Start by contacting the insurer or servicing agent and asking for the current reinstatement application, the lapse date on record, the filing deadline, and a written amount-due statement. A reinstatement package commonly includes a signed application and payment of overdue premiums, interest, and any policy debt. The insurer may also require evidence of insurability or health information, but no universal medical exam or physician-record requirement applies to every life policy.
Do not assume that sending money restores coverage. Reinstatement depends on the policy contract, the governing state law, the carrier’s underwriting decision, and your circumstances. Obtain written approval and the effective date before treating the policy as active.

What to request from the insurer first
Ask the insurer or servicing agent to send the policy-specific reinstatement packet rather than relying on a form found online. The packet and the contract determine which documents, signatures, health disclosures, and payments are required.
- The current reinstatement application and all accompanying forms.
- The lapse date the insurer has recorded.
- The last date on which a reinstatement request can be filed under the policy and applicable law.
- A written, itemized statement of the amount required to reinstate the policy.
- Instructions for signatures, delivery, payment method, and supporting records.
- Confirmation of the address, email, or portal to which the completed package must be sent.
Have your policy number, insured’s full legal name, date of birth, and current contact information ready. If ownership, beneficiary, or address information has changed, tell the carrier before submitting the application so it can explain whether additional forms are needed.
#1 Best Overall
MetLife’s current variable-universal-life FAQ describes a process in which an eligible policyholder receives an amount-due letter and an application; the completed application must be returned with the payment due. That is an example of one carrier’s process, not a nationwide form.
Documents a reinstatement package may contain
Signed reinstatement application
The application is the core document. It normally asks you to confirm the policy, explain or acknowledge the lapse, answer questions about health and other underwriting matters, and sign declarations that the information is complete and accurate. Read every question literally and answer for the period requested. An incomplete answer can delay review or lead to a refusal.
Proof of payment
The carrier will specify whether it accepts a check, electronic transfer, debit or credit transaction, or another method. Keep the confirmation number, receipt, and a copy of the amount-due statement. A payment that cannot be matched to the policy, arrives after the deadline, or is short of the required amount may not reinstate coverage.
Identity, ownership, or authority documents
The insurer may ask for information needed to verify the policyholder or the person signing. If a trustee, attorney-in-fact, guardian, or other representative signs, ask what proof of authority is required. Do not send an unrequested medical-record release or physician statement; wait for the carrier’s written instructions.

Policy-specific supporting records
Depending on the contract and the lapse circumstances, the carrier may request an evidence-of-insurability statement, medical questionnaire, attending-physician statement, laboratory results, or an authorization to obtain records. The available sources do not establish a universal list for individual life policies. Submit only the records the insurer identifies, using its required form and time frame.
When health information or a medical exam is required
A reinstatement clause can make evidence of insurability a condition of approval. “Evidence of insurability” is a broad underwriting concept: it may be a signed statement about your health, answers to a detailed questionnaire, or additional medical documentation. It does not automatically mean that every applicant must undergo a medical exam.
Answer every health question accurately
Disclose diagnoses, symptoms, hospitalizations, injuries, treatment, medications, and consultations exactly as the form requests. If a question asks about a period or a type of treatment, do not substitute a general statement such as “healthy.” Keep copies of your answers and note the names and contact details of treating providers if the form requests them.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsDo not order tests before the carrier tells you what it accepts
The governing sources establish that evidence of insurability may be required, but they do not set a universal test panel, exam threshold, or medical-record standard for an individual policy. Arranging an exam or paying for records before receiving instructions can create expense and delay without satisfying the carrier’s requirements.

Why forms can differ
An official Prudential group hospital indemnity form illustrates how one carrier can use different requirements for different lapse windows: one section combines a request with payment, while another requires a good-health attestation addressing illness, injury, treatment, and treating providers. That is group supplemental coverage, not proof of what an individual life policy requires. Your own policy and insurer control.
How the amount due is calculated
Request a current reinstatement or payoff quote; do not estimate the figure from the old premium notice. State-law examples identify several components that may be included:
- Premiums that became overdue before the lapse.
- Interest on those arrears, calculated under the contract or applicable statute.
- Policy indebtedness, such as an outstanding loan or other debt, including payment or reinstatement of that debt where required.
- Charges needed to bring the policy back into the status required by its terms.
Florida’s statute describes reinstatement as potentially involving overdue premiums and payment or reinstatement of policy indebtedness, with interest subject to statutory terms. South Dakota’s text similarly refers to arrears and policy debt. Those provisions do not produce a single national price; only the insurer can state the amount for your policy on the date you apply.

Universal-life cash-flow issues
For MetLife universal-life products, the FAQ says the amount due can include the next two monthly charges after the payment is received. It also warns that, after approval, the policyholder may need to maintain planned premium payments and may need to increase premiums or reduce the amount of life insurance to retain coverage. These are MetLife-specific terms and should not be applied to other carriers without confirmation.

Deadlines: how long you may have
The filing window comes from the policy, state law, and the type of coverage. A commonly cited three-year period is only an example. Check the contract and ask the insurer to state the final date in writing.
| Authority or situation | Illustrated reinstatement period | Important qualification |
|---|---|---|
| Florida Statutes § 627.459 (2026 text) | Within three years after premium default | The statute requires a policy provision and includes exceptions and additional conditions. |
| South Dakota codified text | Three years for ordinary life policies | Contract and statutory requirements still apply. |
| South Dakota codified text | Two years for industrial life policies | This shorter period applies to the industrial-life category identified in the statute. |
| Texas Insurance Code, Chapter 1106 | One year for a qualifying mental-incapacity case | The request and proof must be filed within one year; when the statute’s conditions are met, evidence of insurability may not be required. |
The Texas rule is a narrow exception for specified mental-incapacity circumstances, not a general way to extend every reinstatement deadline. If incapacity may have caused the lapse, obtain legal or professional help promptly and ask the insurer which proof the statute requires.
Step-by-step reinstatement process
- Confirm the status. Ask whether the policy is lapsed, still within another contract status, surrendered, or otherwise terminated. Request the recorded lapse date and the last reinstatement date.
- Obtain the packet and quote. Request the current application, health forms, payment instructions, and an itemized amount due with a validity date.
- Review the contract. Locate the reinstatement clause, requirements for good health or insurability, treatment of policy loans, interest provisions, and the clause describing when reinstatement becomes effective.
- Gather only requested information. Assemble policy details, contact information, signatures, and any medical or authority documents the carrier lists. Do not commission an exam or records release in advance.
- Complete health answers carefully. Use the requested dates, diagnoses, providers, and treatments. Correct an error immediately through the carrier rather than crossing out or concealing information.
- Pay the exact amount through the approved channel. Include every component shown on the quote. Retain proof of delivery and payment.
- Track the review. Ask when the package is considered received, whether anything is missing, and whether underwriting has requested more information.
- Obtain the decision in writing. Keep the approval notice, the effective date, any conditions, and the schedule of future premiums or coverage changes.
- Verify ongoing status. After approval, confirm that planned premiums are set up correctly and that any required increase, debt treatment, or reduction in coverage is reflected in the policy record.
When payment does not immediately restore coverage
An application or check may be only an offer to reinstate. One court decision quoting a particular policy stated that approval had to occur during the insured’s lifetime and good health and that other company requirements had to be satisfied before reinstatement. That language is a warning to read your own contract, not a universal clause.
Recommended Free Tools
Until the carrier confirms approval and an effective date, ask how claims, payments, and any death during review will be treated. Do not represent the policy as active to a beneficiary, lender, or estate planner based solely on a receipt.

Common problems and how to address them
The quote is higher than expected
Ask the insurer to separate arrears, interest, policy debt, charges, and any future premium component. If the quote expires, request an updated figure rather than sending the old amount.
The insurer requests more health information
Ask for the exact form, date range, providers, and deadline. Sign only the authorization the carrier supplies and send records through the specified secure channel.
You are close to the deadline
Request the deadline and submission method in writing the same day. Ask whether a partial package or payment reserves a filing date; do not assume it does. Send the completed materials with delivery tracking and keep the confirmation.
The policyholder cannot sign
Tell the carrier who will sign and ask what evidence of guardianship, power of attorney, trusteeship, or other authority is required. A representative’s signature does not automatically satisfy the insurer’s identity and underwriting requirements.
The application is denied
Request the written reason, the contract provision used, and the date of the decision. Ask whether any alternative status, conversion, or new-application route exists, but treat a new policy as a separate underwriting process rather than assuming it preserves the lapsed policy’s terms.
Questions to settle before you submit
- What exact date does the insurer treat as the lapse and what exact date is the filing deadline?
- What amount is due today, and how long is that quote valid?
- Is evidence of insurability required, and if so, which forms or records will satisfy it?
- When will coverage become effective if the application is approved?
- Will future premiums, policy debt, or the face amount change after reinstatement?
- How will the insurer notify you if the package is incomplete or approval is conditional?
The safest approach is prompt, complete communication with the carrier: obtain its forms and deadline, disclose health information accurately, pay the itemized amount, and wait for written approval and an effective date.
