Medicare Advantage vs. Medigap: Which Coverage Is Better for You?

Neither option is universally better. Medicare Advantage is usually the better fit if you want a potentially low plan premium, built-in prescription coverage, extra benefits, and a defined annual limit on covered Medicare services—and you can use the plan’s network and follow its authorization rules. Original Medicare with Medigap is often better if you value nationwide access to participating providers, fewer utilization restrictions, and more predictable supplemental cost sharing. Your ZIP code, doctors, prescriptions, travel, budget, and willingness to manage plan rules should decide the choice.

What “better” means in this decision

Medicare Advantage and Medigap are not two versions of the same policy. They organize Medicare coverage in fundamentally different ways.

As an Amazon Associate I earn from qualifying purchases.

  • Medicare Advantage (Part C) is a private, Medicare-approved alternative to Original Medicare. A plan generally provides Part A hospital coverage, Part B medical coverage, and usually Part D prescription coverage through one plan.
  • Original Medicare plus Medigap keeps Parts A and B with the federal program. A Medigap policy helps pay specified Medicare cost sharing, while prescription coverage is purchased separately through Part D.

You cannot enroll in Medicare Advantage and use a Medigap policy at the same time. CMS states: “When you’re getting started with Medicare, you can either buy Medigap or enroll in a Medicare Advantage Plan, but you can’t have both.” If you join Medicare Advantage, an existing Medigap policy generally will not pay the plan’s deductibles, copayments, or coinsurance.

The practical question is therefore not which label is superior. It is which cost structure, provider-access model, drug coverage, travel rules, and enrollment risk fit your expected care.

Side-by-side comparison

Question Medicare Advantage Original Medicare with Medigap
How coverage is delivered Private Medicare-approved plan replacing the way you receive Parts A and B; most plans include Part D. Federal Original Medicare for Parts A and B, plus a private standardized Medigap policy; Part D is separate.
Provider access Usually requires use of the plan’s network and service area for non-emergency care. Out-of-network rules depend on the plan type. Generally use any doctor or hospital that accepts Medicare nationwide.
Premium pattern Some plans charge a $0 plan premium, but you still generally pay the Part B premium and the plan’s copayments, coinsurance, drug costs, and other charges. You pay a Medigap premium in addition to the Part B premium and normally add a separate Part D premium.
Annual spending protection Every plan has a yearly limit for covered Medicare services, but the amount varies by plan and year. No single Medigap limit applies to every policy. Your exposure depends on the letter plan, its deductibles, and premiums.
Prior authorization and referrals Plans may require referrals and prior authorization for services, procedures, or medicines. Original Medicare usually does not require prior approval for covered services, although some services have Medicare coverage rules.
Prescription drugs Most plans include Part D in the same plan; verify the formulary and pharmacy rules. Medigap policies sold after 2005 do not include drug coverage, so you generally need a separate Part D plan.
Extra benefits Plans may offer dental, vision, hearing, fitness, transportation, or other supplemental benefits, subject to plan limits. Medigap primarily covers Medicare cost sharing and generally does not cover routine dental, routine vision, or hearing aids.
Travel Plans have service areas and generally limited coverage outside the United States; emergency and urgent-care rules vary. Original Medicare is accepted by participating providers nationwide. Some Medigap policies provide limited foreign emergency coverage.
Ability to switch later You can change plans during permitted election periods, but moving to Medigap later may involve medical underwriting unless a protected right applies. The six-month Medigap Open Enrollment Period beginning when Part B starts offers the strongest protection against medical underwriting.

How Medicare Advantage works

One private plan coordinates most Medicare benefits

When you join Medicare Advantage, the plan receives a Medicare payment to provide your Part A and Part B benefits. You remain enrolled in Medicare and generally continue paying the Part B premium, but the plan sets the network, cost-sharing schedule, and procedures you use to obtain care. Most Medicare Advantage plans also include Part D, so you do not normally buy a separate drug plan.

Plan designs differ. Health maintenance organization plans commonly require in-network care except for emergencies, urgent care, or approved out-of-network services. Preferred provider organization plans may allow out-of-network care at a higher cost. Private fee-for-service and special needs plans have their own rules. Read the specific plan’s Evidence of Coverage rather than relying on the plan type alone.

Network and service-area rules are central

CMS warns that you may need doctors and other providers who are in the plan’s network and service area for non-emergency care. Before enrolling, check every physician, hospital, laboratory, imaging center, and therapist you expect to use. A doctor’s office may participate with one plan from an insurer but not another, and a hospital network can differ from its affiliated physician groups.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Most plans require you to live in the service area for at least part of the year. A permanent move can trigger a Special Enrollment Period, but routinely spending months in another state can make a local-network plan inconvenient even when it is available in your home ZIP code.

Referrals and prior authorization can affect timing

A plan may require a primary-care referral before you see a specialist. It may also require prior authorization before approving a hospital admission, advanced imaging, skilled nursing stay, durable medical equipment, or a high-cost drug. Authorization is a utilization-management decision, not a guarantee that the service will be free. Ask the plan and the treating office who submits the request, how long a decision normally takes, and how to appeal a denial.

The annual limit protects against unlimited covered-service costs

Medicare Advantage plans set a maximum amount you pay in a calendar year for covered Medicare Part A and Part B services. After you reach that plan-specific limit, the plan pays 100% of covered services for the remainder of the year. The limit does not automatically include premiums, Part D drug spending, non-covered services, or care from an unauthorized provider. Compare the limit with the copayments for the services you are most likely to need; a low premium does not make every plan low cost.

“$0 premium” does not mean zero cost

A $0 plan premium means the plan does not charge an additional monthly premium for that plan, not that Medicare is free. You generally still owe the Part B premium. You can also face medical and hospital copayments, coinsurance, drug deductibles or copayments, premiums for optional benefits, and the plan’s annual maximum. Review a plan’s estimated yearly drug and medical costs, not just its advertised premium.

How Original Medicare with Medigap works

Original Medicare provides broad provider choice

With Original Medicare, you can generally see any doctor or use any hospital nationwide that accepts Medicare. You do not select a private network for covered services, which is useful if you live in more than one state, travel frequently, or want access to specialists without changing plan networks. Providers can decline Medicare assignment or charge limiting amounts in certain situations, so confirm participation before care.

Medigap pays defined portions of Medicare cost sharing

Medigap policies are standardized by letter. A Plan G sold by one insurer has the same federally defined benefits as Plan G sold by another insurer, but the premium can be very different. CMS notes that “there can be big differences in the premiums that different insurance companies charge for the same coverage,” so compare policies with the same letter rather than comparing letters and insurers at the same time.

Medigap helps with covered Part A and Part B deductibles, coinsurance, and copayments according to the letter plan. It does not turn every service into a covered service: routine dental care, routine eye exams and glasses, hearing aids, long-term custodial care, and private-duty nursing are generally outside standard Medigap benefits. You continue paying the Part B premium and normally need separate Part D coverage for prescriptions.

2026 Medigap figures to know

CMS’s 2026 Medigap chart lists these figures:

  • For high-deductible Plan F or Plan G, you pay the first $2,950 in 2026 for Medicare-covered costs before the policy begins paying according to its terms.
  • Plan K has an $8,000 annual out-of-pocket limit in 2026.
  • Plan L has a $4,000 annual out-of-pocket limit in 2026.
  • The Medicare Part B deductible referenced for Plans K and L is $283 in 2026.

These are plan-year figures, not universal Medigap premiums or a prediction of your personal spending. High-deductible policies trade a lower premium for more initial exposure. Confirm the current chart, premium, and household discounts with the insurer licensed in your state.

Which option usually has lower out-of-pocket costs?

There is no national answer because Medicare Advantage plans and Medigap premiums vary by ZIP code, insurer, plan design, age-rating method, health status where underwriting is permitted, and the care you use.

Cost to model Medicare Advantage Original Medicare plus Medigap
Monthly premiums Plan premium, often advertised as $0, plus the Part B premium; add any Part D or supplemental premiums the plan charges. Part B premium, Medigap premium, and separate Part D premium.
Routine care Plan-specific copayments or coinsurance for primary care, specialists, therapy, imaging, and outpatient procedures. Medicare cost sharing after the Medigap policy pays its specified portion; the exact result depends on the letter plan and whether a deductible applies.
Hospital or major illness Inpatient and outpatient cost sharing continues until the plan’s annual Part A and Part B maximum is reached. Covered cost sharing is more predictable under comprehensive letter plans, but premiums and uncovered services remain your responsibility.
Prescription drugs Formulary tiers, deductibles, copayments, coinsurance, and pharmacy rules under the plan’s Part D benefit. Separate Part D premium and drug cost sharing; Medigap itself does not pay for prescriptions.
Worst-case planning Use the plan’s annual medical out-of-pocket maximum, then add premiums, drugs, and excluded or out-of-network care. Add annual premiums, Part D exposure, the letter plan’s deductibles or limits, and services Medicare does not cover.

For a meaningful comparison, enter your expected doctors, procedures, medications, and pharmacy into Medicare.gov’s plan comparison for your ZIP code. Compare the total annual premium, estimated medical spending, estimated drug spending, and maximum medical exposure. A plan with a higher premium can cost less overall if you use frequent specialist care or expensive medications; a low-premium plan can be economical when your care is light and in network.

Prescription drugs: the decision many people underestimate

Most Medicare Advantage plans include Part D, but the formulary is plan-specific. Check the exact drug name, dose, quantity, tier, prior-authorization requirement, step-therapy rule, preferred pharmacy, and mail-order price. A plan can cover a medication while charging substantially more than another plan.

Medigap policies sold after 2005 do not include prescription coverage. You generally need a stand-alone Part D plan, which means checking two separate contracts: the Medigap policy for medical cost sharing and the Part D plan for medicines. Delaying Part D without other creditable drug coverage can lead to a late-enrollment penalty, so establish whether your existing coverage is creditable before declining Part D.

Extra benefits versus predictable medical cost sharing

Medicare Advantage plans may include dental cleanings, limited restorative dental work, routine vision allowances, hearing aids, fitness programs, transportation, over-the-counter allowances, or meal benefits. Each extra has eligibility rules, dollar caps, provider restrictions, and expiration terms. Treat them as potential value, not as cash equivalents. Verify whether your dentist, optometrist, or hearing provider participates and whether the allowance covers the service you actually need.

Medigap’s purpose is different: it mainly reduces Medicare-approved Part A and Part B cost sharing. If you want routine dental, vision, or hearing coverage with Original Medicare, you generally must buy separate coverage or pay directly. The trade-off is a simpler medical provider-access model rather than a package of ancillary benefits.

Travel and living in more than one place

Original Medicare’s nationwide participation can be valuable for snowbirds, people who routinely visit family in another state, and patients who want access to specialized hospitals outside a local network. Some Medigap policies also cover limited foreign emergency care, subject to policy conditions and limits; read the certificate rather than assuming all international treatment is covered.

Emergency and urgent care are covered under special rules, but planned routine care away from the service area may be unavailable or cost more. If you move, report the move promptly because your election options and plan availability can change.

Enrollment timing and the risk of switching later

Medigap Open Enrollment is a six-month window

Your Medigap Open Enrollment Period starts when you are both 65 or older and enrolled in Part B. During this six-month period, you generally have the strongest federal protections: insurers cannot use medical underwriting to reject you or charge more because of health conditions for the available policies. The period is one-time and does not restart because you switch policies later.

Waiting to buy Medigap can mean medical questions, a higher premium, or a denial outside a protected period, depending on your state and circumstances. State law may provide additional rights, but do not assume a later return from Medicare Advantage will be guaranteed.

Guaranteed-issue situations

Federal guaranteed-issue rights can apply in specific circumstances, such as losing certain employer or union coverage, a plan leaving Medicare, or using a trial right that allows a return to Original Medicare. The policy choices, deadlines, and documentation requirements vary by event. Contact your State Health Insurance Assistance Program or Medicare before the right expires.

Moving between Medicare Advantage and Medigap

You can leave or change Medicare Advantage only during an applicable election period, such as the Annual Election Period or a Special Enrollment Period. Returning to Original Medicare does not automatically give you unrestricted access to Medigap. Outside protected situations, an insurer may ask health questions, apply state-specific rules, charge more, or decline an application. Confirm Medigap acceptance and the effective date before terminating Advantage coverage.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If you first joined Medicare Advantage when you were initially eligible for Medicare and decide within the trial-right period that it is not suitable, you may have a right to buy certain Medigap policies. Keep written evidence of enrollment dates and qualifying events.

A practical decision process

  1. List your non-negotiables. Write down doctors, hospitals, prescriptions, travel destinations, preferred pharmacies, and any upcoming surgery or specialist treatment.
  2. Check Original Medicare participation. Ask each provider whether they accept Medicare and whether they are accepting new patients. If a provider does not participate, estimate the possible limiting-charge exposure.
  3. Compare local Advantage plans. In Medicare.gov’s comparison tool, enter your ZIP code, medications, pharmacies, and preferred providers. Open each plan’s network directory, formulary, Evidence of Coverage, premium, copayments, prior-authorization rules, and annual medical maximum.
  4. Price Medigap by letter. Compare the same letter across insurers in your state. Record the premium method, household discount, rate-increase history if supplied, underwriting questions, high-deductible option, and effective date. Do not compare a cheap Plan N with a comprehensive Plan G as though they provide identical benefits.
  5. Add Part D to the Medigap scenario. Run your prescriptions through the stand-alone Part D comparison, including preferred pharmacies and mail order. Confirm that your current drug coverage is creditable if you are delaying enrollment.
  6. Calculate three totals. Estimate annual premiums, likely routine-care and drug costs, and a bad-year maximum. Include the Part B premium in both scenarios and exclude benefits you would not use.
  7. Verify before submitting. Call the plan for written confirmation of provider participation, formulary status, authorization requirements, travel rules, and effective dates. Save the plan documents and confirmation numbers.

Common situations and the likely fit

You want the broadest doctor choice

Original Medicare with Medigap is usually the stronger starting point when access to any participating provider nationwide matters more than bundled extras. Confirm that the Medigap premium and separate Part D cost fit your budget.

You have a tight monthly budget and use local providers

Medicare Advantage may fit when a low plan premium, local network, included Part D, and extra benefits are more valuable than unrestricted access. Model specialist, hospital, therapy, and drug copayments before enrolling.

You take several expensive medications

Do not choose based on the medical premium. Compare each plan’s formulary tier, utilization rules, pharmacy network, deductible, and annual drug spending. A plan that excludes one important medicine or requires a burdensome authorization may be a poor fit even if its premium is $0.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

You travel or divide the year between states

Start with provider access and service-area rules. Original Medicare’s nationwide participation may be easier to use, while an Advantage plan may work only if its travel, emergency, telehealth, and out-of-area provisions match your routine.

You are approaching or just past Part B enrollment

Put the six-month Medigap Open Enrollment timeline on your calendar. Obtain Medigap quotes and confirm eligibility before choosing Advantage if you think you may want Medigap soon; changing your mind later may expose you to underwriting.

Pitfalls that lead to expensive surprises

  • Choosing on the premium alone: add Part B, drug costs, copayments, coinsurance, and the annual medical maximum.
  • Assuming an insurer’s network is universal: verify the exact plan, medical group, hospital, and year.
  • Ignoring prior authorization: ask whether planned services and current medicines require approval and who handles appeals.
  • Using a Medigap letter as a price ranking: same-letter benefits are standardized, but premiums and rate structures differ by insurer and location.
  • Forgetting separate Part D: Medigap policies sold after 2005 do not include drug coverage.
  • Assuming you can buy Medigap later: outside Open Enrollment or a guaranteed-issue event, underwriting may apply.
  • Counting unused extras as savings: dental, vision, hearing, and other allowances have limits and provider rules.
  • Failing to update after a move: a new ZIP code can change Advantage availability, networks, premiums, and enrollment rights.

Where to verify current 2026 information

Plan premiums, formularies, provider directories, service areas, and benefit limits change annually and vary by ZIP code. Use Medicare.gov’s current plan comparison with your actual prescriptions and pharmacies. Read the plan’s Evidence of Coverage and Annual Notice of Change, and contact Medicare or your State Health Insurance Assistance Program when an enrollment right, guaranteed-issue protection, or appeal deadline is unclear. No national premium or universal out-of-pocket figure can substitute for a local, plan-year quote.
