Short answer: Apollo Hospitals is the strongest choice for investors prioritising scale, diversification and a disclosed balance-sheet reference point. Rainbow Children’s Medicare suits investors who specifically want paediatric, maternity and obstetrics/gynecology exposure. Shalby offers a smaller, orthopaedic-led growth story but carries greater concentration risk. The final choice depends on the valuation you pay: an excellent hospital operator can still be an unattractive stock when its multiple assumes years of exceptional growth.
What each company actually represents
These are not interchangeable hospital businesses. Apollo Hospitals Enterprise is a broad healthcare platform with a large hospital network and multiple service lines. Rainbow Children’s Medicare is a focused paediatric and women’s-care operator. Shalby is a smaller, more concentrated network with an orthopaedic-led identity. Comparing only reported revenue or the latest share-price move can therefore produce a misleading conclusion.

The useful comparison has five parts: operating scale, specialty mix, growth from new capacity, financial quality and the valuation embedded in the share price. A diversified operator may deserve a premium for resilience, while a specialist may justify a premium only if its niche produces superior occupancy, pricing and returns on new beds.

A publication-day valuation check is essential because prices and multiples move continuously. The following table is deliberately limited to figures that can be dated and verified at publication; do not treat an undated multiple as current.

Apollo Hospitals: the scale and diversification leader
What the FY2025 numbers show
Apollo’s FY2024-25 annual report records 8,025 operating beds and 10,187 total beds, with 68% occupancy. Consolidated revenue was ₹217,940 million and consolidated EBITDA was ₹30,219 million. The network reported 604 thousand discharges and 7.3 million outpatients. Those figures establish a materially larger operating base than the other two companies’ disclosed revenue figures.
Scale can improve purchasing power, clinician recruitment, referral flows, digital reach and the ability to spread central costs. Diversification across locations and clinical departments can also reduce dependence on one city, one surgeon group or one diagnosis. It does not eliminate execution risk: a large network can contain mature hospitals, ramping facilities and lower-return assets at the same time.

1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesBalance-sheet reference point
Apollo reported a consolidated debt-equity ratio of 0.26 for FY2025. That is a useful reference point when assessing future expansion, but it is not a complete solvency analysis. Investors should still inspect lease liabilities, debt maturities, interest coverage, capital expenditure commitments and cash generated after maintenance spending.
What to monitor
- Whether occupancy can rise without excessive discounting or a deterioration in case mix.
- Revenue per occupied bed and EBITDA margin by mature versus newly opened facilities.
- Capital expenditure per added bed and the time required for new capacity to reach mature utilisation.
- Cash conversion after working-capital movements and expansion spending.
- How much earnings comes from hospitals versus other healthcare activities in the consolidated group.
Rainbow Children’s Medicare: a focused paediatric and women’s-care thesis
Specialisation is the investment case
NSE describes Rainbow’s business segment as “the business of running multi-specialty pediatric and obstetrics and gynecology hospital.” That wording makes Rainbow the most focused specialty exposure among the three. The model can benefit from referral relationships, specialised clinical teams, brand recognition among families and repeat demand in maternity and paediatric care.

Specialisation also narrows the addressable case mix. A paediatric and maternity operator is more exposed to local birth rates, insurance coverage, referral patterns, clinician availability and competition from general hospitals that offer similar services. Investors should therefore judge the company on the quality and durability of its niche rather than assume that a smaller network will automatically grow faster.
FY2025 growth context
A FY2025 summary reported revenue of ₹489.80 crore, up 14.2% year over year. Revenue growth alone does not show whether the increase came from mature-hospital pricing, higher occupancy, new beds, acquisitions or a one-off mix change. The next checks are occupancy, average revenue per occupied bed, contribution from recently opened facilities and operating cash flow.
Historical capacity and ARPOB context
Axis Capital’s FY2021 peer table recorded Rainbow at 14 hospitals and 1,475 beds, with FY2021 average revenue per occupied bed (ARPOB) of ₹40.9 thousand. These are historical figures, not current capacity. They are useful only for understanding that Rainbow’s specialised case mix produced higher ARPOB than Shalby in that older comparison; they should not be used as a present-day operating estimate.
Shalby: smaller scale, orthopaedic concentration and a higher-execution thesis
What supports the case
Shalby has an orthopaedic-led identity and a smaller operating base. A FY2025 industry summary reported consolidated revenue of ₹1,086.96 crore, up 16.4% from ₹934 crore in FY2024. Growth from a smaller base can be meaningful if new hospitals fill efficiently, established facilities gain share and specialised procedures support pricing.
Orthopaedics can provide a recognisable clinical franchise and elective procedures may offer planning visibility. However, concentration increases sensitivity to surgeon retention, implant costs, payer negotiations, local competition and changes in procedure volumes. A single weak facility or a disruption in a key clinical team can have a larger effect on group results than it would at a diversified operator.

Historical peer context
In Axis Capital’s FY2021 table, Shalby had 11 hospitals and 2,012 beds, compared with Rainbow’s 14 hospitals and 1,475 beds. Shalby’s FY2021 ARPOB was ₹27.4 thousand versus Rainbow’s ₹40.9 thousand. The comparison is dated and must not be presented as current capacity or current pricing, but it illustrates how specialty mix and patient complexity can produce different economics even when hospital counts are similar.

What to monitor
- Occupancy and ARPOB in orthopaedic versus non-orthopaedic departments.
- Implant and consumable costs as a percentage of procedure revenue.
- Dependence on a small number of surgeons, locations or institutional customers.
- Bed additions, ramp-up time and return on capital at newer facilities.
- Operating cash flow relative to reported EBITDA and reported net profit.
How to compare hospital operators properly
Occupancy is necessary but not sufficient
Occupancy measures how much available bed capacity is used. Higher occupancy generally improves fixed-cost absorption, but an apparently strong percentage can hide low pricing or a less profitable case mix. Compare occupancy with ARPOB, length of stay, procedure mix and EBITDA margin. Apollo’s disclosed FY2025 occupancy is 68%; equivalent current figures for Rainbow and Shalby are not stated in the available figures, so no reliable ranking can be made on occupancy alone.
ARPOB reveals pricing and case mix
ARPOB, or average revenue per occupied bed, helps distinguish volume growth from higher-value care. It is affected by specialty, acuity, payer mix, length of stay and non-bed revenue. A rising ARPOB can be positive when driven by complexity and pricing, but less reassuring when it reflects unusually long stays or a temporary mix shift.
Bed additions create a timing problem
New beds usually incur staff, marketing and infrastructure costs before reaching mature utilisation. Ask whether management is adding capacity ahead of demand, converting existing space or acquiring an operating hospital. The relevant return is not the number of beds announced; it is the cash return after ramp-up, maintenance capital expenditure and financing costs.
Margins need a like-for-like basis
Compare EBITDA margin using the same period and accounting perimeter. A group with pharmacies, diagnostics or other services may report a different consolidated margin from a pure hospital operator. Separate mature hospitals from start-up facilities where disclosures permit, because early losses can depress group margins while creating future capacity.

Cash conversion tests accounting quality
Hospital groups can report profit while cash is absorbed by receivables, inventory, deposits, expansion or acquisitions. Review operating cash flow, free cash flow after maintenance capital expenditure, working-capital days and the relationship between EBITDA and cash from operations. Cash conversion is especially important when a company is funding rapid bed additions.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Valuation: the factor that decides the stock, not the hospital story
Apollo’s scale, Rainbow’s specialty focus and Shalby’s smaller-base growth can all be attractive. None is automatically the better investment at every price. The correct question is whether the current enterprise value or market capitalisation is reasonable relative to sustainable earnings and the capital required to produce them.
Use consistent multiples
- Record each company’s share price and date from the same exchange session.
- Calculate market capitalisation using the current diluted share count.
- Add net debt and other debt-like obligations to derive enterprise value.
- Use the same trailing or forward EBITDA, or the same trailing or forward earnings, for all three stocks.
- Check whether exceptional items, acquisitions or start-up losses distort the denominator.
EV/EBITDA is useful for comparing different leverage levels, while P/E is intuitive when depreciation and interest burdens are stable. For hospital companies, neither multiple should be read without examining maintenance capital expenditure and returns on incremental beds.
How to interpret a premium
- Apollo premium: may be defensible if diversification, network effects and cash generation support durable growth.
- Rainbow premium: may be defensible if paediatric and maternity demand, clinician depth and new-hospital returns remain strong.
- Shalby premium: requires confidence that orthopaedic growth and expansion can overcome concentration risk.
If the market price already discounts years of high occupancy, strong pricing and successful expansion, even a small execution miss can compress the multiple. Conversely, a lower multiple is not automatically cheap if hospitals are underutilised, leverage is rising or cash conversion is weak.
Decision matrix for different investor priorities
| Investor priority | Best fit to investigate first | Why | Main qualification |
|---|---|---|---|
| Maximum scale and diversification | Apollo Hospitals | Largest verified FY2025 revenue, bed base, discharges and outpatient volume | Large networks require disciplined capital allocation and can contain lower-return assets |
| Paediatric and maternity specialisation | Rainbow Children’s Medicare | Focused business segment covering paediatric and obstetrics/gynecology hospitals | Concentration makes demand, referrals and clinician availability more important |
| Smaller-base growth and orthopaedics | Shalby | Orthopaedic-led identity and reported FY2025 revenue growth of 16.4% | Smaller scale creates greater concentration and execution risk |
| Most visible disclosed leverage reference | Apollo Hospitals | FY2025 debt-equity ratio of 0.26 is reported | All balance-sheet analysis still requires current debt, leases and cash-flow review |
| Lowest valuation risk | Cannot be determined without dated prices and multiples | Valuation changes daily and is decisive | Refresh the valuation table before acting |
Risks that can change the ranking
Regulation and payer pressure
Price controls, insurance reimbursement changes, government-scheme rates and compliance requirements can affect revenue per case and margins. A specialist with high exposure to one payer or procedure category may feel a change faster than a diversified group.

Clinician concentration
Hospital economics depend on doctors who attract patients and perform procedures. Recruitment, retention and incentive costs can alter both growth and profitability. Check whether disclosures indicate dependence on a small number of physicians or whether the network has depth across departments.
Expansion and acquisition risk
Buying or building capacity can increase revenue while reducing near-term returns. Examine purchase consideration, integration costs, lease commitments, renovation spending and the time needed for acquired hospitals to reach target occupancy.
Reputation and clinical quality
Patient outcomes, infection control, accreditation, adverse events and data privacy can affect referrals and utilisation. A single incident can have consequences beyond the immediate financial charge, particularly for a focused regional brand.

Interest rates and leverage
Debt-funded expansion becomes less attractive when borrowing costs rise or occupancy ramps slowly. Compare net debt with EBITDA, maturity schedules and interest coverage rather than relying on a single debt-equity ratio.
A practical checklist before buying any of the three
- Download the latest annual report and most recent results presentation from the company’s investor-relations or exchange filing page.
- Write down current beds, occupancy, ARPOB, EBITDA margin, operating cash flow and capital expenditure for the same period.
- Separate mature hospitals from facilities opened or acquired recently.
- Identify the dominant specialties, payer mix and geographic concentration.
- Calculate market capitalisation, enterprise value, EV/EBITDA and P/E using a dated quote.
- Stress-test the thesis with lower occupancy, slower ARPOB growth, higher staff costs and a delayed ramp-up.
- Compare free cash flow after maintenance capital expenditure with the dividend or expansion funding requirement.
- Set a purchase price that leaves room for execution mistakes instead of assuming the base case will occur.
Bottom line
Apollo is the strongest all-round candidate when the priority is scale, diversification and a comparatively clear FY2025 operating and leverage record. Rainbow is the differentiated choice for an investor who deliberately wants paediatric, maternity and obstetrics/gynecology exposure and accepts specialty concentration. Shalby is the higher-risk, smaller-base option for an investor who believes its orthopaedic franchise and expansion can compound faster than the larger platforms.
There is no defensible universal winner without the publication-date valuation table. Compare each stock’s price with sustainable EBITDA, cash generation, bed-level returns and balance-sheet commitments on the same basis. If one company’s multiple already assumes flawless expansion, choose the stock whose expected return still works after applying conservative occupancy, margin and ramp-up assumptions.
