employment tax returns and medical documents under SSA-10.
  • Confusing payment details with eligibility evidence: Bank information supports direct deposit. It does not establish your relationship to the worker or your right to benefits.
  • Leaving out prior marriage information: SSA may ask for other marriage-history details in spouse and surviving-divorced-spouse claims. Write down accurate dates and names before your interview.
  • Assuming one checklist fits every survivor: A child’s claim, a dependent-parent claim and a spouse’s claim can require different relationship evidence. Confirm the list with SSA.
  • Missing the lump-sum deadline: The $255 payment has a two-year application window, even though monthly survivor-benefit rules are separate.

What to have ready for SSA’s questions

Besides documents, prepare the claimant’s full identifying information, the deceased worker’s identifying information and the basic facts of the relationship. Be ready to explain whether you are a spouse, surviving divorced spouse, child or dependent parent, and to identify any record you could not obtain. Clear answers help SSA determine which additional evidence applies.

A simple folder—paper or digital—is sufficient; no special organizer is required, and no folder is endorsed by SSA. Keep originals protected while you submit the versions SSA requests.

Bottom line

Start with proof of death, your birth and identity records, and the marriage, divorce or child relationship evidence that matches your claim. Add status, earnings, military, child and banking records only when relevant, use originals or certified copies where required, and contact SSA promptly even if something is missing. File by phone or through a local office, not online, and ask about the separate $255 payment before its two-year deadline.
