How to Protect Retirement Income From Inflation and Rising Healthcare Costs

Protecting retirement income takes a coordinated plan, not one product. Match essential spending with reliable income, keep part of the portfolio tied to long-term inflation and time horizon, budget Medicare’s premiums and cost sharing separately, and create a distinct funding plan for long-term custodial care that Medicare generally does not cover.

Why inflation and healthcare costs require one retirement plan

Inflation reduces what each dollar buys, while healthcare expenses can rise unevenly and arrive as premiums, deductibles, coinsurance, prescriptions, insurance premiums, or years of assistance with daily activities. A portfolio that handles ordinary living costs may still be exposed to a large care bill. Conversely, an insurance purchase designed for care does not automatically preserve spending power for food, housing, utilities, or transportation.

The practical objective is to give each risk a job in the plan:

  • Reliable income covers a core level of recurring expenses and addresses the possibility of living longer than expected.
  • Inflation-aware assets help preserve purchasing power over a retirement that may last decades.
  • Healthcare reserves and coverage choices handle Medicare premiums, deductibles, plan cost sharing, and expenses Medicare excludes.
  • Flexible spending and periodic rebalancing keep withdrawals aligned with changing markets, prices, health, and household needs.

No universal stock-and-bond mix, withdrawal percentage, Medicare plan, or long-term-care policy fits every household. Your location, tax situation, health, assets, income sources, and tolerance for market losses determine the appropriate design.

1. Map spending before choosing investments

Start with a written spending and income map. Separate expenses you must pay from those you can delay, reduce, or skip in a difficult market year. Then compare those expenses with income that is expected to continue.

Classify expenses by flexibility and inflation exposure

Expense group Examples Planning question
Essential recurring costs Housing, food, utilities, transportation, insurance and basic healthcare Which income sources can cover these bills without relying on a favorable stock market?
Flexible lifestyle costs Travel, gifts, dining, hobbies and large discretionary purchases What could be postponed if investments fall or prices rise faster than expected?
Irregular reserves Home repairs, vehicle replacement, family support and deductibles Where will a one-time bill come from without disrupting essential income?
Healthcare and care costs Medicare premiums, deductibles, plan cost sharing, prescriptions and possible long-term services Which costs are covered, which are capped, and which require a separate funding source?

Mark expenses especially sensitive to price increases. Housing, food, utilities, transportation and healthcare may not rise in the same pattern as a published inflation index. Your household’s inflation rate is the change in the particular goods and services you buy, not necessarily the national average.

List income by reliability

Write down Social Security, pension payments, annuity payments that are actually guaranteed under their contracts, employment or part-time income, interest and dividends, and planned portfolio withdrawals. For each source, note whether it has an explicit inflation adjustment, can be reduced, depends on market performance, or ends when a person dies. The gap between essential spending and dependable income is the amount your investments or other resources must support.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Stress-test the map

Review the budget under several conditions: prices rise faster than expected, markets fall early in retirement, one spouse dies, or care needs increase. The exercise is not a forecast. It identifies which expenses can be changed and which assets should remain liquid so that a temporary market decline does not force a sale at an unfavorable price.

2. Understand what Social Security’s COLA does—and does not—do

Social Security benefits receive an annual cost-of-living adjustment (COLA). For 2026, the COLA is 2.8%, based on the change in the Consumer Price Index for Urban Wage Earners and Clerical Workers (CPI-W) from the third quarter of 2024 to the third quarter of 2025. The Social Security Administration describes COLAs as a way for benefits to keep pace with inflation.

That adjustment follows CPI-W, not your personal spending pattern. A retiree whose budget has unusually high rent, property taxes, medical premiums, or prescription costs may experience faster increases than the COLA. A COLA also does not create a ceiling on healthcare costs or guarantee that every expense will be covered.

Use the COLA in a conservative income plan

  • Apply the announced adjustment to your benefit estimate, then compare the result with your actual essential-expense list.
  • Do not assume that a single year’s COLA predicts future adjustments; the percentage changes with the CPI-W measure.
  • Keep a reserve for expenses that can rise faster than the index used for the adjustment.
  • Coordinate claiming decisions and survivor-income needs with your household plan; the COLA alone does not resolve the timing or adequacy of benefits.

3. Use inflation-linked securities with their limits in mind

How Treasury Inflation-Protected Securities work

Treasury Inflation-Protected Securities (TIPS) are U.S. Treasury securities whose principal adjusts with changes in the Consumer Price Index. They are issued in 5-, 10- and 30-year terms. Interest is paid every six months, and the amount redeemed at maturity is no less than the original principal. These features can help match part of a future spending need to CPI inflation.

TIPS are not a guarantee that every personal expense will keep pace. Their adjustment is linked to CPI, while your rent, food, insurance or medical costs may move differently. The price also depends on the purchase yield and the market’s interest-rate expectations.

Account for timing and market value

Although TIPS have a maturity value tied to their inflation-adjusted principal, they can be sold before maturity. The market price at that time may be higher or lower than the amount invested. Selling during a period of rising interest rates can produce a loss even if the security’s inflation adjustment is working as designed. A ladder of maturities can reduce the need to sell a long-term holding to meet a near-term bill, but the maturities still need to match your spending schedule.

Know the tax treatment

TreasuryDirect states that federal tax applies annually to TIPS interest and may also apply to increases in principal, even though the principal increase is generally received at maturity. State and local taxes do not apply according to TreasuryDirect. The account type and your tax bracket affect the practical result, so include the tax payments in cash-flow planning rather than treating the quoted interest as spendable income.

When TIPS may fit

  • A known future expense is linked reasonably well to CPI and the holding period can extend to maturity.
  • You want a direct Treasury inflation mechanism rather than relying solely on companies to raise dividends or prices.
  • You can tolerate price changes if you might need to sell early.

TIPS are one component, not a complete retirement portfolio. They do not replace liquid cash, growth assets needed for a long retirement, or a plan for healthcare costs that do not track CPI.

4. Keep cash for stability, not as your only inflation defense

Cash and cash equivalents are useful for near-term spending, emergencies and a source of withdrawals during market declines. Their main long-term risk is inflation: if the return does not keep up with rising prices, purchasing power falls. Holding only cash can therefore make a long retirement less secure even though the account balance appears stable.

Match allocation to the withdrawal timeline

Investor.gov frames asset allocation around time horizon, goals and risk tolerance. Money needed soon generally has less capacity for market volatility than money that may not be spent for many years. A retiree who must sell a volatile asset to pay an immediate bill faces a different risk from one who can leave that asset invested through a downturn.

Diversification spreads exposure among asset categories rather than depending on one security or one economic outcome. Rebalancing restores the intended mix after market movements change it. Rebalancing is a discipline, not a promise of higher returns, and it can create taxes or transaction costs in some accounts.

Avoid a one-size-fits-all allocation

There is no evidence-based universal percentage for stocks, bonds, TIPS or cash that applies to every retiree. Consider:

  • the years until each withdrawal is likely to occur;
  • the amount of essential spending already covered by reliable income;
  • how much loss you could tolerate without changing necessary spending;
  • tax location and account withdrawal rules; and
  • whether a spouse, beneficiary or charitable goal changes the time horizon.

Review the plan when spending, income, health or market conditions change, rather than moving the entire portfolio in response to headlines.

5. Address longevity risk with guaranteed income carefully

Longevity risk is the possibility of outliving your assets. Investor.gov explains that a pension lifetime annuity protects against that risk for the pension income promised. By contrast, people saving in defined-contribution accounts bear investment risk and must make savings last.

That distinction does not mean every retail annuity is appropriate. Guarantees depend on the issuing insurer and the specific contract. Before considering a retail annuity, compare the following items in writing:

Feature Questions to answer
Inflation protection Does the payment rise with an index, increase by a fixed percentage, or remain level?
Guarantee Which payment is contractually guaranteed, and by whom?
Fees and charges What explicit fees, surrender charges or adjustments reduce value?
Liquidity How much can be withdrawn, and what happens if you need a large amount unexpectedly?
Survivor benefits What income, if any, continues for a spouse or beneficiary?
Inflation and healthcare fit Will the payment keep up with the expenses you are trying to protect?

Guaranteed income can cover a portion of essential bills, while a liquid portfolio may handle emergencies and flexible spending. The balance depends on your existing pension and Social Security, assets, health, household obligations and preferences.

6. Budget Medicare as a recurring, changing expense

Medicare reduces the cost of covered care, but it does not make healthcare free or impose one universal annual spending cap. For 2026, the standard Medicare Part B premium is $202.90 per month, and the Part B deductible is $283. These are Centers for Medicare & Medicaid Services figures for 2026; higher-income enrollees may owe a larger Part B premium.

Part D prescription coverage and Medicare Advantage costs vary by plan and location. Original Medicare generally has no annual out-of-pocket limit unless you also have supplemental coverage or enroll in Medicare Advantage. Premiums, deductibles, copayments, coinsurance, provider networks and drug formularies therefore belong in the household budget.

Build a Medicare cost worksheet

  1. Record the Part B premium for each person and check whether an income-related adjustment applies.
  2. Compare available Part D or Medicare Advantage plans in your service area, including premiums, deductibles, cost sharing, covered drugs, networks and annual limits.
  3. If evaluating Original Medicare, price supplemental coverage and verify what it pays before assuming the gap is covered.
  4. List recurring prescriptions, expected specialist care, dental or vision services, and travel needs that may not fit neatly into a standard premium estimate.
  5. Verify the current year’s premiums and plan details during each annual review; the 2026 figures are not a permanent forecast.

Do not use the $202.90 Part B premium as a complete healthcare-retirement budget. It is one standard premium, before any income-related adjustment and before other coverage, treatment and care expenses.

Coordinate Medicare costs with withdrawals

Set aside a predictable source for premiums and routine cost sharing. Keep additional liquidity for deductibles, a sudden procedure or a change in prescription needs. If portfolio withdrawals determine taxable income, ask a qualified tax professional how the timing could affect future Medicare premiums; the appropriate answer depends on personal tax facts that cannot be determined from a general rule.

7. Plan separately for long-term custodial care

Medicare.gov states that Medicare does not pay for long-term care. Most long-term care is custodial assistance—help with bathing, dressing, eating, toileting, transferring or other daily activities—rather than treatment intended to improve an acute medical condition.

This exclusion is different from Medicare coverage for eligible hospital services or skilled nursing care, which has its own eligibility requirements and limits. A short period of covered skilled care should not be treated as a plan for years of custodial assistance.

Evaluate the funding paths

  • Personal resources: savings, investments, home equity or income may pay for care, but withdrawals can reduce the assets available to a surviving spouse or for other goals.
  • Medicaid: eligibility and covered services are governed by state rules and financial requirements. Do not assume eligibility without reviewing your state’s current standards.
  • Private long-term-care insurance: policy benefits, elimination periods, inflation riders, triggers, exclusions, premium changes and renewal terms vary by contract. Compare the policy to the type and duration of care you would actually want.

Care costs vary widely by setting, geography and level of assistance. The available evidence does not establish a universal reserve amount or a standard premium, so use local estimates and your own health and family circumstances when modeling this risk.

Write a care decision before a crisis

Discuss preferred settings, who may help manage finances, which assets could be used first, and how a spouse’s income would be protected. Document powers of attorney and other legal arrangements with appropriate professional advice. A written preference does not guarantee availability or affordability, but it gives the household a starting point before an emergency.

8. Put the pieces together in an annual process

At retirement or during the first planning year

  1. Create the essential, flexible, irregular and healthcare spending categories.
  2. List reliable income and identify the gap that investments must cover.
  3. Assign near-term withdrawals to liquid resources and review whether longer-term assets are diversified for the remaining horizon.
  4. Decide whether an inflation-linked allocation, such as a TIPS ladder, fits specific future expenses and tax constraints.
  5. Compare Medicare options in your area and record premiums, deductibles, networks, drug coverage and out-of-pocket limits.
  6. Choose a long-term-care funding approach and identify what would happen if care lasted longer or cost more than expected.
  7. Document beneficiaries, account ownership, emergency contacts and who can make financial and healthcare decisions.

Every year

  • Replace estimates with the latest Social Security COLA and Medicare figures.
  • Reconcile actual spending with the plan, paying special attention to healthcare and housing.
  • Rebalance only when the portfolio has moved materially from its intended allocation or your circumstances have changed.
  • Check TIPS maturities, cash reserves and upcoming large withdrawals.
  • Review Medicare plan changes during the applicable enrollment period and confirm that medications and providers remain covered.
  • Revisit the long-term-care plan after a diagnosis, family change, move or major asset change.

Common mistakes and how to correct them

Relying on one inflation number

Problem: treating the Social Security COLA or a national CPI measure as a guarantee for your household budget.

Correction: track your own major spending categories and maintain flexibility for costs that rise faster than the index.

Holding all retirement assets in cash

Problem: avoiding market volatility but allowing inflation to erode purchasing power over a long retirement.

Correction: reserve cash for near-term needs and use a diversified allocation suited to your time horizon and risk tolerance.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Assuming TIPS cannot lose value

Problem: forgetting that a TIPS sale before maturity is priced by the market and that CPI may not match personal expenses.

Problem: budgeting only the standard 2026 premium of $202.90 per month.

Correction: add the deductible, plan premiums, prescriptions, cost sharing, potential income-related adjustments and services outside the selected coverage.

Expecting Medicare to pay for extended custodial care

Problem: confusing covered skilled or hospital services with long-term assistance with daily activities.

Correction: choose a separate strategy involving personal resources, possible Medicaid eligibility or private insurance.

Buying guaranteed income without comparing trade-offs

Problem: focusing on the payment amount while overlooking inflation adjustment, fees, liquidity, survivor benefits and the strength of the guarantee.

Correction: compare the contract with existing Social Security and pension income and determine which essential expenses the guarantee would actually cover.

Questions to take to a professional

  • Which expenses are essential, and how much of them is covered by income that is expected to continue?
  • How would a prolonged market decline affect withdrawals and discretionary spending?
  • Which TIPS maturity, if any, matches a known future liability after taxes and liquidity needs?
  • How will Medicare premiums and plan cost sharing fit the household’s cash flow in the current year?
  • What happens to a spouse’s income and assets if one person needs extended custodial care?
  • What contract provisions make a proposed lifetime-income product different from existing pension or Social Security guarantees?

A fiduciary investment adviser, benefits specialist, insurance professional or tax professional may address different parts of this plan. Ask how each person is paid, what they are licensed to advise on, and which assumptions drive the recommendation.
