Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →The affordable coverage choice after an interstate retirement depends first on two facts: whether you are under 65 or eligible for Medicare, and the destination state and ZIP code. Those facts determine which enrollment window you can use, which doctors and hospitals are available, what prescriptions cost, and whether your current plan can continue.
Start by confirming your coverage end date and move date. Then check the destination network and medications, estimate a full year of premiums and cost sharing, verify the enrollment deadline and effective date, and document every confirmation before you leave. A low monthly premium is not affordable if your specialists are out of network, your prescriptions are excluded, or coverage starts after your old plan ends.

1. Identify the coverage path before comparing plans
Do not compare Marketplace and Medicare plans as though they follow the same rules. Your age, Medicare eligibility, household income, current coverage and destination determine the available paths.
If you are under 65 and losing employer coverage
A retiree who loses job-based insurance can generally apply for an individual plan through the Health Insurance Marketplace. Household income and size affect eligibility for premium tax credits, reduced out-of-pocket costs and Medicaid. The savings are based on the applicable household and income rules, not simply on being retired.

HealthCare.gov says a Special Enrollment Period related to loss of job-based coverage may be available from 60 days before to 60 days after separation from work. Treat that as a deadline to verify, not an automatic approval: confirm that your event qualifies, submit any requested documents and check the plan’s effective date. A plan can be selected on time yet still begin later than you expect.
If you are eligible for or already enrolled in Medicare
Medicare choices are different. Original Medicare generally allows you to use any doctor or hospital that takes Medicare anywhere in the United States. Medicare Advantage plans can limit non-emergency care to a network and service area. Moving to a new state can therefore require a plan change even when your Medicare eligibility has not changed.
Check whether you have Part A and Part B, whether you need a Part D prescription plan, and whether you carry Medigap. If you are approaching 65, coordinate the move with your initial Medicare enrollment dates rather than waiting for an employer plan to end.
If you have retiree or former-employer coverage
Employer or union retiree coverage follows its own contract. It may continue nationwide, use a national network, coordinate with Medicare or end when you move. Ask the plan administrator for the written rule that applies to your destination. Voluntarily dropping retiree coverage can affect access to a Marketplace Special Enrollment Period and the savings you could receive, so verify the consequences before canceling.
2. Lock down the destination details
Insurers price and administer coverage by location. Write down the new street address, county and ZIP code, the lease or closing date, and the date you will physically move. A temporary stay, vacation or trip for medical treatment is not the same as moving to a new home for Marketplace enrollment purposes; HealthCare.gov says those temporary situations do not qualify as a move-based Special Enrollment Period.
Ask your current plan whether your existing address is inside its service area and whether the destination is covered. A plan directory may show a provider in the right city but not the exact location or accepting status. Use the destination ZIP code when obtaining quotes and checking networks, because a plan offered in one county may not be offered in the next.

3. Compare the four main coverage paths
| Path | Geographic access | Cost structure | Questions to answer |
|---|---|---|---|
| Original Medicare, optionally with Part D and Medigap | Generally any U.S. doctor or hospital that takes Medicare | Part B premium plus Medicare cost sharing; Medigap adds a policy- and location-dependent premium; Original Medicare alone has no annual out-of-pocket maximum | Do desired providers accept Medicare? Is a separate drug plan needed? Which Medigap policies and purchase rights apply in the new state? |
| Medicare Advantage | Non-emergency care can depend on the plan network and service area; out-of-network care may cost more | Part B premium plus any plan premium, deductibles and copays; the plan has an annual limit for covered Medicare services | Does the plan serve the destination? Are doctors, hospitals, pharmacies and drugs covered? Are referrals or prior authorization required? |
| Marketplace individual plan, usually before Medicare eligibility | Plan and provider network are location-specific | Premium and cost sharing vary; income and household size affect Marketplace savings, and Medicaid may be available | Does the move or loss of coverage create a Special Enrollment Period? What is the effective date? Are providers and prescriptions included? |
| Retiree or former-employer coverage | Depends on the employer plan’s contract and network | Employer-specific premiums and cost sharing | Does it continue after the move? How does it coordinate with Medicare? What happens if you drop it? |
These are comparison categories, not a universal recommendation. Health needs, medications, preferred clinicians, income, state law and the current employer plan can change the result.

4. Calculate the full-year cost, not just the advertised premium
Build a 12-month estimate for each realistic option. Include:
- Every monthly premium, including the Medicare Part B premium and any plan, Part D, Medigap or retiree premium.
- The annual medical deductible and prescription deductible.
- Copays and coinsurance for primary care, specialists, emergency care, hospital stays, imaging, therapy and recurring treatments.
- Prescription costs at the pharmacies you will actually use, including different tiers and any quantity limits.
- The plan’s annual out-of-pocket maximum for covered services, where one applies.
- Travel or out-of-network exposure if your care will occur outside the destination.
Original Medicare by itself has no yearly out-of-pocket maximum. Medicare Advantage plans do have a yearly limit for covered Medicare services, but premiums and cost sharing still vary by plan and location. Do not treat that limit as a cap on premiums, non-covered services or every prescription cost.
Known 2026 Medicare figures
For 2026, Medicare.gov lists Part A premiums of $311 or $565 per month for people who do not qualify for premium-free Part A; most people do qualify for premium-free Part A based on work history. The applicable amount depends on the person’s work record.
For a Part B-covered service, a beneficiary generally pays 20% of the Medicare-approved amount after the Part B deductible when the provider accepts assignment. That is a general cost-sharing rule, not a prediction of total annual spending. Your actual exposure depends on services, providers and supplemental coverage.
Use scenarios instead of one optimistic estimate
Create at least three budgets: routine care, a year with a major outpatient episode, and a year with hospitalization or intensive treatment. Use the plan’s published deductible, copays, coinsurance and out-of-pocket limit for each scenario. For prescriptions, price every medication for a full year at the intended pharmacy and test whether a formulary change or coverage phase alters the total.

5. Verify doctors, facilities and treatment before enrolling
Make a written list of people and services you cannot easily replace:
- Primary-care clinician and every specialist.
- Preferred hospital, emergency department and ambulatory surgery center.
- Laboratories, imaging centers, physical or occupational therapy and home-health agencies.
- Infusion sites, dialysis centers, behavioral-health clinicians and other recurring treatment locations.
- Pharmacies, mail-order services and specialty pharmacies.
Check each item in the current directory using the destination ZIP code, then call the provider’s billing or insurance office. Ask whether the provider accepts the specific plan and network, whether the listed location is correct, and whether new patients are being accepted. Directory information can lag behind contracts, and a provider who accepts Medicare is not necessarily in a particular Medicare Advantage network.
Confirm treatment rules
Ask the insurer whether referrals, prior authorization, step therapy, site-of-care rules or quantity limits apply. Obtain written instructions for transferring an active authorization, scheduled surgery, infusion series or durable medical equipment order. Request copies of recent clinical notes, medication lists, imaging reports and lab results so the new clinician can continue care without an avoidable delay.

6. Check prescriptions in detail
For each medication, record the exact name, strength, dosage, quantity and refill frequency. Verify the plan’s formulary, tier, preferred pharmacy and mail-order terms. Ask whether the drug needs prior authorization, has a quantity limit or requires a specialty pharmacy. A plan that covers a drug in one state or year may have a different formulary after a move or renewal.
Plan the refill around the move. Obtain an adequate supply that your current plan permits, transfer prescriptions to the destination pharmacy, and keep the prescriber’s contact information available. If a medication is not covered, ask about the plan’s exception or appeal process before the first refill is due.
7. Understand Original Medicare, Part D and Medigap together
Original Medicare
Original Medicare’s broad provider reach is often valuable for retirees who travel or split time between states. Medicare.gov states: “You can use any doctor or hospital that takes Medicare, anywhere in the U.S.” Confirm that each desired provider accepts Medicare and, when relevant, accepts assignment. Original Medicare alone does not set an annual out-of-pocket ceiling, so budget for deductibles and coinsurance or consider supplemental coverage.

Part D
Original Medicare does not include routine outpatient prescription coverage. If you need drug coverage, compare Part D plans available at the destination. A move can create a plan-change opportunity, but the permitted timing and effective date depend on your circumstances. Review the formulary and pharmacy network every year rather than assuming the current plan follows you unchanged.

Medigap policies help pay some Original Medicare cost sharing. If you remain on Original Medicare, you can generally keep an existing Medigap policy after moving. Keeping it is different from replacing it: a new policy can cost more or require medical questions when you are outside a protected enrollment right.
Do not cancel an existing Medigap policy until the replacement is accepted, the effective date is confirmed and you understand the state’s rules. A move does not automatically create an unrestricted right to buy every Medigap policy. Contact the destination state’s insurance department or a state-licensed counseling service to verify available plans and guaranteed-issue rights for your exact situation.
8. Evaluate Medicare Advantage service-area rules
Medicare Advantage plans must serve the county or service area where you live. Non-emergency care may be limited to network providers, and out-of-network care can cost more or be unavailable except under the plan’s rules. Before moving, ask the plan what happens when you permanently change your address and whether the plan will disenroll you or provide a special election period.
Compare the destination plans using the same doctors, hospital, pharmacy and medications. Check referral and prior-authorization requirements, urgent and emergency coverage while traveling, telehealth availability and the annual maximum for covered Medicare services. A plan with a low premium can be a poor value if your specialist or hospital is outside the network.

If you are under 65, a move to a new home in a new ZIP code or county may qualify for a Marketplace Special Enrollment Period when other requirements are met. Loss of job-based coverage can also trigger a Special Enrollment Period, with HealthCare.gov guidance describing a window from 60 days before to 60 days after separation. Verify your eligibility, provide proof of the prior coverage or move if requested, and select an effective date that prevents a gap.

Marketplace savings depend on projected household income and size. Recalculate when retirement changes wages, pension income, withdrawals or other taxable income. Medicaid eligibility can also depend on income and state rules. Plans purchased outside the Marketplace do not qualify for Marketplace premium tax credits or other income-based savings.
10. Prevent a coverage gap during the move
- Record the old plan’s end date. Obtain written confirmation from the employer, insurer or Marketplace plan. Ask whether coverage ends on the last workday, the last day of that month or another date.
- Confirm your move date and destination address. Keep proof such as a lease, closing document or utility record if an insurer requests it.
- Verify the enrollment window. Write down the first and last day to apply, the documents required and the date the new plan can begin.
- Apply early. Submit the application before the deadline and save the confirmation number, submitted forms and notices.
- Confirm the effective date in writing. Do not rely solely on an application screen or an agent’s verbal statement.
- Pay the first premium on time. Enrollment is not complete until the insurer receives any required initial payment.
- Coordinate medications and care. Transfer prescriptions, send records and schedule appointments after the network and start date are confirmed.
- Keep a transition file. Store plan documents, provider confirmations, authorization numbers, premium receipts and every call’s date, department and representative.
11. Common mistakes and how to correct them
Choosing the lowest premium
Correct the mistake by calculating annual premiums, cost sharing, drug spending and the applicable out-of-pocket limit. Include the cost of a provider outside the network.
Assuming Medicare Advantage travels like Original Medicare
Correct it by checking the plan’s service area, network and travel rules before the move. Ask what happens to ongoing treatment and prescriptions at the new address.
Dropping Medigap before a replacement is secure
Correct it by keeping the current policy until the new policy is issued and effective, unless a qualified counselor confirms a different protected process.
Using a temporary address to obtain a plan
Correct it by applying with the address where you actually live. A vacation or move solely for medical treatment does not establish the move-based Marketplace Special Enrollment Period described by HealthCare.gov.
Relying on an outdated provider directory
Correct it by calling the provider and insurer and documenting the confirmation for the exact plan, location and network.
Ignoring income changes in retirement
Correct it by updating projected household income and household size when applying for Marketplace coverage and when circumstances change. Incorrect estimates can change savings and later tax reconciliation.
12. A practical decision sequence
- Classify eligibility: under 65 and losing employer coverage, newly Medicare-eligible, already on Medicare, or covered by a retiree plan.
- Map the destination: state, county, ZIP code, move date and the plan service area.
- List care needs: clinicians, facilities, pharmacies, drugs and recurring treatments.
- Filter viable plans: remove options that exclude a required provider, medication or treatment.
- Price a full year: premiums, deductibles, copays, coinsurance, prescriptions and annual limits.
- Check protections: Special Enrollment Period rules, Medicare election dates, Medigap rights and retiree-plan consequences.
- Confirm implementation: effective date, first premium, provider acceptance, authorizations and prescription transfers.
13. Questions to ask before you sign up
- What is the plan’s exact service area for my new ZIP code?
- Are my primary doctor, specialists, hospital and treatment facilities in network or accepting Medicare?
- Which pharmacies and mail-order services are preferred?
- Are all medications covered, and what authorization, quantity or step-therapy rules apply?
- What are the deductible, copays, coinsurance and annual out-of-pocket limit?
- What is the premium for every component, including Part B, Part D, Medigap or retiree coverage?
- When does coverage end under my current plan, and when does the new plan become effective?
- What documents must I submit, and when is the first payment due?
- If I change or drop coverage, what enrollment or guaranteed-issue rights do I lose?
Free Medicare counseling is available through the State Health Insurance Assistance Program, but the appropriate office and services vary by state. Verify the destination state’s program rather than relying on a generic contact.
