India vs. US retirement healthcare: costs, coverage, and planning for parents

The right comparison starts with geography, not currency conversion. A parent who lives and receives care in India is governed mainly by Indian eligibility and hospital rules; a parent treated in the United States faces Medicare parts, premiums, deductibles, networks and supplemental coverage. India’s 70-plus AB PM-JAY expansion offers eligible citizens a defined annual hospitalization benefit, while U.S. Medicare combines several forms of insurance with recurring premiums and service-by-service cost sharing.

Use the framework below to identify what each parent can use, what it will cost, which bills remain uncovered and how to plan for long-term care.

As an Amazon Associate I earn from qualifying purchases.

Start with where each parent will receive care

India and the United States do not offer interchangeable retirement-healthcare products. A program normally pays according to its own eligibility rules, participating providers and covered services. Before comparing premiums, write down each parent’s expected place of treatment for the next year: India, the United States or both.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Parent living and treated in India: Check AB PM-JAY eligibility, the state’s implementation, empanelled hospitals and the treatment package for the planned service.
  • Parent living and treated in the United States: Check Medicare eligibility, enrollment in Parts A and B, drug coverage and whether a Medicare Advantage, Medigap or retiree plan changes the bill.
  • Parent splitting time between countries: Confirm where routine, specialist, hospital and prescription care will actually occur. Do not assume that a benefit in one country follows the parent to the other.

Location also determines provider access. A generous headline benefit is not useful if the parent cannot reach a participating hospital or the needed service is outside the applicable package.

India: what the 70-plus AB PM-JAY expansion provides

Eligibility after the 2024 expansion

The Union Cabinet approved AB PM-JAY coverage for all Indian citizens aged 70 and older, regardless of socioeconomic status. The expansion took effect on October 29, 2024, according to the Ministry of Health and Family Welfare’s November 2024 update. The age threshold and citizenship requirement are central: a younger parent cannot qualify through this expansion merely because the family has high medical expenses.

People already covered by schemes such as CGHS, ECHS or Ayushman CAPF may choose to continue with their existing scheme or use AB PM-JAY. The government announcement also said that people with private health insurance or Employees’ State Insurance are eligible. Those statements do not eliminate the need to check the parent’s current enrollment record, state process and hospital participation.

Annual benefit and family structure

The announced benefit is up to ₹5 lakh per year. It is a defined scheme benefit, not an unrestricted reimbursement account and not unlimited insurance.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Parent’s situation How the announced benefit is described What to verify before treatment
70-plus senior in a family already covered by PM-JAY An additional top-up for the eligible senior; the government described it as not being shared with younger family members. Whether the senior is correctly tagged in the family record, the available balance and the hospital’s authorization process.
Newly eligible 70-plus senior Coverage on a family basis, up to the announced annual amount. Which family members are included, how the state administers the benefit and whether another family member’s use affects the available amount.

The ₹5 lakh figure should therefore be read together with the words “up to,” the family or top-up structure and the package rules. It does not by itself tell you what a particular operation, cancer treatment, intensive-care stay or follow-up service will cost the family.

What is covered and where

The Health Ministry’s November 2024 update described cashless benefits for 1,961 procedures across 27 medical specialties in the then-current national package. That is a dated description of the package, not a promise that every procedure is available to every parent at every hospital today. Package lists, authorizations and state implementation can change.

Before admission, ask the hospital to confirm all of the following in writing or in its authorization system:

  • That the hospital is currently empanelled for the parent’s scheme and state.
  • That the diagnosis and proposed treatment map to an available package.
  • Which pre-authorization documents are required.
  • What is included in the cashless package and what the family may still have to pay.
  • Whether post-discharge medicines, tests or follow-up visits are included, limited or excluded.

“Cashless” describes the approved pathway for covered services; it does not mean every medical expense associated with an illness is automatically paid.

United States: how Medicare costs are assembled

Part A hospital insurance

Part A helps cover inpatient hospital care and has a deductible and cost sharing tied to benefit periods. Most people with sufficient Medicare-tax work history qualify for premium-free Part A. People without enough qualifying work history may owe a Part A premium; the applicable amount depends on their record and is not a single national figure for every beneficiary.

Because the deductible and cost sharing are attached to benefit periods, a family should not budget Part A as if it were simply one annual deductible. Ask the hospital or Medicare plan how a new admission, readmission or extended stay is treated under the current benefit-period rules.

Part B medical insurance

For 2026, Medicare lists a standard Part B premium of $202.90 per month. The premium can be higher for beneficiaries subject to income-related adjustments. The 2026 annual Part B deductible is $283. After the deductible, beneficiaries usually pay 20% of the Medicare-approved amount for covered Part B services.

2026 Original Medicare Part B item Amount or rule Qualification
Standard monthly premium $202.90 Standard amount; higher-income beneficiaries can pay more.
Annual deductible $283 2026 figure.
Typical cost after deductible 20% of the Medicare-approved amount for covered Part B services Applies to usual covered Part B services; provider assignment and service rules matter.

Part B covers many physician, outpatient and preventive services, but the family’s bill depends on what Medicare approves, whether the service is covered and whether another policy pays some of the patient share.

Prescription drug coverage

Drug-plan premiums, deductibles, copayments and coinsurance vary by the plan selected and the prescriptions used. There is no responsible single “Medicare drug cost” for an unspecified parent. Build the estimate from the parent’s medication list, pharmacy choices and the plan’s current formulary rather than from an average.

Original Medicare, Medigap and Medicare Advantage

Original Medicare generally allows a beneficiary to use any doctor or hospital that takes Medicare anywhere in the United States. Original Medicare by itself has no yearly out-of-pocket limit. A Medigap policy can pay some deductibles, coinsurance or copayments, subject to the policy’s terms and premiums.

Medicare Advantage plans are private Medicare plans with their own premiums, copayments, coinsurance, provider networks and annual out-of-pocket limits. Non-emergency care may require network providers, and some plans require referrals. Costs and rules differ by plan and service area, so one plan’s figures cannot be presented as representative of the entire country.

U.S. coverage arrangement Provider access Out-of-pocket protection What to compare locally
Original Medicare alone Generally any U.S. doctor or hospital that accepts Medicare. No yearly out-of-pocket limit. Part A and B cost sharing, drug coverage and ability to pay uncapped bills.
Original Medicare plus Medigap Usually retains Original Medicare provider access. Medigap may pay specified Medicare cost sharing; policy premiums and benefits vary. Premium, plan letter, enrollment timing, medical underwriting rules where applicable and drug coverage.
Medicare Advantage Often network-based for non-emergency care; referrals may be required. Plan has an annual out-of-pocket limit, with the amount and covered services varying. Network hospitals and doctors, referral rules, premium, copays, coinsurance, drug formulary and annual limit.

India and U.S. coverage compared by the decisions that matter

Decision point India: AB PM-JAY 70-plus expansion United States: Medicare
Who may qualify Indian citizens aged 70 or older under the expansion, regardless of income; verify records and state implementation. Eligibility and enrollment depend on Medicare rules, age or qualifying disability, work history and other circumstances.
Headline benefit Up to ₹5 lakh per year; family or senior top-up structure matters. No single annual medical benefit amount; Parts A and B pay covered services subject to deductibles, coinsurance and plan rules.
Hospital access Use participating, empanelled hospitals and an applicable package. Original Medicare generally permits any U.S. provider accepting Medicare; Medicare Advantage may use networks.
Routine outpatient care Confirm whether the needed service is in the current package and available through the state process. Part B generally covers covered outpatient and physician services after its deductible, usually with 20% coinsurance.
Prescription drugs Whether medicines are included depends on the approved package and local rules; verify before discharge. Drug-plan premium and cost sharing vary by plan and prescriptions.
Annual cap or limit Defined annual scheme amount, subject to eligibility, package and family structure. Original Medicare alone has no annual out-of-pocket cap; Medicare Advantage has a plan-specific limit; Medigap changes cost sharing.
Long-term custodial care Not established by the figures above; plan separately for ongoing personal assistance and uncovered services. Medicare does not pay for long-term care; Medicaid, state eligibility and private long-term-care insurance may be relevant.

The table is a decision aid, not a conversion between rupees and dollars. The two systems define eligibility, covered packages and patient liability differently, so an exchange-rate comparison can produce a misleading budget.

Long-term care is a separate retirement risk

Medicare states plainly: “Medicare doesn’t pay for long-term care.” Long-term care includes medical and non-medical help for chronic illness or disability, often assistance with dressing, bathing, using the bathroom or other daily activities. Most long-term care is non-medical, and Medicare and most health insurance, including Medigap, do not pay for it.

A short-term skilled nursing benefit or care connected to a covered hospital episode should not be treated as a promise to fund indefinite custodial care. For a U.S.-based parent, possible funding routes include Medicaid subject to state eligibility and private long-term-care insurance. For a parent in India, the family must examine public eligibility, private policies, savings and the cost of paid home or facility assistance in the parent’s city. The practical question is not only “Will insurance pay?” but also who will provide and finance daily help if independence declines.

Retiree and employer coverage can change the Medicare decision

A parent with retiree health insurance should ask the former employer or plan administrator how that plan coordinates with Medicare. Medicare says a person becoming eligible may need both Part A and Part B to receive full retiree-coverage benefits.

  1. Ask whether the retiree plan pays primary or secondary after Medicare eligibility.
  2. Confirm whether Part A, Part B or both must be enrolled for the retiree plan to pay as designed.
  3. Ask whether delaying Medicare enrollment creates a coverage gap or a late-enrollment penalty.
  4. Obtain the coordination rules and drug-coverage notice in writing.
  5. Use the State Health Insurance Assistance Program (SHIP) for free, individualized Medicare counseling, including questions about whether Medigap is appropriate.

Do not cancel a retiree plan, decline Part B or buy supplemental coverage based only on a general rule. The employer plan’s documents and the parent’s enrollment dates control the outcome.

A practical planning process for families

1. Create a parent-by-parent profile

Record citizenship, age, residence, expected treatment country, current public or employer coverage, private insurance, medication list, major diagnoses and likely procedures. Include the parent’s preferred hospitals and the family member who can handle authorizations.

2. Verify eligibility and enrollment

For India, confirm the 70-plus record, state implementation, family or top-up status and empanelled hospitals. For the United States, confirm Medicare eligibility, Part A work-history status, Part B enrollment, drug coverage and any Medicare Advantage, Medigap or retiree policy.

3. Map the likely care pathway

Separate routine visits, specialist consultations, diagnostic tests, hospital admissions, prescriptions, rehabilitation and daily-living assistance. A plan that is adequate for an emergency admission may not cover medicines, dental care, hearing services, home help or long-term custodial support.

4. Price the parent’s actual exposure

Use the known figures where they apply: the 2026 standard Part B premium is $202.90 monthly and the Part B deductible is $283, followed usually by 20% coinsurance for covered Part B services. Add the parent’s Part A status, drug-plan costs, supplemental premiums, plan copays and transportation. For India, start with the approved package and any amount above the ₹5 lakh annual scheme benefit, rather than assuming every expense is cashless.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. Check provider and authorization rules before a crisis

Save the names and telephone numbers of participating Indian hospitals, U.S. Medicare providers, the Medicare Advantage plan, retiree administrator and the person responsible for prior authorization. Ask what happens outside the network or when a package is unavailable.

6. Build a long-term-care reserve

Estimate several levels of help: occasional home assistance, daily personal care, skilled nursing and residential care. Decide which assets, income, family support, public programs or insurance would be used at each level. Keep this reserve separate from the deductible fund for an acute hospitalization.

7. Review annually and after a move

Recheck Indian package and hospital participation, Medicare plan documents, drug formularies, retiree coordination and the parent’s residence. A move to another state or country can change provider access and available coverage even when the parent’s age and diagnosis remain the same.

Assuming Original Medicare has a spending ceiling

Original Medicare alone has no yearly out-of-pocket limit. A family needs a deliberate choice about Medigap, Medicare Advantage or the ability to absorb uncapped cost sharing.

Assuming “cashless” means every related bill is free

Cashless treatment applies to an approved pathway and package. Ask about exclusions, medicines, tests, follow-up and non-covered supplies before admission.

Confusing skilled nursing with permanent custodial care

Medicare’s statement that it does not pay for long-term care means families must plan separately for ongoing help with daily activities.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ignoring retiree-plan coordination

Declining or delaying Medicare without checking the employer plan can create a gap or penalty. Get the coordination rule from the administrator and seek SHIP counseling.

What a realistic family budget should include

No single India-versus-U.S. annual cost estimate is valid for an unspecified household. A parent’s total depends on age, location, income where relevant, work and retiree coverage history, health conditions, services used, provider choice and the ability to obtain covered care.

Use four separate budget lines for each parent:

  • Access costs: premiums, enrollment fees if applicable, transportation and administrative expenses.
  • Routine care: physician visits, tests, medicines, preventive services and supplies.
  • Acute-event exposure: deductibles, copayments, coinsurance, package exclusions and amounts above an annual scheme limit.
  • Long-term support: home aides, equipment, modifications, respite, nursing or residential care.

Then stress-test the plan against an ordinary year, one major hospitalization and a year in which the parent needs daily assistance. The exercise reveals whether the family is relying on a benefit that covers only one part of the risk.

Decision checklist before choosing or changing coverage

  • Where will the parent receive most care during the next 12 months?
  • Is the parent an Indian citizen aged 70 or older, and has the AB PM-JAY record been verified?
  • Which Indian hospitals are currently empanelled for the required package?
  • Is the parent enrolled in Medicare Parts A and B, and what is the Part A premium status?
  • For 2026, has the family budgeted the $202.90 standard Part B monthly premium and $283 Part B deductible, while checking for income-related premiums?
  • Does the U.S. parent have Original Medicare, Medigap, Medicare Advantage, drug coverage or retiree insurance?
  • Are network, referral, formulary and prior-authorization rules written down?
  • What costs remain when a service is excluded, a package limit is reached or a provider is unavailable?
  • Who will pay for daily personal assistance if the parent cannot bathe, dress or use the bathroom independently?
  • When will the family review the plan again?

The most defensible plan is the one that matches the parent’s actual care location, verifies eligibility and provider access, budgets the documented cost sharing, and reserves money for care that neither public program covers.
