How to Verify a Mortgage Insurance Notice Before You Pay

Do not pay an unfamiliar mortgage-insurance notice until you have independently confirmed it with the company currently servicing your mortgage. Use the phone number or website on a recent mortgage statement or coupon book—not the contact details printed on the notice—to ask whether the notice is genuine, what type of insurance it concerns, the dates and amount involved, and the approved payment method. Then compare the answer with your loan and insurance records. Keep making your normal mortgage payment on schedule while you investigate.

What to do first

Treat the notice as an unverified claim, not as proof that you owe money and not as proof that it is a scam. Mortgage-related insurance charges can be legitimate, but “mortgage insurance” can mean several different products with different rules. A notice may also arrive after a servicing transfer, when the company handling your account has changed.

As an Amazon Associate I earn from qualifying purchases.

  • Do not click its link, scan its QR code, call only the number on the notice, or send money to a new payment address before independent confirmation.
  • Do not stop your regular mortgage payments because a letter tells you to. A payment dispute does not automatically suspend your loan obligations.
  • Do not ignore a possible coverage problem. If the notice is real, a delay could leave your servicer without proof of required property insurance or could cause a legitimate deadline to pass.

The safest sequence is: record the notice details, identify the current servicer from a trusted record, call or contact that servicer through an independently verified channel, ask specific account questions, compare the response with your insurer and loan records, and dispute any unexplained charge in writing.

Step 1: Record exactly what the notice claims

Before contacting anyone, make a copy or photograph for your records. Write down the following fields exactly as they appear:

#1 Best Overall

  • Clear Print Quality: Displays high-resolution printing intended to remain legible through regular use and cleaning
  • Sender’s legal name and mailing address
  • Date of the notice
  • Your property address
  • Loan or account reference number (do not disclose it to an unverified caller or website)
  • The insurance name or description used, such as PMI, mortgage insurance, hazard insurance, homeowners insurance, or lender-placed insurance
  • The alleged lapse, cancellation, transfer, renewal, or other event
  • Coverage dates or the date range in question
  • Amount due, whether it is a premium, fee, estimated annual cost, or past-due balance
  • Payment deadline and requested action
  • Payment address, bank details, telephone number, email address, web address, QR code, or other payment route

These details give the verified servicer a precise description to investigate. They do not authenticate the document. An impostor can copy an account number, logo, mailing style, or a real company’s name.

Step 2: Find the current mortgage servicer independently

Your mortgage servicer is the company that handles day-to-day account administration and receives mortgage payments. It may be different from the lender that originally made the loan, and it can change over time. The Consumer Financial Protection Bureau’s guidance on working with a mortgage servicer recommends using your latest monthly statement or coupon book to locate the current contact information.

  1. Find the most recent statement or coupon book that you obtained before the unfamiliar notice.
  2. Use the telephone number, mailing address, or web address printed on that trusted record.
  3. If you no longer have a statement, locate the servicer through a source you can verify independently, such as the servicer’s established official website or a prior statement saved in your records.
  4. Ask the verified servicer whether your account has transferred and, if so, which company was responsible during the dates listed in the notice.

Do not rely solely on the notice’s phone number, email address, QR code, link, or payment instructions. If a transfer occurred, compare the notice with the official transfer notices you received. CFPB guidance says a transfer notice should identify when the old servicer stops accepting payments, when the new servicer starts, the new servicer’s contact information, and the transfer date. Review later statements as well; the first document you receive after a transfer may not explain every historical charge.

Step 3: Ask the servicer account-specific questions

Once you reach the servicer through a trusted channel, ask the representative to look at your actual loan record. Do not accept a general answer about what the company “usually” does.

Confirm the notice and charge

  • Did your company issue this notice, and is it associated with my mortgage account?
  • What exact charge or action does your system show?
  • What is the amount due, how was it calculated, and what is the due date?
  • Which payment channel should I use—my normal payment portal, an established lockbox address, or another method?

Identify the insurance

  • Is this private mortgage insurance (PMI), FHA mortgage insurance, VA-related mortgage insurance or funding charges, lender-paid mortgage insurance, or force-placed property or hazard insurance?
  • What coverage dates does your record show?
  • What event triggered the notice: a new premium, an alleged policy lapse, insufficient coverage, a servicing transfer, a cancellation or termination, or a correction?

Resolve a claimed property-insurance gap

  • What proof of homeowners or hazard coverage do you need?
  • Where should my insurer send the declarations page, certificate, or other evidence?
  • Can you confirm receipt after it is sent?
  • If my policy was active, what dates does your system believe were uncovered or insufficient?

Document the conversation

Record the date and time, the number you dialed, the representative’s name or identification number, the conclusions, and any reference number. CFPB guidance on resolving mortgage payment and fee problems recommends detailed notes and requesting a reference number. Save the notice, statements, insurer correspondence, uploaded documents, and delivery receipts together.

Step 4: Compare the notice with independent records

Verification is strongest when three records agree: the servicer’s account, your insurance records, and your mortgage statements. Check each item rather than looking only for a matching company name.

Item to compare Records to use What a mismatch may mean
Borrower and property name/address Mortgage statement, loan closing records, insurance declarations Clerical error, wrong account, or impersonation
Insurance type Loan documents, insurer policy, servicer explanation PMI may have been confused with property insurance, or a different loan rule may apply
Policy or coverage period Declarations page, renewal notice, cancellation notice, servicer account history Coverage may have lapsed, proof may not have reached the servicer, or the wrong dates may be recorded
Amount Statement, escrow history, insurer premium, servicer calculation New premium, estimated force-placed cost, duplicate charge, or incorrect fee
Payment destination Current statement or verified servicer instructions Old payment address, transfer-related confusion, or scam attempt
Reason and requested action Notice, policy correspondence, transfer notice, account notes Missing documentation, servicing error, or a notice that does not belong to your loan

If your homeowners policy was active for the dates at issue, ask the insurer to confirm that in writing and send the requested evidence directly to the verified servicer. Ask both companies how they will reconcile duplicate or conflicting records. Keep the insurer’s policy number and contact information private when communicating with an unverified sender.

Know which kind of “mortgage insurance” the notice concerns

The term is not a single product. Identifying the category is essential because a charge that is normal for one loan may be wrong for another.

Coverage or charge What it generally does Questions to ask
Private mortgage insurance (PMI) Usually protects the lender against the risk of borrower default on a conventional loan. It is not homeowners coverage for damage to your property. Is PMI still required under my loan terms? What is the scheduled cancellation or termination date? Is the amount shown the regular premium?
FHA mortgage insurance Applies under FHA loan rules and does not follow the same cancellation framework as conventional PMI. Which FHA insurance charge is shown, and what rule in my loan or program governs it?
VA-related mortgage insurance or charges VA-backed loans have different program requirements and may involve charges that are not conventional PMI. What specific VA-related charge is this, and why is it appearing now?
Lender-paid mortgage insurance The lender pays the insurance cost, often through the loan’s pricing or interest structure, rather than billing the borrower as a separate PMI premium. Does my note or servicing record authorize a separate charge? Is this notice actually about property insurance instead?
Force-placed homeowners or hazard insurance The servicer obtains coverage when it lacks evidence that required property insurance is active or adequate. CFPB says this generally protects the lender rather than the homeowner. What coverage gap is alleged? What period and premium are involved? What proof will remove or prevent the charge?

Ask the servicer to name the coverage in its system rather than assuming that a notice using the words “mortgage insurance” refers to PMI.

Force-placed insurance: what a real notice should explain

A force-placed notice can be legitimate when the servicer does not have evidence of required property coverage. It may follow an insurer cancellation, an unpaid renewal premium, a policy that does not meet the loan’s requirements, a failed transfer of documents, or a servicing-record error.

CFPB guidance says a servicer must warn the borrower at least 45 days before charging for force-placed insurance. Federal model notices in Appendix MS-3 to Regulation X show notices that identify the coverage period, the premium or estimated annual premium, possible coverage limitations, and a way to contact the servicer. Those features are useful questions to ask, but the appearance of a letter or the presence of a model-form phrase does not prove that a particular notice is genuine.

Questions specific to a claimed lapse

  • What insurer and policy number does your system show, if any?
  • Which exact date was the policy allegedly cancelled, expired, or found insufficient?
  • What date would force-placed coverage begin, and what is the premium or estimated annual premium?
  • Does the policy cover only the lender’s interest, and what property risks or limits are excluded?
  • What evidence will you accept to cancel the placement or correct the record?

Force-placed coverage can cost more than coverage you purchase yourself and may protect only the lender’s interest. That is a reason to verify the record and submit proof promptly—not a reason to pay an unverified person or link.

PMI cancellation is a different issue

A notice about PMI should not be handled as though it were a homeowners-insurance lapse. CFPB guidance last reviewed August 28, 2026, describes the federal Homeowners Protection Act framework for many conventional loans.

  • An eligible borrower may generally request PMI cancellation when the scheduled principal balance reaches 80 percent of the home’s original value, subject to conditions such as a written request, a good payment history, current payments, and no junior lien.
  • PMI generally must be automatically terminated at the scheduled 78 percent point when the borrower is current.
  • These thresholds do not apply in the same way to FHA and VA loans. Ask the servicer about the specific program and note terms.
  • CFPB examination guidance says the servicer must notify the borrower in writing no later than 30 days after PMI is cancelled or terminated and should provide written grounds when a cancellation request is denied. That examination page was published in 2012, so use the current servicer response and governing loan documents for account-specific timing.

If a notice says PMI is being added, removed, or changed, ask the servicer for the amortization or balance calculation, the original-value figure used, and the contractual or statutory basis for the change. Do not infer that a PMI notice proves your homeowners policy is missing.

Warning signs of a payment scam

CFPB foreclosure-relief scam guidance identifies several high-risk demands. Its wording includes a request that a borrower “make your payments to someone other than your lender or servicer.” Treat that instruction as a stop-and-verify event.

  • The sender demands immediate payment or threatens an unusually short deadline without allowing normal account verification.
  • You are told to pay an individual, unrelated company, gift-card provider, wire recipient, cryptocurrency address, or bank account that does not match your verified servicer.
  • The notice tells you to stop making regular mortgage payments or to send the entire payment to a third party.
  • The sender refuses to identify the insurance type, coverage period, account record, or acceptable proof of coverage.
  • A web address, email domain, telephone number, or payment portal differs from the information on your trusted statements.
  • The notice contains an unfamiliar design, unusual spelling, or a request for sensitive information before the issuer will answer basic account questions.

An unfamiliar design by itself does not establish fraud. Servicers can change forms, vendors, addresses, and branding. The decisive test is whether the current servicer independently confirms the issuer, charge, dates, and payment route.

What to do when the servicer confirms the notice is real

Ask the verified servicer to state the next action clearly. Depending on the insurance type, that may mean paying through your normal mortgage channel, sending proof of active insurance, correcting a servicing record, or waiting for a written calculation.

  1. Confirm the amount, due date, coverage period, and payment destination during the verified contact.
  2. Ask whether the amount will be collected through escrow, added to the loan account, billed separately, or handled by the insurer.
  3. If proof of coverage is required, ask for the exact document and submission address. Have your insurer send it when possible, and request confirmation of receipt.
  4. Obtain a reference number or written confirmation of the instructions.
  5. Pay only through the confirmed channel and retain the confirmation, receipt, and account statement.

If the servicer confirms that its records show a lapse but your insurer says the policy was active, ask both parties to reconcile the same dates and policy number. A payment may not cure a record error, and sending money before the records are corrected could create a duplicate charge.

What to do when the notice is wrong or cannot be explained

If the servicer confirms an error, cannot explain the charge, or fails to correct conflicting information, use the servicer’s designated written address for a notice of error or a request for information. CFPB guidance says that special address may appear on your monthly statement, coupon book, notice, or the servicer’s website. If one is designated, use it rather than a general customer-service address.

Include enough detail to locate the account

  • Your name and property address
  • Loan or account number, using the identifying information requested by the servicer
  • The date and description of the disputed notice or charge
  • The insurance type and coverage dates you believe are correct
  • The correction or information you are requesting
  • Copies—not originals—of statements, declarations pages, notices, and prior correspondence

Keep a complete copy and proof of delivery. Send documents in a way that lets you confirm when the servicer received them. Continue your normal scheduled mortgage payments while the written request is pending, as CFPB guidance advises. Do not reduce or redirect the regular payment unless the verified servicer gives you account-specific written instructions.

For a consumer mortgage problem that remains unresolved, CFPB guidance says you can submit a complaint. A complaint can create an additional review channel, but it is not a substitute for checking the payment deadline, sending time-sensitive proof of insurance, or obtaining account-specific legal advice when your rights or foreclosure risk are involved.

How servicing transfers change the answer

A legitimate notice may look unfamiliar because the company managing the loan changed. The transfer can affect where payments go and which company held the insurance records on a particular date.

  • Identify the transfer date and the last date the old servicer accepted payments.
  • Confirm the first date the new servicer accepted payments and its official contact information.
  • Ask which company was responsible for receiving insurance evidence during the disputed period.
  • Compare the notice with both the transfer notice and statements from after the transfer.
  • Verify any new payment destination independently before using it.

If the notice concerns dates before the transfer, the new servicer may need to research records obtained from the old servicer. Ask for a reference number and submit your request in writing if the phone representative cannot resolve it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A practical decision framework

What the verified servicer says Your next step What not to do
The notice is genuine and the amount and payment route match the account Follow the confirmed instructions and retain proof of payment or submission. Do not use a different payment route printed on a later email or text without confirming it.
Your policy was active, but proof is missing from the account Ask your insurer to send the declarations page or required evidence to the verified servicer and confirm receipt. Do not assume that having a policy automatically means the servicer received proof.
The charge is force-placed insurance and you have no active required policy Ask about the 45-day warning, coverage period, cost, limitations, and how to obtain compliant coverage or submit proof. Do not buy coverage from an unverified caller or let the issue sit until the deadline passes.
The notice concerns PMI, but the amount or termination status is wrong Request the balance calculation, original-value figure, loan-program rule, and written correction process. Do not treat PMI as property insurance or stop your mortgage payment.
The servicer denies issuing the notice or cannot match the account Do not pay through the notice. Preserve it, report the suspected impersonation through appropriate channels, and monitor your mortgage account. Do not reply with account credentials or personal information.
The servicer confirms an error but does not correct it Send a notice of error or information request to the designated written address and keep delivery proof; consider a CFPB complaint if unresolved. Do not discard statements or rely only on an undocumented phone call.

Records to keep after the issue is resolved

  • The original notice and the date you received it
  • Mortgage statements before and after the notice
  • Insurance declarations pages, renewal notices, cancellation notices, and proof of payment
  • Servicing-transfer notices
  • Call notes, representative names or identifiers, reference numbers, and written confirmations
  • Copies of notices of error, information requests, attachments, and delivery receipts
  • Payment confirmations and any corrected statement showing the final account balance

These records help distinguish a real coverage lapse from a missing-document problem, a transfer error, a duplicate charge, or an impersonation attempt.

Questions that should be answered before you pay

  • Which company is the current servicer, and did I verify its contact information independently?
  • What exact insurance product or charge is involved?
  • What event and dates does the account record show?
  • Does the amount match the servicer’s calculation and my loan or policy records?
  • Is the payment destination the same as the verified servicer’s established channel?
  • If a property-policy lapse is alleged, what proof will the servicer accept and where should it be sent?
  • If servicing transferred, which company controlled the account during the disputed dates?
  • Have I kept making my normal mortgage payments while the issue is reviewed?

When professional help may be appropriate

General federal guidance cannot determine whether a particular letter, fee, deadline, policy lapse, or payment destination is valid. FHA and VA rules, lender-paid insurance terms, state law, the wording of your note, and your individual account history can change the result. If the issue is tied to a threatened foreclosure, a large disputed balance, an active coverage cancellation, or a servicing transfer that the companies will not resolve, consider a qualified housing counselor or attorney. Ask the verified servicer what documents it needs and keep communicating in writing while you obtain help.
