Neither option is automatically better. A HELOC can be less expensive and keeps the borrower in control of how much is drawn, but it normally requires monthly payments and an income-and-credit approval. An FHA-insured Home Equity Conversion Mortgage (HECM) reverse mortgage can defer required monthly repayment and offer a line of credit, monthly proceeds or a lump sum, yet it has substantial fees, causes the balance to grow and can become due when the homeowner moves to long-term care.

Start by identifying the care setting, expected funding gap and whether the homeowner will continue living in the property. Check Medicare, Medicaid, long-term-care insurance and other assistance before borrowing. Then compare written, personalized HELOC and HECM terms with a HUD-approved counselor and the state Medicaid agency.

HELOC and HECM reverse mortgage: the essential differences

A HELOC is revolving credit secured by home equity. You can draw repeatedly up to a credit limit; repaying principal restores available credit. It is usually a variable-rate debt, so the payment can change as the balance or interest rate changes. A HELOC is generally a second lien if a first mortgage remains, meaning the borrower has two housing debts.

The most common reverse mortgage is the FHA-insured HECM. Instead of making scheduled principal-and-interest payments while living in the home, an eligible borrower can receive proceeds through a line of credit, monthly payouts or a single disbursement. Interest and fees are added to the loan balance. The balance therefore grows as money is borrowed and charges accumulate.

Issue HELOC HECM reverse mortgage
Access to funds Repeated draws up to a credit limit; paying down the balance restores availability. Line of credit, monthly payout or single disbursement, subject to the loan’s terms.
Interest Usually adjustable; charged on the outstanding balance. Accrues on amounts advanced. For a HECM line of credit, CFPB guidance says interest is charged on amounts used; a lump-sum disbursement can accrue interest before all proceeds are spent.
Required monthly repayment Normally yes, under the lender’s draw-period and repayment-period rules. Required monthly principal-and-interest payments can generally be deferred while occupancy and other obligations are met, but interest and fees increase the balance.
Typical qualification focus Income, credit, debt obligations, equity and lender underwriting. Borrower age, principal-residence status, property eligibility, federal-debt and financial assessment, counseling and the ability to meet property charges.
Effect of leaving for care Leaving the home does not by itself create a special HELOC payoff trigger, but payments remain due under the contract. Moving to a nursing home, assisted-living facility or another residence for most of the year typically makes the HECM due, subject to co-borrower and eligible non-borrowing-spouse protections.
Upfront and ongoing cost Fees and interest vary by lender and market; current offers must be compared. Origination, closing costs, FHA mortgage insurance, interest and possible servicing charges. CFPB lists origination fees of $6,000 or less and an annual mortgage-insurance premium of 0.5% of the outstanding balance on its page last reviewed May 28, 2024; these are not a current quote.

A home-equity loan is a different product: it normally provides one lump sum with scheduled repayment. Do not treat its terms as interchangeable with a HELOC.

How the choice affects senior-care cash flow

HELOC: lower initial borrowing, but a payment obligation

A HELOC can fit a temporary or uncertain expense because the borrower draws only what is needed. Unused capacity remains available, and interest generally applies to the amount outstanding rather than the entire limit. The trade-off is a continuing payment obligation. Payments can rise when the index or lender margin produces a higher rate, and the transition from a draw period to a repayment period can increase the required amount even if the rate does not change.

Retirement income may make approval difficult. Lenders commonly assess documented income, credit history, existing debts and the property’s available equity. A family member who plans to help pay care bills cannot assume the lender will count that support unless it meets the lender’s underwriting rules. Ask for the minimum payment, the payment at several higher-rate assumptions and the date on which repayment terms change.

HECM: less required monthly repayment, growing balance

A HECM is designed for an older homeowner who wants to convert equity without selling immediately. The borrower may select a line of credit, regular monthly proceeds, a single disbursement or a combination permitted by the loan. Deferring monthly principal-and-interest payments can protect a tight retirement budget, but it is not free financing: interest, mortgage-insurance premiums and other charges are added to the balance.

Using a lump sum can create a larger balance than a line of credit because interest may accrue on money before it is spent. Borrowing only the amount needed, when it is needed, can limit that carrying cost, although the available payout and draw rules must be confirmed in the loan disclosures.

Who can qualify?

HECM eligibility and conditions

General HECM requirements include:

  • At least one borrower is 62 or older.
  • The home is the borrower’s principal residence and meets the program’s property standards.
  • Any existing mortgage balance is paid off at closing, using HECM proceeds or other funds.
  • The borrower satisfies federal-debt and financial-assessment requirements.
  • The borrower has funds available, or a set-aside is established, for property taxes, homeowners insurance, maintenance and required repairs.
  • The borrower completes counseling with a HUD-approved reverse-mortgage counselor before closing.

Age alone does not guarantee approval or a particular payout. The home’s value, the youngest borrower’s age, interest-rate assumptions, existing liens and the lender’s assessment affect proceeds. Request written disclosures that show the available amount, fees, set-asides and conditions rather than relying on an advertisement or an informal estimate.

HELOC underwriting

A HELOC lender normally evaluates income, credit score and history, debt-to-income measures, property value, existing liens and repayment capacity. A retiree may qualify using pension, Social Security, investment or other documented income, but the lender’s calculation controls. A low rate or large line advertised to a working borrower may not be available to someone living on retirement income.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ask whether the line can be frozen or reduced, what happens if the home’s value falls, whether an annual fee applies and whether the lender requires a minimum initial draw. Read the agreement for the index, margin, rate-change limits, draw period, repayment period, late charges and any balloon or final-payment provision.

Costs, rates and the balance you may leave behind

HECM costs

The Consumer Financial Protection Bureau says reverse mortgages are typically more expensive than other home loans. Potential upfront charges include an origination fee, closing costs and FHA mortgage insurance. Ongoing charges can include interest, mortgage insurance and servicing fees. The CFPB cost page, last reviewed May 28, 2024, lists an origination fee of $6,000 or less and an annual mortgage-insurance premium of 0.5% of the outstanding balance. Those figures are product-cost details from that page, not a promise about a current loan or a particular borrower’s total cost.

Because charges are added to the balance, the debt can rise even when the borrower takes no additional cash. The amount owed is generally limited by the home’s value under the HECM insurance structure, but the home and estate still face the repayment process when the loan becomes due. A loan officer should provide an amortization illustration for the expected period of care and explain which charges are paid in cash, financed or placed in a set-aside.

HELOC costs and rate risk

A HELOC may have lower upfront costs than a reverse mortgage for some borrowers. The CFPB states that “A home equity loan or a home equity line of credit might be a cheaper way to borrow cash against your equity.” The word “might” matters: the agency does not say a HELOC is always cheaper. A HELOC can have an application or appraisal fee, annual fee, closing cost, variable interest and a payment that changes with the balance and rate.

Compare offers using the same assumptions. Request the current index and margin, the maximum and minimum rate, how often the rate can reset, the minimum payment at today’s rate and at higher rates, and the total interest if the line is used for the expected care period. No universal HELOC rate or cost is established here; lender quotes change over time and by borrower.

What happens if the homeowner moves into care?

HECM occupancy rules

A HECM generally becomes due when the borrower dies or moves out. Moving to a nursing home, assisted-living facility or another residence for most of the year can therefore trigger repayment. The question is not only whether care is needed, but whether the homeowner will continue to occupy the mortgaged property as the principal residence under the loan’s rules.

A co-borrower who remains in the home can generally continue living there if the loan’s obligations are met. A non-borrowing spouse may qualify for protections under HUD rules, including in some extended healthcare-facility absences, but eligibility depends on specific facts and documentation. Before relying on a spouse protection, verify the current loan terms with the servicer and speak with a HUD-approved counselor.

HELOC occupancy and repayment

A HELOC contract usually does not impose the same reverse-mortgage occupancy trigger. However, moving does not cancel the debt: scheduled payments, variable-rate exposure and the lender’s lien continue. If the borrower can no longer make payments, the home can still be at risk of foreclosure, whether the property is occupied by the borrower, a spouse or another family member.

This distinction is central when a care plan expects a permanent move. A HECM may be poorly matched to that plan if repayment would be required soon after funds are borrowed and no qualifying co-borrower or protected spouse will remain. A HELOC may avoid that specific trigger but can impose unaffordable payments. Review the expected move timeline before selecting either product.

Check public benefits and other funding first

Medicare and nursing-home care

Medicare generally does not pay for long-term nursing-home care. It continues to cover certain hospital, physician, prescription-drug and medical-supply services, but most people entering a nursing home initially pay out of pocket for long-term custodial care. Medicare.gov’s nursing-home guidance was accessed October 13, 2026; coverage rules and individual circumstances should be confirmed directly.

Medicaid and facility participation

Medicaid can help eligible people, but income, asset and other rules vary by state. Not every nursing home accepts Medicaid, and a facility’s private-pay rate may differ from its Medicaid arrangement. Contact the state Medicaid office and ask the specific facility whether it accepts Medicaid and what happens if a resident’s private funds run out.

The treatment of reverse-mortgage proceeds under Medicaid rules is not uniform enough to state a national rule here. Ask the state Medicaid agency or a qualified benefits counselor how an advance, undrawn line of credit and loan balance would be treated in the relevant state and month.

Other resources to evaluate

  • Long-term-care insurance benefits and elimination periods.
  • Employer, veterans’ or local aging-program assistance.
  • Home-care services that cost less than facility care for the needed level of support.
  • Downsizing or selling the home, if a move is likely and the family can plan safely.
  • Cash savings or investments, while preserving an emergency reserve and funds for future care.

Identify the type of care—skilled nursing, assisted living, home care or another service—the monthly shortfall and the likely duration before deciding how much equity to use.

A step-by-step decision process

  1. Describe the care plan. Write down the setting, expected start date, monthly cost, amount covered by insurance or benefits and a realistic range for duration. Record whether the homeowner expects to remain in the property.
  2. Confirm benefit sources. Check Medicare coverage, state Medicaid eligibility, facility Medicaid participation, long-term-care insurance and local programs. Obtain answers in writing when possible.
  3. Test the household budget. Include ordinary housing costs, taxes, insurance, maintenance, utilities, existing mortgage payments, care payments and a reserve for rate increases. Decide what monthly payment the household could sustain if care lasts longer than expected.
  4. Request a complete HELOC proposal. Ask for the index, margin, rate caps, draw and repayment periods, minimum payment, higher-rate payment examples, fees, lien position and conditions that could freeze or reduce the line.
  5. Obtain HECM counseling and disclosures. Use a HUD-approved counselor. Request the payout options, initial and ongoing fees, mortgage-insurance charges, property-charge set-asides, occupancy requirements, spouse treatment and an illustration of the projected balance.
  6. Model the care-period outcome. Compare monthly cash flow, total debt, remaining equity and the amount heirs or a surviving spouse might need to repay or preserve. Use the same care duration and draw schedule for each option.
  7. Plan for a move or death. Ask the servicer what notice is required, how repayment is calculated, who may remain in the home and how the family would sell or refinance if the loan becomes due.
  8. Choose the smallest workable borrowing amount. Borrowing less, delaying draws and preserving other resources can reduce interest and protect options for later care, but do not leave essential care unpaid.

How the decision changes in common situations

Short, uncertain period of home care

If the homeowner expects to remain at home and needs funds in stages, a HELOC’s revolving structure may avoid paying interest on unused proceeds. It is suitable only if the household can make the required payments under higher-rate scenarios and still meet taxes, insurance and care bills. A HECM line of credit may reduce monthly repayment pressure for an eligible borrower, but its upfront costs and balance growth may outweigh that advantage for a brief borrowing period.

Permanent move to a nursing facility

If the homeowner is likely to leave the property for most of the year, examine the HECM occupancy trigger before borrowing. A required payoff can arrive when the home’s sale or refinance is difficult. A HELOC does not remove the payment burden, and a sale or other plan may be more practical if keeping the home no longer serves the household. The right choice depends on the move timeline, co-borrower or spouse status and ability to carry the property.

One spouse needs care while the other remains home

Document who is a borrower, who is a non-borrowing spouse and who will occupy the home. A co-borrower who remains can generally stay if obligations are met; a non-borrowing spouse’s protection is fact-specific. Obtain counselor and servicer confirmation before signing, because an assumption about marital or occupancy status can expose the home to an unexpected repayment demand.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Retired borrower with limited monthly income

A HELOC payment may be the decisive constraint even when the interest rate looks attractive. A HECM can defer required monthly principal-and-interest payments, but property taxes, insurance, maintenance and repairs still must be paid, and the balance grows. Set aside funds for those obligations and test whether the household can sustain them for the full care period.

Risks that apply to both choices

  • Home at risk: Failure to meet a HELOC’s payments or a HECM’s taxes, insurance, maintenance and other obligations can lead to foreclosure.
  • Less equity later: Money used for care is equity that cannot be used for a later emergency, a future move or an inheritance.
  • Longer care than expected: A shortfall that lasts years can exhaust a credit line, increase a reverse-mortgage balance or force a sale.
  • Changing terms: HELOC rates and payments can change; lender policies, program charges and public-benefit rules can change as well.
  • Family misunderstandings: Relatives who expect to inherit the home or make payments should understand the lien, repayment trigger and documentation requirements.

Questions to ask before signing

Questions for a HELOC lender

  • What are the index, margin, current rate, maximum rate and rate-reset frequency?
  • How long is the draw period, when does repayment begin and what is the payment at several higher rates?
  • Can the line be frozen or reduced, and under what conditions?
  • What are all application, appraisal, closing, annual, inactivity and early-termination fees?
  • What happens if the homeowner moves to a care facility or dies?

Questions for a HECM lender or counselor

  • How much can be received under each payout option, and when can draws be made?
  • Which origination, closing, mortgage-insurance, servicing and interest charges apply?
  • What property-tax, insurance, repair or maintenance set-aside is required?
  • Exactly what occupancy event makes the loan due?
  • Does a co-borrower or non-borrowing spouse qualify for continued occupancy under the current documents?
  • How would the estate, spouse or heirs repay, sell or refinance when the loan becomes due?

Bottom line for a senior-care funding decision

Choose based on the household’s care timeline and cash flow, not on the label “reverse” or “home-equity.” A HELOC can be a lower-cost, flexible source for a borrower who qualifies and can reliably make variable payments. A HECM can conserve monthly cash flow for an eligible homeowner, but fees, balance growth and a possible payoff after a move make occupancy and long-term plans critical. Before using home equity, verify public benefits, obtain written offers, complete required counseling and model what remains for the homeowner, spouse and heirs.

The Bottom Line

There is no universal winner. Compare personalized HELOC and HECM terms against the expected care duration, monthly budget, occupancy plan, benefit eligibility and equity needed for later life.
