Map a mortgage QC finding by tracing verified file evidence to the rule that governs the loan’s delivery, insurance, or guarantee, then reassessing the underwriting decision with corrected facts. A label such as “major,” “high,” or “eligibility defect” is not a universal result. The loan may remain eligible after a data correction, require a price adjustment, violate a program rule, or reveal only a documentation or process-control weakness.
The defensible conclusion is a documented chain: issue, evidence, applicable guide and effective date, corrected analysis, outcome, internal and external classifications, owner, remediation, and closure proof.

What a mortgage QC crosswalk must establish
A crosswalk converts an observation from a file review into a conclusion that another reviewer, manager, investor, or regulator can reproduce. It should answer five questions in order:
Free tools Windows power users keep installed
One-click scans. No signup required.
- What exactly is wrong, and which documents or data prove it?
- Which investor, agency, insurance, or guarantee rule governs this loan?
- Does the corrected information change underwriting eligibility, pricing, compliance, collateral acceptance, or only process quality?
- How should the issue be categorized and severity-rated for internal reporting and any applicable external taxonomy?
- What action, owner, due date, notification, and closure evidence are required?
Do not start with a severity label and work backward. “Missing document,” “incorrect data,” and “unsupported underwriting judgment” are different observations even when they concern the same subject. Preserve the original document, the version reviewed, the data used in underwriting, and the recalculation that supports the final disposition.

Finding versus defect
A finding is an observed condition. A defect is a finding that, under the governing rule and verified facts, creates a specified consequence. A control gap may indicate weak procedures without making the delivered loan ineligible. Conversely, an apparently small data error can become an eligibility defect if the corrected debt-to-income ratio, loan-to-value ratio, occupancy status, or other measure falls outside program limits.
Why the rule set matters
Fannie Mae delivery eligibility and post-closing QC requirements are not interchangeable with FHA policy violations and remedies. A lender may aggregate results in one enterprise report, but the authority, terminology, remedy, notification channel, and deadline remain program-specific. Current Freddie Mac or private-investor requirements must be checked in their own guides before assigning a specific external category or remedy.
Step 1: State the issue precisely and preserve evidence
Write the finding so a person who has not seen the file can understand the alleged condition without inference. Identify the loan, product, channel, review phase, sample type, and documents examined.
Use an evidence-based finding statement
A useful statement names the expected condition, the observed condition, and the supporting evidence. For example:
“The final application omitted a recurring obligation shown in the borrower’s bank statements and condominium documents. The payment was not included in qualifying debt-to-income calculations. Recalculation is required using the verified monthly obligation.”
This is stronger than “undisclosed debt—high severity,” because it distinguishes the source document, the data omission, and the analysis still required. Attach or reference the relevant application, credit report, income and asset documents, appraisal or condominium records, closing package, and underwriting calculations.
Distinguish common evidence states
- Absent document: The file does not contain evidence required by the applicable rule. Do not assume the underlying fact was false.
- Inaccurate data: A value in the application, underwriting system, appraisal data, or closing record conflicts with verified evidence.
- Unsupported judgment: The decision may be reasonable, but the file lacks the analysis or documentation required to support it.
- Contradictory evidence: Documents point to different facts, requiring resolution rather than selection of the most favorable item.
- Process failure: A required review, approval, escalation, or control did not occur, even if the loan ultimately remained eligible.
Record the review context
Include whether the review was pre-closing, post-closing, servicing, targeted, random, or discretionary; the acquisition or review period; and whether the sample was designed to estimate a population or investigate a known risk. A defect rate or ranking without that context is misleading.

Step 2: Identify the governing investor or agency rule
For each issue, identify the delivery or insurance program, the guide or handbook section, and the version or effective date applicable to the loan. The rule in force when the loan was underwritten, closed, delivered, or reviewed may differ, so preserve the relevant dates and transition provisions.
Build the authority record
- Program and counterparty: For example, a Fannie Mae conventional delivery or an FHA-insured Title II loan.
- Product and channel: Include refinance or purchase, construction or standard execution, delegated or correspondent channel, and any applicable special feature.
- Rule location: Record the guide, handbook, bulletin, mortgagee letter, or contract section, not merely a topic name.
- Version and effective date: The principal Fannie Mae QC sections used for this workflow show an April 1, 2026 section date. FHA’s revised taxonomy under Mortgagee Letter 2025-01 became effective for Title II reviews on January 15, 2025.
- Required outcome: Note whether the authority addresses eligibility, pricing, documentation, compliance, collateral, servicing, repurchase, indemnification, or another remedy.
Never use an agency label as if it were a universal industry definition. Preserve the exact external category when one exists, while retaining the lender’s enterprise category for aggregation.
Fannie Mae post-closing QC expectations
Fannie Mae’s QC guidance requires verification of the accuracy and integrity of information used to support the credit decision, confirmation that the decision is supported, and assessment of compliance and eligibility for delivery. For loans evaluated through Desktop Underwriter, discrepancies must be addressed against permitted tolerances; corrected data outside tolerances must be resubmitted, and the lender must determine whether the corrected loan remains eligible. The decision and supporting analysis must be documented.
FHA’s distinct framework
HUD describes its FHA defect taxonomy as a categorization of loan-level FHA policy violations and corresponding remedies communicated through the Loan Review System. Mortgagee Letter 2025-01 revised the taxonomy and added servicing-review content. HUD also noted that the servicing-review method had not previously been published other than in draft form, so the taxonomy should not be treated as an exhaustive statement of every servicing-review detail.

The key decision test is: After the verified facts are corrected and the applicable rule is applied, does the loan remain eligible as delivered? Document the calculation, inputs, tolerances, and result rather than relying on a conclusion alone.
Reperform the affected analysis
- Replace the disputed value with the best-supported fact and identify the document or source used.
- Recalculate every dependent measure, such as debt-to-income ratio, loan-to-value ratio, qualifying income, reserves, payment, assets to close, or occupancy-related underwriting inputs.
- Apply the program’s eligibility rules, tolerances, and documentation requirements that were effective for the loan.
- Record whether the corrected result remains eligible, becomes eligible only with a pricing adjustment, requires additional documentation, or is ineligible.
- Preserve both the original and corrected calculations, including the person who performed the reassessment and the date.
Separate possible outcomes
| Outcome | Meaning | How to report it |
|---|---|---|
| Control or documentation finding | The file or process is deficient, but corrected facts support the original decision and eligibility. | Describe the missing or weak control, required documentation, owner, and corrective action. Do not call the loan ineligible. |
| Pricing or adjustment issue | The loan may remain deliverable when corrected data are submitted and the applicable adjustment is paid. | Identify the corrected data, adjustment, delivery treatment, and evidence of payment or correction. |
| Eligibility defect | Corrected facts fail a program requirement or the loan was not eligible as delivered. | Cite the rule, show the corrected analysis, state the delivery consequence, and follow the investor or agency notification path. |
| Compliance defect | A law, regulation, disclosure, or fair-lending requirement may have been violated, regardless of secondary-market eligibility. | Escalate through compliance procedures and preserve the legal analysis separately from the investor disposition. |
| Collateral or valuation issue | Property data, appraisal analysis, or collateral documentation is inaccurate or insufficient. | Identify the affected property fact, required review or correction, and whether collateral acceptance changes. |
Fannie Mae describes a price-adjusted loan as one with defects that could remain eligible for delivery if correct data are submitted and the applicable loan-level price adjustment is paid. That is a Fannie Mae explanation, not a category that every investor uses identically.
Step 4: Design a taxonomy crosswalk that can be trended
A multi-investor lender should retain two classifications: the enterprise category used for internal reporting and the external investor or agency category used by the governing rule. A one-to-one mapping is not always defensible. One internal finding may map to multiple external categories, or no exact equivalent may exist.

Recommended crosswalk fields
| Field | What to capture |
|---|---|
| Loan and review identity | Loan identifier, product, delivery channel, review phase, sample type, and acquisition or review period. |
| Finding statement | Plain-language issue tied to specific file evidence. |
| Evidence references | Document names, dates, data fields, calculations, and unresolved contradictions. |
| Authority | Investor or agency, guide or handbook section, version, and effective date. |
| External category | Exact investor or agency taxonomy label, when supplied. |
| Internal category | Enterprise category used across products and counterparties. |
| Severity | Internal severity definition, with the rule or threshold that supports it. |
| Impact | Eligibility, pricing, compliance, documentation, collateral, servicing, or process-control effect. |
| Root cause | People, procedure, system, vendor, training, data, or governance cause supported by analysis. |
| Disposition | Eligible as delivered, eligible after correction or adjustment, unresolved, or ineligible. |
| Action and ownership | Corrective action, accountable business owner, due date, escalation, and closure evidence. |
| Notification status | Self-report, rebuttal, investor response, submission date, and outcome where applicable. |
Use versioned mappings
Store the effective date of each mapping and retain superseded versions. When an agency changes terminology or remedy, historical reports should continue to reflect the taxonomy applicable to the reviewed loan. Add a “no direct equivalent” option instead of forcing a misleading match.
Fannie Mae states that lenders may use their own taxonomy. Its QC guidance nevertheless requires a taxonomy sufficient to track and trend loan-level defects, identify root causes, and support effective action planning. The lender’s system therefore needs consistent categories and severity definitions even when external labels differ.
Step 5: Apply severity without turning it into an eligibility conclusion
Severity should communicate risk and urgency; it should not substitute for the rule analysis. Define severity levels in writing and test inter-rater consistency. A practical model is:
- Critical: Verified issue creates a material eligibility, legal, safety, fraud, or delivery risk requiring immediate escalation.
- Major: Corrected facts may change eligibility, pricing, material underwriting conclusions, or required investor notification.
- Moderate: The issue affects documentation, calculation support, or a control and requires correction, but current evidence does not establish ineligibility.
- Minor: A limited administrative or procedural deficiency with no identified effect on the decision, eligibility, or compliance.
These are internal examples, not universal agency labels. A “major” internal finding may remain eligible after correction, while a lower-severity documentation issue can become serious if it conceals an eligibility failure. Record the rationale and the impact separately.
Worked example: occupancy misrepresentation
Occupancy findings require a holistic review. A single inconsistent clue does not establish that the borrower falsely represented the subject property as a primary residence.

- Borrower’s current housing circumstances and the distance and location of the subject property.
- Tax returns and bank statements for rental income or other rental indicators.
- Credit history showing multiple mortgages, insurance declarations, or other properties.
- The status of another owned home after closing.
- A separate borrower attestation, reinforced before closing, addressing intended occupancy.
Crosswalk conclusion
State which facts were verified, which remain unresolved, and how the corrected occupancy classification changes underwriting, pricing, or delivery. Fannie Mae identified occupancy as the leading driver of its 2025 price-adjusted loans across cited samples, particularly loans delivered as primary residences that were actually investment properties. That observation describes Fannie Mae’s samples, not a market-wide rate.
Worked example: omitted liability and debt-to-income ratio
An omitted obligation is not automatically an ineligible loan. The reviewer must establish the obligation, determine why it was excluded, and recalculate the ratio.

Evidence to evaluate
- All qualification documents, including applications, credit reports, income records, and asset statements.
- Borrower explanations for obligations that do not appear on the credit report.
- Condominium or appraisal documents for solar payments, assessments, liens, or other recurring obligations.
- Recurring payments visible in asset-account activity.
Fannie Mae describes an omitted-liability defect as an obligation found in application or other file documents that was excluded from debt-to-income calculations without documentation, where inclusion would push the ratio beyond eligibility thresholds. Your crosswalk should show the monthly amount, the original ratio, the corrected ratio, the applicable threshold, and the resulting disposition.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Fannie Mae reporting, self-reporting, and action plans
Fannie Mae requires written QC reporting to management monthly. Reports must summarize findings, communicate defects to responsible departments or business units, and use consistent methodology and terminology, including defect categories and severity levels. Post-closing reporting must include at least three months of trending, distinguish applicable-law compliance defects from underwriting and eligibility defects, and include the highest-severity defect rate against the lender’s target at least quarterly.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteWhen trends are identified, the lender must create a written action plan that identifies the corrective action, expected resolution, and implementation and completion timeframe. The plan should name an accountable owner, define the control change, state how completion will be evidenced, and schedule a validation review to test whether repeat defects decline.
Fannie Mae ineligible-as-delivered notification
For a Fannie Mae loan determined to be ineligible as delivered, the lender must notify Fannie Mae within 30 days of the defined confirmation date and provide a summary and relevant supporting documents through the self-report process. The guide defines the confirmation date by reference to publication of the monthly post-closing report to management in which the loan appears. Do not apply this 30-day requirement to FHA, Freddie Mac, or private investors without checking their current rules.
FHA findings: keep policy violations and remedies separate
For FHA loans, classify the issue under the FHA policy and remedy framework applicable to the review. Mortgagee Letter 2025-01 revised the FHA taxonomy and added servicing content, effective January 15, 2025, for Title II reviews. The finding should identify the policy provision, the evidence, the corresponding remedy, and the Loan Review System reporting path where applicable.
An enterprise report may show FHA and conventional findings together for trend analysis, but the external category, remedy, and escalation must remain tagged to FHA. Do not convert an FHA policy violation into a Fannie Mae delivery conclusion or use Fannie Mae’s price-adjusted-loan concept as an FHA disposition.

How to trend investor defects without misleading management
Trend reports should show counts and rates with the population, sample design, period, and denominator. Separate random and discretionary samples, initial and final findings, loan-acquisition periods, and review-completion periods. A list of the most frequent findings from a targeted sample is not a market-wide incidence estimate.

Fannie Mae’s June 29, 2026 Quality Insider examples illustrate the importance of qualification. In random-sample initial significant defects for loans acquired in the third and fourth quarters of 2025, listed issues included primary-occupancy misrepresentation, income misrepresentation, incorrect rental-income or loss calculation, undisclosed liability, missing debt documentation, and monthly-payment calculation issues. In discretionary reviews completed from October 2025 through March 2026, listed issues included undisclosed liability, borrower not employed, interested-party contributions exceeding borrower costs, undisclosed mortgages, and occupancy misrepresentation.
For a separate random sample of loans acquired from July through December 2025, Fannie Mae listed inadequate or unadjusted comparable sales, insufficient assets to close, missing, incomplete, or ineligible asset documentation, and inaccurate subject-property physical-feature reporting. Fannie Mae described these as issues that do not themselves create eligibility defects but can signal control gaps. Present all such lists with the stated sample and period; never call them universal rankings.
Common mapping failures and how to correct them
Failure: treating every finding as an eligibility defect
Correction: Recalculate with verified facts and cite the rule before assigning an ineligible disposition.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Failure: using one taxonomy for every counterparty
Correction: Retain the enterprise category and the exact applicable investor or agency category in separate fields.
Failure: losing the original evidence after correction
Correction: Preserve the original document, corrected document, calculation versions, reviewer notes, and decision approval.
Failure: reporting severity without impact
Correction: Explain whether the issue affects eligibility, pricing, compliance, documentation, collateral, servicing, or only process control.
Failure: mixing sample populations
Correction: Display sample type, acquisition or review period, and denominator beside every rate or ranking.
Failure: closing corrective actions without testing
Correction: Require closure evidence and perform a later targeted or routine review to verify that the control works.
A practical review checklist
- Have we stated the issue in one sentence tied to specific evidence?
- Did we distinguish an absent document, inaccurate data, unsupported judgment, contradiction, and process failure?
- Is the correct investor, agency, product, channel, guide section, version, and effective date recorded?
- Did we recalculate the affected underwriting measures using verified facts?
- Did we test eligibility, pricing, compliance, collateral, and documentation outcomes separately?
- Are internal severity and external taxonomy stored in separate fields?
- Does the disposition say eligible as delivered, eligible after correction or adjustment, unresolved, or ineligible?
- Are owner, corrective action, due date, notification status, and closure evidence assigned?
- Does the trend report identify sample type, population, period, denominator, and severity target?
- Have recurring trends produced a written action plan and a later effectiveness test?
Bottom line for QC managers
The reliable crosswalk is not a lookup table of labels. It is an evidence-to-rule-to-outcome record. Verify the file facts, identify the governing guide and date, redo the underwriting analysis, separate eligibility from pricing and process effects, preserve both internal and external classifications, and report the result through the correct investor or agency channel. That discipline prevents overcalling a documentation gap as ineligibility while ensuring that a genuine delivery defect is escalated, self-reported, and remediated on time.
