Premium-free Medicare Part A often starts retroactively—usually up to six months before you apply for Part A or Social Security benefits—so many people must stop both payroll and employer HSA contributions six months before applying. Your exact final contribution month depends on your Medicare eligibility date, birthday, application timing, and whether you keep working.
The rule that ends HSA contributions
Internal Revenue Service (IRS) Publication 969 states: “Beginning with the first month you are enrolled in Medicare, your contribution limit is zero.” This applies to employee salary-reduction contributions, direct contributions you make yourself, and amounts your employer deposits. Medicare’s 2026 handbook similarly says, “You aren’t eligible to make contributions to an HSA after you have Medicare.”

The critical date is the month Medicare coverage begins, not necessarily the day you submit an application or the day you receive an approval notice. If Medicare coverage is made retroactive, every month in that retroactive period is treated as a month in which you were enrolled. HSA contributions allocated to those months are excess contributions under IRS rules.
You can remain employed, covered by a high-deductible health plan, and otherwise satisfy the HSA eligibility tests after age 65. Those facts do not preserve contribution eligibility once Medicare enrollment starts.

Why Medicare Part A can require an earlier stop
Premium-free Part A commonly reaches back six months from the month you enroll in Part A or apply for Social Security or Railroad Retirement Board (RRB) benefits. The retroactive period cannot begin before your first month of Medicare eligibility. The Social Security Administration describes the rule this way: “Part A coverage begins up to 6 months before the month you apply if you are over 65.”
That look-back is why Medicare.gov tells workers with HSAs to stop both their own and their employer’s contributions six months before they retire or apply for Social Security or RRB benefits. Stopping only your payroll deduction is not enough if your employer continues depositing money.
Part B has different enrollment mechanics. Someone with qualifying current-employment coverage may have an eight-month Special Enrollment Period to enroll in Part B after that employment or coverage ends. That Part B window does not eliminate the Part A retroactivity risk or create a separate period in which HSA contributions are allowed. Coordinate the Part A decision, Social Security or RRB application, retirement date, and payroll cutoff as one timeline.
Find your last allowable contribution month
| Situation | Medicare timing to examine | Practical last-contribution guidance | Important qualification |
|---|---|---|---|
| Initial Enrollment Period (IEP), ordinary birthday timing | Medicare’s 2026 handbook generally uses the month before you turn 65. | Make the final HSA contribution in the month before your 65th birthday month. | Confirm the actual Part A effective month on your Medicare notice. |
| Birthday on the first day of a month | Coverage generally starts on the first day of the prior month. | The safe final contribution month may be two months before the month in which you turn 65. | The first-of-month birthday changes the coverage month; do not rely on the ordinary IEP example. |
| Applying six or more months after turning 65 | Premium-free Part A can generally be retroactive six months from the application month. | Stop contributions six months before the month you apply for Medicare. | Part A cannot be retroactive before your first month of Medicare eligibility. |
| Still working after 65 | Current-employment coverage may let you delay Part B, but Part A and Social Security/RRB timing can still trigger retroactivity. | Medicare.gov recommends stopping employee and employer contributions six months before retirement or applying for Social Security/RRB benefits. | Confirm how your employer plan, intended retirement date, and Medicare application interact. |
| Medicare eligibility based on a circumstance other than turning 65 | Your eligibility and enrollment month may differ from the standard age-65 schedule. | Use the first month of actual Medicare enrollment as the contribution cutoff. | Ask Medicare and a tax professional to verify the effective date before changing contributions. |

If you enroll during your Initial Enrollment Period, Medicare’s 2026 handbook says you can generally avoid an HSA tax problem by making your last contribution in the month before you turn 65. This is a planning rule, not a substitute for checking your Part A effective date. A contribution for the month Medicare starts is not allowed, even if the contribution was deposited before you received your Medicare card.

Birthdays on the first of the month
When your birthday falls on the first day of a month, Medicare coverage generally begins on the first day of the preceding month. For example, if your 65th birthday is the first day of July, the relevant Medicare coverage month may be June. Under the handbook’s guidance, the safe final HSA contribution month may therefore be May. Verify the effective date shown by Medicare before your payroll cutoff.
Applying six or more months after age 65
People who wait at least six months after turning 65 face the clearest retroactivity trap. Medicare’s 2026 handbook says they can generally avoid a tax penalty by stopping HSA contributions six months before the month they apply. That recommendation tracks the usual six-month retroactive Part A period.
For instance, an application submitted in October can cause premium-free Part A to reach back to April, subject to the rule that coverage cannot begin before the first month of Medicare eligibility. Contributions assigned to April through September would be treated as contributions for Medicare months. The illustration shows why the application month—not the date you expect to retire—must drive the payroll cutoff.
Working past 65 does not automatically preserve eligibility
Continuing to work can allow you to postpone some Medicare enrollment decisions, particularly Part B when you have qualifying current-employment coverage. It does not, by itself, allow HSA contributions after Medicare enrollment begins.

Before choosing a delayed-enrollment strategy, establish all four dates:
- The month your employer coverage ends or you retire.
- The month you will apply for Social Security or RRB benefits, if applicable.
- The month Part A will begin, including any retroactive months.
- The month payroll and employer HSA deposits will actually stop.
Medicare.gov’s working-past-65 guidance says you and your employer should stop contributing six months before retirement or an application for Social Security or RRB benefits. Give that instruction to payroll and your benefits administrator in writing, and request confirmation of the final deposit month.
What counts as an HSA contribution for this purpose?
The zero limit applies regardless of who supplied the money. Review all of these sources:
Recommended Free Tools
- Pre-tax salary deferrals through your employer’s payroll system.
- Employer-funded HSA deposits, including recurring contributions that continue after you change your own election.
- Direct contributions you send to the HSA custodian and later claim on your tax return.
- Any year-end or incentive deposit that is allocated to a month in which Medicare coverage applies.
Ask the custodian how it dates contributions. Some deposits are made later but allocated to an earlier tax year; the allocation and Medicare coverage months both matter when identifying an excess.

What to do if Medicare is retroactive and you contributed anyway
IRS Publication 969 expressly treats contributions made during retroactive Medicare months as excess contributions. Act promptly rather than waiting for a tax notice.
- Confirm the effective date. Use your Medicare award or enrollment notice to identify the first month of Part A and any retroactive months. Keep the application date and, if relevant, the Social Security or RRB filing date.
- Inventory every deposit. Obtain HSA transaction records and payroll statements. Separate employee deferrals, employer contributions, and direct deposits, and identify the tax year to which each was assigned.
- Contact the HSA custodian. Ask whether a timely return of excess contribution is available for the affected tax year and what documentation it requires. Do not assume that simply withdrawing the balance corrects the tax reporting.
- Tell your tax professional. Form 8889 may need to reflect the excess and the correction. The correct treatment depends on the contribution type, tax year, and custodian processing, so obtain advice specific to your records.
- Stop future deposits. Confirm that payroll has ended salary deferrals and that the employer has stopped automatic or matching HSA deposits.
- Retain the paper trail. Keep the Medicare notice, applications, payroll records, HSA statements, custodian correspondence, and tax forms together.
A practical planning checklist
- Identify your first month of Medicare coverage, especially Part A.
- Determine whether premium-free Part A will be retroactive from a Part A, Social Security, or RRB application.
- Check whether a first-of-the-month birthday changes the expected coverage month.
- Tell payroll and the HSA administrator the exact final contribution month.
- Stop employer contributions as well as your own salary deferrals.
- If you contributed during retroactive Medicare months, contact the custodian and a tax adviser promptly about excess-contribution correction and Form 8889.
- Save the Medicare award notice, application dates, payroll records, and HSA contribution history.
Common planning mistakes
Using the Medicare card arrival date
The card may arrive after coverage has started. The effective month on the enrollment notice controls the HSA limit.
Stopping only personal contributions
An employer deposit is still an HSA contribution. Payroll, human resources, and the HSA custodian should all receive the cutoff date.
Assuming an eight-month Part B window allows HSA deposits
The Special Enrollment Period concerns Part B enrollment after current-employment coverage ends. It does not override the zero contribution limit that begins with Medicare enrollment or the retroactive Part A rule.

Applying for Social Security without adjusting HSA payroll
A Social Security or RRB application can establish the point from which premium-free Part A reaches backward. Coordinate the application and HSA stop date before filing.
Waiting until tax preparation to investigate
Once you discover retroactive coverage, gather records and contact the custodian promptly. Correction options and reporting depend on timing and the tax year.
Bottom line for your situation
There is no universal “age 65” cutoff. The controlling rule is zero HSA contribution room beginning with the first month of Medicare enrollment, including retroactive Part A months. For many people enrolling around age 65, the last contribution is the month before the 65th-birthday month; a first-of-month birthday can move that back another month. For someone applying six or more months after turning 65, stopping six months before the application month is the Medicare handbook’s general approach. If you keep working, coordinate employer coverage and the eight-month Part B Special Enrollment Period without overlooking Part A retroactivity.
