To keep Medicaid active, watch for notices from your state, keep your mailing address and other contact details current, and return every requested form or document by the deadline printed on the notice. Each state runs its own renewal process. For your case status, deadline and submission instructions, contact your state Medicaid agency.
Start with these actions
Medicaid renewal is handled by the state that manages your case, not by one nationwide portal or a single national deadline. The safest approach is to treat every state notice as time-sensitive even if you expect to remain eligible.

- Check your contact information. Make sure the state has your current mailing address, phone number and email address if your state uses email. A move, returned letter or changed phone number can prevent you from seeing a renewal notice.
- Watch every channel your state uses. Check postal mail, your state benefits account and any text or email alerts you have signed up to receive.
- Read the entire notice. Find the renewal date, the people listed, the information requested and the exact way the state wants you to respond.
- Return what is requested by the stated deadline. Follow the notice’s instructions for an online submission, mail, fax, drop-off or another method. The notice controls your practical deadline.
- Keep proof of your response. Save a copy of the completed form and documents, plus a confirmation number, upload receipt, fax confirmation or mailing record when available. Keeping records is a practical safeguard, not a federal filing requirement.
- Call the state agency when anything is unclear. Ask whether the renewal was received, whether more information is needed and whether coverage remains active.
Use the Centers for Medicare & Medicaid Services (CMS) state contact directory to find the agency for your state. The agency can see your case and give the state-specific instructions that a general federal page cannot.

Your state tries to renew you using information it already has
Before asking you to complete paperwork, a state must first try an “ex parte” renewal. That means checking reliable information in your account or other current information available to the agency to determine whether you still qualify. CMS states:

“States must make a redetermination of eligibility for all Medicaid beneficiaries without requiring information from the individual if able to do so based on reliable information contained in the individual’s account or other more current information available to the agency.”
This requirement appears in CMS’s November 26, 2024 informational bulletin. If the state can confirm eligibility from reliable information, you may receive a notice that coverage continues without having to send a form.
A form is sent when available information is not enough
If the state cannot establish continued eligibility from the information it has, it sends a renewal form and may request documents. The form can be prepopulated with information the state already knows. You must still check every line, correct anything that changed and provide the requested proof by the date on the notice.

Do not assume that no form means no action is needed. Read notices even when you believe the state can renew you automatically. If you are uncertain whether a response is required, ask the state Medicaid agency to check your case.
Most MAGI eligibility is reviewed every 12 months
For people whose eligibility is determined using modified adjusted gross income (MAGI) rules, federal guidance describes renewal once every 12 months and no more frequently than once every 12 months. This is a general federal interval, not a promise that every household will receive a notice on the same date or through the same delivery channel. The interval is described in CMS’s December 4, 2020 renewal requirements.

Prepare before a renewal notice arrives
Keep your case file current
Report changes through the method your state provides, such as its benefits account, call center or local office. In particular, update your address before a move or as soon as possible afterward. Also update phone and email details if your state uses them for alerts. A notice sent to an old address can still carry a deadline even if you never see it.
Know which agency and account to use
Bookmark the official state Medicaid or combined benefits website and keep the agency phone number from an official notice. The CMS state directory is a starting point, but your state’s own notice and account are the authority for your case. Be cautious with unsolicited callers or websites asking for payment or sensitive information; verify contact details through the official agency.
Gather information you may need to verify
A renewal form may ask you to confirm household members, income, address, health-insurance information or other eligibility facts. The exact questions and documents vary by state and by eligibility category. Do not send documents that the notice does not request, and do not substitute a document the notice says is unacceptable without asking the agency first.
Plan for help if you need it
If reading forms, using an online account or meeting a deadline is difficult, arrange help before the notice arrives. A trusted person can help you understand instructions, but the state will tell you what authorization is needed for someone else to discuss or change your case. Ask the agency about accessible formats, language assistance or another way to submit information if the standard process does not work for you.
What to do when a renewal notice arrives
- Identify the notice. Confirm that it is about Medicaid or CHIP renewal, not a separate change-reporting request. Note the date printed on the notice and the response deadline.
- Check each person listed. Renewal decisions are made for individuals. Make sure the notice includes the correct household members and identify whose coverage is being reviewed.
- Read the response instructions. Use the submission channel named by the state. If an online account is required, sign in through the official state site rather than a link from an unexpected message.
- Review prefilled information. Correct income, household or contact details that are wrong or outdated. Explain changes in the space provided or through the method the notice gives.
- Attach every requested document. Check that pages are legible and that your name or case identifier appears where the state requires it. Keep the originals unless the notice specifically requires an original.
- Submit before the deadline. Do not wait for the final day if mail, scanning or an online outage could delay delivery. If the deadline has passed, submit as soon as possible and call the agency.
- Confirm receipt. Save the online confirmation, upload receipt, fax report, postal tracking or a dated note of an in-person drop-off. Ask the agency to confirm that the response is in your case file.
If the notice is confusing, the deadline is close or the agency says it has no record of your response, contact the state Medicaid agency immediately. Explain what you sent, when and how, and ask what additional step is required to prevent a lapse.

Federal response periods: what the numbers mean
Federal rules set minimum protections in specific situations, but your notice still supplies the operative deadline and instructions.

The 30-day minimum applies to the situations identified above; it is not a universal deadline for every Medicaid notice. Eligibility category, state procedures and the wording of your notice matter.
Renewals are handled for each person, not automatically for the whole household
CMS requires states to conduct renewals at the individual level. If one household member does not respond, that person’s failure to respond should not automatically end another member’s eligible coverage. CMS describes this requirement in its August 30, 2023 letter on individual-level renewals.
When a household notice arrives, check whose eligibility is listed and whether separate responses are required. If a child, parent or other household member appears to have been terminated because another person did not respond, call the state promptly and ask the agency to review each person’s record separately.
If you missed the renewal request or your coverage ended
Contact the state immediately
A missed form or document request can result in a procedural termination. Do not assume that you must start over. Tell the state when you received or discovered the notice, why the response was late and what information you can provide now. Ask whether the case can be reconsidered and exactly which documents the agency needs.

The 90-day MAGI reconsideration pathway
CMS’s March 15, 2024 renewal reminders say that a MAGI individual who responds to a renewal request within 90 days after a procedural termination must not be required to submit a new application. CMS’s individual-level renewal guidance also describes returning the renewal form within 90 days of termination for reconsideration without a new application. These protections apply to the specified MAGI circumstances; the state must determine whether your case fits them and explain its procedure.
Use the agency’s official contact route and ask three direct questions: whether the termination was procedural, whether you are still within the 90-day period, and how to submit the renewal information for reconsideration. Keep the confirmation that you responded.
Do not wait for the 90-day period
The 90-day window is not a reason to delay. A state may need time to review documents, correct an account or explain another eligibility pathway. Contact the agency as soon as you learn coverage ended and follow the instructions in the termination notice.

The state must check other Medicaid eligibility bases first
Before terminating coverage because the current eligibility basis no longer applies, the state must consider whether you qualify under another Medicaid basis. This requirement is described in CMS’s March 15, 2024 renewal requirement reminders. A change in income, age, disability status, pregnancy, household composition or another factor can affect which rules apply, so ask the agency what eligibility basis it evaluated.

You should receive advance notice and fair-hearing information
CMS says a state must provide advance notice before adverse action, including an explanation of fair-hearing rights. Read the termination notice carefully. It should explain the reason, effective date and how to request a hearing. Appeal deadlines are controlled by the notice and state rules; there is no single deadline that applies nationwide.

Appeal when the facts or process are wrong
If income, household information or another fact was recorded incorrectly, tell the agency and follow the appeal instructions in the notice. Submit evidence in the format and by the date required by your state. Ask whether coverage can continue while a timely appeal is pending; the notice and state rules determine what is available.
Ask about other coverage if Medicaid cannot continue
If the state confirms that you no longer qualify for Medicaid, ask what other coverage options may be available based on your circumstances. Possibilities can include another Medicaid eligibility category, the Children’s Health Insurance Program (CHIP), Marketplace coverage or employer coverage. Eligibility, enrollment periods and costs differ, so obtain current instructions from the state agency or the appropriate official program.
Common renewal problems and the practical fix
- No notice arrived: Verify your address and contact details with the state, ask whether a renewal is pending and request another copy of any notice.
- The notice requests information you already provided: Resend it using the specified channel, include a copy of your prior confirmation if you have one and ask the agency to mark the case file.
- The online account will not accept an upload: Take a screenshot of the error, note the date and time, then use another submission method listed on the notice or call the agency before the deadline.
- You cannot obtain a document by the deadline: Contact the agency before the deadline, explain the problem and ask whether an alternative document, extension or other process is available. Do not assume an extension exists without confirmation.
- The notice lists the wrong household member or status: Call the agency and ask for an individual-level review. Send corrections through the official method and keep proof.
- You moved between states: Contact the Medicaid agency in the state where you now live and the agency that handled the old case. Do not assume coverage transfers automatically; each state controls its own case and eligibility process.
- You receive suspicious renewal messages: Verify the sender through the official state website or the CMS state directory before sharing personal information. Use the phone number printed on a genuine notice when possible.
A simple renewal calendar
Use this sequence whenever your state’s annual review approaches:
- Before any notice: update your address, phone and email information and make sure you can access the state benefits account if your state uses one.
- When the state reviews your case: read every letter or electronic alert. The state may complete an ex parte renewal without asking you for information.
- If a form arrives: record the deadline, review the prefilled information, gather the requested documents and submit them through the stated channel.
- After submission: retain proof and check the account or call the agency to confirm receipt and the expected decision status.
- If coverage is terminated: read the reason and appeal instructions, then contact the agency immediately about reconsideration, other eligibility bases and any available hearing rights.
Federal guidance provides the framework, but the state notice remains the practical checklist for your case. For status questions, use the CMS directory to reach your state Medicaid agency.
What to remember
Keeping Medicaid active is mainly a matter of communication and deadlines: keep your case information current, read state notices, respond completely by the stated date and preserve proof. Some people will be renewed automatically from reliable information already available to the state, but everyone should monitor notices and verify status when uncertain. If coverage ends after a missed request, contact the state promptly and ask about the 90-day MAGI reconsideration pathway, individual-level review and the appeal rights described in your notice.
