Short answer: A cash-out refinance replaces your current mortgage with a larger one and gives you the difference in cash. A HELOC usually leaves your first mortgage in place and adds a separate, revolving credit line secured by your home. Choose only after comparing written offers for the amount you need, total cost over the time you expect to borrow, payment variability, tax treatment and the value of keeping your existing mortgage terms. Both options can lead to foreclosure if you cannot repay.
How the two debts are structured
Cash-out refinance
A cash-out refinance pays off your existing first mortgage and replaces it with a new, larger mortgage. After the old balance, transaction costs and any other payoff amounts are covered, the remaining proceeds are delivered to you as a lump sum. You receive one mortgage payment, but the new loan can have a different interest rate, term, payoff date and closing-cost structure.

Because the entire mortgage is replaced, the new rate applies to the full balance, not just the cash you take out. If your current mortgage has an unusually low rate or favorable remaining term, replacing it can be expensive even when the new monthly payment appears lower. The Consumer Financial Protection Bureau (CFPB) notes that cash-out refinances generally have higher closing costs, may take longer to pay off and may carry a higher rate than the existing mortgage. See the CFPB’s guidance on home-equity borrowing and mortgage costs.

A home equity line of credit is normally a second mortgage when you already have a first mortgage. Your original mortgage remains in place, and the HELOC gives you a credit limit from which you can draw, repay and draw again during the availability period. The CFPB describes it as a line of credit similar to a credit card, except that your home secures the borrowing: CFPB explanation of HELOCs.

HELOCs are typically variable-rate products. Your rate and required payment can change as the underlying index changes and as your outstanding balance changes. Many contracts have a draw period followed by a repayment period; the payment can rise when the draw period ends, when the balance begins amortizing or when a promotional rate expires. You normally make a separate HELOC payment in addition to your first-mortgage payment.
Cash-out refinance versus HELOC
| Decision point | Cash-out refinance | HELOC |
|---|---|---|
| Existing first mortgage | Paid off and replaced by a larger mortgage. | Usually remains unchanged; the HELOC is an additional lien. |
| How you receive money | One lump-sum disbursement at closing. | Draws from a credit line, potentially multiple times during the draw period. |
| Rate structure | May be fixed or variable, depending on the offer. | Typically variable; the rate and payment may change. |
| Monthly payments | One new mortgage payment. | A separate HELOC payment plus the existing mortgage payment. |
| Best structural fit | A known, one-time amount when replacing the current mortgage is acceptable. | Expenses that arrive over time or require a reusable line. |
| Main cost questions | Closing costs, points, prepaid items, new term, total interest and payoff balance. | Opening and ongoing fees, draw and repayment terms, rate adjustments and payoff balance. |
| Collateral risk | Your home secures the replacement mortgage. | Your home secures the additional line; the balance may be due under the contract when the home is sold. |
Exact fees, rate caps, adjustment frequency, minimum or required draws, draw availability, repayment length and early-closure provisions vary by lender. Treat the table as a structural comparison, not a quote. Ask for the same borrowing amount and the same expected borrowing period in every offer.
Start with the mortgage you already have
Before requesting new credit, write down your current principal balance, interest rate, monthly payment, remaining term, escrow amount and any prepayment conditions. A refinance gives up this loan, while a HELOC normally does not. Calculate the value of retaining your current rate and the number of years left before the mortgage is paid off.

Also define the cash requirement. A single, known expense may align with a lump-sum refinance; a renovation paid in stages, an irregular business need or a reserve for uncertain costs may align with a line you draw as needed. This is a cash-flow description, not proof that either option is affordable. Borrow only an amount that remains manageable if your income falls or the rate rises.

Compare total cost, not the headline payment
Costs to include in a refinance
Request a written Loan Estimate and add lender charges, origination fees, points, appraisal and other third-party costs, government charges, title services and prepaid interest, taxes or insurance. A lower monthly payment can simply reflect a longer repayment term, a larger principal balance or costs financed into the loan. Compare the projected balance and total interest at the date you expect to sell or repay, not only the first payment.
Points and lender credits change the trade-off between upfront and ongoing cost. The CFPB explains how to evaluate them over different holding periods in its guidance on discount points and lender credits. A break-even calculation can be useful when the only material difference is upfront refinance cost and monthly savings: divide the additional upfront cost by the monthly savings. It is not a complete answer when the loan term, balance, rate structure or planned holding period differs, so also compare balances and cumulative interest at several dates.
Costs to include in a HELOC
Ask for every opening fee, annual or maintenance charge, transaction or draw fee, appraisal cost, inactivity charge, conversion fee and early-closure fee. Confirm whether there is a minimum initial draw or minimum balance. Then obtain the draw-period length, repayment-period length, rate index, margin, adjustment frequency, periodic and lifetime caps, and the payment formula.

Use a common time horizon
For both products, model the balance and total dollars paid after a short, medium and long period that match your plans—for example, the date you might sell, refinance again or finish the project. Include the cash actually received, all fees, interest, principal reduction and the remaining payoff balance. A product with a lower first payment can cost more over the period you keep it.
A step-by-step comparison process
- Document the current loan. Record the balance, rate, term remaining, payment and payoff conditions. Obtain a current payoff statement so the refinance comparison uses the real amount.
- Specify the amount and timing. Separate money needed immediately from money that may be needed later. Do not request a larger line merely because the approved limit is available.
- Request comparable written offers. Ask for a cash-out refinance estimate and a HELOC disclosure for the amount actually needed. Request the same assumptions about property value, occupancy and repayment horizon from each lender.
- Read the rate mechanics. For a refinance, identify whether the rate is fixed or adjustable, the term and any reset rules. For a HELOC, identify the index, margin, adjustment dates, caps, draw period and repayment period.
- Stress-test payments. Have the HELOC lender illustrate payments after a meaningful rate increase, at a high permitted rate and after the draw period ends. For a refinance, compare payments if taxes, insurance or an adjustable rate change; those items may not be included in the advertised principal-and-interest figure.
- Calculate total cost at several dates. Add upfront and ongoing charges, cumulative interest, principal paid and the projected payoff balance. Include points or lender credits and any cost financed into the balance.
- Evaluate flexibility and behavior. Decide whether a revolving line would make repeated borrowing too easy, whether a single disbursement could leave unused cash accruing interest, and whether you can reliably make two payments if you choose a HELOC.
- Review tax use before closing. Keep a separate record of each draw and how it was spent. Ask a qualified tax professional how the intended use, your filing status and debt limits affect eligibility.
- Check the contract before signing. Confirm the right to cancel where applicable, draw expiration, minimum payments, rate caps, early-closure terms, lien position and what happens if the home is sold.
Does a HELOC change your mortgage rate?
Usually not. A HELOC normally leaves the first mortgage’s rate and payment unchanged, while its own variable rate applies to the amount drawn. Your total housing debt payment is the two payments combined. A cash-out refinance does change the mortgage rate because it replaces the original loan; the new rate applies to the entire refinanced balance.
That distinction matters when your first mortgage rate is substantially below current offers. Keeping it can preserve a valuable term, but a HELOC’s variable payment and second-lien costs must still fit your budget. Conversely, if the existing mortgage is expensive and the refinance offer materially improves the rate after all costs, replacing it may be worth analyzing rather than assuming that a second lien is cheaper.
Tax treatment of interest
For tax year 2025, IRS Publication 936 says interest on home-equity debt generally qualifies for the home-mortgage-interest deduction only when the proceeds are used to buy, build or substantially improve the home that secures the debt, and only when you itemize and satisfy other applicable limits. The rules are explained in IRS Publication 936 (2025), Home Mortgage Interest Deduction.

Using a HELOC for credit-card bills, tuition, a vehicle or other personal spending does not automatically make the interest deductible. Keep invoices, draw dates and bank records that connect qualifying expenditures to the secured home. A refinance of acquisition debt generally retains that character only up to the old mortgage principal balance immediately before refinancing; additional cash-out may be treated differently depending on its use. Tax rules and your facts can change, so obtain current professional advice rather than relying on an advertised deduction.

Your home secures both debts
Failure to pay either loan can lead to foreclosure. A HELOC may also be frozen or reduced if the lender determines that property value or your financial condition has changed. Selling the home generally requires paying off liens, and the contract may require repayment of the HELOC at sale.
Variable payments can rise
With a HELOC, a higher index or a larger drawn balance can increase the required payment. The transition from draw to repayment can produce a separate payment shock. Maintain a cash reserve and test the payment at rates above today’s quote before borrowing.

Existing payment trouble
If your mortgage payment is already difficult, adding a lien or replacing the loan may worsen the problem. The CFPB advises speaking with a housing counselor before taking a home-equity loan or HELOC to explore alternatives that may make better financial sense: CFPB consumer guidance.
Which option fits common needs?
You need a known lump sum
Compare a cash-out refinance with a fixed-rate home-equity loan as well as a HELOC. A refinance may provide one payment, but replacing a low-rate first mortgage can raise the cost of every dollar already borrowed. A HELOC avoids that replacement but may leave you with variable payments and two liens.
You expect staged expenses
A HELOC’s ability to draw, repay and draw again can match expenses that arrive over time. Confirm that the draw period lasts long enough, that undrawn availability cannot be canceled unexpectedly under the contract and that you can handle the repayment-period payment.
Recommended Free Tools
You plan to sell soon
Short ownership favors low upfront cost and a clear payoff calculation. Large refinance costs may not be recovered before sale. Check HELOC early-closure charges and make sure sale proceeds will cover both liens and transaction costs.
You want to consolidate unsecured debt
Converting credit-card or personal debt into debt secured by your home changes the consequence of nonpayment. Compare the total interest savings with fees, the repayment term and the risk of foreclosure. Do not assume that tax benefits apply to personal-use proceeds.
Questions to put to every lender
- What is the annual percentage rate and how is it calculated?
- Which fees are charged at opening, annually, per draw or at early closure?
- What is the minimum initial draw or minimum balance?
- For a HELOC, what are the index, margin, adjustment frequency and rate caps?
- How long are the draw and repayment periods, and what payment is due after the draw period?
- For a refinance, what are the points, lender credits, total closing costs and new payoff date?
- What balance and total dollars paid should I expect after the dates I am considering?
- Can the line be frozen, reduced or accelerated, and what happens if I sell the home?
- Which disclosures, cancellation rights and prepayment terms apply to this contract?
Keep the offers, disclosures and payment scenarios together. Compare like with like, verify every assumption and have an attorney or qualified financial professional review unusual provisions before you commit.
