Marketplace members pay the insurance company according to its bill and payment instructions; insurers may or may not offer quarterly billing. Medicare beneficiaries who receive a premium bill generally get Part B-only bills every three months and Part A bills monthly, while some premiums are deducted from Social Security or Railroad Retirement Board benefits.
Use your current invoice or official plan account as the controlling source. Pay the first Marketplace premium directly to the insurer—coverage does not begin until it is paid—and allow enough processing time for any automatic debit, online payment, bank bill pay, or mailed check.

First identify who bills you
The right payment method depends on the program, not simply on whether you prefer monthly or quarterly installments. Before scheduling anything, read the latest invoice, sign in to the plan’s official portal, or call the number on the bill.
Marketplace plans
For a plan purchased through HealthCare.gov or a state Marketplace, the insurer—not HealthCare.gov—collects the premium. HealthCare.gov’s payment guidance says insurers handle payments differently and that members should follow the insurer’s instructions about when and how to pay. The Marketplace account may send you to the insurer’s payment site, but the insurer’s bill controls the due date, available frequencies, accepted payment methods, and any fees.

Medicare premiums
Medicare’s federal billing rules are more standardized. A person billed for Part B only generally receives a bill every three months. Part A bills are monthly. Many beneficiaries do not receive a bill because Part B premiums are deducted from Social Security or Railroad Retirement Board benefits. A bill’s frequency can change if the premiums being collected change, so check each notice.
| Coverage or billing situation | Typical schedule described by the official guidance | Who sets the payment details |
|---|---|---|
| Marketplace plan | May be monthly or another schedule offered by the insurer; quarterly payment is not guaranteed | Your insurance company |
| Medicare Part B only, when Medicare sends a bill | Generally every three months | Medicare |
| Medicare Part A bill | Monthly | Medicare |
| Medicare premiums deducted from benefits | Withheld from Social Security or Railroad Retirement Board payments according to the benefit schedule | The benefit agency and Medicare |
Can you choose monthly or quarterly installments?
Sometimes, but only when the program or insurer offers that option. An individual Marketplace policy does not automatically give you a right to pay quarterly. Some insurers accept only the frequency shown on the invoice; others may let you pay several months in advance, enroll in recurring monthly payments, or use a different arrangement after you contact billing.
Ask the insurer these specific questions before sending a large payment:
- Does the plan accept a quarterly payment, or must premiums be paid monthly?
- Will an advance payment be applied to future months or returned?
- What exact amount is due, including any subsidy change or past-due balance?
- What date must the payment post, rather than merely be mailed or submitted?
- Are card, electronic-check, phone, or bank-bill-pay fees charged?
- How will the insurer notify you if the premium changes?
For Medicare, the bill itself states the amount and due date. Medicare offers manual payments and a recurring monthly bank debit, but a beneficiary cannot turn a monthly Part A bill into a quarterly bill simply by choosing a different payment channel.
How to pay a Marketplace premium
Pay the first premium before coverage can start
After enrolling, pay the first premium directly to the insurance company. HealthCare.gov states that coverage will not start until the first premium is paid. The Marketplace cannot accept that premium on the insurer’s behalf.

- Open your Marketplace enrollment confirmation and identify the insurer.
- Use the insurer’s website, telephone number, or paper instructions to create or access its member account.
- Confirm the effective date, premium amount, subsidy, and due date shown by the insurer.
- Submit the first payment using an offered method, such as electronic bank payment, card, phone payment, or mail.
- Save the confirmation number or receipt and verify in the insurer portal that the payment posted.
If your Marketplace account provides a “pay premium” link, it may route you to the insurer’s payment system. Follow the redirected instructions rather than assuming that a payment made inside the Marketplace account completed the transaction.
Set up recurring monthly payments when offered
In the insurer’s billing portal, look for labels such as “autopay,” “recurring payments,” or “automatic premium payment.” Enter the checking-account or card information requested, choose the debit date if the insurer permits a choice, and read the authorization terms. Record the account used, the first debit date, and how to cancel or change the authorization.
Quick wins for a faster PC:
Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Automatic payment is not proof that coverage is active. Confirm that the first debit succeeded and that later payments are marked “posted” or “paid.” A changed bank account, expired card, insufficient funds, or a revised premium can stop a recurring payment without changing the policy’s due date.
Request quarterly billing instead of assuming it
Call the number on the invoice and ask whether the insurer will accept three months of premiums in one transaction. Get the answer in writing through the portal or a billing message. If quarterly payment is unavailable, use monthly autopay or schedule three separate payments in advance, each for the amount and due date the insurer specifies. Do not send an arbitrary three-month amount: a subsidy adjustment, deductible-related change, or prior balance can make it incorrect.
California Marketplace example: why state rules do not generalize
Covered California illustrates a common arrangement but is not a rule for every state or insurer. Its billing guide says the first and future bills come from the insurance company; future bills arrive monthly and explain the due date and online, phone, or mail payment methods. A Covered California enrollee should therefore use the insurer’s bill even though enrollment occurred through the state Marketplace. Enrollees elsewhere should check their own insurer’s instructions rather than copying this schedule.

How to pay a Medicare premium bill
Medicare lists four practical routes: a secure Medicare account, Medicare Easy Pay, online bill pay through a bank, and mail. The amount and due date on the Medicare bill remain controlling.
| Method | How it works | Timing or cost considerations |
|---|---|---|
| Medicare account | Sign in to the secure Medicare account and submit a checking or savings payment | Medicare says checking or savings payments take about five business days to process; submit at least five business days before the due date |
| Medicare Easy Pay | Free recurring monthly debit from a checking or savings account | Deductions occur on the 20th or next business day; initial setup can take six to eight weeks |
| Bank online bill pay | Use your bank’s bill-pay service and follow Medicare’s payee and account-number instructions | Your bank may charge a service fee and may require additional delivery time |
| Send payment using the remittance instructions and address on the Medicare bill | Allow postal delivery and processing time; retain the payment record |
Pay through a Medicare account
- Sign in to your secure Medicare account.
- Open the premium-billing or payment area and review the current balance and due date.
- Enter the checking or savings information requested and submit the payment.
- Keep the confirmation and check the account later to make sure the payment posted.
Medicare’s guidance says online checking or savings payments take about five business days to process. Submit at least five business days before the due date; submitting on the due date is not a safe substitute for allowing processing time.
Use Medicare Easy Pay for automatic monthly deductions
Medicare Easy Pay is a free service that automatically deducts the premium from a checking or savings account each month. Medicare schedules the debit for the 20th of the month or the next business day.
- Request Medicare Easy Pay through Medicare’s official payment instructions.
- Provide the bank details and authorization requested on the form or account setup.
- Note the setup date and keep paying by another method while the request is processed.
- Watch the bank account and Medicare notices for the first successful debit.
Setup can take six to eight weeks. Until deductions actually begin, continue paying each bill by another method. If a debit fails, Medicare sends a bill; pay that bill through another method rather than assuming Easy Pay will retry automatically.
Use bank bill pay
Your bank’s bill-pay service may send an electronic payment or a check. Enter Medicare exactly as the payee shown in the Medicare instructions, include the required account or billing number, and schedule delivery early enough for the bank and Medicare to process it. Some banks charge a service fee. A “scheduled” status in your bank account does not necessarily mean Medicare has received or posted the payment.

Mail a payment
Use the remittance slip and mailing address printed on the Medicare bill. Write the required identifying information exactly as instructed, allow for postal delivery and Medicare processing, and keep a copy of the check, money order, or other payment record. Do not mail cash.
How to set up a reliable monthly payment routine
A monthly routine works whether the insurer requires monthly bills or you choose monthly autopay for budgeting control.
- Calendar the real due date. Record the date shown on each bill, not a date copied from a prior month.
- Choose a funding source. A checking account generally avoids card-expiration problems, while a card may provide a different transaction record; use only a source with enough available funds.
- Schedule early. For Medicare online checking or savings payments, allow at least five business days. For an insurer, use the processing lead time it states.
- Verify posting. Check the portal or account after the expected processing window and save the confirmation.
- Review changes. Compare the next bill with the prior amount. Update an authorization when the premium or bank information changes.
- Keep a backup method. If an automatic debit fails, make a manual payment before the policy’s deadline and then correct the failed authorization.
What happens when a payment is late?
Marketplace grace-period rules
HealthCare.gov says a Marketplace plan using the advance premium tax credit usually has a three-month grace period when the member has already paid at least one full month’s premium during the benefit year. The period starts with the first missed month. Paying a later month does not, by itself, erase the unpaid first month.
If the past-due amount remains unpaid, coverage may end retroactively to the first missed month. Claims submitted during later grace-period months can be held or handled differently, so ask the insurer how claims are treated while the account is delinquent. A person who loses Marketplace coverage for nonpayment generally cannot obtain a Special Enrollment Period solely because of that termination. People who do not receive the advance premium tax credit may have different grace-period rules; check the insurer and state insurance department.
A grace period is not a quarterly installment option. It is a conditional protection after a missed payment and does not cancel the unpaid premiums.

Medicare late-payment concerns
For Medicare, follow the due date and instructions on the bill. If an automatic debit fails or a bank payment is delayed, make another payment method your immediate fallback and contact Medicare using the information on the bill. Keep records of the failed transaction and the replacement payment.
Choosing monthly versus quarterly payment
| Priority | Monthly payment | Quarterly payment, when the payer offers it |
|---|---|---|
| Cash flow | Smaller, regular outlay; easier to align with a monthly budget | Requires three months of premium available at once |
| Control | More frequent chances to review premium changes | Fewer transactions, but a change can affect a larger advance payment |
| Automation | Usually fits recurring monthly debit programs | May require manual requests or may not be available |
| Processing risk | Each payment must post before its monthly due date | One delayed or misapplied payment can affect several months |
| Fees | Check for card or bank fees on each transaction | Confirm whether advance payments or a chosen payment channel carry fees |
Choose monthly payments when income arrives monthly, the premium changes often, or you want to inspect every bill. Consider quarterly payment only after the insurer or program confirms that it accepts it, tells you how it applies the money, and provides the exact amount and due date.
Troubleshooting common payment problems
The first Marketplace payment was made, but coverage still shows as pending
Check whether the payment actually posted to the insurer rather than merely being submitted through a Marketplace link. Compare the confirmation number, amount, and effective date with the insurer’s account. Contact the insurer’s billing department with the receipt before assuming the policy is active.

Medicare Easy Pay has not started
Six to eight weeks may be needed for setup. Continue paying bills by another method during that period. If the first expected debit is missed, check for a Medicare notice and pay the bill manually.
A bank payment says “sent,” but the account remains unpaid
Ask the bank for the delivery date and confirmation details, then give Medicare or the insurer the information from that record. A bank’s scheduled or sent status does not establish that the payer posted the money.
The premium changed after autopay was enabled
Read the new invoice, verify the debit amount, and update the authorization if the insurer requires it. Do not cancel a payment without arranging a replacement; an unpaid balance can trigger the plan’s delinquency process.
You cannot find quarterly billing
Look at the payment-frequency language on the invoice and ask billing directly. If quarterly installments are not offered, use the available monthly method and keep three months of premiums in a separate budgeting category rather than sending an unauthorized lump sum.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsPayment-safety and recordkeeping checklist
- Use only the insurer, Medicare, or official Marketplace account reached from a trusted statement or government site.
- Confirm the web address and avoid sending payment details through an unsolicited message.
- Save the invoice, confirmation number, date submitted, amount, and account used.
- Review bank and card statements for the posted transaction.
- Update billing information promptly after changing banks, cards, addresses, or benefit deposits.
- Call the number printed on the bill when a payment is missing, duplicated, reversed, or applied to the wrong member.
Questions to ask before sending an installment
Have the invoice in front of you and ask the payer to confirm the amount, due date, posting deadline, payment frequency, accepted methods, processing time, fees, and procedure for changing or canceling automatic payments. For a Marketplace plan, also ask how a payment will affect the effective date and how claims are handled during any delinquency. For Medicare Easy Pay, ask whether the authorization is active yet and continue manual payments until it is.
