There is no India-wide rule that automatically puts a loan EMI, a school fee or a medical bill first. When money cannot cover everything, protect urgent medical care and basic household needs, then use dates, minimum amounts, written policies and the cost of each option to decide what can be paid, delayed or renegotiated. Contact every lender, school and healthcare provider before a missed deadline and ask for specific terms in writing.
This guide is for an India-oriented household. RBI directions, a school’s policy, your loan agreement, local law and the patient’s clinical urgency all matter. A payment arrangement is never guaranteed merely because you request it.

What to do in the next 24 hours
Protect urgent treatment and essentials
If someone needs emergency or otherwise necessary treatment, ask the treating clinician what cannot safely wait. Ask the hospital billing or social-work desk about clinically suitable lower-cost options, a government facility, insurance authorization and available assistance. Do not postpone necessary care solely to make a routine payment.
Reserve enough for food, housing, utilities and transport to care. This is practical triage, not a legal priority rule. A payment that preserves a roof, electricity or access to treatment may be more important to household safety than an obligation whose timing can be negotiated, but the decision depends on the actual contracts and deadlines.

Make a dated cash-flow snapshot
Use ordinary notes, a spreadsheet or paper. Record only money that is actually available or has a reliable payment date.
| Item to record | Details to capture |
|---|---|
| Cash and income | Balance available today, expected income, date it will arrive and any amount that is already committed. |
| Essential spending | Food, housing, utilities, transport to treatment and other costs that keep the household safe and functioning. |
| Loan obligations | Each EMI, due date, lender contact, amount you can pay now and the consequences stated in the agreement. |
| School charges | Fee deadline, amount requested, balance after a part-payment, late charges and the institution’s written policy. |
| Medical costs | Amount due now, expected near-term treatment cost, insurance share, assistance application deadline and what the clinician says is time-sensitive. |
RBI borrower guidance discusses assessing cash flows when a borrower is in financial difficulty. The exercise is not about finding a perfect budget; it gives you precise facts for a conversation with each organisation.
Classify each amount by urgency and flexibility
For every item, answer three questions: What happens clinically or practically if it is not paid today? What is the last date before a written consequence applies? What arrangement can the organisation offer, and what will that arrangement cost in total? Put the answers beside the amount rather than relying on memory or pressure from the latest phone call.

Handling medical bills without delaying necessary care
Ask for the lowest safe route
Speak with the treating team, billing office and hospital social-work or patient-assistance desk. Ask whether the same clinically appropriate treatment is available through a government hospital, a lower-cost ward, a generic medicine or an outpatient route. The clinician must decide whether an alternative is medically suitable; a cheaper option is not automatically safe.

Ask your insurer about pre-authorisation, network rules, exclusions, co-payments and the documents needed for reimbursement. Request the answer, estimated patient share and submission deadline in writing. If a claim is pending, ask the hospital what amount is required immediately and whether the remainder can wait while the insurer decides.
Check public assistance carefully
A 2022 Press Information Bureau release describes assistance through Rashtriya Arogya Nidhi for qualifying poor patients with specified life-threatening illnesses treated at government hospitals. It is not a universal medical-bill benefit. Eligibility, covered conditions, hospital requirements and application procedures can change, so confirm the current rules with the relevant government authority or hospital before counting on the money. Ask when an application must be filed and whether treatment can begin while it is being considered.
Put healthcare-cost statistics in context
A NITI Aayog article using National Sample Survey 75th Round data from 2017–18 examined 4,564 hospitalised children aged 0–4. In that analysis, “catastrophic” out-of-pocket healthcare expenditure meant household health spending above 10% of household consumption expenditure. The figure describes that study population and research definition; it is not a personal threshold for deciding to defer treatment or for predicting your bill.
Managing loan EMIs when income is short
Contact the lender before a missed payment
RBI guidance treats an inability, or likely inability, to meet debt service as a possible sign of financial difficulty. The difficulty can be identified even when no instalment is yet overdue. Use a verified official channel and ask for the hardship, servicing or collections team. Explain the temporary shortfall, the amount you can pay and the date you expect income or funds. Ask the lender to list every available option under your specific agreement and to send the terms, charges and effect on the repayment schedule in writing.

Possible arrangements include a revised payment date, a short-term payment plan or a formal restructuring. Their availability depends on the lender’s policy, your agreement and an assessment of your circumstances. RBI defines restructuring as “an act in which a lender, for economic or legal reasons relating to the borrower’s financial difficulty, grants concessions to the borrower.” That definition does not create an automatic right to a concession.
Understand floating-rate EMI reset choices
For floating-rate, EMI-based personal loans covered by the applicable RBI direction, the lender must communicate rate-related changes and provide choices at reset. The direction states: “The borrowers shall also be given the choice to opt for (i) enhancement in EMI or elongation of tenor or for a combination of both options; and, (ii) to prepay, either in part or in full, at any point during the tenor of the loan.”
Ask for the reset notice, the revised amortisation schedule and any service or prepayment charge. A lower monthly amount achieved by extending the tenor can increase total interest and keep the debt outstanding for longer. A higher EMI may end the loan sooner but can leave too little for essentials. A fixed-rate switch may be offered under the lender’s policy, but it is not a universal entitlement; compare its rate and fees with the alternatives.
Compare lender proposals on the same facts
| Proposal | What to ask | Trade-off to calculate |
|---|---|---|
| Change the payment date | Is the change one-time or recurring? Is there a fee, and will the next instalment be larger? | Whether the new date matches reliable income without creating two large payments together. |
| Short-term arrangement | How many instalments are affected? Are unpaid amounts added later, and at what rate? | Immediate relief versus the later instalment amount and total additional cost. |
| Tenor extension | New end date, revised EMI, interest rate, processing fee and total repayment. | Lower monthly outgo versus extra interest and a longer obligation. |
| EMI increase or mixed reset | What amount is due each month and from which date? Is the schedule based on the current rate? | Faster repayment versus whether essentials remain affordable in a bad-income month. |
| Fixed-rate switch, if offered | Eligibility, fixed rate, conversion fee, lock-in terms and prepayment rules. | Payment certainty versus the fixed-rate cost and reduced flexibility. |
| Part or full prepayment | Minimum amount, charge, effect on EMI or tenor and how the lender will apply the money. | Interest saved versus cash needed for treatment, food and other near-term obligations. |
| Formal restructuring | Whether the account is formally restructured, all concessions, reporting treatment and total cost. | Potential affordability relief versus contractual, reporting and classification consequences. |
Know the consequence of formal restructuring
Restructuring terms are concessions, not a guaranteed approval. Under the applicable RBI prudential guidance, a restructured standard account is immediately downgraded to non-performing-asset classification. The precise effect depends on the rules that apply to your lender and product, so ask the lender to explain the classification and reporting treatment in your written offer before accepting it. Do not choose an arrangement solely because its first EMI is smaller.

Addressing school fees before the deadline
Ask the institution for its written policy
Contact the school or relevant educational institution before the due date. Ask about instalments, an extension, a part-payment, late charges, hardship review and the date by which the balance must be cleared. Request the answer by email or on an official letterhead, including the amount needed now and the consequences of each missed date.
There is no nationwide rule in the material available here that assigns ordinary school fees a fixed priority or guarantees a deferral, continued attendance or a waiver. A policy in one school or state should not be treated as a rule for every institution. Read the enrolment agreement, fee schedule and any written notice you have received.
Compare school arrangements
| Arrangement | Questions to put in writing | Decision point |
|---|---|---|
| Part-payment now | How much is required to keep the account current, and what balance and date remain? | Whether the part-payment leaves enough for urgent care and essentials. |
| Instalments | Number and dates of instalments, administrative fee, late charge and missed-instalment consequence. | Whether each future instalment fits reliable income rather than optimistic income. |
| Extension | New due date, interest or late fee, and whether the full balance becomes due later in one sum. | Whether the later lump sum is realistic. |
| Hardship review | Documents needed, decision date and what is payable while the review is pending. | Whether applying now changes any immediate deadline; do not assume it does. |
Separate school charges from an education loan
A school’s fee policy is different from a bank’s education-loan servicing process. If the problem is a bank education loan, use the lender complaint route described below. If it is the school’s own charge, communicate with the institution and keep the contract and its written responses.
When all three demands arrive together
Use a one-page priority worksheet
After reserving urgent treatment and essentials, list the remaining demands in date order. For each one, write the minimum amount that prevents the next stated consequence, the next date, the cost of delaying, and the arrangement requested. This prevents a caller who demands immediate payment from silently deciding your entire household allocation.

| Obligation | Minimum needed now | Next date | Written consequence | Arrangement requested |
|---|---|---|---|---|
| Medical | Amount the provider says is required for clinically necessary care | Treatment or billing date | Clinical, discharge or billing consequence stated by provider | Insurance approval, assistance, staged payment or clinically safe lower-cost route |
| Loan EMI | Amount lender confirms for the proposed arrangement | Contractual due date or agreed date | Only the consequence stated in the agreement or written lender notice | Payment-date change, reset choice, short arrangement or restructuring terms |
| School fee | Amount institution says is needed now | Fee deadline | Late charge or other written policy consequence | Instalment, extension, part-payment or hardship review |
Decide by total household impact
Compare each proposed payment against the cash left for essentials and the next reliable income date. A plan that works only if overtime, a pending claim or a promised loan arrives is not yet affordable. If two organisations offer flexibility, prefer the arrangement with clear written terms, a sustainable payment and the lower total cost after fees and interest. If none is sustainable, tell each organisation that fact and seek qualified, India-specific financial counselling rather than stacking an unaffordable new loan on top of the old one.

Escalating a bank education-loan service complaint
For a service complaint about an education loan, RBI’s stated sequence is:
Free tools Windows power users keep installed
One-click scans. No signup required.
- Complain to the bank first, using its official grievance channel, and keep the complaint number and documents.
- Allow one month for a response. If there is no response within that period, or the response is unsatisfactory, preserve the reply and your original complaint.
- Use the RBI Integrated Ombudsman Scheme and its Complaint Management System, supplying the correspondence and relevant account records.
This route addresses a bank’s service complaint. It is not a guaranteed method to erase a valid fee, cancel a loan balance or compel a concession that the agreement and applicable rules do not provide.
Final checks before accepting any arrangement
- Is the amount due now compatible with food, housing, utilities, transport and urgent treatment?
- Does the document show every fee, interest change, revised EMI, new end date and total repayment?
- Does it say whether the account is formally restructured and how the lender will classify or report it?
- Can you prepay later, and are there restrictions or charges?
- For a medical route, has a clinician confirmed that the cheaper or delayed option is safe?
- For a school arrangement, are the remaining balance and every future date written down?
- Have you verified insurance, public assistance and application deadlines rather than assuming approval?
- Do you know what must be paid while a request is pending?
The practical goal is not to declare one category the winner. It is to keep the household safe, make the time-sensitive payment that cannot safely wait, and turn every negotiable obligation into a documented, affordable plan before the next deadline.
