Medicare Advantage Provider Networks Explained: In-Network, Out-of-Network, and Referrals

Whether you can keep your doctors in Medicare Advantage depends on your specific plan, not simply on whether a doctor accepts Medicare. An HMO generally limits covered care to its network, a PPO usually pays for covered out-of-network care at a higher cost, and a PFFS plan uses different rules based on whether a Medicare-approved provider accepts the plan’s payment terms. Medicare Advantage Special Needs Plans (SNPs) follow the network rules of their underlying HMO or PPO design, while Medical Savings Account (MSA) plans generally have no network for services covered by Original Medicare.

Before scheduling non-emergency care, confirm that the provider participates in your exact plan, what you will owe, whether a specialist referral is required, and whether the service needs prior authorization. A referral and prior authorization are separate requirements.

What a Medicare Advantage provider network is

A provider network is the group of doctors, hospitals, specialists, laboratories, pharmacies, and other clinicians that have a contract or other participation arrangement with a Medicare Advantage plan. The network determines where you can receive covered care and how the plan shares the cost with you.

Networks are plan-specific. Two plans offered by the same insurer can have different hospitals, medical groups, referral rules, and out-of-network benefits. A provider who participates in Medicare, or even in one Medicare Advantage plan, is not automatically in-network for another plan. Ask the plan or use its current provider directory for the exact contract you hold, then confirm with the provider’s office before the appointment.

“Accepts Medicare” is therefore not a complete answer to the question, “Can I use this doctor?” Original Medicare participation and Medicare Advantage network participation are different. The exception is a plan whose rules allow use of Medicare-approved providers under specified conditions, such as many PFFS and MSA arrangements.

How each Medicare Advantage plan type handles networks

Plan type Network rule Out-of-network care Specialist referrals
HMO Generally requires network providers for routine covered care. Usually not covered, except for emergency care, out-of-area urgent care, and temporary out-of-area dialysis. The plan may have additional exceptions. Usually required. Some services, including yearly mammogram screenings, can be obtained without a referral.
HMO Point-of-Service Uses an HMO network for most care. Some plans cover specified out-of-network services, generally with higher cost sharing. Check the plan’s HMO and point-of-service rules; referral requirements can apply.
PPO Network providers normally cost less, but the plan permits broader choice. Covered services are generally allowed out of network, usually at a higher cost. Contact the plan first to confirm medical necessity and coverage. No referral is required to see a specialist.
PFFS You may use Medicare-approved providers who accept the plan’s payment terms and agree to treat you. Out-of-network providers may charge you more and can decline to treat you. Generally not required.
HMO SNP Generally requires network care, subject to plan and emergency exceptions. Usually follows HMO limits; verify any specific exception with the plan. Generally required, subject to the plan’s stated exceptions.
PPO SNP Network providers usually have lower cost sharing. Can cover Medicare-covered services out of network under the plan’s PPO rules. No referral is required.
MSA Generally has no provider network for services covered by Original Medicare. You can generally use Medicare-approved providers for those covered services; confirm that the provider accepts Medicare. No referral is required.

This table describes the usual structure, not a guarantee for every contract. Your Evidence of Coverage and member-service department control the rules for your plan and the service you need.

In-network care: what it means for your bill

When a doctor or facility is in network, it has agreed to the plan’s contract terms. The provider bills the plan, and you pay the applicable deductible, copayment, coinsurance, or other cost sharing listed for that service. In-network care is usually the least expensive option available under an HMO or PPO.

Network status can differ by location and service. A hospital may be in network while an emergency physician, anesthesiologist, radiologist, or laboratory working there is not. Ask which clinicians and facilities will bill separately, particularly before planned surgery, imaging, infusion treatment, or inpatient care.

Out-of-network care: when it is covered and what can change

HMO and HMO SNP plans

Routine out-of-network care is generally not covered by an HMO. The important Medicare-recognized exceptions are emergency care, urgent care when you are outside the plan’s service area, and temporary out-of-area dialysis. A plan can also authorize an exception when needed care is unavailable in its network. Do not assume that paying the provider yourself will make the claim payable; call the plan before non-emergency treatment whenever possible.

HMO Point-of-Service plans

An HMO-POS plan may include a separate benefit for specified out-of-network services. That benefit normally has higher cost sharing than the in-network option and may have its own deductible, authorization, or service limits. The provider must still be willing to treat you and submit the claim under the plan’s rules.

PPO and PPO SNP plans

A PPO generally lets you use an out-of-network provider for a covered service, but you will usually pay more. The plan may apply a separate out-of-network deductible or coinsurance, and the provider may bill at a rate that produces a larger balance for you under the plan’s terms. Medicare recommends contacting the plan first to confirm that the service is medically necessary and covered.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

PFFS plans

PFFS plans do not operate like a conventional HMO network. You may see a Medicare-approved provider only if that provider accepts the plan’s payment terms and agrees to treat you for the service. An out-of-network provider can refuse the plan’s terms or charge you more, so ask the office to verify acceptance for the particular visit before care is delivered.

MSA plans

MSA plans generally do not maintain a network for services covered by Original Medicare. You still need a Medicare-approved provider, and the provider must accept Medicare for the service. The plan’s account and deductible rules determine your financial responsibility, so confirm how the claim will be processed.

Emergency, urgent, and dialysis exceptions

Emergency care is covered under Medicare Advantage even when you are outside the network. PPO members also have coverage for urgent care, while HMO rules specifically recognize out-of-area urgent care as an exception to the usual network requirement. Temporary out-of-area dialysis is another HMO exception. If you face a true emergency, seek immediate treatment first; network questions can be handled after you are safe.

For a non-emergency problem while traveling, call the number on your member ID card to learn whether the situation qualifies as urgent care, which facility to use, and how the plan will process the claim. Keep discharge papers and receipts if the provider asks you to pay at the time of service.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Referrals and prior authorization are different

What a referral does

A referral is a direction from a primary-care or other plan provider to a specialist or service. Most HMOs require one for specialist visits, and HMO SNPs generally follow that approach. PPOs, PPO SNPs, PFFS plans, and MSA plans generally do not require a specialist referral. A plan can waive a referral for particular preventive services; a yearly mammogram is one example identified by Medicare.

What prior authorization does

Prior authorization is the plan’s advance approval for a particular service, procedure, drug, supply, or setting. Medicare says members typically need it before the plan will cover certain services or supplies. The requirement can apply in a PPO even when no referral is needed, and it can apply after a specialist has recommended treatment.

A referral is not proof that prior authorization has been granted. Ask the plan exactly which approval is required, who must request it, whether the approval is for a specific provider and date range, and what happens if the plan denies it. Request the answer in writing or through the plan’s secure message system when timing or cost is significant.

Plan-directed care and out-of-network referrals

Medicare uses the term plan-directed care when a plan provider sends you for a covered service or to a provider outside the network without first obtaining an organization determination. In most cases, you will not have to pay more than the plan’s usual cost sharing for that directed care. Because the details depend on the plan and service, ask member services to confirm that the referral was plan-directed and how the claim will be paid.

Keep the referral, appointment instruction, and any authorization number. If the out-of-network provider later bills you at a higher amount, those records help the plan investigate whether its direction should have protected you from additional cost sharing.

How to verify a provider before non-emergency care

  1. Identify the exact plan. Use the plan name, contract or plan identifier, and coverage year shown on your member ID card. Network rules can change when you switch plans or when a new contract year begins.
  2. Check the plan’s directory. Search for the individual clinician, practice location, facility, and specialty. A directory listing is a starting point, not a substitute for confirmation.
  3. Call the plan. Ask whether the provider is in network for your exact plan and whether the planned service will be treated as in-network. Ask about separate professionals who may bill at a hospital or surgery center.
  4. Call the provider’s billing office. Give the plan name and member information, and ask the office to verify participation for the date and service. Record the representative’s name, the date, and any reference number.
  5. Confirm the financial rules. Ask for the in-network and out-of-network copayment or coinsurance, deductible, and any service-specific limit. Ask how the plan handles a provider who is not in network.
  6. Ask about referrals. Find out whether your primary-care clinician must issue one, whether it must identify a particular specialist, and how long it remains valid.
  7. Ask about prior authorization. Confirm whether the service, test, supply, drug, or care setting needs approval and who submits the request. A specialist’s recommendation alone may not satisfy this step.
  8. Save the evidence. Keep directory screenshots, written messages, referral documents, authorization numbers, and call notes until the claim is processed and any appeal period has passed.

What to do when covered treatment is unavailable in network

Tell the plan that the covered treatment or qualified specialist is not available within its network. Ask whether the plan will arrange an out-of-network appointment, issue a single-case agreement, or authorize another exception. Ask how the service will be priced and whether you will pay the plan’s usual in-network cost sharing.

Do not book an expensive out-of-network procedure first and request approval afterward unless the plan tells you to do so. Ask for the decision and any organization-determination or authorization number before treatment. If the plan refuses a service you believe should be covered, use the appeal instructions in your Evidence of Coverage.

Questions to ask member services

  • Is my doctor, hospital, facility, and each separately billing clinician in network for this exact plan?
  • What will I pay in network, and what will I pay out of network for this service?
  • Does this plan cover out-of-network care, and are there separate deductibles or limits?
  • Do I need a referral for this specialist or service?
  • Does the service require prior authorization, and who is responsible for obtaining it?
  • What should I do if no in-network provider can provide the covered treatment?
  • How are emergency, out-of-area urgent care, and temporary dialysis claims handled?
  • Can you provide the answer in writing or give me a call-reference number?

Common situations and the safest response

Your doctor accepts Medicare but is not listed for your plan

Do not assume the visit will be covered at the in-network rate. Ask the plan whether the doctor participates in your contract, whether an exception is available, and what the out-of-network cost would be. If the plan is an HMO, routine care may not be covered outside the network.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A PPO specialist says no referral is needed

That may be correct because PPOs generally do not require specialist referrals, but ask whether the planned test, procedure, drug, or facility needs prior authorization. The absence of a referral requirement does not remove other coverage conditions.

An HMO sends you to an outside specialist

Ask whether this is plan-directed care and request confirmation of the cost-sharing treatment before the appointment. Keep the plan’s direction and any authorization documentation in case the provider later bills you.

You need care while traveling

For an emergency, obtain immediate care. For an urgent but non-emergency problem, contact the plan to identify the covered option and claim process. HMO members should pay particular attention to the out-of-area urgent-care exception and temporary dialysis rules.

Bottom line

Medicare Advantage network rules are controlled by the plan type and the specific contract. Verify network status and expected cost before routine care; treat referrals and prior authorization as separate checks; and document any plan-directed or exception-based care. That preparation is the best way to avoid an unexpected out-of-network bill.
