What Documents Do You Need to Apply for Social Security Disability Benefits?

Do not wait until you have every record. To apply for Social Security disability benefits as an adult, you need detailed information about your identity, medical care, work history and other benefits. The Social Security Administration (SSA) may later request proof such as a birth certificate, tax records, medical reports or workers’ compensation documents. Submit the application with what you have, identify your providers and dates as accurately as possible, and respond to any follow-up request. SSA says, “Do not delay applying for benefits because you do not have all the documents. We will help you get them.”

What you should have ready before starting

The application asks for more than a single medical diagnosis. Prepare a folder—paper or digital—with the information below. Some items are facts you report on the form; others are supporting documents SSA may request to verify your answers.

As an Amazon Associate I earn from qualifying purchases.

  • Your full name, date and place of birth, Social Security number and contact information.
  • Names, addresses, telephone numbers and approximate treatment dates for every doctor, hospital, clinic and other medical source.
  • Your conditions, symptoms, medicines, prescribers, tests and dates, and medical reports or test results already in your possession.
  • Recent earnings, employers and a detailed work history.
  • Information about workers’ compensation, black-lung benefits, government disability payments, settlements or similar benefits.
  • For Supplemental Security Income (SSI), records of income, bank accounts, property, household expenses and living arrangements.

This preparation list applies primarily to an adult claim. A person applying for a child may be asked for different school, developmental and household information, so use the adult or child version of SSA’s current Disability Starter Kit that matches the claim.

Identity and personal documents

Information entered on the application

Have your date and place of birth and Social Security number available. Depending on the questions in your application, you may also need a spouse’s or former spouse’s name and identifying information, children’s details and your preferred contact method. If you want payments by direct deposit, have your bank routing number and account number ready.

Documents SSA may request

SSA can ask for a birth certificate or another acceptable proof of birth. If you were born outside the United States, it may request proof of U.S. citizenship or lawful alien status. The exact document depends on your circumstances and the instructions SSA gives you.

SSA generally needs to examine original versions of many identity documents. It returns originals after review. Do not write on an original document, and do not mail a foreign birth record or another document that would be difficult or impossible to replace. Take those items to a Social Security office for examination instead. Follow the current instructions for the specific document rather than assuming a photocopy will be accepted.

Medical information and evidence

List every relevant medical source

Make a provider list before you begin. For each doctor, hospital, clinic, therapist or other source, record the name, complete address, telephone number and approximate dates you were treated. Include emergency departments, surgeries, rehabilitation, mental-health care and other treatment related to the conditions that limit your ability to work.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Also write down each condition, when it began, how it affects daily activities and work, the medicines you take and who prescribed them, and tests or procedures with their dates. SSA asks about abilities such as walking, sitting, standing, lifting, carrying, understanding and remembering instructions. Describe what you can do, what you cannot do reliably, how long an activity is possible and what happens afterward.

Provide records you already possess

Send medical reports, imaging summaries, laboratory results, hospital discharge papers and other evidence you already have. Photocopies of medical documents are accepted. You do not need to pay a service to assemble a complete chart before filing.

SSA’s disability determination service (DDS) normally requests records from the treatment sources you identify. Doctors are asked for medical evidence; they are not asked to decide whether you meet SSA’s disability standard. A provider’s statement can describe diagnoses, treatment and functional limitations, but the legal disability decision belongs to SSA.

Uploading additional medical documents

If SSA asks you to use its Upload Documents service, the agency’s FAQ dated January 13, 2026, states that one submission can contain no more than 50 files and that each file can be no larger than 25 MB. Those are technical limits, not a limit on the amount of evidence in your claim, and portal instructions can change. Check the current upload screen before preparing files, keep confirmation that a submission went through, and follow any case-specific directions.

Your past jobs

SSA asks for a job history reaching as far as five years before you became unable to work. For each job, gather the job title, type of business, employer, start and end dates, hours and days worked, pay, main duties and why the job ended. Note physical and mental demands—such as standing, lifting, operating equipment, concentrating or interacting with customers—because those details help SSA evaluate how your conditions affect work.

A Social Security Statement can help you check your earnings record and work-history context, but it does not replace the detailed answers in the application. If you were self-employed, have your recent self-employment tax returns available; SSA may request them to verify earnings.

Workers’ compensation, government benefits and military service

Other disability or injury payments

Report workers’ compensation, black-lung benefits and similar temporary or permanent payments. SSA may request award letters, payment stubs, settlement agreements or other proof. The application guidance also asks about certain federal, state, local and military disability benefits. Answer each question as written: veterans’ benefits and military disability benefits are not automatically treated as the same category.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Military records

If you served in the U.S. military before 1968, SSA may request your military discharge papers. Keep the original safe and provide it only in the manner SSA directs.

Extra paperwork for SSI applicants

SSDI and SSI use the same core disability evidence, but SSI also evaluates financial eligibility and living arrangements. SSI applicants should expect questions and possible document requests in four areas:

Earned income

  • Recent payroll stubs.
  • Self-employment tax returns or other records of business income.

Unearned income

  • Benefit or award letters.
  • Bank statements, court orders or receipts showing support, alimony, pensions or other payments.

Resources

  • Statements for checking, savings and other accounts.
  • Records for real estate, vehicles, insurance, burial arrangements and investments.

Living arrangements and household costs

  • A lease, rent receipt or deed.
  • Information about who lives with you and who pays rent, utilities, food or other household expenses.

SSA’s SSI list is not all-inclusive. Another document may substitute for an item on the examples, and SSA will tell you what evidence is needed for your case. Keep current statements and receipts because financial circumstances can change while an application is pending.

SSDI and SSI: how the document requests differ

Document issue SSDI SSI
Eligibility basis Work history and Social Security-covered earnings, in addition to meeting the disability rules. Income, resources and living arrangements, in addition to meeting the disability rules.
Medical evidence Names and contact details for providers, treatment dates, conditions, medicines, tests and records already available.
Work information Recent earnings and a detailed history of jobs, duties, dates and hours.
Extra financial records Usually not the central eligibility documentation, although other-benefit information may be requested. Payroll and tax records, unearned-income proof, account statements, property and insurance records, and household-cost or housing documents.
Who is applying Adult and child applications use different starter kits and may require different supporting information.

This table is a document-planning guide, not an eligibility test. SSA may request additional evidence in either program.

Information versus documents: what SSA actually needs first

Many applicants assume they cannot file until they possess every underlying record. That is not how the process is designed. At filing, complete the application with the most accurate information you can provide:

  1. Identify each medical source and give approximate treatment dates, even if you do not have the chart.
  2. Describe medicines, tests, symptoms and functional limits from your own records and recollection.
  3. List employers, dates, duties, hours and pay for the requested work period.
  4. Report other benefits, settlements and military service when the form asks.
  5. Upload or submit copies of records already in your possession.

DDS can request records directly from providers. If a provider no longer has a file, tell SSA promptly and identify another source, such as a hospital, imaging center or pharmacy, that may document the treatment.

How to submit documents safely

Use the current application and release forms

SSA’s adult guidance directs applicants to complete the Disability Benefit Application and the Medical Release Form. The free Adult Disability Starter Kit includes a fact sheet, a document-and-information checklist and a worksheet for organizing answers. Use the current versions because form names, online screens and technical instructions can be updated.

Choose an application route

You can apply online, by calling SSA or through a local Social Security office. The route you choose does not change the underlying need to provide accurate medical, work and financial information.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Mailing, uploading or bringing records

  • SSA accepts photocopies of W-2 forms, self-employment tax returns and medical documents.
  • For many identity and civil-status records, SSA says it must see the original and will return it.
  • If you mail documents, include your Social Security number on a separate sheet so staff can match the material to your claim.
  • Do not write on originals, and do not mail irreplaceable foreign birth records; bring them to an office for examination.
  • Before uploading or mailing sensitive records, check the current SSA instructions for file types, addresses and any case-specific cover sheet.

What to do when a record is missing

File the application with the information and evidence available. SSA’s medical-evidence guidance says, “Do not delay sending your application while gathering evidence.” After filing, monitor notices, voicemail and your online account, and answer requests by the deadline. If you cannot obtain a document, explain why and provide any substitute evidence SSA accepts.

Delay can affect when benefits begin. For SSI, SSA says the earliest payable month is generally tied to the application or contact filing date and the date eligibility is met. Contact SSA promptly rather than waiting for a complete medical file or assuming a later filing date will have no financial effect.

A practical final checklist

  • Social Security number, birth information and contact details.
  • Citizenship or lawful-status evidence if applicable.
  • Bank routing and account numbers if requesting direct deposit.
  • Provider names, addresses, phone numbers and treatment dates.
  • Conditions, symptoms, medicines, prescribers, tests and available reports.
  • Current and prior-year earnings and employer information.
  • Up to five years of job duties, dates, hours, pay and reason each job ended.
  • Workers’ compensation, black-lung, government disability or settlement records.
  • Military discharge papers for pre-1968 service, if applicable.
  • For SSI: income, bank, property, insurance, vehicle, housing and household-expense records.

The most important preparation step is accurate, complete information—not waiting for a perfect stack of paperwork. Use SSA’s current starter kit, keep copies of everything you submit, and follow the individualized requests in your case.
