How to Use Home Equity to Pay for Senior Living Without Selling Your Home

Home equity can help pay for senior living, but the financing method must match where the homeowner will live. A home equity loan, HELOC or cash-out refinance can provide money while the owner remains responsible for monthly repayment. An FHA-insured HECM reverse mortgage can provide a lump sum, monthly advances or a line of credit without a required regular mortgage payment, but it generally becomes due when the last borrower (and any protected eligible non-borrowing spouse) dies, sells the home or stops using it as a principal residence. A permanent move to assisted living can therefore defeat the goal of keeping the home.

Before borrowing, estimate the care horizon and ongoing home costs, verify the senior-living setting your insurance will cover, and obtain HUD-approved reverse-mortgage counseling if a HECM is under consideration. The right choice depends on occupancy, income, existing debt, equity, health, expected duration of care and what you want heirs to receive.

Start with the occupancy question

Senior living is an umbrella term. Independent living, assisted living, memory care and a continuing-care retirement community can involve very different contracts and payment schedules. The National Institute on Aging explains that families commonly use a combination of personal funds, income, insurance and assistance to pay for long-term care (NIA, “Paying for Long-Term Care”). Home equity can be one part of that plan, but the first question is not “How much equity do we have?” It is “Will the homeowner continue to occupy this house as a principal residence?”

  • Staying at home while receiving services: A loan secured by the home may fit if income can cover required payments and property expenses.
  • Moving permanently to assisted living or a continuing-care community: A HECM’s principal-residence requirement may make the loan due, so it may not finance a plan to leave the house indefinitely.
  • Temporary rehabilitation or medical stay: The length of the absence and who remains in the home matter. A healthcare-facility stay lasting more than 12 consecutive months can trigger repayment rules when no co-borrower remains, subject to eligible non-borrowing-spouse protections.

Read the specific promissory note and security instruments before assuming that a move is allowed. A lender’s answer should be in writing, and a HUD-approved counselor can explain how the HECM rules apply to the household.

HECM reverse mortgage Lump sum, scheduled advances or a line of credit, subject to program limits. No required regular mortgage payment, but interest and fees accrue and increase the balance. The home must remain the borrower’s principal residence, and taxes, insurance and maintenance must be paid. Moving away can make the balance due. Will the borrower remain in the home? How long might care last? Can property charges be paid? How much equity should remain for heirs?
Home equity loan One lump-sum advance secured by the home. Requires scheduled repayment. If a first mortgage remains, this is generally a second mortgage paid alongside it. Can income support the payment during care? Is the rate fixed or adjustable? What is the total interest cost?
HELOC Repeated draws up to an approved credit limit. Usually has an adjustable rate, so payments can change. The line is secured by the home and must be repaid under its terms. Is spending phased or uncertain? What payment could result if rates rise? Can the borrower qualify on current income?
Refinance with equity extraction Replaces the existing mortgage and may provide cash, depending on the product and appraisal. Creates new rate, term and payment obligations. Extending the term or replacing a low-rate mortgage can raise total interest. How do the new payment, payoff date, closing costs and total interest compare with the current loan?
Other funds or a move Long-term-care insurance, savings, pension or investment income, assistance programs, downsizing or a sale. Coverage, eligibility and tax or estate consequences vary by policy and state. Selling or downsizing changes where the household lives and how much equity is available. Does existing coverage pay for this care setting? Would a smaller or sold home reduce risk more safely?

CFPB describes a reverse mortgage as borrowing against the home as security (CFPB, “What is a reverse mortgage?”). It describes a home equity loan as a specific amount borrowed against home equity and distinguishes it from a HELOC’s revolving credit structure (CFPB, “Home equity loan vs. HELOC”).

Using an FHA-insured HECM while keeping the home

What a HECM can provide

The Home Equity Conversion Mortgage (HECM) is the FHA-insured reverse-mortgage program for eligible older homeowners. Proceeds may be arranged as a lump sum, scheduled payments, a line of credit or a combination, within program limits. Unlike a conventional mortgage, the borrower generally does not make a required monthly principal-and-interest payment. Interest and fees are added to the balance instead, so the debt grows over time.

“No monthly mortgage payment” does not mean “no housing cost.” The borrower remains responsible for property taxes, homeowners insurance, utilities, maintenance and repairs. Failure to keep those charges current can put the loan in default and lead to foreclosure. Set aside a realistic reserve for these expenses before using proceeds for care.

Basic eligibility and proceeds

For an FHA-insured HECM, borrowers generally must be at least 62, occupy the property as a principal residence, own it outright or have enough equity to pay existing liens at closing, and demonstrate the resources needed to maintain property charges. HUD says the available amount depends on the youngest borrower’s or eligible non-borrowing spouse’s age, current interest rates and the applicable home-value limit. The home is collateral, but title remains with the borrower.

HECM counseling is required before closing. A counselor can review alternatives, explain obligations and help identify whether a spouse or another resident has protection under the program. Use HUD’s program information at HUD’s FHA Reverse Mortgage for Seniors (HECM) and confirm the counselor is HUD-approved.

Why a permanent move can make the HECM due

CFPB states that a reverse mortgage generally becomes due when the last surviving borrower or eligible non-borrowing spouse dies, sells the home or no longer lives there as a principal residence (CFPB, “When do I have to pay back a reverse mortgage loan?”). If assisted living is a permanent relocation, the borrower may no longer meet the occupancy condition even though the family intends to keep the property.

An absence for healthcare can also matter. When the borrower is in a healthcare facility for more than 12 consecutive months and no co-borrower remains in the home, the loan typically must be repaid or otherwise satisfied, subject to the program’s rules for an eligible non-borrowing spouse. The exact facts—who signed as borrower, who occupies the home, the dates of absence and the documents on file—control. Ask the servicer before the absence reaches a contractual deadline.

Couples and eligible non-borrowing spouses

For a couple, identify whether both partners are borrowers or whether one is an eligible non-borrowing spouse. A spouse who is not a borrower may have continued-occupancy protections if the HECM requirements are met, but those protections are not automatic for every person living in the home. Ask the counselor and lender to document how a move by one partner affects the other, what notice is required and what happens if the borrower dies or permanently relocates.

What happens to equity and heirs

The reverse-mortgage balance consists of advances plus accrued interest and fees. If the home is sold, the balance is paid from sale proceeds; the owner or heirs retain any remaining value after the debt and selling costs. Heirs can generally keep the home by paying the balance or sell it to repay the loan. If the balance exceeds the home’s value, the HECM process described by CFPB limits repayment to 95% of the appraised value when heirs choose to satisfy the loan that way; heirs do not automatically receive an unencumbered house. CFPB explains these choices at “Can heirs keep or sell a home with a reverse mortgage?”

When a home equity loan may fit

A home equity loan delivers a defined amount at closing, often useful when the first months of a care contract require a known deposit or entrance fee. The predictable lump sum can simplify budgeting, but repayment begins under the loan agreement. The payment competes with assisted-living charges, medications, taxes, insurance and everyday expenses. If the borrower has an existing first mortgage, the new loan is generally a second lien with its own payment and closing costs.

  • Request the annual percentage rate, fixed or adjustable-rate terms, amortization period, monthly payment and total scheduled interest.
  • Ask whether the lender can call the loan due if the home is no longer the borrower’s principal residence.
  • Stress-test the budget for a surviving spouse, a longer care period and reduced income.
  • Check prepayment penalties, late-fee rules and what happens if the property is sold.

When a HELOC may fit

A HELOC lets the borrower draw only what is needed, which can suit care that begins gradually or has uncertain duration. During the draw period, the borrower may be able to reuse available credit as principal is repaid. Rates are usually adjustable, and payments can rise when the index or lender margin changes. The line can also be frozen or reduced under the agreement, so do not treat the full limit as guaranteed lifetime funding.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Before opening a HELOC, obtain an example payment at today’s rate and at a materially higher rate. Confirm the length of the draw period, the repayment period, minimum draw, annual or inactivity fees, appraisal charges and the lender’s rules if the owner moves to a facility. A HELOC secured by the home can lead to foreclosure if required payments are missed.

When refinancing could be safer—or more expensive

A refinance can replace an existing mortgage and, if the new loan is large enough, release some equity as cash. It may be considered when the current mortgage has a high rate, the household needs a different term or a single payment is easier to manage. It can be a poor trade when the existing rate is low, closing costs are high or the new term extends debt into retirement.

Compare the current loan and proposed loan side by side:

  • interest rate and whether it is fixed or adjustable;
  • monthly principal-and-interest payment;
  • remaining term versus new term;
  • cash received after payoff and closing costs;
  • total interest if each loan is carried to maturity;
  • occupancy, sale and early-payoff provisions.

Do not evaluate a refinance solely by the cash received at closing. A lower initial payment can hide a longer repayment period or a higher lifetime cost.

A practical decision process

  1. Price the care plan. Obtain the community’s fee schedule, entrance or move-in charges, monthly services, medication management fees, memory-care supplements and refund rules. Ask how prices can change and what happens if the resident outlives the initial deposit.
  2. Map household cash flow. List Social Security, pensions, investments, wages, savings, existing mortgage payments, credit balances, insurance premiums, taxes, utilities, maintenance and the costs of an empty home.
  3. Define the occupancy plan. Decide whether the homeowner expects to remain in the house, return after rehabilitation, or leave permanently. For a HECM, verify the principal-residence rule and the 12-month healthcare-facility issue before applying.
  4. Check insurance and assistance first. Read long-term-care insurance definitions for covered settings, elimination periods, daily or monthly limits, inflation protection and benefit duration. The NIA notes that coverage and eligibility vary; obtain state-specific advice for Medicaid and estate-recovery rules rather than assuming another state’s program applies.
  5. Gather lien and title information. Identify the current mortgage payoff, home-equity lines, tax liens, judgments, co-owners and anyone whose occupancy could affect a loan.
  6. Request comparable offers. For each HECM, home equity loan, HELOC or refinance, record the rate, all upfront and ongoing fees, payment schedule, draw rules, servicing terms, occupancy requirements and effect on heirs.
  7. Complete required counseling and independent review. HECM counseling is mandatory. Consider an elder-law attorney or fiduciary financial professional when capacity, a spouse’s rights, Medicaid planning or estate objectives are involved.
  8. Build a property-charge reserve. Keep funds earmarked for taxes, insurance, repairs and maintenance. For a reverse mortgage, ask whether a set-aside is required or advisable and how it changes available proceeds.
  9. Plan the exit. Decide who will monitor notices, how a future sale would be handled, and where repayment funds would come from if the borrower dies or no longer occupies the home.

Questions to take to a counselor or lender

  • What event makes this loan due if the homeowner enters assisted living, memory care or a continuing-care community?
  • How is a temporary healthcare stay measured, and what notice must the borrower give?
  • Who is a borrower, co-borrower or eligible non-borrowing spouse on the documents?
  • What are the total upfront costs, ongoing servicing charges, interest-rate adjustments and projected balance under different care durations?
  • Which property expenses remain the borrower’s responsibility, and what happens after a missed tax or insurance payment?
  • Can the borrower make voluntary payments, and how would payments affect the balance or available line of credit?
  • What choices will heirs have if the balance is greater than the home’s value?
  • Will a trust, power of attorney, co-owner or resident who is not a spouse change eligibility or protections?

Common mistakes and how to avoid them

Treating “no mortgage payment” as free money

With a HECM, interest and fees increase the debt, while taxes, insurance and upkeep continue. Put those recurring costs in the care budget and assign someone to pay them if the homeowner is no longer managing bills.

Assuming the house can remain empty indefinitely

A vacant house or permanent move can violate principal-residence requirements. Get written guidance before moving belongings, signing a long-term residency contract or renting the property.

Borrowing the maximum amount

Taking more equity than needed increases interest and can reduce funds available to heirs or to cover a later care need. Compare a staged HELOC, scheduled HECM advances and a smaller lump sum with the actual payment calendar.

Ignoring the surviving spouse

Loan status and occupancy protections can change after the borrower dies or moves. Confirm documentation and contingency steps before closing, not after a health crisis.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Using national assumptions for public benefits

Medicaid eligibility, estate recovery and assistance programs are state-specific and fact-dependent. Obtain guidance in the state where the care and property are located.

Choosing a loan by rate alone

Fees, rate adjustments, payment duration, draw restrictions, due-and-payable triggers and inheritance effects can outweigh a small difference in the advertised rate. Compare the complete documents and an amortization illustration.

Alternatives that may reduce the amount borrowed

Home equity is not the only source of funding. Review long-term-care insurance, annuities or pension income, taxable and retirement-account withdrawals, family contributions documented in writing, state or local assistance, and whether a smaller home would lower taxes, insurance and maintenance. A sale or downsizing can provide more certain liquidity but changes housing and estate plans. A reverse-mortgage alternative guide from CFPB discusses considering other ways to meet financial needs (CFPB, “Consider alternatives”), while its home-equity guide outlines trade-offs among borrowing choices (CFPB, “Using home equity to meet financial needs”).

Do not assume that long-term-care insurance pays every senior-living bill. Confirm whether the policy covers assisted living, memory care, a continuing-care facility, home care or only certain levels of licensed care, and check benefit limits and waiting periods.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Bottom line for keeping the home

If the homeowner will remain in the house as a principal residence and can reliably pay property charges, a HECM, HELOC, home equity loan or refinance may convert equity into care funding without an immediate sale. If the homeowner is moving permanently to assisted living, scrutinize a HECM first because the move may make the loan due; a conventional equity loan or HELOC still requires repayment and may be unaffordable once care costs begin. Compare the full household plan—including insurance, public benefits, downsizing and a possible sale—before pledging the home.
