Start by getting the exact date your employer plan ends, then pursue both options on their separate deadlines. You generally have 60 days after losing job-based coverage to apply for a Marketplace plan. COBRA generally gives you 60 days from the later of the coverage-ending date or the date its election notice is provided or mailed. COBRA can preserve your existing group plan and doctors, but you usually pay up to 102% of the plan cost. A Marketplace application may qualify your household for premium tax credits, cost-sharing reductions, or Medicaid/CHIP, so its net cost can be lower. Compare the actual COBRA quote with Marketplace premiums, deductibles, networks, prescriptions, coverage dates, and each family member’s needs before electing or ending coverage.
What to do first after your job-based insurance ends
Job separation and insurance termination are not always the same event. Some employers keep coverage through the end of the month; others end it on the last day worked or another date in the plan documents. Ask the former employer’s benefits office or plan administrator for the last day every covered person is insured and request the COBRA election notice. Do not rely on a termination letter or a payroll record alone.

- Confirm the final covered day. Get the date in writing and ask whether dependents have the same end date.
- Record two independent deadlines. HealthCare.gov says to apply for Marketplace coverage within 60 days after losing job-based coverage. The U.S. Department of Labor says the COBRA election period generally lasts 60 days from the later of coverage ending or provision or mailing of the election notice.
- Request the full COBRA quote. Ask for the monthly amount for employee-only, employee-plus-dependents, and family coverage; the payment address or portal; the election deadline; and the first-payment deadline.
- Apply through the Marketplace. Use your estimated income for the entire calendar year, including wages already earned before the job ended.
- Compare coverage and total exposure. A lower premium can still be a poor choice if your doctors, hospital, prescriptions, or ongoing treatment are not covered.
These deadlines run separately. Applying for Marketplace coverage does not elect COBRA, and receiving a COBRA notice does not apply for a Marketplace plan.

What COBRA preserves
COBRA is temporary continuation of an eligible employer group health plan. When you elect it, the coverage is generally the same group plan you had immediately before the qualifying event, subject to the plan’s normal rules, network, formulary, deductibles, and benefit changes. That continuity can be valuable when a specialist is treating you, a pregnancy is in progress, a surgery is scheduled, or a prescription is difficult to replace. Verify the exact plan year, network, and benefit terms with the administrator; COBRA does not freeze every plan feature forever.

Who can elect
A former employee and other qualified beneficiaries may have independent election rights. A spouse or dependent child can sometimes elect COBRA even when the former employee does not. Ask the administrator how each family member is listed, what elections are available, and whether anyone has a different qualifying event or coverage end date.
COBRA deadlines and first payment
The ordinary election period is 60 days, measured from the later of the date coverage ends or the date the election notice is provided or mailed. The U.S. Department of Labor says the initial premium is generally due within 45 days after election. The notice controls the payment method, grace periods, and retroactive-payment procedure, so follow its instructions rather than relying on a general calendar calculation. Missing a required payment can jeopardize continuation.
Why COBRA can cost more than your payroll deduction
While employed, the company commonly paid part of the premium. Under COBRA, you usually pay the employee share, the former employer’s share, and an administrative charge of up to 2%—as much as 102% of the plan’s total cost. The Department of Labor summarizes this as: “You usually pay the full premium amount, unless your employer agrees to cover some or all of it as part of your separation.” A severance agreement or other employer subsidy can reduce what you are billed; ask whether any contribution is conditional and when it ends.
How long COBRA lasts
COBRA continuation is usually available for up to 18 months. Certain qualifying events and disability-related circumstances can permit extensions, but eligibility, notice requirements, and maximum periods vary. Ask the plan administrator which extension rules apply to your event and household. State continuation laws may offer additional rights, especially for plans or employers outside federal COBRA’s scope; those rules are state-specific.
How Marketplace coverage works after job loss
Special Enrollment Period
HealthCare.gov states that losing job-based insurance creates a Marketplace Special Enrollment Period even when the job ended because you quit or were fired. The relevant event is the loss of coverage, not the reason employment ended. Apply within 60 days after that loss. The application will ask for the event date and may request documents showing prior coverage.

Effective date
The loss-of-coverage guidance generally describes Marketplace coverage beginning on the first day of the month after employer coverage ends. Your eligibility notice and selected plan establish the actual effective date, so check them before assuming there is no gap. If the dates do not match your needs, contact the Marketplace promptly and ask what options are available.
Financial help is based on the full-year household estimate
The Marketplace evaluates estimated tax-household income for the calendar year, not just the wages you expect after the layoff. Include salary, bonuses, unemployment compensation when applicable, self-employment income, and other countable income already received or reasonably expected. The application can screen for premium tax credits, cost-sharing reductions, and Medicaid or CHIP. Update the Marketplace if your income, household, address, or access to another employer plan changes.

An affordable employer plan that meets applicable standards—including a spouse’s offer—can affect eligibility for Marketplace premium tax credits even if you decline that offer. The Marketplace determination uses the household facts you submit. Do not assume that being unemployed automatically guarantees a subsidy.
What changes when you choose a Marketplace plan
A Marketplace plan is new individual-market coverage. You must verify its provider network, hospital participation, drug formulary, prior-authorization rules, deductible, copays or coinsurance, out-of-pocket maximum, and effective date. A plan can cover the same categories of care as your former group plan yet have very different doctors, referral rules, and cost-sharing.
COBRA and Marketplace plans compared
| Decision point | COBRA continuation | Marketplace plan |
|---|---|---|
| What it is | Temporary continuation of eligible employer group coverage. | A new individual-market policy selected through the Marketplace. |
| Enrollment deadline | Generally 60 days from the later of coverage ending or provision or mailing of the election notice. | Apply within 60 days after loss of job-based coverage. |
| Typical effective-date rule | If elected and paid under the plan’s rules, continuation can apply back to the loss-of-coverage date. | Loss-of-coverage guidance generally points to the first day of the month after employer coverage ends; confirm the date in your eligibility notice. |
| Premium basis | Usually the full group-plan cost, including the former employer share, plus up to 2% administration; a separation subsidy may reduce the bill. | Plan premium and out-of-pocket costs vary by plan, household, location, and eligibility for financial help. |
| Provider and prescription continuity | Often retains the former plan’s network and formulary; verify current terms. | Requires a new check of doctors, facilities, prescriptions, formularies, and utilization rules. |
| Financial assistance | Ordinary COBRA pricing is not reduced by Marketplace premium tax credits. An employer contribution can change the amount charged. | The application assesses premium tax credits, cost-sharing reductions, and Medicaid/CHIP eligibility. |
| Duration | Usually up to 18 months, with possible extensions for certain qualifying events. | Continues under the plan’s terms while premiums are paid and eligibility remains; later plan changes generally require Open Enrollment or another qualifying event. |
| Changing later | COBRA exhaustion, loss of an employer subsidy, or an still-open original loss-of-coverage window may support Marketplace enrollment. Voluntarily ending COBRA early generally does not create a new Special Enrollment Period by itself. | After enrollment, a later move to COBRA or another plan depends on the applicable qualifying event and plan rules. |
A step-by-step way to choose
1. Build a coverage timeline
Write down the last day of employer coverage, the date the COBRA notice was provided or mailed, the COBRA election deadline, the date the first COBRA payment is due, the Marketplace application deadline, and the expected effective date of any Marketplace plan. Keep the notice envelope, emails, and confirmation numbers.

2. Get an itemized COBRA quote
Request the exact monthly premium for every coverage tier your household might use. Ask whether dental or vision continuation is separate, whether the quote includes the administrative charge, whether an employer subsidy applies, and how long that subsidy lasts. Confirm whether the first payment must cover retroactive months.
3. Apply to the Marketplace before comparing subsidies
Submit the application with a realistic full-year tax-household income estimate. Include income earned before the layoff and expected income for the remainder of the year. Review the resulting premium-tax-credit, cost-sharing-reduction, and Medicaid/CHIP determinations. If the estimate is uncertain, update it when circumstances change to reduce the risk of an unexpected tax reconciliation.
4. List care that cannot be interrupted
- Primary-care doctors, specialists, therapists, and preferred hospitals.
- Scheduled procedures, pregnancy care, or active cancer, dialysis, or behavioral-health treatment.
- Daily prescriptions, specialty drugs, insulin, durable medical equipment, and prior authorizations.
- Deductible progress and out-of-pocket spending already accumulated during the current plan year.
Ask each insurer whether a current authorization transfers, whether continuity-of-care exceptions exist, and what documentation is needed. Do not assume a new plan will credit amounts paid under the former group plan.
5. Compare annual—not just monthly—cost
For each option, estimate annual premiums plus likely deductibles, copays, coinsurance, prescription costs, and the out-of-pocket maximum. Consider a low-use year and a high-use year. A plan with a higher premium can be cheaper when substantial care is expected; a lower-premium plan can expose you to more cost before insurance pays.
6. Check each family member separately
COBRA and Marketplace choices do not have to be identical for everyone. One person may need the former group network while another qualifies for Medicaid or a subsidized Marketplace plan. Ask whether dependents can elect COBRA independently and compare the household’s total cost under mixed coverage.

7. Investigate other enrollment routes
A spouse’s or parent’s employer plan may offer a special enrollment right after loss of coverage, often with a shorter request period than the Marketplace window. Contact that plan immediately for its deadline and required proof. Medicaid and CHIP applications are available year-round for people who qualify.
8. Confirm the new plan before ending old coverage
Do not cancel COBRA or stop paying premiums until you have confirmed the Marketplace or other plan’s approval, effective date, first premium, network, and prescriptions. If you need COBRA only as a bridge, ask how to terminate it and whether the termination will affect future enrollment rights.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Switching between COBRA and Marketplace coverage
When moving from COBRA to the Marketplace may be possible
Marketplace enrollment may be available when COBRA is exhausted, when an employer subsidy for COBRA ends and the resulting loss qualifies, or when the original loss-of-coverage Special Enrollment Period is still open. A qualifying event and the timing of the application determine the result. Obtain the Marketplace eligibility decision before relying on a switch.
Why simply dropping COBRA is risky
Voluntarily ending COBRA early, or merely stopping premium payments, generally does not itself create a new Special Enrollment Period. You could end up uninsured until Open Enrollment or another qualifying event. Ask the Marketplace in writing whether your specific event qualifies and what date coverage would begin before you terminate.

How COBRA can bridge a gap
Because an elected COBRA continuation can apply back to the loss-of-coverage date when required premiums are paid, some people elect it while waiting for a Marketplace plan to become effective. Confirm the retroactive terms, payment deadline, and cancellation procedure with the administrator. A bridge is useful only if the total cost and timing are acceptable.

Other coverage routes to check
Spouse or parent employer coverage
Loss of job-based coverage can trigger special enrollment in a spouse’s plan, and a child may have rights through a parent’s plan. Employer plans often impose a shorter request period than the Marketplace, so contact the plan’s benefits office immediately. Ask whether the new coverage starts the day after the old plan ends or on another date.
Medicaid and CHIP
Medicaid and CHIP can be available year-round to eligible applicants. Eligibility depends on household circumstances, income methodology, state, age, pregnancy, disability, and other factors. The Marketplace application can screen for these programs and route an eligible household to the appropriate agency.
State continuation laws
Some states extend continuation rights beyond federal COBRA or cover plans and employers outside federal COBRA’s scope. The rules, deadlines, duration, and plan types differ by state. Ask your state insurance department or plan administrator which continuation law applies; do not assume federal COBRA is the only option or that state rules match it.

Common mistakes and how to avoid them
- Counting from the wrong date: Use the actual coverage-ending date for Marketplace timing and the later-of coverage-ending date or notice date for the COBRA election period.
- Comparing payroll deductions with COBRA: Compare the full COBRA quote, including the employer share and possible 2% administration charge, with the Marketplace net premium after any approved savings.
- Using only post-layoff income: Marketplace assistance uses the estimated full-calendar-year tax-household income, including earnings before job loss.
- Assuming quitting or firing removes Marketplace eligibility: HealthCare.gov says loss of job-based coverage can qualify regardless of whether you quit or were fired.
- Ignoring network and formulary differences: Check each doctor, facility, prescription, dosage, prior authorization, and specialty pharmacy in the exact plan.
- Letting COBRA lapse while undecided: Missing the initial payment can jeopardize continuation. Follow the notice’s payment instructions while you make a decision.
- Stopping COBRA to force a Marketplace window: Voluntary early termination generally does not create a new Special Enrollment Period.
- Assuming family coverage must be all-or-nothing: Dependents may have independent COBRA rights, and different family members may qualify for different Marketplace, Medicaid, CHIP, or employer options.
- Forgetting employer subsidies: A severance contribution may lower COBRA temporarily; confirm its end date and conditions.
- Expecting the new plan to honor old spending: Ask whether deductibles, out-of-pocket amounts, authorizations, and continuity-of-care arrangements transfer. They often follow the new plan’s rules.
Documents and questions to keep together
Create one folder—digital or paper—with the termination letter, proof of the last covered day, COBRA election notice and envelope, premium quote, plan summary, Marketplace application and eligibility notice, selected plan documents, payment confirmations, medication list, and provider information.
When calling an administrator or insurer, ask:
- What exact date does coverage end for each person?
- What is the COBRA election deadline, and when is the first payment due?
- What is the monthly cost by coverage tier, and does it include the administrative charge?
- Can each dependent elect separately?
- What is the earliest effective date of the alternative plan?
- Are my doctors, hospitals, prescriptions, and current authorizations covered?
- Will any employer subsidy continue, and when does it stop?
- If I end COBRA, what event—if any—opens a Marketplace enrollment period?
When to get individualized help
Federal guidance cannot determine your household’s eligibility, subsidy, network, state continuation rights, or the correct termination date. Contact the former employer’s plan administrator for COBRA questions, HealthCare.gov or your state Marketplace for enrollment and financial-help decisions, your state insurance department for state continuation rules, and the insurer for network and prescription confirmation. Keep written responses and confirmation numbers, especially when a deadline is close.
The practical decision rule
Choose COBRA when preserving the existing plan is worth its full premium—often because care is already in progress, the network is unusually important, or a temporary employer subsidy makes the cost manageable. Choose a Marketplace plan when its approved net premium and cost-sharing are lower and its network, formulary, and effective date meet your needs. In either case, protect the deadlines first, verify the actual dates and costs, and do not end one form of coverage until the replacement is confirmed.
