Mortgage Payment Relief Options for Families With a Seriously Ill Child

By TheFinanceBase Team 9 min read

If your child’s illness has made the mortgage unaffordable, call your mortgage servicer before you miss a payment and request a formal loss-mitigation review. Explain the medical-related income loss or expenses, ask which options apply to your loan, and get every proposed term in writing. A HUD-approved housing counselor can help you prepare the application and communicate with the servicer at no or little cost.

There is no general federal mortgage benefit that automatically applies because a child is seriously ill. Relief depends on the loan’s owner or insurer, the servicer’s rules, your finances, and sometimes state programs. The diagnosis alone does not establish eligibility, but illness-related medical costs, reduced work hours, or caregiving can be documented as a financial hardship.

What to do first when a payment problem is foreseeable

  1. Call the servicer immediately. Use the phone number on your monthly statement or the servicer’s official website. Tell the representative that your household is experiencing a hardship connected to your child’s serious illness. Describe whether the problem is likely to be temporary or ongoing, how income and expenses have changed, and what assets are available. Ask for the mortgage-assistance or loss-mitigation application and the complete list of options for your loan. The Consumer Financial Protection Bureau (CFPB) advises borrowers who may miss a payment to call right away: “If you can’t pay your mortgage or are worried about missing a mortgage payment, call your mortgage servicer right away.”
  2. Contact a HUD-approved housing counselor. Counselors can explain the servicer’s proposals, help organize documents, and communicate with the mortgage company. CFPB says this help is free or often available at little cost. Use the CFPB housing-counselor locator or call the HOPE Hotline at 888-995-HOPE (4673).
  3. Create a written file. Keep the application, pay stubs, benefit statements, medical-expense records, bank statements, letters, and every message from the servicer. Record the representative’s name, the date, and what was promised after each call. Ask for decisions, deadlines, and repayment terms in writing. Continue checking statements and confirm that escrow is still paying property taxes and homeowners insurance.

CFPB’s Mortgage help resource also directs homeowners to official assistance and warning signs of scams.

Which mortgage-relief options to discuss

The servicer may evaluate refinancing, a loan modification, a repayment plan, forbearance, payment deferral, a short sale, or a deed-in-lieu of foreclosure. No borrower automatically qualifies for all of them. The loan type, investor or insurer, delinquency status, income, expenses, and ability to resume payments determine what can be offered.

Option How it can help What happens to the unpaid amount Main decision issue
Forbearance Temporarily pauses or reduces payments Must be repaid later; interest may continue Choose a workable repayment arrangement before the pause ends
Repayment plan Spreads past-due amounts over a limited period Added in portions to regular payments Can make monthly payments temporarily higher
Payment deferral Allows some borrowers who can resume the normal payment to move missed amounts to the loan’s end Generally due when the home is sold or refinanced, or at maturity, under the written terms Eligibility is loan-specific; available on some Fannie Mae or Freddie Mac loans
Loan modification Changes loan terms to make the payment more manageable Handled under the modified balance, rate, term, or other approved structure Requires a full review and may change the payoff date or total interest
Refinancing Replaces the existing mortgage with a new loan Old loan is paid off by the new loan Requires qualifying credit, income, equity, and closing costs
Short sale Sells the home for less than the mortgage balance with servicer approval Deficiency treatment depends on the approval and applicable law Home is sold; obtain legal and tax advice before signing
Deed-in-lieu Transfers the home to the lender to avoid a foreclosure sale Handled under the agreement and applicable law Home is surrendered; review any remaining liability with a qualified adviser

Forbearance: temporary breathing room, not forgiveness

CFPB describes forbearance as “a process that can help if you’re struggling to pay your mortgage.” It may let the servicer pause payments or accept smaller payments while medical bills, reduced earnings, or caregiving demands disrupt the household budget. The missed or reduced payments do not disappear. Interest can continue to accrue, and the agreement must state how and when the amount will be repaid.

For loans backed by Fannie Mae or Freddie Mac, CFPB notes that a lump-sum payment is not required at the end of forbearance. Other federal, agency-backed, and private loans can have different rules. Ask the servicer to identify the investor or insurer and explain the exact exit option before accepting the pause.

Repayment plan: catch up through higher payments

If the family can afford more than the normal payment for a limited period, the servicer may add part of the past-due balance to each monthly bill until the account is current. This can avoid a single large bill, but the temporary payment may be substantially higher. Compare that amount with the household’s expected income after treatment, leave, or caregiving changes.

Payment deferral: move the arrears to the end

Some Fannie Mae and Freddie Mac borrowers who can resume the regular payment may be able to defer missed amounts to the end of the loan. Depending on the written terms, the deferred balance may become due when the home is sold or refinanced. This is not automatic and does not necessarily apply to other loans. Confirm whether the deferral changes the maturity date, accrues interest, or affects a future refinance.

Loan modification: a longer-term payment change

A modification can alter one or more loan terms so the ongoing payment is more manageable when the family cannot return to the old payment. The servicer may review income, expenses, assets, hardship documentation, and the loan’s program rules. A lower payment can come with a longer term, a different interest rate, a changed principal balance, or more total interest. Do not compare only the first monthly payment; request the projected balance, payoff date, and total payments under the proposed modification.

Refinancing, short sale, and deed-in-lieu

Refinancing may help only when the borrower can qualify for a new loan and the new rate and closing costs improve the situation. A short sale and a deed-in-lieu are alternatives that involve leaving the home. They can be appropriate in some cases but have credit, tax, deficiency, and housing consequences. Seek individualized housing counseling and, where relevant, legal and tax advice before agreeing to either.

Questions to get answered in writing

Before signing an agreement or relying on a phone promise, ask the servicer these questions:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Which options can temporarily reduce or suspend payments, and what application, income records, medical-expense records, or other documents are required?
  • Does interest continue on missed or reduced payments? Are late fees or other fees waived?
  • When do payments resume, and what will the exact monthly payment be then?
  • How will the paused amount be repaid: a repayment plan, deferral, modification, a lump sum, or another method?
  • Will the interest rate, loan term, maturity date, principal balance, or payoff amount change?
  • What happens to the deferred amount if the home is sold or refinanced?
  • Will escrow continue paying property taxes and homeowners insurance? Could an escrow shortage raise later payments?
  • Are homeowners-association charges included, and must the family keep paying them separately?
  • What happens if the child’s treatment lasts longer, caregiving increases, or income falls again?
  • What is the deadline to accept, reject, or appeal the decision, and where should completed documents be sent?

CFPB advises homeowners to plan for the end of forbearance before it ends and to understand the repayment method rather than assuming either that a lump sum will be required or that it will not. Its guidance on working with servicers is available at How to work with your mortgage servicer.

Keep escrow, insurance, and credit from creating a second crisis

An approved arrangement does not automatically resolve every housing bill. Continue monitoring the mortgage statement during any pause. Confirm that the servicer is applying escrow funds to property taxes and homeowners insurance and ask how any shortage will be collected. A later escrow adjustment can raise the payment even after the hardship arrangement ends. Homeowners-association dues are usually separate from the mortgage and may need a separate payment arrangement.

Do not stop paying simply because a company says an application is pending. Unless the servicer has approved a forbearance or another arrangement, missed payments can be reported and can have lasting credit consequences. If the servicer says a payment is not required, request the effective dates and repayment terms in writing and keep the confirmation with your records.

Help tied to the illness, and what it can and cannot do

The general mortgage pathways above can address a hardship caused by unexpected medical costs or lost income, but the reviewed federal guidance does not establish a dedicated mortgage entitlement for families with a seriously ill child. Ask the servicer and a housing counselor whether your state or locality has a homeowner-assistance program and whether your loan is eligible. Rules and funding can change, so verify the program directly.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Hospital social workers, oncology or pediatric-care coordinators, and disease-specific nonprofit organizations may know about help with medical bills, transportation, lodging, meals, utility costs, or caregiver support. Those resources can free cash for the mortgage, but do not describe them as mortgage assistance unless the program expressly allows mortgage payments as an eligible expense. CFPB notes that some relief may remain available through a servicer or state even when a loan lacks federal or government-sponsored-enterprise backing; ask what restrictions and documentation apply.

  • Eligibility and paperwork: What facts about this loan and household support approval, and what must be updated?
  • Failure scenario: What happens if income does not recover on schedule?
  • Housing outcome: Does the option preserve ownership, or does it require selling or transferring the home?

A HUD-approved counselor can help turn these answers into a budget that includes treatment-related expenses, transportation, childcare, food, utilities, insurance, taxes, and other debts. A proposal that looks affordable on the mortgage statement may still fail if it leaves no room for essential care.

Recognize foreclosure-rescue scams

Use the servicer, a HUD-approved counselor, and legitimate legal-aid channels. CFPB warns against companies that demand upfront fees, guarantee a mortgage change or foreclosure prevention, tell you to stop paying the servicer, ask you to sign over title, or direct payments to an account other than the mortgage company. Official housing counseling is available at no or low cost; start with the CFPB locator or the HOPE Hotline rather than an unsolicited caller.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not sign documents you do not understand, especially a deed, power of attorney, or agreement that changes who owns the home. Verify every phone number and payment address against your statement, and independently call the servicer if a third party claims to have negotiated a deal.

If the hardship continues or the application is denied

Tell the servicer before the approved period ends if treatment or caregiving prevents recovery. Ask whether a new review, modification, or another loss-mitigation option is available and submit updated income and expense information promptly. A denial letter should explain the reason and any appeal or reconsideration deadline; give the letter to your housing counselor.

If keeping the home is no longer affordable despite a documented review, discuss an orderly sale, short sale, or deed-in-lieu before foreclosure proceedings advance. Get advice about relocation, remaining debt, taxes, credit effects, and whether state law changes the outcome. Acting early generally gives the family more choices than waiting for a foreclosure notice.
