How to Use Form SSA-44 to Lower Medicare Premiums After a Retirement Income Drop

If you retire, reduce your work hours, or lose pension income and your household income falls, Form SSA-44 lets you ask Social Security to recalculate your Medicare income-related monthly adjustment amount (IRMAA). An approved request can reduce the surcharges added to both Medicare Part B and Part D premiums.

You must prove two things: the qualifying life-changing event and the resulting reduction in income. The current form is SSA-44 (12-2025). You can submit it online, by mail or fax, or with help from a Social Security appointment. A newer-income determination generally starts January 1 of the year you request it, although enrollment timing and a reduction that begins the following year can change the effective date.

What Form SSA-44 changes

IRMAA is an income-based surcharge on Medicare Part B medical-insurance premiums and Part D prescription-drug premiums. Social Security generally calculates it from your modified adjusted gross income (MAGI), using federal tax information from two years earlier. If your income was high while you were working but dropped after retirement, that older return can leave you paying a surcharge that no longer reflects your finances.

SSA-44 is the official request for a new initial determination based on a qualifying life-changing event. It is not a general request to negotiate a premium and it does not erase the standard Medicare premium. Instead, Social Security reviews your more recent or expected lower income and may reduce or eliminate the IRMAA amounts.

The form can affect both parts of the surcharge. A favorable decision may lower the Part B adjustment, the Part D adjustment, or both, depending on the income tier and the year used. It does not change your underlying Medicare enrollment, deductible, coinsurance, or the standard Part B premium itself.

For 2026, the standard Part B premium is $202.90, according to Social Security’s 2026 figures. For an individual, MAGI above $109,000 and up to $137,000 adds $81.20 to Part B and $14.50 to prescription-drug coverage. For married couples filing jointly, the listed IRMAA thresholds begin above $218,000. These are 2026 amounts; your own surcharge depends on filing status, MAGI, enrollment, and the applicable year’s tables. A lower SSA-44 determination changes the income-related addition, not necessarily every Medicare cost you pay.

When retirement qualifies as a life-changing event

The retirement-related categories on SSA-44 are Work Stoppage, Work Reduction, and Loss of Pension Income. Social Security’s Medicare guidance also recognizes stopping work or reducing hours, loss of pension income, certain disaster-related losses of income-producing property, and qualifying employer settlement payments.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Work stoppage

Use this category when you stopped working, such as retiring from a job. The date is the date the work ended, not necessarily the date you first began receiving Social Security or Medicare. If you retired from one job but still receive wages from another, describe the actual work stoppage and include the remaining earnings in your income estimate.

Work reduction

Use it when you continued working but reduced hours, shifts, or compensation enough to lower your household income. A partial reduction must still be tied to the lower MAGI you report. A schedule change, reduced salary, or permanent move from full-time to part-time work is easier to document when the employer confirms the effective date and new pay arrangement.

Loss of pension income

This category can apply when a pension payment ends or is materially reduced. Keep the pension administrator’s notice showing the change and its effective date. If other pensions, annuities, or retirement distributions continue, include them in the AGI estimate rather than treating the ended payment as your only income.

Other listed events

The form also lists marriage, divorce or annulment, death of a spouse, loss of income-producing property, and an employer settlement payment. Those events qualify only when they cause the income reduction that you document. For example, a death-of-spouse claim should connect the date of death with the loss of the spouse’s income, while a property-loss claim should show how the income-producing asset stopped generating income.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Retirement by itself is not enough. Social Security requires evidence of the event and evidence that your income fell (or will fall) because of it. A claim that simply says “I am retired” without an event date or lower-income figures can lead to a request for more information.

A quick eligibility check

Before completing SSA-44, confirm that all of these statements fit your situation:

  • You received an IRMAA notice, or your Medicare premium is being increased because of income.
  • A listed life-changing event occurred or is expected to occur.
  • The event caused, or is expected to cause, a meaningful reduction in household MAGI.
  • You can identify the tax year and filing status for the lower-income figures.
  • You can provide event evidence and income evidence, or a supportable estimate when the return is not yet available.

If the issue is that Social Security used the wrong filing status, tax return, or other fact and there was no qualifying event, an appeal may be the better route. If retirement caused the change, describe it as a new initial-determination request and provide the evidence.

Information and evidence to gather first

Collect documents before completing the form so the dates and tax figures agree across your submission. Organize the material in the same order as the form; that makes it easier for an office representative to see both the event and its financial effect.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Event proof: an employer letter confirming retirement or reduced work, a retirement notice, a pension administrator’s statement, or another document showing what happened and when.
  • Income proof: a signed federal tax return for the relevant year, wage or pension statements, and records supporting your estimate if the return has not yet been filed.
  • Tax details: adjusted gross income (AGI), tax-exempt interest, the tax year, and your federal filing status.
  • Identity and Medicare details: the information shown on your Social Security and Medicare records, so the request can be matched to your account.
  • Change-of-pay details: final pay information, severance, bonuses, pension changes, and known retirement distributions that could affect the reported year.

SSA says it needs documentation verifying both the life-changing event and the income reduction. POMS guidance allows an estimated MAGI statement under its procedures when a tax return is not available. Estimates should be supportable and updated if your actual income changes. A practical estimate lists each expected source, the amount already received, the amount expected for the rest of the year, and any tax-exempt interest.

How to complete SSA-44

Step 1: Record the life-changing event

Mark the event that applies and enter the date. For a retirement, select Work Stoppage; for reduced hours, select Work Reduction; for an ended or reduced pension, select Loss of Pension Income. If more than one event applies, identify the event most directly responsible for the income decline and explain the connection in the space provided.

Use a specific date, such as the last day worked or the date the pension reduction took effect. If the event is scheduled but has not happened, state the expected date and use the anticipated-income section. Do not use the date you received an IRMAA notice as the event date unless that is also the date the qualifying event occurred.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 2: Report income that has already fallen

Use this section when the lower-income year has occurred or you can document current figures. Enter:

  • the tax year;
  • your AGI;
  • tax-exempt interest; and
  • your federal filing status.

AGI plus tax-exempt interest is the MAGI figure Social Security uses for IRMAA purposes. Do not substitute taxable income for AGI, and do not omit tax-exempt interest simply because it is not taxed on your return. Use the AGI line from the federal return for the stated year, then add tax-exempt interest as the form directs.

If you filed jointly, use the joint return’s figures and filing status. If you filed separately or expect to change status, identify the status that applies to the reported or anticipated year and explain any uncertainty. A filing-status mismatch can place the request in the wrong IRMAA threshold.

Step 3: Report an anticipated lower-income year

If the retirement or reduction will occur later, complete the anticipated-income section. Enter your estimated AGI, estimated tax-exempt interest, the tax year, and the filing status you expect to use. Base the estimate on known wages, severance, bonuses, pensions, investment income, and other taxable or tax-exempt amounts for that year.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Separate one-time items from recurring income. A large severance payment, bonus, or retirement-account distribution can keep MAGI higher than expected even after you stop working. Conversely, do not count wages for months after a documented work stoppage. Keep a worksheet showing how you reached the estimate so you can explain it if Social Security asks.

Step 4: Attach evidence and sign

Attach copies, not irreplaceable originals, unless Social Security specifically instructs otherwise. Include proof of the event and the income figures. Sign and date the form. Keep a complete copy of the signed form and every attachment.

Check that the name, Social Security information, tax year, event date, filing status, and amounts are consistent throughout the packet. An unsigned form, missing tax year, or attachment that does not identify whose income it represents can delay review.

Ways to submit the request

Method Convenience Questions and guidance Document handling Access outside the United States
Online Usually the quickest way to start if you can use Social Security’s online sign-in route. Limited while completing the form; you may need a later contact if evidence is unclear. Upload or provide documents as the online instructions require, and save confirmation records. Useful for people abroad who can access the online system, subject to account and document limitations.
Mail or fax Works without an online account. No live assistance during completion. Make legible copies, include all pages, and retain fax confirmation or mailing evidence. May be practical internationally, but confirm how Social Security wants documents sent from your location.
Social Security appointment Slower to arrange but useful for complicated income or event histories. You can ask questions and clarify which evidence applies. Bring the completed form and organized copies; the office may request additional material. Call first to learn what service is available from outside the United States.

Social Security publishes an online sign-in route and accepts a completed SSA-44 with evidence by fax or mail to a Social Security office. You can schedule an appointment at 1-800-772-1213; TTY users can call 1-800-325-0778. POMS describes SSA-44 as optional when requesting a new initial determination, so you can begin by contacting Social Security if you need help identifying the correct filing route.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Choose the channel that matches your needs. Online filing can reduce postage and creates an electronic submission trail, but you still must preserve confirmation and copies of uploads. Mail or fax may be preferable when you already have a complete paper packet; use legible pages and retain delivery or transmission evidence. An appointment is useful when several income sources, multiple events, or an anticipated tax year make the form difficult to interpret. The appointment does not remove the need for written evidence.

When a lower premium can begin

Under 20 CFR § 418.1201, a determination using a more recent tax year with significantly reduced MAGI is generally effective January 1 of the year you make the request. For example, a request made during 2026 based on a qualifying reduction and lower 2026 income would generally be effective January 1, 2026, if the other requirements are met.

If the qualifying reduction will not happen until the following year, the determination generally begins January 1 of that following year. For example, if a scheduled work stoppage and income reduction will not occur until 2027, the newer-income determination generally does not begin in 2026 merely because you filed early. Part B enrollment timing can produce a different start date, so a person enrolling after the request should ask Social Security how the rule applies to the enrollment month.

The effective date is not the same as the processing date. Social Security does not promise a universal processing time or a fixed refund schedule. Continue paying the billed premium while the request is pending unless Social Security tells you otherwise. If the decision lowers amounts already collected, ask how any credit or refund will be handled; do not assume it will arrive on a particular date.

SSA-44 versus an appeal

Choose the procedure that matches the problem:

Situation Appropriate path What you are asking Social Security to do
Your income fell because you retired, reduced work, lost a pension, or experienced another listed event. SSA-44 new initial determination Recalculate IRMAA using newer or expected lower income.
You believe an existing IRMAA decision is wrong even though there is no qualifying life-changing event, or you disagree with how Social Security applied the rules. Appeal Review and overturn or correct the existing determination.

POMS instructs staff to treat an alleged life-changing event as a request for a new initial determination unless the beneficiary insists on an appeal. Explain that your income changed because of retirement and provide the supporting evidence. If your disagreement concerns a separate factual or legal error, state that clearly. Filing the wrong type of request can obscure the reason your income changed, so identify the retirement event in your cover explanation even when you also believe the notice contains another error.

How Social Security generally determines the income year

Social Security generally receives the most recent federal tax return supplied by the Internal Revenue Service. For 2026 determinations, that commonly means tax-year 2024 data supplied during 2025. That timing explains why a recent retirement may not appear in the return that triggered your surcharge and why SSA-44 exists for qualifying changes.

Use the tax year requested on the form, not simply the year you filed the return. If you file jointly, report the joint filing status and the applicable joint MAGI. If you expect to file separately or change filing status, explain the expected status and retain records supporting the estimate.

Do not assume that the year of your retirement is automatically the year Social Security will use. The relevant year is the more recent completed or anticipated tax year supported by the event and the income evidence. A retirement in late 2026 may leave substantial 2026 wages, while the full reduction may be visible in 2027. State the timing plainly and attach figures that match it.

Common filing mistakes and fixes

Submitting only a retirement letter

A retirement letter proves the event but not the resulting MAGI. Add the tax return or a well-supported estimate showing the lower income.

Reporting taxable income instead of AGI

Use AGI from the federal return and add tax-exempt interest. Taxable income after deductions is a different number.

Using the wrong tax year

Identify whether you are reporting an already completed lower-income year or estimating the year in which the reduction will occur. Enter that year consistently in the form and attachments.

Forgetting filing status

IRMAA thresholds differ for individual and married-filing-jointly taxpayers. Select the status for the tax year you report or expect to use.

Free tools Windows power users keep installed

Ignoring remaining income

Retirement does not necessarily eliminate all income. Include continued wages, pension or annuity payments, investment income, severance, and other amounts that affect AGI. Omitting them can make an estimate unreliable.

Sending incomplete copies

Assuming approval is automatic

Social Security must verify both the event and the lower income. Be prepared to answer a follow-up request or provide a corrected estimate.

Expecting an immediate premium change

Examples of when to file

Retired during the current year

You stop working in June and expect wages to be much lower for the year. File using Work Stoppage, provide the employer’s retirement confirmation, and report the year’s estimated AGI and tax-exempt interest. Include wages earned before June and any severance, bonus, pension, or investment income expected later in the year. If the estimate is for the following tax year because the reduction will not occur until then, the start date generally shifts to January 1 of that following year.

Moved from full-time to part-time work

You continue working but reduce hours permanently. Select Work Reduction, document the effective date and new schedule or pay, and show how the change lowers the reported MAGI. If the employer changes compensation again, retain the updated statements and be ready to revise the estimate.

Pension payment ended

A pension administrator stops payments after a plan event. Select Loss of Pension Income, attach the administrator’s notice, and include the lower-income tax figures or a supportable estimate. Report other retirement income that continues so the estimate reflects household MAGI rather than only the lost payment.

Spouse’s income ended

A spouse dies and the household loses the spouse’s wages or pension. The death-of-spouse category may apply. Attach the death certificate or other requested proof, identify the date income ended, and calculate the filing status and MAGI expected for the relevant tax year.

What to do after sending SSA-44

  1. Save the submitted form, attachments, confirmation number, fax receipt, or mailing proof.
  2. Watch for a Social Security request for clarification or additional evidence.
  3. Continue checking your Medicare premium notices while the decision is pending.
  4. Review the written determination when it arrives, including the effective date and both Part B and Part D amounts.
  5. Compare the decision with the tax year, AGI, tax-exempt interest, and filing status you reported.
  6. If the decision does not address the qualifying event or contains an error, contact Social Security promptly and ask whether a correction or appeal is appropriate.

Frequently asked questions

Can I use SSA-44 if I simply stopped working?

Yes, a work stoppage such as retirement is a listed event, but you must also show that it caused a lower MAGI. Submit the work-end evidence and the lower or expected income figures.

Does SSA-44 lower both Part B and Part D?

It can. IRMAA applies to both programs, and an approved newer-income determination can reduce the applicable surcharges for Part B and prescription-drug coverage. Review both amounts in the written decision.

What if I have not filed the lower-income tax return?

Provide a supportable estimated MAGI statement under Social Security procedures, along with evidence of the event and the income used in the estimate. Keep records and update the figures if circumstances change.

What numbers belong on the form?

Enter AGI, tax-exempt interest, the relevant tax year, and filing status. AGI plus tax-exempt interest is the MAGI figure used for IRMAA; taxable income is not a substitute.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can I file before retirement occurs?

If the work stoppage or other reduction is scheduled for a later date, use the anticipated-income section and identify the expected event date. The effective date generally follows the year in which the qualifying reduction occurs.

Is SSA-44 the same as an appeal?

No. SSA-44 requests a new initial determination because a qualifying life-changing event reduced income. An appeal challenges an existing determination. Social Security guidance says staff should treat an alleged life-changing event as a new initial-determination request unless you insist on an appeal.

How fast will my premium change?

There is no universal processing-time or refund promise. Keep paying billed amounts unless instructed otherwise, monitor notices, and ask Social Security how any credit or refund will be handled after a favorable decision.

Can someone outside the United States submit the form?

Online access, mail, fax, and appointment availability can differ outside the United States. Confirm the appropriate document-delivery method with Social Security and retain proof of submission.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Key takeaway

Form SSA-44 is the correct route when retirement, reduced work, or lost pension income lowers the MAGI used for your Medicare IRMAA. File with proof of both the event and the income drop, report AGI plus tax-exempt interest for the correct year and filing status, and preserve every submission record. The reduction usually applies from January 1 of the request year, but future-year reductions and Part B enrollment can alter that date.

Frequently Asked Questions

Can I use Form SSA-44 after I retire?

Yes. Select Work Stoppage, document the date work ended, and show the resulting lower or expected MAGI.

What proof does Social Security need?

You need evidence of both the life-changing event and the income reduction, such as an employer or pension letter plus a signed tax return or supportable estimate.

Will SSA-44 reduce Part D premiums too?

An approved newer-income determination can reduce IRMAA surcharges for both Part B and Part D.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When does the lower premium start?

Generally January 1 of the request year; if the reduction begins the following year, the determination generally starts January 1 of that following year. Enrollment timing can affect Part B.

What if my tax return is not filed yet?

Social Security procedures may allow an estimated MAGI statement supported by income records and event evidence.

Should I appeal instead?

Use SSA-44 for a qualifying income-reducing event. Use an appeal for disagreement with an existing determination unrelated to such an event.

The Bottom Line

Retirement-related income drops can make an older IRMAA calculation inaccurate. Use SSA-44, document the event and lower MAGI, and retain every submission and response from Social Security.
