Start by locating the mismatch: the servicer’s account, the recorded mortgage or deed of trust, a payoff or release, or a title search. Then send the current servicer or lienholder a written explanation with the loan number and your supporting records. If the public record is wrong or a release is missing, check both the lienholder’s filing history and the county recorder’s records. A title professional or local real-estate attorney can help when a sale, refinance, foreclosure, or ownership issue is involved.
The procedure depends on the lien type and local recording rules. An FHA-insured Home Equity Conversion Mortgage (HECM) is HUD’s reverse-mortgage program, but private reverse mortgages and other recorded liens follow different contacts and procedures.

First identify what is actually wrong
“A reverse mortgage is still on my title” can describe several different problems. Separating them prevents you from sending a county-recording question to a loan-servicing department, or a billing dispute to a recorder that cannot change a lender’s ledger.
| Where the mismatch appears | Typical issue | First contact | Useful proof |
|---|---|---|---|
| Servicer account | Balance, payoff figure, payment history, borrower or property data, or account status conflicts with your documents | Current mortgage servicer | Account statements, payoff statement, payment confirmations, correspondence |
| Recorded instrument | Mortgage or deed of trust contains a wrong name, legal description, amount, beneficiary, or recording reference | Lienholder or its recording counsel; county recorder for the public copy | Recorded instrument, recording number and date, later assignments, deed and legal description |
| Release or satisfaction | Loan was paid, but no satisfaction or reconveyance appears, or the filed release is defective | Lienholder or servicer, then county recorder to confirm receipt | Payoff quote, payment proof, release document, recorder’s index or certified copy |
| Title search or commitment | Title company reports an open lien, missing assignment, or broken chain of title | Title professional coordinating with the lienholder and recorder | Current title search or commitment, all recorded instruments and payoff records |
Keep these categories distinct. A recorder maintains the public index and recorded documents; it generally cannot correct a lender’s balance or create a release that the lienholder has not supplied.

How to prepare before contacting anyone
Build one dated file so every party can see the same chain of events. Gather:
- The recorded mortgage or deed of trust, including its recording number, book and page or instrument number, and recording date.
- Every recorded assignment, amendment, substitution of trustee, or other document that changes the lienholder or servicing chain.
- The latest account statement and payoff statement, including the date through which the payoff was calculated.
- Proof of payment, such as a wire confirmation, cashier’s-check receipt, escrow disbursement, or the servicer’s written payoff confirmation.
- A current title search or title commitment if a title company has identified the problem.
- Letters, emails, call notes, and delivery receipts showing when you asked for a correction.
Use the exact borrower name, property address, loan number, and legal description shown in the records. A title commitment is especially useful because it identifies the document that the title examiner believes is still affecting the property.
The Federal Deposit Insurance Corporation’s document list is aimed at failed-bank situations, not a universal checklist for every reverse mortgage. If the original lender failed, use the institution-specific successor or FDIC instructions; otherwise, follow the current lienholder’s requirements.

For a wrong balance, payoff amount, payment application, borrower detail, or account status, begin with the current servicer. The Consumer Financial Protection Bureau (CFPB) states: “If you think your mortgage servicer has made an error or you need information about your mortgage loan, you can call or write a letter to your servicer.” A call can identify the right department, but a written notice creates a clear record.
What to put in the written notice
- Identify the borrower, property address, and loan account number.
- Describe the specific error in plain language. State what the statement or payoff says, what your records show, and the date of the event.
- Attach copies, not irreplaceable originals, of the payoff statement, payment confirmation, account statement, or recorded document that supports your position.
- State the correction you are requesting, such as a revised payoff, corrected account balance, written confirmation that the loan is paid, or instructions for recording a release.
- Ask for a written response and retain a complete copy of what you sent.
Send the notice using the address or method the servicer designates for error notices or information requests, rather than assuming the general payment address is correct. Keep proof of delivery and note the date the servicer received it.

Response periods
Under current CFPB guidance, a servicer generally must respond to a notice of error or information request within 30 days, excluding weekends and legal public holidays. In specified cases it may notify you that it needs an additional 15 days, also excluding weekends and legal public holidays. These are response periods for the covered notice process, not a promise that a county recorder will update its index on the same schedule.
Regulation X §1024.35 describes the required content and categories for a notice of error. For a legal dispute about whether a particular problem falls within that rule, use the current controlling regulation or obtain legal advice.

How to remove a paid-off reverse mortgage lien from the public record
Paying the loan does not by itself guarantee that the public index immediately shows a release. The lienholder normally prepares and records a satisfaction, release, or reconveyance with the same recording official that recorded the original mortgage or deed of trust. The exact document name varies by state.
Follow both sides of the filing
- Ask the servicer or lienholder whether it prepared the satisfaction or release after receiving the payoff.
- Request the recording date and instrument number, or a copy of the document it says was filed.
- Search the county recorder’s index under the borrower, property, original lienholder, and successor names. Ask the recorder whether a document was received but rejected, misindexed, or recorded under a different name.
- If the recorder has no filing, ask the lienholder what corrected or replacement document it will submit and when.
- If a filing is defective, obtain the recorder’s rejection reason and give it to the lienholder or its recording agent so the replacement addresses the exact defect.
- After recording, obtain a certified copy or updated index entry and give it to the title company handling any transaction.
The FDIC’s lien-release guidance emphasizes recording the release with the same official that recorded the original mortgage or deed of trust. That guidance is most directly relevant to failed-bank matters; local law controls the form, fees, and acceptance process for other liens.
When the lienholder and recorder disagree
Ask the lienholder for documentary proof of submission, not only a verbal assurance. Ask the recorder for the intake or rejection information if available. A title professional can compare the legal description and recording references and identify whether the issue is a missing filing, a mis-indexed document, or a genuine chain-of-title break. Do not ask the recorder to alter a document’s substantive terms; that correction normally must come from the lienholder through a new or corrective instrument.

Special handling for FHA-insured HECM and HUD-held liens
HUD describes HECM as FHA’s reverse-mortgage program. HUD guidance applies to HECM and HUD-held liens, not automatically to every loan marketed as a reverse mortgage. A private lender’s lien may have a different servicer, release form, payment address, and escalation path.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsHUD-held Partial Claim releases
HUD’s Partial Claim servicing fact sheet dated February 25, 2026 describes a specific process: after payoff, HUD’s contractor prepares and files a satisfaction. The fact sheet asks borrowers to allow 15 business days for preparation and filing, with additional time possible for the county recorder’s processing. That estimate applies to the HUD-held Partial Claim process; it is not a universal deadline for every HECM, private reverse mortgage, or county.
That process calls for the payoff quote and payoff confirmation. Keep both, along with the recorded satisfaction once available. If the 15-business-day preparation and filing period has passed without a recording reference, contact the HUD servicing route or contractor identified for the lien and ask whether the document was prepared, submitted, rejected, or returned for correction.
Use the correct HUD servicing route
HUD publishes separate servicing information for HECM payoff and release matters. Identify whether the document is a HUD-held Partial Claim, an FHA-insured HECM serviced by a lender, or another lien before choosing a contact. Giving the wrong department a payment receipt can delay the request because the party holding the lien may not be the company that sends monthly statements.
Protect yourself from payoff fraud
HUD warns that it does not accept wire transfers to pay Secretary-held liens. Its official statement is: “FHA does not accept wire transfers to payoff Secretary-held Liens.” A Secretary-held lien payoff statement containing wiring instructions may be fraudulent.

Do not rely on an unsolicited email, changed wiring instruction, or phone number printed on an unexpected document. Verify the payoff through HUD’s official contact channels and request a corrected statement from the FHA Resource Center when the lien is Secretary-held. This warning is specific to Secretary-held liens; it does not mean every private lender uses the same payment method. Independently verify any lender’s instructions using a trusted statement or previously verified contact.

Servicer complaint
If the servicer does not correct the account or provide a meaningful response after the written notice period, submit a complaint to the CFPB with the notice, delivery proof, statements, payoff records, and response. The complaint file should show the precise correction you requested and the documents supporting it.
Housing counseling
CFPB directs borrowers toward HUD-approved housing counseling for help understanding reverse-mortgage servicing and available options. A counselor can help organize the account history and identify the appropriate HUD or lender route, but cannot change a county record or provide a legal determination.
Attorney or title professional
Consult a local real-estate attorney or title professional promptly when:
- A sale, refinance, estate transfer, or other closing is scheduled.
- The title commitment shows an unreleased lien that could prevent insurability.
- The legal description, borrower name, or lienholder chain is wrong.
- You have received foreclosure papers, a demand, or another legal deadline.
- The correction affects ownership, heirs, probate, or priority between liens.
State and county rules can control corrective instruments, recording fees, notice requirements, and deadlines. National agency guidance cannot substitute for advice about a particular property and jurisdiction.

Choose the resolution path by record location and urgency
| Situation | Primary path | Documents to lead with | Urgency |
|---|---|---|---|
| Servicer statement conflicts with your payment or payoff records | Written notice to the servicer; CFPB escalation if unresolved | Account statement, payoff quote, payment confirmation, dated correspondence | Routine unless a payoff or transaction is pending |
| Paid lien does not appear released in the county index | Confirm filing with lienholder and recorder; obtain a replacement or corrective filing if needed | Recorded original, payoff proof, release copy or recording reference, title search | Time-sensitive when a closing is scheduled |
| HUD-held Partial Claim paid but satisfaction is not visible | Use the HUD contractor or servicing route and ask for filing status | Payoff quote and payoff confirmation | Allow HUD’s stated 15 business days plus county processing time |
| Wrong name, legal description, or lienholder appears in the record | Have the lienholder and title professional coordinate a corrective instrument and chain-of-title review | Recorded instrument, assignments, deed, legal description, title commitment | Urgent if ownership or insurability is affected |
A practical call-and-letter checklist
Use this sequence so each conversation produces an actionable record:
- Write a one-sentence description of the mismatch and identify the document where it appears.
- Call the servicer or lienholder to confirm the responsible department and the approved written-notice address.
- Send the written notice with the loan identifier, requested correction, supporting copies, and delivery proof.
- Separately contact the recorder to verify whether a satisfaction or corrective instrument was received, recorded, rejected, or misindexed.
- Record every response with the employee’s name or department, date, promised action, and reference number.
- Set a follow-up date based on the applicable response period or the transaction deadline, whichever is earlier.
- Escalate to the CFPB, HUD-approved counselor, title professional, or attorney according to the type of unresolved problem.
Do not send original deeds, payment receipts, or certified documents unless the receiving institution specifically requires them and provides a secure method. Keep a complete chronological file until the corrected account statement and the public recording both match.
What a successful correction looks like
For a servicing error, success is a written explanation and corrected ledger or payoff statement that agrees with the payment evidence. For a public-record error, success is a properly recorded satisfaction, release, or corrective instrument with a recording reference that the title examiner can verify. For a title-chain problem, success may require several linked assignments or corrective filings rather than one deletion.
