How to read UnitedHealth Group’s earnings report and key financial metrics

By TheFinanceBase Team 10 min read

To read a UnitedHealth Group earnings report, work from the consolidated income statement to medical-cost trends, margins, reported versus adjusted earnings, cash flow, the balance sheet, business segments and management’s outlook. Revenue growth is only the starting point: it does not tell you whether costs grew faster, whether profit improved, or how much cash the business generated.

The latest results covered in this guide are UnitedHealth Group’s second quarter of 2026 release dated July 16, 2026. The company scheduled its third-quarter 2026 results for before market open on October 13, 2026, but that release is not included in the source set for these figures. Check the company’s investor-relations filing index for the newest release before using any number as current.

1. Identify exactly which UnitedHealth report you are reading

Start by recording the period, release date and document type. UnitedHealth Group publishes earnings releases, detailed quarterly reports, annual reports, proxy materials and Securities and Exchange Commission filings. An earnings release is a summary; Form 10-Q and Form 10-K filings provide the accounting detail, definitions and reconciliations behind the headline numbers.

  • Period: Note whether the figures cover a quarter, six months or the full fiscal year.
  • Release date: Results are dated when the company publishes them, not when the underlying services were provided.
  • Document: Distinguish an earnings release from Form 10-Q or Form 10-K. The filing normally contains fuller footnotes and segment disclosures.
  • Basis: Mark every number as reported (GAAP) or adjusted (company-defined non-GAAP).
  • Comparison: Use the same period a year earlier for a year-over-year comparison; use the immediately preceding quarter only when seasonality and timing make that comparison meaningful.

For this guide, the evidenced periods are full-year 2025 and the second quarter of 2026. Do not infer third-quarter 2026 figures from the October 13 schedule or from management’s earlier guidance.

2. Read the consolidated income statement before the headline

The income statement shows how revenue turns into operating earnings and then net income. Read each line in sequence rather than jumping directly to earnings per share.

Revenue: the top line, not a profit measure

Revenue is the amount UnitedHealth reports from its businesses during the period. For fiscal 2025, total revenue was $447.567 billion, up 12% from 2024. That increase says the company sold or administered more services, collected more premiums or otherwise recorded more revenue; it does not establish that profit, margins or cash flow increased.

Revenue can rise while earnings fall when medical costs, administrative expenses, restructuring charges, interest, taxes or other costs rise faster. In 2025, the company reported lower operating earnings and earnings per share despite the 12% revenue increase. Always read revenue alongside the cost lines and the earnings measures below it.

Medical costs and other operating costs

Medical costs are especially important in the insurance businesses because they represent benefits paid or expected to be paid for members’ care. Other operating costs include the expenses required to administer plans and run the broader company. A useful first check is whether each cost category grew more slowly or more quickly than revenue.

Do not assume that a lower medical care ratio means every cost fell. A ratio can improve because pricing, benefit design or member mix changed, while technology, operations or community-support investments increase other operating expenses.

Operating earnings, non-operating items and net income

Operating earnings measure profit from the company’s operating activities before interest and other non-operating items and taxes. For 2025, operating earnings were $18.964 billion. After non-operating items and taxes, net earnings attributable to common shareholders were $12.056 billion.

Those are different layers of profitability. If operating earnings improve but net income does not, inspect interest expense, investment results, taxes or other items below operating income. Conversely, a favorable non-operating item can lift net income without indicating stronger underlying operations.

Diluted EPS

Diluted earnings per share divides earnings attributable to common shareholders by the diluted share count, which reflects potential dilution from instruments such as employee awards. UnitedHealth reported $13.23 diluted EPS for 2025. EPS is useful for relating earnings to one share, but it inherits every limitation of the income statement and can also be affected by changes in the share count.

3. Understand UnitedHealth’s medical care ratio (MCR)

UnitedHealth defines its medical care ratio (MCR) as medical costs divided by premium revenue. It is an insurance-cost ratio: it measures how much of premium revenue was consumed by medical costs. It is not calculated using all consolidated revenue and it is not the group’s consolidated profit margin.

How to interpret a higher or lower MCR

  • A higher MCR means a larger share of premium revenue went to medical costs, all else equal, leaving less of that premium revenue to cover operating expenses and profit.
  • A lower MCR means medical costs consumed a smaller share of premium revenue, all else equal.
  • The ratio alone does not identify the cause. Pricing, benefit design, member mix, utilization, medical-cost trend, contract performance and reserve development can all matter.

For 2025, reported MCR was 89.1%, compared with 85.5% in 2024. The annual filing discusses Medicare funding reductions, elevated medical-cost trend, member mix and losses on value-based-care contracts, lower favorable development, Medicare Part D and market morbidity among the factors affecting the change, along with offsets. Use the company’s stated drivers rather than assigning a cause from the ratio alone.

The 2025 release also presented an 88.9% adjusted MCR. Reported MCR included a 20-basis-point negative impact from loss contracts. Adjusted MCR is a company-defined presentation; read the reconciliation to see precisely what was removed.

Q2 2026 MCR and reserve development

For the second quarter of 2026, MCR was 86.7%, versus 89.4% in the second quarter of 2025. The release attributed the year-over-year decrease to benefit design and pricing discipline, member mix and medical-cost management initiatives.

The quarter also included $860 million of net favorable prior-period medical development, most related to 2026 dates of service. Favorable development means previously recorded medical-cost reserves were more than sufficient and were reduced, benefiting the current period. Treat that item as context for the reported ratio; do not create an “adjusted MCR” by dividing $860 million by consolidated revenue. MCR uses premium revenue as its denominator and requires the company’s own calculation and reconciliation.

4. Keep MCR, operating cost ratio and profit margins separate

Several percentages appear in the report, but they answer different questions. The 2025 Form 10-K reported an operating cost ratio of 13.3%, an operating margin of 4.2% and a net earnings margin of 2.7%.

Measure What it relates 2025 reported figure What it does not mean
Medical care ratio Medical costs ÷ premium revenue 89.1% Not consolidated profit margin
Operating cost ratio Operating costs relative to the company’s reported revenue base 13.3% Not medical-cost share of premiums
Operating margin Operating earnings relative to consolidated revenue 4.2% Not the amount left after taxes and non-operating items
Net earnings margin Net earnings attributable to shareholders relative to consolidated revenue 2.7% Not 100% minus MCR

It is incorrect to calculate UnitedHealth’s profit margin as 100% minus MCR. MCR uses premium revenue and medical costs, while consolidated results include Optum, other business lines, different revenue types and expenses outside medical benefits.

In Q2 2026, the company reported $8.0 billion of operating earnings, a 4.9% net margin and a 12.7% operating cost ratio, compared with 12.3% in the year-ago quarter. Management said the increase in the operating cost ratio reflected targeted investments in technology, operations and community support. That explanation is why a ratio should be read with the accompanying narrative, not in isolation.

5. Compare reported EPS with adjusted EPS

Reported EPS is the GAAP result attributable to common shareholders. Adjusted EPS is a non-GAAP, company-defined measure intended to remove specified items that management believes make period-to-period operating performance easier to analyze. Neither should be ignored.

What the 2025 figures show

For 2025, UnitedHealth reported $13.23 reported diluted EPS and $16.35 adjusted EPS. The release disclosed a $1.78 diluted-EPS impact from specified portfolio-divestiture, restructuring and other items.

Read the reconciliation next to the adjusted figure. It identifies each exclusion and shows its tax and per-share effect. An adjustment is not automatically irrelevant or non-recurring: restructuring can happen again, divestitures can alter future results, and a recurring type of charge may still be economically important.

Q2 2026 EPS comparison

In Q2 2026, reported EPS was $6.04 and adjusted EPS was $6.38. Present the reported number first, then use the release’s reconciliation to explain the difference. When comparing quarters, compare reported with reported and adjusted with adjusted; mixing the two bases can create a false improvement or decline.

6. Check operating cash flow and the balance sheet

Accounting earnings and cash generated are related but distinct. Operating cash flow starts with accounting results and adjusts for non-cash items and changes in operating assets and liabilities. It helps you judge whether the business is producing cash from its operations, but a single quarter is not a reliable annual run rate.

Cash-flow figures to record

  • Full-year 2025: operating cash flow was $19.7 billion, or 1.5 times net income.
  • Q2 2026: operating cash flow was $11.1 billion, or 1.9 times net income.

The 1.9-times figure is a second-quarter relationship, not a promise that cash conversion will remain 1.9 times net income for the year. Investigate large working-capital movements, reserve changes and timing effects in the cash-flow statement and footnotes.

Debt and capital

At June 30, 2026, UnitedHealth reported a 41.2% debt-to-capital ratio. Record the date with the percentage: balance-sheet ratios change as debt, cash, equity and earnings change. Compare the ratio with prior periods and with management’s stated capital priorities rather than treating one quarter-end figure as a permanent level.

7. Use UnitedHealthcare and Optum segments to explain the consolidated result

UnitedHealth Group reports through UnitedHealthcare and Optum, with Optum containing distinct businesses. Segment revenue, operating earnings and margin help show which part of the company is driving a consolidated change.

For Q2 2026, UnitedHealthcare reported $86.0 billion of revenue, $3.9 billion of operating earnings and a 4.6% operating margin. Compare those figures with the segment’s year-ago results in the release, then examine Optum’s businesses and the corporate or elimination items that reconcile segment results to the group total.

Segment mix matters when interpreting MCR and margins. A change in the proportion of insurance revenue, health-services revenue or business with different cost structures can move consolidated percentages even when an individual operation’s economics are unchanged.

8. Treat management guidance as a dated outlook

Guidance is management’s expectation at a particular date, not a realized result. As of the July 16, 2026 Q2 release, the full-year 2026 outlook was:

Outlook measure Q2 2026 release guidance How to use it
Reported EPS $18.45–$18.95 Compare later reported EPS with the range; do not call the midpoint an actual result.
Adjusted EPS $19.50–$20.00 Compare with adjusted results on the same non-GAAP basis.
Operating cash flow Approximately $24 billion Check the eventual full-year cash-flow statement against this dated expectation.

When a newer release is available, replace these ranges with the latest guidance and note whether management raised, lowered or reaffirmed it. A guidance change can reflect new medical-cost experience, policy or funding changes, acquisitions, divestitures, reserve development or other assumptions; read the explanation rather than treating the direction alone as a verdict.

9. A practical reading sequence you can repeat each quarter

  1. Open the newest investor-relations filing index. Confirm the release date, reporting period and whether a newer Form 10-Q, Form 10-K or earnings release has superseded the figures used here.
  2. Write down consolidated revenue, operating earnings, net earnings and diluted EPS. Record the year-ago values beside them and calculate the direction of change without treating revenue growth as profit growth.
  3. Read medical costs and the MCR definition. Confirm that the numerator is medical costs and the denominator is premium revenue. Note the company’s explanation for utilization, pricing, benefit design, member mix and reserve development.
  4. Record operating cost ratio, operating margin and net margin separately. Check the denominators and do not convert one into another.
  5. Read the reported-to-adjusted EPS reconciliation. List every exclusion, its per-share effect and whether management describes it as unusual, ongoing or related to a strategic action.
  6. Review segment tables. Compare UnitedHealthcare and Optum revenue, operating earnings and margins, then reconcile the segments to consolidated results.
  7. Read operating cash flow and the balance sheet. Note cash generated, its relationship to net income, debt-to-capital and any material changes in working capital or reserves.
  8. Finish with guidance and management commentary. Label the outlook with its release date and compare subsequent actual results with the same reported or adjusted basis.

10. Common interpretation mistakes and how to avoid them

Calling revenue growth profit growth

Revenue rose 12% in 2025, but operating earnings and EPS were lower. The fix is to read the income statement from revenue through net income and identify which costs changed faster than sales.

Calling MCR a company-wide margin

MCR applies to medical costs and premium revenue. It does not include every revenue stream or expense in UnitedHealth Group. Use operating and net margins for consolidated profitability.

Using adjusted EPS without the reconciliation

Adjusted EPS is not a second GAAP result. Read the named exclusions, preserve the reported EPS in your notes and do not assume adjustments are permanently non-recurring.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Annualizing one quarter’s cash flow

Q2 2026 operating cash flow was $11.1 billion, but seasonality, reserve movements and working-capital timing can make quarters uneven. Wait for the year-to-date and full-year statements before forming an annual cash-generation view.

Ignoring favorable or unfavorable reserve development

The $860 million of net favorable prior-period medical development in Q2 2026 helped the reported period. Identify whether a change comes from current-period medical experience or the release of earlier reserves.

Comparing unlike periods or bases

Compare Q2 with Q2 for a year-over-year view, and compare reported with reported or adjusted with adjusted. Sequential comparisons can be useful, but insurance utilization and policy timing make them more seasonal and require explanation.

11. What a complete earnings note should contain

After reading the release, your notes should answer these questions without relying on the headline:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • What period and release date do the figures cover?
  • Did revenue, operating earnings, net income and diluted EPS move in the same direction?
  • What happened to MCR, and what drivers did management identify?
  • How did operating costs, operating margin and net margin change?
  • What is the difference between reported and adjusted EPS, and which exclusions explain it?
  • Which segment produced the change in revenue or operating earnings?
  • Did operating cash flow support accounting earnings, and what happened to debt-to-capital?
  • What guidance was in effect on the release date, and has a newer release changed it?

Answering those questions produces a more reliable picture than repeating a single ratio or the first sentence of the press release. UnitedHealth’s own filings and reconciliations are the authoritative place to verify definitions and update every figure when the next report arrives.
