W-2 Box 3 vs. Box 5: How to Check Your Social Security and Medicare Wages

By TheFinanceBase Team 10 min read

Box 3 is your Social Security wages; Box 5 is your Medicare wages and tips. They are often identical, but Box 3 is limited by the annual Social Security wage base while Box 5 has no wage-base cap. For 2026, that cap is $184,500. Once wages exceed it, Box 3 can stop increasing while Box 5 continues to show Medicare-taxable pay. A difference can therefore be completely correct—but you should check the tax year, tips, payroll records and your Social Security earnings record before assuming either the W-2 or your record is wrong.

What do W-2 Box 3 and Box 5 mean?

Employers use Box 3 to report wages subject to Social Security tax and Box 5 to report wages and tips subject to Medicare tax. Both amounts are generally calculated from taxable compensation before many payroll deductions, but the two federal payroll taxes use different limits.

W-2 field What it reports Important limit or treatment
Box 3 Social Security wages Social Security wages, together with Social Security tips in Box 7, are limited to that year’s Social Security wage base.
Box 5 Medicare wages and tips There is no annual wage-base limit. Medicare wages can continue rising after Social Security wages reach the cap.
Box 7 Employee-reported Social Security tips These tips count toward the Social Security wage-base calculation and should be considered when comparing Box 3 and Box 5.
Box 8 Allocated tips Allocated tips are not included in Boxes 1, 3, 5 or 7. They are a separate information amount.

The Internal Revenue Service General Instructions for Forms W-2 and W-3 (2026) state that wages and tips subject to Medicare tax are the same as those subject to Social Security tax (Boxes 3 and 7), except that Medicare has no wage-base limit. That distinction explains most legitimate differences.

Why are my W-2 Box 3 and Box 5 different?

Your pay exceeded the Social Security wage base

For tax year 2026, the Social Security wage base is $184,500. Social Security tax applies only up to that amount for the year. Medicare tax continues on every dollar of Medicare-taxable wages, so Box 5 may be higher.

The IRS illustrates the result with an employee paid $199,750 during 2026: Box 3 is $184,500, while Box 5 is $199,750. This is an IRS example, not an average or statistical result. See the wage-base discussion in the 2026 W-2 and W-3 instructions.

Some compensation is subject to Medicare but not Social Security

Not every item of compensation necessarily receives identical treatment under both payroll taxes. Taxable benefits, retirement-plan amounts, tips and other forms of compensation can affect the boxes differently. The employer-facing W-2 instructions do not promise that every line on an individual pay stub will map one-for-one to a W-2 box. If the difference remains after you account for the wage base and tips, ask payroll to identify the specific compensation item.

Tips are being compared in the wrong fields

Employee-reported Social Security tips appear in Box 7 and reported tips are included in Box 5. Allocated tips appear in Box 8 and are not included in Box 3, Box 5 or Box 7. A tipped employee should therefore review all four fields and the employer’s tip records rather than judging the W-2 from Box 3 and Box 5 alone. The IRS treatment is described in the 2026 W-2 instructions and the Instructions for Form 8027.

Should Box 3 and Box 5 be the same?

Usually, yes—when your total wages are at or below that year’s Social Security wage base and the wages are subject to both Social Security and Medicare tax. The IRS says the boxes will generally match in that situation. They can differ legitimately when wages exceed the cap, when tips or other compensation have different treatment, or when a payroll reporting error exists.

Your situation What you would generally expect What to check
Wages below the year’s Social Security wage base; no special compensation Boxes 3 and 5 generally match. Confirm the W-2 year, employer and taxable year-to-date pay.
Wages above the year’s wage base Box 3 may stop at the wage base; Box 5 can be higher. Use the wage base for that exact tax year, not a prior-year figure.
Reported tips Box 5 includes reported tips; Box 7 separately identifies Social Security tips. Reconcile tip reports, Box 7 and the employer’s payroll records.
Allocated tips Box 8 can contain an amount that is absent from Boxes 3, 5 and 7. Do not add Box 8 to Box 3 or Box 5 as if it were reported cash wages.
Taxable benefits or unusual pay A difference may result from the particular tax treatment of the item. Ask payroll which item caused the difference and whether it was reported correctly.

How to check your Social Security and Medicare wages

1. Confirm the basic identifiers

  1. Make sure the form is a W-2 for the correct calendar tax year.
  2. Verify the employer name and employer identification information.
  3. Check that your name and Social Security number are correct. A typographical error or an unreported name change can keep earnings from being credited to your Social Security record.

2. Gather your year-to-date payroll records

Collect final pay stubs, payroll summaries and any separate tip reports for the year. Look for year-to-date Social Security wages, Medicare wages, reported tips, taxable benefits and any adjustment or correction issued by the employer. A pay stub may use labels that do not exactly match the W-2 boxes, so compare the underlying taxable-pay totals and ask payroll about any unexplained mapping.

3. Apply the correct year’s Social Security wage base

For a 2026 W-2, compare Social Security wages with the $184,500 2026 wage base. For an older W-2, use the wage base published for that year; do not copy the 2026 amount into a prior-year calculation. The IRS’s Topic No. 751 and the applicable year’s W-2 instructions provide payroll-tax information.

4. Compare the boxes with the cap in mind

  • If your Social Security wages are below the annual cap and the wages are subject to both taxes, Boxes 3 and 5 should generally be equal.
  • If total wages exceed the cap, Box 3 can be limited to the cap while Box 5 continues to include Medicare-taxable wages above it.
  • If Box 3 is below the cap but Box 5 is higher, look for reported tips, taxable benefits or another compensation category before treating the difference as an error.

5. Include tips in the reconciliation

For reported tips, check Box 7 and your tip declarations. The Social Security wage calculation uses Box 3 together with Box 7, subject to the annual wage base. Box 5 includes reported tips subject to Medicare tax. Box 8’s allocated-tip amount is informational and is not included in Boxes 1, 3, 5 or 7, so do not use it as a substitute for reported tips.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

6. Ask payroll for an explanation when the math still does not work

Give payroll a copy of the relevant pay stubs and identify the exact boxes and periods that do not reconcile. Ask whether the difference is caused by the Social Security cap, tips, a taxable benefit, a prior payroll adjustment or an incorrect wage report. Keep the written response and supporting records with your tax documents.

Why is my Medicare wage amount higher than my Social Security wage amount?

The most common reason is that your pay passed the Social Security wage base. Social Security tax stops applying once the year’s covered wages reach the cap, but Medicare tax continues without that cap. Thus, a higher Box 5 is expected for a high earner even when every payroll report is correct.

For example, with $199,750 of 2026 wages subject to both taxes and no complicating tip issue, Box 3 can show $184,500 and Box 5 can show $199,750. The amounts are different because the taxes have different limits, not because Medicare wages are necessarily being overstated.

Additional Medicare Tax is a separate check

Do not use Box 3 to calculate Additional Medicare Tax. Employers generally begin withholding Additional Medicare Tax when wages paid to an employee exceed $200,000 during 2026 and continue withholding for the rest of that calendar year. That $200,000 figure is the employer’s withholding trigger; your final liability depends on filing status and the wages reported on your tax return.

The IRS uses Box 5 Medicare wages and tips for the individual calculation on Form 8959. Review the Instructions for Form 8959 rather than trying to derive the tax from Box 3. The employer withholding rules are also explained in the 2026 Form 941 instructions and IRS Topic No. 761.

What to do if the W-2 appears wrong

Contact the employer or payroll department first

Explain whether the issue is a wrong dollar amount, an omitted tip, an incorrect tax year, a name or Social Security number mismatch, or a difference that does not follow the wage-base rules. Supply pay stubs, tip records and the W-2, and ask the employer to review the wage report it submitted.

Understand Form W-2c

If the employer confirms that a submitted W-2 is incorrect, the employer—not the employee—corrects the filing with Form W-2c. The Social Security Administration explains that employees do not need to make a separate earnings-record correction when their employer has submitted the W-2c. Keep the original W-2, the corrected W-2c and any updated W-2 for your records. See the SSA guidance on whether an employee must do anything after an employer submits a W-2c and the SSA Employer Correction Request Notices.

Do not alter the W-2 yourself

Do not cross out amounts, write in replacement figures or submit a self-created W-2 as a correction. If the employer will not respond, document your requests and contact the appropriate tax or Social Security agency for guidance while retaining your evidence.

Ask the employer to correct an employer-submitted error

If the W-2 itself is wrong, ask the employer to submit Form W-2c. Once the employer has submitted that correction, SSA says you do not need to file a separate correction for the same earnings. Follow up with SSA if the corrected earnings do not appear after processing time.

Observe the ordinary correction deadline

SSA generally allows a request to correct an earnings record until three years, three months and 15 days after the end of the year in which the wages were paid. Exceptions apply to specified types of errors and missing wages, so the deadline is not absolute. Contact SSA promptly, especially for an older year, and ask which exception or evidence rules apply to your situation.

Gather proof before you contact SSA

  • W-2 or W-2c for the affected year
  • Final pay stubs and year-to-date payroll summaries
  • Employment records showing dates and pay
  • Tip declarations or other records supporting reported tips
  • Documents showing a legal name change, if the name on the wage record differs

What if my name or Social Security number is wrong?

A mismatch can prevent SSA from crediting wages to your record. SSA identifies typographical errors, an unreported name change and inaccurate or incomplete employer records as possible causes. Compare the name and number on your W-2 with your Social Security card and payroll records. Ask the employer to correct a submitted W-2 with Form W-2c, then verify that SSA receives and credits the correction. The employer-correction process is described in the SSA’s employee correction guidance.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How the W-2 and SSA record fit together

Document or record What it tells you Who normally fixes an error
W-2 The employer’s reported wages, tips and withholding for the tax year. The employer issues a corrected W-2c when the submitted form is wrong.
Pay stubs and payroll reports Detailed year-to-date amounts that help explain how the W-2 was assembled. Employer payroll staff should explain or correct discrepancies.
SSA earnings record The wages SSA has credited to your work history for benefit calculations. You can ask SSA to correct a missing or incorrect entry, using the employer’s W-2/W-2c and other evidence.

A W-2 discrepancy and an SSA-record discrepancy are related but not identical problems. First determine whether the employer’s form is wrong, whether SSA has failed to post a correct form, or whether your identifying information prevented crediting. That diagnosis determines whether payroll, SSA or both need to act.

A practical decision path

  1. Check the year. Identify the W-2 tax year and find that year’s Social Security wage base.
  2. Check the cap. If wages exceeded the cap, a higher Box 5 may be expected.
  3. Check tips. Reconcile Box 7, reported tips and Box 8 allocated tips separately.
  4. Check compensation details. Ask about taxable benefits or other pay that may be subject to Medicare but not Social Security.
  5. Check identity details. Verify your name and Social Security number.
  6. Contact payroll. Request an explanation and a Form W-2c if the submitted W-2 is wrong.
  7. Check SSA. Review your earnings record and submit evidence if wages are missing or miscredited, observing the ordinary correction period and its exceptions.

Key takeaways

  • Box 3 is Social Security wages; Box 5 is Medicare wages and tips.
  • Only Social Security wages have an annual wage-base cap. The 2026 cap is $184,500.
  • When wages are below the cap and subject to both taxes, Boxes 3 and 5 generally match.
  • Above the cap, Box 5 can exceed Box 3 without an error.
  • Reported tips affect Box 7 and Box 5; allocated tips in Box 8 are excluded from Boxes 1, 3, 5 and 7.
  • An employer corrects an incorrect submitted W-2 with Form W-2c, while SSA handles a separate request to correct its earnings record.
