How to File a Health Insurance Claim When Your Policy Has a Deductible

You usually do not have to file a health insurance claim yourself just because you have not met your deductible. A claim is a request for payment that you or your provider sends to the insurer. In most in-network situations, the provider submits it. The insurer then processes the claim under your policy, and the deductible may leave you responsible for some or all of the plan’s allowed amount.

First ask the provider’s billing office whether it filed the claim. Then check your plan’s instructions, wait for the explanation of benefits (EOB), and compare that EOB with the provider’s bill. If your plan requires you to submit the claim, use the insurer’s current form and deadline. Original Medicare has a specific member-filing process that is different from the rules for commercial insurance.

What a claim and a deductible actually mean

A claim is a payment request

HealthCare.gov defines a claim as “A request for payment that you or your health care provider submits to your health insurer when you get items or services you think are covered.” Filing a claim is therefore an administrative step. It does not mean the insurer will pay the entire bill, and it does not mean you must pay before the insurer reviews the service.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The provider can submit the request electronically or on paper. A member may need to submit it when the provider does not bill the plan, when the service was received out of network, or when the plan’s terms specifically require member filing. The exact form, submission channel and deadline depend on the insurer and policy.

A deductible applies to covered services subject to it

HealthCare.gov describes a deductible as the amount you pay for covered health care before the plan starts paying for services subject to that deductible. That definition does not mean every service is subject to one deductible or that the provider’s full advertised charge is the amount used.

  • Some services can be covered before the deductible is met.
  • A plan can have separate deductibles for categories such as prescription drugs or out-of-network care.
  • Family coverage can track both an individual deductible and a family deductible.
  • Services that are excluded, not medically necessary under the policy, or otherwise outside coverage are handled under different rules.

Consequently, “I have not met my deductible” does not, by itself, tell you whether a particular service is covered or exactly what you owe. Your summary of benefits, certificate of coverage and member portal contain the controlling terms.

Do I have to file my own health insurance claim?

Usually, no. Ask the provider’s billing office whether it submitted the claim and, if so, request the submission date and claim number or reference number. In-network providers commonly agree to bill the plan as part of their network contract. Out-of-network providers may or may not submit claims, depending on the plan and the provider’s practice.

Do not assume that a bill arriving at your home means no claim was filed. Providers often send a statement while the claim is still pending, or after the insurer has assigned the allowed amount to your deductible. Conversely, do not assume a provider filed simply because it has your insurance card. Confirm the status with both parties if no EOB appears.

Step-by-step: filing and checking a claim

1. Confirm who is expected to submit it

  1. Call or message the provider’s billing office using the contact information on the statement.
  2. Ask whether the claim was sent to your insurer, the date it was sent, and the claim reference number if available.
  3. Ask which billing entity submitted it if the practice uses a separate laboratory, facility or physician group.
  4. If the office has not submitted the claim, ask whether it will do so and what information it needs from you.

Write down the representative’s name, the date and time, and any promised action. If the provider refuses or cannot submit the claim, contact the insurer before preparing a member claim so you do not send duplicate or conflicting requests.

2. Check your plan’s current instructions

Use the member portal, policy documents and the customer-service number on your insurance card. Ask the insurer to explain the rules for the specific service and date of care:

  • Was the provider in network on the date of service?
  • Is the service covered, and is prior authorization or a referral relevant?
  • Does the service use the general deductible or a separate deductible?
  • How much of the applicable individual or family deductible remains?
  • Must the member submit the claim, or will the provider submit it?
  • What form, attachments, electronic portal or mailing address must be used?
  • What is the member filing deadline?

Ask the representative to identify the policy section supporting the answer. Commercial plans do not share one universal claim form, address or deadline.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

3. Gather the required documents

If the insurer requires a member-submitted claim, follow its current instructions exactly. Typical materials can include a completed claim form, an itemized bill, proof of payment when relevant, clinical records, a referral or authorization, and an explanation of why the member is filing. The plan may ask for different or additional documents.

Keep a complete copy of everything you send. Save the submission confirmation, postal tracking or portal receipt, the date submitted, and every letter or secure message from the insurer. Never mail your only original receipt or medical record unless the plan explicitly requires an original and you have retained a copy.

4. Submit through the specified channel

Use the insurer’s member portal, designated email, fax or mailing address shown in the current instructions. Check that the patient name, member identification number, group number, provider information, dates of service and diagnosis or procedure information match the itemized bill. A wrong member number or missing page can delay processing.

Do not use the Original Medicare form or address for a commercial plan. Medicare’s CMS-1490S process is a separate example described below.

5. Read the explanation of benefits

After processing, the insurer sends an EOB or posts one in the member portal. CMS’s consumer guidance states, “An explanation of benefits isn’t a bill.” It is the insurer’s explanation of how the claim was adjudicated, not a demand for payment.

Check these fields against your records:

  • Patient and member information
  • Provider and facility name
  • Date of service and service description
  • Claim number and processing status
  • Provider charge
  • Plan allowed charge
  • Amount paid by the insurer
  • Amount assigned to deductible, copayment or coinsurance
  • Patient balance shown by the plan
  • Remark or denial codes and their explanations

The allowed charge is the amount the plan recognizes under its rules; it can differ from the provider’s listed charge. The EOB shows how that allowed amount was divided between the insurer and your cost sharing.

6. Reconcile the EOB with the provider’s bill

Compare the service lines, dates and amounts line by line. CMS advises that a provider bill should not exceed the patient balance shown on the EOB for the processed claim. If the bill is higher, ask the provider to place the account on hold and review its coding and ledger. Call the insurer about any adjudication discrepancy, and keep notes from both conversations.

Document or amount What it represents What to do
Provider charge The amount the provider originally billed Do not assume this is the amount used to calculate your deductible.
Allowed charge The amount recognized under the plan’s pricing rules Use this figure when checking deductible and other cost sharing.
Insurer payment The portion the plan paid Confirm that it matches the coverage and network status shown on the EOB.
Patient balance on EOB The amount the plan says may be assigned to you Compare it with the provider’s bill; ask for correction if the bill is higher.

Why insurance applied the claim to your deductible

If the service is covered and subject to a deductible, the plan may apply some or all of the allowed charge to the portion of your deductible that remains. The insurer can process the claim correctly even when its payment is zero. The claim still establishes the allowed amount and updates your deductible accumulator.

That result is different from a denial. An EOB should distinguish an amount assigned to deductible from a service that is excluded, not authorized, not medically necessary under the plan’s rules, or otherwise denied. Read the remark code and its explanation rather than treating every $0 insurer payment as a rejection.

Out-of-network care can add another layer: the plan may use a separate out-of-network deductible, pay a different percentage, or leave you exposed to balance billing where permitted. Ask the insurer how network status affects this exact claim and whether the provider has agreed to accept the plan’s allowed amount.

How to submit an out-of-network health insurance claim

Start by asking the out-of-network provider whether it will bill your plan. If it will, request the claim number after submission. If it will not, contact the insurer for the member claim form and filing deadline before sending documents.

Original Medicare: a separate filing process

The broad rule that a provider usually submits a claim should not be used to describe every Medicare situation. This section applies to Original Medicare; Medicare Advantage plans have their own member services and claim instructions.

When a patient may need to file

Medicare says patients generally need to file only in rare situations. If a provider has not filed, first ask that provider to submit the claim. If it still does not, contact Medicare and ask what action is required and which deadline applies.

CMS-1490S and supporting documents

When Medicare instructs you to file, use form CMS-1490S. Medicare identifies these materials:

  • The completed CMS-1490S form
  • An itemized bill
  • A letter explaining why you are filing the claim
  • Relevant supporting records requested for the claim

Mail the packet to the state-specific Medicare Administrative Contractor address printed in the form’s instructions. Do not substitute a commercial insurer’s form or mailing address.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Medicare’s general time limit

Medicare states a general filing limit of 12 months, or one full calendar year, after the service date, unless an exception applies. Confirm the applicable deadline with Medicare because an exception or a particular claim circumstance can change the result. Retain a copy of the packet and proof of mailing.

What to do when a claim is denied or processed incorrectly

Distinguish a deductible allocation from a denial

An amount assigned to your deductible means the plan processed the service and allocated the allowed charge to your cost sharing. A denial means the plan decided not to cover or pay the service for a stated reason. The remedy depends on the reason shown on the EOB or denial notice.

Use the plan’s appeal instructions

  1. Read the denial notice for the precise reason and filing deadline.
  2. Check the policy language cited by the insurer.
  3. Ask the provider for corrected billing, medical records or a supporting statement when those materials address the stated reason.
  4. Submit the internal appeal through the method and address in the notice.
  5. Keep the appeal request, attachments, EOB, denial letter and notes of every call.

Many plans offer internal review and, where applicable, external review. Procedures and deadlines vary by coverage type and current notice, so rely on the instructions attached to your determination rather than an old general timetable.

Do not confuse an insurance appeal with a medical-bill dispute

CMS’s medical-bill guidance distinguishes the federal No Surprises Act dispute route from an insurance claim appeal. People who used insurance generally do not qualify for that particular bill-dispute process. A bill that reflects an unmet deductible is not, by itself, a No Surprises Act violation. If the insurer misprocessed a covered service, use the plan’s appeal process; if you believe a specific surprise-billing protection applies, follow the separate complaint or dispute instructions for that situation.

Common problems and practical fixes

No EOB appears

Confirm with the provider whether and where it sent the claim. Then ask the insurer to search by patient, provider and date of service. Verify that the insurer has your current address and that the provider used the correct member identification number. If neither party can locate the claim, ask the insurer whether a member claim is required and obtain the deadline in writing or through a secure message.

The provider says the insurer paid, but the bill is still high

Request the EOB and compare each line. A large patient balance can result when the allowed amount was assigned to a deductible or coinsurance. If the provider’s invoice exceeds the EOB patient balance, ask the billing office to correct it and escalate the discrepancy to the insurer.

The claim has the wrong date, provider or service

Ask the provider to submit a corrected claim and tell the insurer that a corrected submission is coming. Keep the original EOB, corrected claim reference and all correspondence so the account history is clear.

The provider asks for full payment before the claim is processed

Ask whether the amount is an estimate or a final bill, and request an itemized statement. Tell the provider that the claim is pending and ask whether it can wait for the EOB. You can still owe the allowed amount after processing if the deductible applies, but an estimate should not be presented as the insurer’s final patient responsibility.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The insurer says the service is not covered

Read the exclusion or limitation cited in the denial. If you believe the service meets the plan’s coverage terms, appeal by the deadline and include records that address the stated reason. If the service is genuinely excluded, a deductible does not make it covered.

Records to keep until the account is settled

  • Itemized provider bill and any corrected bills
  • Claim form and every attachment submitted
  • Portal confirmation, fax confirmation or postal tracking
  • EOB and denial notices
  • Receipts and payment records
  • Names, dates and reference numbers from calls
  • Appeal requests, supporting records and decisions

Keep these records until the claim is fully resolved, any appeal period has passed and the provider confirms the account balance. A spreadsheet or dated folder can help you track multiple claims without mixing service dates.

Decision guide: who files and what happens next

Situation Likely next step Important qualification
In-network provider, routine covered care Provider submits; wait for the EOB. Confirm submission if no EOB appears.
Provider will not submit Ask insurer whether member filing is required and obtain its form and deadline. Requirements vary by policy.
Out-of-network care Determine whether provider or member submits and which deductible applies. Network, allowed-charge and balance-billing rules differ.
Original Medicare and provider did not file Ask provider to file; if necessary, follow Medicare’s CMS-1490S process. The general limit is 12 months after service unless an exception applies.
EOB and bill disagree Ask provider to reconcile and insurer to review adjudication. The provider bill should not exceed the EOB patient balance for the processed claim.
Payment denied Follow the denial notice’s appeal instructions. Deadlines and escalation rights depend on the coverage and notice.
