Do not assume your U.S. health plan or a policy labeled “travel insurance” will pay every medical bill overseas. Your regular health insurance, travel medical insurance, evacuation coverage, and trip-cancellation insurance are separate benefits with different triggers, limits, and claims rules. A regular plan may pay for some emergency or routine treatment abroad, while a trip policy may protect only prepaid reservations. Medical evacuation is a separate question again.
Before departure, confirm your destination, dates, activities, medical conditions, payment method, preauthorization rules, and evacuation destination with the insurer. The terms of your own plan and the policy issued for this trip control what you can recover.

The four kinds of coverage travelers often confuse
“Travel insurance” is a broad marketing label rather than a single benefit. The U.S. Department of State says, “There are different kinds of travel or trip insurance that can protect your health, your trip, and your finances.” Read the policy schedule and definitions to identify which protection you actually bought.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problems| Coverage | Primary purpose | Questions to verify |
|---|---|---|
| Existing health insurance | Medical benefits under your normal U.S. plan; overseas treatment is plan-specific. | Does it apply in each destination? Are emergency and routine services covered? Are foreign providers accepted? Must you pay first? What are limits, preapproval rules, and claim deadlines? |
| Travel medical insurance | Short-term benefits for eligible medical care during a trip, either standalone or bundled with other travel benefits. | Covered services, medical maximum, deductible, trip-length limit, geography, pre-existing-condition rules, activities, exclusions, assistance line, and payment process. |
| Medical evacuation | Eligible emergency transportation to an appropriate facility, potentially including return to the United States. | What “medically necessary” means, who authorizes and arranges transport, where the patient may be taken, transport limits, companion or repatriation benefits, and exclusions. |
| Trip cancellation or interruption | Reimbursement of certain prepaid travel costs after a covered event forces cancellation or a change. | Covered events, reimbursement basis, documentation, notice deadlines, and exclusions. This benefit generally does not pay foreign medical bills. |
One category does not automatically include another. Medicare specifically warns that travel insurance does not necessarily include health insurance, so read conditions and restrictions rather than relying on the product name.

What your current health insurance may do overseas
Some U.S. health plans cover typical medical costs in other countries; others exclude foreign care or cover only narrow emergencies. The State Department’s August 11, 2025, travel-insurance guidance and international checklist tell travelers to review the individual plan or call the insurer because the plan document controls.
Emergency versus routine treatment
Ask separately about emergency care and routine care. A plan might reimburse an emergency-room visit after an accident but exclude a planned consultation, follow-up appointment, physical therapy, or care for a condition that was not urgent. Request a written explanation for every destination on your itinerary, including stopovers and cruises.
Provider acceptance and payment
Foreign hospitals may not recognize a U.S. insurance card or may require a deposit before treatment. Confirm whether the insurer has a local network, can issue a guarantee of payment, or expects you to pay the provider and submit a claim. Ask which currency and documents are acceptable, whether itemized bills must be translated, and how the insurer calculates reimbursement.
Limits, authorization, and exclusions
Clarify annual or trip-specific limits, deductibles, coinsurance, emergency definitions, preauthorization requirements, and exclusions for activities such as scuba diving, mountaineering, motorcycling, or organized competition. Ask how the policy handles pre-existing conditions and whether a medical-assistance center must approve admission, surgery, or transfer.
Travel medical insurance: what it is designed to cover
Travel medical insurance is intended to fill gaps for eligible treatment during a defined trip. Depending on the policy, benefits can include emergency physician services, hospital care, urgent prescriptions, diagnostic tests, and medically necessary transport to a nearby facility. The policy wording determines the covered services and the maximum payable amount.

Key policy fields
- Geography: Check every country, territory, and transit point. A policy may exclude a destination under a government travel warning or exclude domestic treatment after you return home.
- Trip length: Confirm the exact departure and return dates, grace periods, and extension procedure if you are delayed.
- Medical maximum and deductible: A large maximum does not eliminate deductibles, coinsurance, sublimits, or service-specific caps.
- Pre-existing conditions: Look for a definition, stability period, waiver requirements, and exclusions for complications of an existing diagnosis.
- Activities: Verify coverage for the sports, work, volunteer service, altitude, or equipment use you actually plan.
- Assistance and preauthorization: Record the 24-hour number and learn which treatments require approval before they occur.
- Claims: Note notice deadlines, required medical records, itemized invoices, proof of payment, and rules for duplicate recovery from another plan.
A policy can pay medical expenses without paying to cancel a flight, and cancellation coverage can reimburse a ticket without paying a hospital. Treat each insuring agreement as its own contract.
Medical evacuation is a separate financial risk
Most regular health plans do not pay to return a patient to the United States by air ambulance. Evacuation coverage may be included in a travel medical policy or purchased as an add-on, but the trigger is usually an insurer-approved medical necessity rather than a traveler’s preference.

Pick the symptom - the matching free tool is one click away.
How evacuation decisions are made
Ask who decides that transport is medically necessary, who selects the aircraft and receiving facility, and whether the assistance company must arrange the flight. A policy may transport you to the nearest suitable hospital rather than home. Other policies can authorize repatriation after stabilization, subject to a separate limit.
Questions about the destination and companions
Check whether transport ends at a local facility, a regional center, or a hospital in the United States. Ask about a family member’s fare, an attendant, return transportation for a traveling companion, and repatriation of remains. Verify exclusions for remote locations, war, alcohol-related incidents, and activities that the policy treats as hazardous.
Potential cost
The State Department’s “Medicine and Health” page, last updated August 11, 2025, estimates that an air ambulance to the United States can cost $20,000 to $200,000, depending on location and health condition. That is an agency estimate, not a quote for your route or a promise that any policy will pay it. The same guidance states, “The U.S. government does not pay medical bills abroad.”

Trip cancellation and interruption coverage
Cancellation or interruption insurance addresses prepaid, nonrefundable travel costs when a listed event occurs. Covered events might include a defined illness, death in the family, severe weather, or another event specified in the contract. The policy may reimburse the documented, eligible loss rather than every expense you incurred.
Read the covered-event list, exclusions, documentation rules, notice deadline, and definition of “trip cost.” A cancellation benefit normally does not pay a foreign doctor, hospital, prescription, or evacuation bill. If illness is the reason for canceling, the policy may require medical records and may exclude a condition that was not stable for the required period.
Medicare, Medigap, and Medicaid considerations
Medicare usually does not cover health care outside the United States, and beneficiaries generally pay the full cost. The Medicare page “Travel outside the U.S.” (accessed October 13, 2026) lists limited exceptions, including certain situations in which a foreign hospital is closer than a U.S. hospital and travel through Canada between Alaska and another U.S. state. Those exceptions are narrow and fact-specific.
Some Medigap policies may cover emergency care abroad. Check the individual policy for its lifetime foreign-travel emergency limit, deductible, timing, and geographic conditions. Medicare Part D plans do not cover prescription drugs purchased outside the United States. Medicare explains separate rules for vaccines recommended by the Advisory Committee on Immunization Practices, so confirm vaccine coverage before departure.
The State Department’s checklist cautions that U.S. health insurance, including Medicare and Medicaid, usually does not cover care abroad. Medicaid eligibility and payment rules are state-specific; call the program or plan before relying on it overseas.

How to compare a regular plan with a travel policy
Use the same questions for both contracts. A side-by-side worksheet prevents a high medical maximum from hiding a restrictive exclusion or an evacuation benefit that ends at the nearest hospital.
| Comparison axis | What to record | Why it changes the decision |
|---|---|---|
| Geography | Countries, territories, transit points, cruise waters, and warning-related exclusions | A policy valid in one destination may not apply to another stop. |
| Duration | Covered start and end dates, extension rules, and maximum trip length | A single extra day can create an uninsured period. |
| Care type | Emergency, routine, follow-up, prescriptions, dental, and mental-health services | Emergency-only protection may not meet a long-stay traveler’s needs. |
| Financial terms | Medical maximum, deductible, coinsurance, sublimits, and currency rules | These determine the traveler’s out-of-pocket exposure. |
| Evacuation | Trigger, authorizer, destination, transport limit, repatriation, and companion benefits | “Evacuation included” does not necessarily mean transport home. |
| Medical history | Pre-existing-condition definition, stability period, waiver, and disclosure duty | An undisclosed condition can lead to a denied claim. |
| Activities | Sports, work, volunteer duties, altitude, vehicles, and alcohol exclusions | Ordinary leisure coverage may exclude the activity you booked. |
| Payment and claims | Direct billing, guarantee of payment, receipts, translations, preapproval, and deadlines | You may need substantial cash or a specific call before treatment. |
| Trip benefits | Cancellation, interruption, delay, baggage, and covered-event definitions | These benefits protect travel costs, not necessarily medical bills. |
What to do before you buy or depart
- Call your existing insurer. Ask whether emergency and routine care is covered in every destination, whether local providers are accepted, how a hospital obtains payment, and how reimbursement works if you pay upfront.
- Map the trip against the policy. List countries, dates, transit points, activities, planned excursions, current conditions, medications, and any need for dialysis, oxygen, or other regular treatment.
- Read the exclusions and limits. Mark deductible or cost-sharing, pre-existing-condition rules, covered-event definitions, preauthorization requirements, claim deadlines, and the 24-hour assistance number.
- Analyze evacuation wording. Identify the medical-necessity test, decision-maker, arranging company, receiving destination, transport limit, and whether return to the United States is possible.
- Prepare claim records. Keep the insurance card, policy number, claim forms, assistance number, prescriptions, physician letters, and copies of bills where you can reach them without internet access.
- Check destination health guidance. Learn how local facilities handle deposits and payment, where suitable care is available, and whether medicines legal in the United States are restricted at your destination. The State Department warns that some countries restrict such medicines.
- Confirm the final itinerary. If dates, destinations, or activities change, ask whether the policy must be endorsed or reissued. Do not assume a booking change automatically extends insurance.
What to do if you need care abroad
Before treatment when the situation allows
Call the insurer or assistance center before admission, surgery, evacuation, or an expensive test if the policy requires preauthorization. Ask for a case number, the approved facility, payment instructions, and the documents you must retain. If the facility demands a deposit, ask whether the assistance company can provide a guarantee of payment.
During an emergency
Obtain urgent care first when delay would endanger you. Once stable, contact the assistance number, the regular health plan, or both. Tell the provider which insurer is primary and ask for an itemized bill, diagnosis, treatment dates, receipts, and proof of payment. Keep copies of correspondence and note the names, dates, and case numbers for every call.
After returning home
Submit the claim by the stated deadline. Include the claim form, itemized invoices, medical records, payment evidence, travel dates, and any preauthorization or case number. If another plan may pay first, follow coordination-of-benefits instructions and do not submit altered or duplicate invoices.

- Buying cancellation insurance and assuming medical care is included: Confirm that a medical insuring agreement and evacuation benefit are actually listed.
- Relying on the insurance card: Card possession does not prove that a foreign provider accepts the plan or that the insurer will pay directly.
- Ignoring routine-care needs: Emergency-only protection may leave medication refills, scheduled treatment, or chronic-care visits uncovered.
- Failing to disclose conditions or activities: Compare your medical history and itinerary with the definitions and exclusions before purchase.
- Assuming evacuation means a flight home: Read the authorized destination and medical-necessity language.
- Waiting until the bill arrives to call: Preauthorization and assistance rules can affect eligibility, especially for admissions and transport.
- Overlooking payment logistics: Carry a payment method for deposits and retain every original receipt if reimbursement is required.
- Forgetting medication restrictions: Check destination rules and carry documentation; U.S. legality does not guarantee foreign legality.
Which coverage combination fits different travelers?
Short vacation with strong employer coverage
Ask the employer plan for written destination-specific terms, emergency and routine benefits, foreign-provider payment instructions, and evacuation exclusions. Add travel medical or evacuation protection only for gaps the regular plan leaves, rather than buying a broad package solely because it says “travel insurance.”

Medicare beneficiary
Start with Medicare’s foreign-care exceptions and your Medigap certificate. If ordinary overseas care is excluded or limited, compare a travel medical policy with evacuation protection and review prescription arrangements separately because Part D does not pay for medicines purchased outside the United States.
Remote, adventure, or limited-care destination
Prioritize evacuation authorization, transport destination, activity eligibility, and a medical assistance center that can coordinate local care. The State Department specifically recommends considering evacuation insurance for higher-risk areas or places with limited medical facilities.
Long stay or frequent traveler
Check maximum trip length, renewal or extension rules, routine-care benefits, pre-existing-condition provisions, and whether coverage ends when you return to the United States. A policy built for a two-week vacation may not fit a multi-month stay.
