UnitedHealth Group was larger by consolidated revenue in the latest common quarter available: it reported $112.0 billion for the second quarter of 2026, compared with $106.096 billion for CVS Health. That $5.9 billion gap describes one quarter of reported sales, not profitability, valuation or investment quality. The companies also make money in different ways. UnitedHealth combines UnitedHealthcare insurance with Optum’s care, data and pharmacy businesses; CVS combines Aetna insurance with pharmacy-benefit services, retail pharmacies, clinics and consumer health operations.
The fairest comparison therefore aligns the same quarter, separates GAAP from adjusted measures, and treats insurance metrics, pharmacy activity and customer counts as different measures rather than interchangeable proof of scale. The figures below use results available through June 30, 2026, the latest common reporting period at the October 13, 2026 cutoff.

At a glance: the latest common-quarter comparison
| Measure | UnitedHealth Group | CVS Health | What it tells you |
|---|---|---|---|
| Q2 2026 revenue | $112.0 billion | $106.096 billion | UnitedHealth reported approximately $5.9 billion more consolidated revenue for the quarter. |
| Q2 2026 operating measure | $8.0 billion earnings from operations | $4.703 billion GAAP operating income; $5.157 billion adjusted operating income | CVS presents both GAAP and adjusted figures. A like-for-like comparison should not place CVS adjusted income against UnitedHealth’s reported measure without explaining the difference. |
| Q2 2026 net margin | 4.9% | Not stated in the cited Q2 release | A margin for one company cannot establish relative profitability when the other company’s comparable figure is not being reported on the same basis. |
| Insurance medical-cost metric | Not presented as a directly comparable figure in the cited materials | Health Care Benefits medical benefit ratio (MBR) of 87.4%, versus 89.9% in Q2 2025 | MBR is a segment insurance measure, not consolidated operating margin. |

UnitedHealth Group: UnitedHealthcare plus Optum
UnitedHealth Group describes two complementary businesses. UnitedHealthcare is the health-benefits operation, selling and administering insurance coverage. Optum provides care and health services built around technology and data. In the 2025 Form 10-K, UnitedHealth reports four operating segments:
- UnitedHealthcare: the insurance business, covering employer, individual, government and other health-benefit arrangements.
- Optum Health: care delivery and related health services.
- Optum Insight: data, analytics, technology and advisory services for healthcare organizations.
- Optum Rx: pharmacy-benefit and pharmacy services.
The authoritative segment definitions and segment reporting are in UnitedHealth’s 2025 Form 10-K. The structure lets UnitedHealth earn insurance premiums while also operating businesses that provide care, manage prescriptions or sell technology and analytical services.
CVS Health: Aetna, Health Services and a physical pharmacy network

CVS reported approximately 9,000 retail locations, more than 1,000 walk-in and primary-care clinics, about 87 million pharmacy-benefit-manager plan members and an estimated more than 37 million people served through insurance products and related services at December 31, 2025. These are company-reported indicators, not an independent market-share survey, and they are not directly equivalent to any single UnitedHealth “consumers served” number.

Why revenue is not an apples-to-apples measure of business quality
Revenue is the cleanest first answer to “which company is bigger?” for a specified period, but it is not a direct measure of profit or economic value. UnitedHealth and CVS have different combinations of premiums, pharmacy transactions, retail sales and service fees. Their consolidated statements also include activity between segments that is eliminated when the company reports total revenue.
Insurance premiums and medical costs
Insurance revenue is collected before the company pays medical claims and other healthcare costs. A large premium base can therefore coexist with high medical expenses. The useful question is not simply how much premium revenue was booked, but how much remains after claims, operating expenses, interest, taxes and other costs.

Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Pharmacy-benefit and prescription activity
Pharmacy-benefit businesses may process very large prescription volumes and may report revenue using arrangements that differ from retail pharmacy sales or service-fee contracts. Gross claims activity, amounts paid to pharmacies and the portion retained by the benefit manager do not all represent the same economic value. Comparing headline revenue without reading the accounting presentation can overstate how much external value each dollar represents.
Internal transactions and eliminations
Each company can use one business unit’s services in another unit. When consolidated financial statements are prepared, intersegment revenue is eliminated. As a result, segment totals will not necessarily add to consolidated revenue, and a segment’s reported sales are not automatically external sales to unrelated customers.
For that reason, the $112.0 billion versus $106.096 billion result is a valid consolidated-quarter scale comparison, but it does not prove that UnitedHealth has a superior margin, lower medical costs or a better valuation.

CVS Health
CVS reported $106.096 billion in revenue for the same quarter. It reported $4.703 billion of GAAP operating income and $5.157 billion of adjusted operating income. The two CVS figures are intentionally different: adjusted operating income excludes items defined by the company’s non-GAAP reconciliation. It should not be treated as interchangeable with GAAP operating income.
What can and cannot be ranked
- Consolidated revenue: UnitedHealth was higher by approximately $5.9 billion in Q2 2026.
- Operating income: UnitedHealth’s $8.0 billion reported earnings from operations can be compared with CVS’s $4.703 billion GAAP operating income as a broad reported measure, but the companies’ segment mixes and accounting details still matter.
- Adjusted operating income: CVS’s $5.157 billion adjusted figure is useful for understanding management’s preferred performance presentation, but it should not be ranked against UnitedHealth’s figure unless UnitedHealth’s corresponding adjusted definition is also used.
- Net margin: UnitedHealth’s 4.9% figure is not enough to declare a more profitable company because the cited CVS release does not provide a matching net-margin figure in the supplied comparison.
The correct conclusion from this quarter is narrow: UnitedHealth had higher reported revenue and a higher reported operating-earnings figure than CVS on the measures shown. It is not a conclusion about long-term returns, stock performance or enterprise value.
What CVS’s medical benefit ratio says about insurance performance
CVS’s Health Care Benefits segment reported an 87.4% medical benefit ratio (MBR) in Q2 2026, down from 89.9% in Q2 2025. MBR generally expresses medical costs as a percentage of premium revenue for the insurance segment: a lower percentage means less of each premium dollar was used for medical claims during that measurement period, before the segment’s other expenses.
CVS attributed the year-over-year reduction primarily to improved underlying performance in its Government business and the absence of the prior-year premium-deficiency reserve. That explanation is important because a reserve effect can make a single quarter look unusually strong or weak. The two-quarter comparison should therefore not be presented as proof of a permanent improvement in insurance economics.
Rank #4
MBR is also not the same as consolidated operating margin. It excludes the broader effects of administrative costs, pharmacy and retail operations, corporate expenses, interest, taxes and other items. UnitedHealth’s cited Q2 materials do not provide an identically presented figure for this comparison, so the CVS MBR should be read as a CVS Health Care Benefits indicator rather than a league-table score against UnitedHealth.
Scale indicators: covered people, PBM members and locations are different
CVS’s year-end figures illustrate why operating reach needs labels. About 87 million PBM plan members refers to people in pharmacy-benefit arrangements. More than 37 million people served through insurance products and related services refers to an insurance-related population estimate. Approximately 9,000 retail locations and more than 1,000 clinics are physical sites. None is a substitute for the others.
UnitedHealth’s UnitedHealthcare membership, Optum patient relationships, pharmacy activity and technology customers likewise represent different populations and transactions. A company can have more covered lives but fewer retail locations, or more prescription transactions without having more insured members. The cited filings do not establish a single, standardized “customers served” denominator that makes the two companies directly rankable.

For a full-year analysis, use the audited fiscal-year filings for both companies and label every number fiscal year ended December 31, 2025. UnitedHealth’s 2025 Form 10-K and CVS Health’s 2025 Form 10-K contain annual revenue, earnings, cash flow, balance-sheet, risk-factor and segment information. Do not combine UnitedHealth’s FY2025 revenue with CVS’s Q2 2026 revenue, a trailing-period figure or management guidance.
An annual worksheet should keep these columns separate:

| Comparison field | Required label | Reason |
|---|---|---|
| Revenue | Company, fiscal period and consolidated measure | Prevents quarterly, annual and trailing-period figures from being mixed. |
| Operating result | GAAP operating income or the company’s stated earnings-from-operations measure | Defines what costs and adjustments are included. |
| Adjusted result | Company-specific non-GAAP definition | Adjustments differ, so the labels cannot be silently treated as equivalent. |
| Cash flow | Operating cash flow for the same fiscal period | Profit and cash generation answer different questions. |
| Balance sheet | As-of date, not an average-period figure | Debt, cash and other balances change during the year. |
| Segment performance | Segment name and eliminations policy | Segment totals do not automatically equal external consolidated sales. |
A practical framework for interpreting the comparison
1. Start with the business mix
Ask how much of each company is insurance, pharmacy-benefit administration, care delivery, retail pharmacy, consumer products or technology. UnitedHealth’s Optum structure gives it separately reported care, insight and pharmacy businesses alongside UnitedHealthcare. CVS’s structure includes a large physical retail and consumer-health operation in addition to Aetna and Health Services.
2. Check whether the earnings measures match
Use GAAP operating income for both companies when the goal is a standardized reported comparison. If adjusted figures are shown, place them in a separate row and list the exclusions from each company’s reconciliation. Never call one company “more profitable” solely because its adjusted figure exceeds the other company’s GAAP figure.
3. Examine insurance cost trends in context
For an insurer, medical-cost ratios, enrollment mix and reserve changes can move quarterly results. A favorable MBR can result from underlying claims improvement, a business-mix shift, a reserve release or the absence of a prior charge. Read the explanation accompanying the ratio instead of treating one quarter as a durable trend.

Consolidated revenue is an external scale measure after eliminations. Segment revenue, prescription volume, PBM members, clinic visits and internal service transactions answer different questions. Use the denominator that matches the question being asked.
5. Keep reported results separate from outlook
Results through June 30, 2026 are historical. Management guidance is a forward-looking outlook and should not be blended into the reported-quarter table. CVS raised its full-year 2026 guidance in the cited release, but guidance is not an achieved result.
Why the cutoff date matters
UnitedHealth announced that its third-quarter 2026 release was scheduled for October 13, 2026, in a release-date notice. The comparison cutoff is October 13, 2026 at 06:26:41 UTC, before that scheduled publication was available in the supplied record. Q2 2026 is therefore the latest common reported quarter used here. Any later Q3 release would need to be evaluated separately rather than silently added to a Q2-versus-Q2 comparison.
How management describes UnitedHealth’s priorities
In UnitedHealth Group’s July 16, 2026 Q2 release, chief executive officer Stephen Hemsley said: “Our results and outlook reflect the continuing progress in our work to simplify how we operate, improve both affordability and the healthcare experience for patients and care providers, and apply modern technology to create real improvement for people.” This is company framing about priorities and outlook, not independent evidence that each stated outcome has already been achieved.
Bottom line
UnitedHealth Group was the larger company by Q2 2026 consolidated revenue, at $112.0 billion versus CVS Health’s $106.096 billion. UnitedHealth’s model is centered on UnitedHealthcare plus Optum’s care, data and pharmacy businesses; CVS combines Aetna insurance with pharmacy-benefit services, retail pharmacies, clinics and consumer health operations. CVS’s 87.4% Q2 Health Care Benefits MBR provides a useful insurance-cost signal, but it is not a consolidated margin and its year-over-year improvement includes the effect of a prior-year reserve. The most defensible comparison uses aligned periods, GAAP measures first, adjusted results separately, and clearly labeled operating-reach metrics.
