Original Medicare vs. Medicare Advantage: Coverage, Costs, and Flexibility (2026 Guide)

Which is better for you—Original Medicare or Medicare Advantage? There is no universal winner. Original Medicare generally provides the widest choice of doctors and hospitals, while Medicare Advantage can offer a yearly out-of-pocket ceiling and bundled extras in exchange for plan networks, service-area rules, and potentially prior authorization.

Your best choice depends on the clinicians you use, prescriptions you take, expected medical care, travel habits, and tolerance for plan rules. Compare the specific plans available in your ZIP code, not an abstract “average” price.

Original Medicare

Original Medicare consists of Part A hospital insurance and Part B medical insurance. You can add a separate Part D prescription-drug plan and may buy a Medigap policy to help pay certain Part A and Part B cost sharing. Medicare generally covers medically necessary Part A and Part B services, but it does not cover every service, including most routine dental care and routine eye examinations.

Medicare Advantage

Medicare Advantage, also called Part C, is a Medicare-approved private plan that provides your Part A and Part B benefits. Most plans include Part D drug coverage, and many offer additional dental, vision, hearing, fitness, or other benefits. The plan sets its own copayments, coinsurance, deductibles, network rules, and service area within Medicare requirements.

Medicare’s official side-by-side comparison is available at Medicare.gov’s comparison page. Plan details change by location and calendar year, so read the current Evidence of Coverage, provider directory, and drug formulary for the plan you are considering.

Quick comparison

Decision point Original Medicare Medicare Advantage
Basic structure Part A plus Part B; Part D is separate if you choose it. Private Medicare-approved alternative that includes Part A and Part B; most plans include Part D.
Doctors and hospitals Any doctor or hospital that takes Medicare, anywhere in the United States. A provider who does not accept assignment may charge more. Usually a network and service area for non-emergency care. Out-of-network rules depend on the plan type and may cost more.
Specialist referrals Usually not required. May be required, depending on the plan and its rules.
Part B cost sharing For many covered services, usually 20% of the Medicare-approved amount after the Part B deductible. Copayments, coinsurance, and deductibles vary by plan and service.
Annual medical out-of-pocket limit No yearly limit unless supplemental or other coverage applies. A plan-specific yearly limit applies to covered Medicare Part A and Part B services. Separate in-network and out-of-network limits may apply.
Routine extras Generally does not cover routine dental care, routine eye exams, or routine physical exams. Medigap generally does not add routine dental, vision, or hearing benefits. Some plans offer dental, vision, and hearing benefits, with plan-specific limits and restrictions.
Prior authorization Usually not needed for covered services or supplies. May be required for certain services or supplies.
Prescription drugs Enroll in a separate Part D plan. Most plans include Part D; in most plan types, you cannot also join a separate Part D plan.
Medigap May be available to help pay eligible Part A and Part B cost sharing. Cannot pay Medicare Advantage cost sharing.
Care outside the United States Generally not covered; some Medigap policies may cover emergency care abroad. Generally not covered, although some plans provide emergency or urgently needed travel benefits.

Provider choice and flexibility

Original Medicare’s nationwide access

Medicare states: “You can use any doctor or hospital that takes Medicare, anywhere in the U.S.” That broad access is useful if you live in more than one state, travel frequently, see specialists in different systems, or want to change clinicians without checking a plan directory first. The provider must participate in Medicare. A clinician who does not accept Medicare assignment can charge more than the Medicare-approved amount, subject to Medicare’s rules.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

In most cases, Original Medicare does not require a referral to see a specialist, and prior authorization is usually not required for covered services or supplies. You still need to confirm that a service is covered and medically necessary.

Advantage networks and plan types

Medicare Advantage plans generally use a defined network and service area for non-emergency care. Health Maintenance Organization plans are typically more restrictive. Preferred Provider Organization plans may allow out-of-network care, but usually at a higher cost. Rules differ even among plans with the same label.

Before enrolling, search the plan’s current provider directory for your primary-care clinician, specialists, preferred hospitals, laboratories, imaging centers, rehabilitation facilities, and likely future providers. Ask each office whether it accepts the specific plan—not merely whether it accepts Medicare Advantage generally. Directories can change, so confirm important appointments and review the plan’s continuity-of-care provisions.

Check referral requirements, prior-authorization rules, referral destinations, and what happens if a service is delivered outside the service area. Emergency care is treated differently from routine out-of-network care, but the plan documents control the cost and process.

How the costs work in 2026

Original Medicare’s standard figures

Medicare lists a standard Part B premium of $202.90 per month in 2026. Higher-income beneficiaries may pay an income-related adjustment, so this is not everyone’s premium. The 2026 Part B deductible is $283 for the year. After the deductible, many Part B-covered services involve 20% of the Medicare-approved amount, although the exact amount depends on the service, provider, and coverage circumstances. Medicare’s current cost guidance is at Medicare.gov’s costs page.

Original Medicare itself has no annual limit on your Part A and Part B out-of-pocket spending. A Medigap policy, employer coverage, Medicaid, or another form of supplemental coverage may reduce that exposure, but the terms and premiums are separate from Original Medicare.

Medicare Advantage costs

Each Advantage plan establishes its own premiums, deductibles, copayments, coinsurance, and medical out-of-pocket limit. A plan advertised with a $0 premium can still require the Part B premium and can have substantial cost sharing for hospital stays, specialist visits, imaging, outpatient procedures, or other services. The plan’s annual limit applies to covered Medicare Part A and Part B services; drug costs are handled separately under Part D.

Some plans have distinct in-network and out-of-network limits. Verify the current-year limit, which expenses count toward it, and whether the services you expect are covered at the network level you intend to use. A low premium or extra benefit does not prove that the plan will cost less over a year with frequent care.

Build an annual estimate

Compare total expected spending rather than premiums alone. For each option, list:

  • Part B premiums, including any income-related adjustment.
  • Plan premiums for Advantage, Part D, or Medigap.
  • Deductibles and likely copayments or coinsurance for primary care, specialists, therapy, imaging, outpatient procedures, and hospital care.
  • Prescription costs at the pharmacies you actually use.
  • Whether expected care is in network and whether prior authorization could delay it.
  • The applicable medical out-of-pocket limit and which charges count toward it.

Use the plan’s Summary of Benefits and Evidence of Coverage for estimates. The official coverage-options guidance explains the two pathways and how drug and supplemental coverage fit with them.

Prescription-drug coverage

Separate Part D with Original Medicare

Original Medicare does not include outpatient prescription coverage. You choose a standalone Part D plan. Compare your exact medications, dosages, quantities, preferred pharmacies, mail-order options, formulary tiers, prior authorization, quantity limits, and step-therapy rules. A plan that covers one drug may place it on a tier that costs more than another plan’s tier.

Part D inside most Advantage plans

Most Medicare Advantage plans bundle Part D. In most plan types, joining a separate Part D plan is not allowed and can affect your Advantage enrollment. Review the Advantage plan’s formulary and pharmacy network in the same way you would review a standalone Part D plan.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The 2026 Part D threshold

For 2026, Medicare lists a $2,100 covered Part D out-of-pocket threshold. After you reach that amount for covered Part D drugs, catastrophic coverage begins and you pay no out-of-pocket cost for covered Part D drugs for the rest of that calendar year. The threshold applies to covered drug spending under the applicable Part D rules; it is not a cap on medical services. Details are available at Medicare’s Part D cost page.

Extra benefits: useful, but not automatically valuable

Original Medicare generally does not pay for routine dental care, routine eye exams, or routine hearing services. Medigap generally does not add those routine benefits. Medicare Advantage plans may offer some dental, vision, hearing, fitness, transportation, or over-the-counter benefits, but the design varies considerably.

Read the fine print for annual maximums, covered procedures, frequency limits, provider restrictions, allowances, prior authorization, and your share of the bill. A dental benefit with a narrow network and a small annual maximum may not cover the treatment you expect. Treat extras as one factor in the comparison, not as a substitute for checking medical and drug costs.

Travel and living in more than one place

Travel within the United States

Original Medicare’s nationwide provider rule can be simpler for people who spend extended periods outside their home state. You still need a provider who takes Medicare, and you remain responsible for applicable cost sharing.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Advantage plans usually define a service area. Routine care while traveling may be limited to network providers or cost more out of network, depending on the plan. Some plans offer temporary visitor or travel arrangements, but the plan’s Evidence of Coverage determines whether they apply.

Care abroad

Medicare generally does not cover care outside the United States under either route. Some Medigap policies cover certain emergency services abroad, and some Advantage plans provide emergency or urgently needed travel coverage. Check the exact policy or plan terms before relying on them; do not assume routine international care is covered.

Medigap and the consequences of switching

Medigap can help pay certain Original Medicare deductibles, coinsurance, or copayments. It cannot pay Medicare Advantage cost sharing, and you cannot use a Medigap policy to make an Advantage plan function like Original Medicare.

Timing matters. Medicare says that missing the six-month Medigap open enrollment period may mean you cannot buy a policy later or may have to pay more, subject to state rules and special guaranteed-issue rights. Before leaving an Advantage plan for Original Medicare, investigate whether a Medigap insurer will accept you, what underwriting applies, the premium, and whether you have a guaranteed-issue right. Do not cancel an existing plan until the replacement coverage is approved and its effective date is confirmed.

The reverse move also needs planning. If you leave Original Medicare and Medigap for Medicare Advantage, ask whether you can return to the same Medigap policy later, whether a trial right applies, and what your state permits. The official coverage-options page explains the interaction among Original Medicare, Advantage, Part D, and Medigap.

A practical decision process

  1. Map your care. Write down every clinician, hospital, facility, expected procedure, and location where you regularly receive care. For Original Medicare, confirm Medicare participation. For Advantage, verify network status for the specific plan and year.
  2. Inventory prescriptions. Record each medication, strength, quantity, and preferred pharmacy. Compare the formulary tier, restrictions, pharmacy network, mail-order terms, and projected annual cost for each Part D option.
  3. Model a realistic year. Add premiums, deductibles, copayments, coinsurance, likely hospital or outpatient costs, drug spending, and the relevant medical out-of-pocket limit. Model both an ordinary year and a high-use year.
  4. Read the rules. Check referrals, prior authorization, step therapy, in-network and out-of-network pricing, emergency coverage, service-area boundaries, and appeal procedures.
  5. Evaluate supplemental coverage. If considering Original Medicare, price Medigap and confirm eligibility, underwriting, enrollment timing, and guaranteed-issue rights before leaving an Advantage plan. If considering Advantage, remember that Medigap will not cover its cost sharing.
  6. Verify travel needs. Identify where you spend time, how you would obtain routine care there, and what emergency or urgently needed care is covered outside the plan’s service area or abroad.
  7. Get independent help. Use Medicare’s plan-comparison tools and contact your State Health Insurance Assistance Program (SHIP). SHIP counseling is free, personalized, and not connected to an insurer or health plan. Medicare’s Medicare & You 2026 handbook provides the official year-specific overview.

Common mistakes to avoid

  • Comparing only premiums. A $0 Advantage premium does not eliminate the Part B premium or service cost sharing.
  • Assuming every Advantage plan works the same way. HMO, PPO, and other plan rules differ, including referrals and out-of-network coverage.
  • Checking only your primary doctor. A hospital, specialist, laboratory, imaging center, or rehabilitation facility may follow a different network contract.
  • Ignoring drug details. A plan can cover your medication but impose a high tier, quantity limit, prior authorization, or nonpreferred-pharmacy charge.
  • Treating the medical out-of-pocket limit as a total cap. It generally applies to covered Part A and Part B services, not every premium, drug expense, or noncovered service.
  • Assuming Original Medicare covers everything. Routine dental and vision services are common exclusions, and there is no built-in annual medical out-of-pocket ceiling.
  • Leaving an Advantage plan before checking Medigap. You may not be able to buy a Medigap policy later on the same terms, depending on timing and state protections.
  • Relying on an old directory or benefit summary. Plans, formularies, networks, premiums, and cost sharing can change each year.

Which option fits which situation?

Original Medicare may fit better when

  • You want the broadest possible choice of Medicare-participating doctors and hospitals nationwide.
  • You regularly travel or divide your time between states.
  • You prefer usually not needing referrals and generally having less plan-level prior authorization.
  • You are willing and able to budget for a Part D plan and, if appropriate, Medigap or other supplemental coverage.

Medicare Advantage may fit better when

  • Your preferred clinicians and hospitals are reliably in a plan’s network.
  • You value a defined annual limit for covered Part A and Part B services.
  • You prefer one plan that usually combines medical and prescription coverage.
  • The plan’s extra benefits are genuinely useful after checking their limits, providers, and cost sharing.

Those are decision patterns, not guarantees. A person with the same diagnosis, prescriptions, and budget can reach a different answer because plans and provider networks vary by county and year.

Bottom line

Choose based on the coverage you will actually use. Original Medicare generally maximizes provider flexibility but leaves you to arrange Part D and supplemental protection and has no built-in annual medical out-of-pocket cap. Medicare Advantage can bundle coverage, offer extra benefits, and cap covered medical spending, but it requires careful review of networks, service areas, referrals, prior authorization, formularies, and plan-specific cost sharing. Compare your doctors, drugs, travel pattern, expected care, and complete annual costs for the exact plans available where you live, then use SHIP or Medicare’s official tools before enrolling or switching.
