Medicare for All vs. Medicare: Coverage, Costs, and Eligibility

Medicare is an existing federal health-insurance program, while Medicare for All is a label for proposed systems. The 2023 Senate Medicare for All Act (S.1655) illustrates one design: automatic enrollment of U.S. residents, a much broader benefit package, and little or no cost-sharing for covered care. It has not been enacted. Your answer to questions about eligibility or costs therefore depends on whether you mean Medicare as it operates in 2026 or a specific proposal such as S.1655.

Medicare and Medicare for All are different things

Medicare today is a functioning federal program. Eligibility is centered on age 65, with additional pathways for certain younger people with disabilities, amyotrophic lateral sclerosis (ALS), or end-stage renal disease (ESRD). Benefits are delivered through Original Medicare or private Medicare Advantage plans, and beneficiaries may add prescription-drug or supplemental coverage.

Medicare for All is not one operating program. It is a political term used for multiple universal-coverage proposals. This article uses S.1655, the Medicare for All Act introduced in the Senate in 2023, as a concrete example. The bill proposed to cover all U.S. residents automatically, but it is a proposal rather than current law. Other bills can differ on benefits, private insurance, provider payment, taxes, and transition rules.

How Medicare works now

Medicare.gov summarizes the basic choice this way: “You can choose either Original Medicare or Medicare Advantage (Part C) for your health coverage.” The choice determines which insurers administer services, what networks apply, and how your costs are calculated.

Original Medicare: Parts A and B

Part A generally covers inpatient hospital, skilled-nursing-facility, hospice, and some home-health services. Most beneficiaries owe no monthly Part A premium because of their work record. People who do not have enough qualifying Medicare-tax work history can buy Part A. For 2026, the monthly buy-in premiums listed by Medicare are $311 or $565, depending on work history.

Part B covers medically necessary physician services, outpatient care, durable medical equipment, preventive services, and other outpatient benefits. The standard Part B premium for 2026 is $202.90 per month, and the annual Part B deductible is $283. After the deductible, the general coinsurance for covered services is usually 20% of the Medicare-approved amount. Income-related adjustments can make the Part B premium higher than the standard amount.

Part A also has cost-sharing tied to each benefit period. In 2026, the Part A deductible is $1,736 per benefit period. Hospital coinsurance is $434 per day for days 61 through 90 and $868 per day for lifetime-reserve days 91 through 150. These are 2026 figures and should be checked against the current Medicare cost tables when planning expenses.

Prescription drugs and supplemental coverage

Original Medicare does not include every benefit or every prescription. A beneficiary can enroll in a stand-alone Part D prescription-drug plan and may buy Medigap supplemental insurance to help pay certain Original Medicare deductibles and coinsurance. Premiums and covered services vary by plan and location.

Medicare Advantage (Part C)

Medicare Advantage is an alternative way to receive Part A and Part B coverage through a private plan approved by Medicare. Plans set their own premiums, deductibles, copayments, provider networks, and rules for referrals or prior authorization within federal requirements. They generally include an annual out-of-pocket limit for covered services, unlike Original Medicare. The exact limit and cost structure are plan-specific, so a person cannot determine a Medicare Advantage bill from the national Part B premium alone.

Original Medicare has no general annual out-of-pocket ceiling

Original Medicare generally does not cap your yearly spending on covered Part A and Part B services. A beneficiary with extensive care needs can therefore face repeated deductibles and 20% Part B coinsurance. Medigap, Medicaid eligibility, employer or union coverage, or a switch to Medicare Advantage can change that exposure, but each option has its own eligibility and cost rules.

Who qualifies for Medicare?

People age 65 and older

The main eligibility route is reaching age 65. Eligibility for premium-free Part A commonly depends on a person’s own or a spouse’s Medicare-tax work record. Part B generally requires a monthly premium. Enrollment timing and late-enrollment consequences depend on a person’s circumstances, including whether qualifying employer coverage is in place; check the current official enrollment guidance for a personal deadline.

People under 65 with disabilities

Some people under 65 qualify after receiving Social Security disability benefits for the required period. The exact start date and exceptions depend on the disability program and individual record.

People with ALS

A person with ALS can qualify for Medicare under a special pathway that does not follow the ordinary waiting period applied to many disability beneficiaries.

People with end-stage renal disease

People with ESRD can qualify based on regular dialysis or a kidney transplant, subject to Medicare’s rules for treatment, timing, and coordination with other insurance.

These routes are limited eligibility categories, not universal enrollment. A person who is healthy, under 65, and not in one of the specified pathways generally cannot buy into Medicare simply because they want its coverage.

What S.1655 proposed for eligibility and benefits

Automatic enrollment for residents

The 2023 bill stated that all U.S. residents would be covered and automatically enrolled upon birth or establishment of residency. That is fundamentally different from current Medicare, where age or a qualifying disability, ALS, or ESRD status is generally required.

A broader benefit package

S.1655 described coverage for medically necessary or appropriate care. The listed categories included:

  • Hospital and other institutional services
  • Prescription drugs
  • Mental-health and substance-use-disorder treatment
  • Dental services
  • Vision services
  • Long-term care

The proposal’s language is broader than the current Medicare package, particularly because dental, vision, and long-term care are not generally covered comprehensively by Original Medicare. The bill’s text does not turn that proposal into an available insurance option today.

Limits on patient cost-sharing

For covered benefits, S.1655 proposed no deductibles, coinsurance, copayments, or similar charges. It allowed a narrow prescription-drug cost-sharing schedule set by the Secretary of Health and Human Services. The schedule would have to be evidence-based and patient-centered, exclude preventive drugs, stay below an annual per-person cap adjusted for inflation, and exempt households at or below 250% of the federal poverty line. The bill set the initial cap at $200 per person per year in its 2023 text.

Those rules describe the bill as introduced. They are not a current benefit card, and they do not establish what a future enacted law would charge after legislative changes.

How the costs differ

What a Medicare beneficiary can owe in 2026

A current Medicare budget usually has several layers:

  • Part A: no monthly premium for most people, but a $1,736 deductible per benefit period and daily hospital coinsurance after specified day thresholds in 2026.
  • Part B: a standard $202.90 monthly premium and a $283 annual deductible in 2026, followed generally by 20% coinsurance for covered services.
  • Prescription drugs: a Part D plan or Medicare Advantage drug benefit can add a premium, deductible, and copayments under that plan’s formulary and cost-sharing rules.
  • Supplemental coverage: Medigap, Medicaid, employer coverage, or other assistance can reduce some out-of-pocket costs but may carry its own premiums and eligibility conditions.
  • Medicare Advantage: premiums and copayments vary by plan; the plan has an annual limit for covered services, with the amount set under the plan’s rules.

Your actual annual spending depends on coverage choice, income, work history, services used, providers, and whether a service is covered. The standard Part B premium is therefore not a complete answer to “What does Medicare cost?”

Why a Medicare for All price cannot be stated as one premium

S.1655 did not establish a household premium comparable to the 2026 Part B charge. A universal program could be financed through taxes, employer payments, premiums, or a combination. The household result would also depend on whether workers still paid other health-related charges, how wages and tax rates changed, and which services were newly covered.

A sound comparison must identify the bill, its benefit package, cost-sharing rules, financing mechanism, time period, assumptions about medical use, and provider payment rates. Government spending is not the same measure as a household’s total health costs. Without those inputs and a current, apples-to-apples estimate, it is not responsible to say that Medicare for All would be free, would save every family a particular amount, or would necessarily cost more.

Would Medicare for All cost you less?

It could reduce premiums and point-of-care charges for some households while increasing taxes or other public financing for those same households. A person with high medical needs, expensive prescriptions, or no dental or vision coverage today could value the broader benefits differently from a person who currently has generous employer insurance. The direction and size of the change cannot be calculated from S.1655’s benefit language alone.

Private insurance under each system

Private plans in Medicare today

Private insurers administer Medicare Advantage and stand-alone Part D plans. Medigap insurers sell supplemental policies for people who remain in Original Medicare. These products can fill gaps, but they do not all cover the same services, use the same networks, or charge the same premiums. Choosing a plan means comparing provider access, drug formularies, prior-authorization rules, premiums, deductibles, and maximum out-of-pocket exposure.

Private insurance in S.1655

The bill would restrict private insurance that duplicates benefits provided by the national program. That does not necessarily answer every question about supplemental coverage, employer plans, or private providers. Policy proposals differ on whether private insurers could offer supplemental benefits, how providers would contract, and how existing coverage would transition. Do not treat one proposal’s private-insurance rule as the rule for every Medicare for All plan.

Practical examples

A 67-year-old using Original Medicare

This person may owe the 2026 Part B premium each month, the Part B deductible, and usually 20% coinsurance for covered Part B services. A hospital stay can trigger the Part A deductible and later daily coinsurance. Without Medigap, Medicaid, or another form of supplemental help, there is no general annual ceiling on Original Medicare spending.

A 67-year-old in Medicare Advantage

This person receives Part A and Part B benefits through a private plan. The plan’s premium, network, drug coverage, copayments, prior-authorization rules, and annual out-of-pocket limit determine the practical cost. The national Part B premium may still apply, but it does not reveal the plan’s total cost structure.

A 40-year-old without a qualifying condition

Under current Medicare rules, age alone does not make this person eligible. Under the S.1655 design, U.S. residency would have triggered automatic enrollment regardless of age.

A person needing dental, vision, or long-term care

Original Medicare generally does not provide comprehensive coverage for these categories. S.1655 specifically listed dental, vision, and long-term care among proposed benefits. Whether those services would be fully covered, subject to clinical rules, or delivered under separate administrative arrangements would depend on the final law and regulations.

How to evaluate any future Medicare for All proposal

  1. Identify the exact bill. Record its bill number, introduction date, and current legislative status. Do not use the label alone.
  2. Check eligibility language. Determine whether coverage is automatic, resident-based, age-based, or limited to citizens or other categories.
  3. List covered services. Look specifically for dental, vision, hearing, mental health, prescription drugs, reproductive care, and long-term care rather than assuming they are included.
  4. Read the cost-sharing section. Separate premiums from deductibles, coinsurance, copayments, drug caps, exemptions, and out-of-network charges.
  5. Find the financing provisions. Look for payroll, income, wealth, sales, employer, or other taxes and identify who would pay them.
  6. Compare like with like. Put taxes, premiums, expected out-of-pocket spending, and the value of newly covered services into the same household comparison and the same year.
  7. Check private-insurance rules. Determine whether duplicative coverage is prohibited and whether supplemental plans remain available.

What is known—and not known—about S.1655

Known elements of the 2023 proposal include automatic resident enrollment, a broad medically necessary benefit package, generally no cost-sharing for covered services, a limited prescription-drug cost-sharing exception, and restrictions on duplicative private coverage. The proposal’s initial prescription-drug cap was $200 per person annually, with inflation adjustment and an exemption for households at or below 250% of the poverty line.

That description does not establish current legislative status, a final tax schedule, a typical household’s annual payment, provider reimbursement levels, or implementation dates. Those details would be essential before making a personal affordability estimate.

Bottom line

Medicare is available now mainly to people 65 and older and certain younger people with disabilities, ALS, or ESRD. In 2026, Original Medicare combines a generally premium-free Part A for eligible beneficiaries with a $202.90 standard monthly Part B premium, a $283 Part B deductible, and usually 20% Part B coinsurance; it has no general annual out-of-pocket maximum. Medicare Advantage and supplemental plans change that calculation.

Medicare for All is a family of proposals, not current coverage. S.1655, introduced in 2023, would have automatically enrolled U.S. residents, covered a broader range of services, and largely eliminated patient cost-sharing while allowing a tightly limited prescription-drug charge. Whether that design would lower or raise any particular household’s total costs cannot be answered without a specified financing plan and a current, comparable estimate.
