How to estimate the cost of senior housing and plan for ongoing fees

The reliable way to estimate senior-housing cost is to price the entire care path, not just an advertised monthly rent. Identify the setting, establish a local benchmark, obtain an all-in written quote, separate fixed charges from care-related variables, and model at least five years including a possible move to higher care. National medians are useful starting points, but location, services, contract terms and changing care needs determine the bill your family will actually pay.

Start with the care setting, because “senior housing” is not one price

A person who needs meals and activities has a different cost profile from someone who needs help bathing, secured dementia care or 24-hour skilled nursing. Before requesting prices, write down the services needed now and the changes that are reasonably foreseeable. Include whether the resident needs medication administration, help with activities of daily living (ADLs), incontinence support, transfers, nighttime supervision, therapy or a secured environment.

How the main settings differ

Setting Typical service purpose Charges to expect Planning question
Independent living Housing, meals, activities and convenience services for people who remain largely self-sufficient Apartment or cottage rent, meals, utilities, housekeeping, transportation and optional services What happens if the resident later needs assisted living or memory care?
Assisted living Housing plus help with ADLs, medication routines, meals and supervision Base rent plus a care tier, points or task charges; medication, supplies and escorts may be separate Which tasks trigger a higher tier, and how often can the tier be reassessed?
Memory care Structured, secured care for dementia and related cognitive impairment Room and board, secured-program fee, care level, medication management, incontinence supplies and possible one-to-one supervision Is the quoted rate for a secured unit, and what care is excluded?
Nursing home Skilled nursing, rehabilitation or continuous medical oversight Room rate (semi-private or private), therapy and other covered or non-covered services Is the need short-term rehabilitation, long-term custodial care or both?
Home care Care delivered in the resident’s home rather than a community Hourly caregiver rate, minimum shift, overtime, agency fees and travel or holiday premiums How many hours are needed each week, and who covers gaps when a caregiver is absent?
Continuing-care retirement community (CCRC) A campus offering multiple levels of care under a rental or entrance-fee contract Entrance fee, monthly service fee, care charges and contract-specific refunds or guarantees What care is included later, and what happens if assets run low?

These categories are not interchangeable. A national assisted-living median cannot be used as a memory-care quote, and a home-care hourly rate cannot be compared with a facility bill without converting it to the same hours and services.

Current national benchmarks—and why they are only a starting point

The following figures provide orientation for a U.S. household. They come from different surveys and definitions, so do not average them together. A Place for Mom’s figures primarily reflect actual partner-community move-in costs, while CareScout reports service-rate medians. Both can differ materially from a community’s written proposal.

Service or setting Published median Source and qualification
Independent living $3,200 per month A Place for Mom, 2025 move-in data, national median reported in its 2026 report
Assisted living $5,419 per month A Place for Mom, 2025 move-in data, national median
Memory care $6,690 per month A Place for Mom, 2025 move-in data, national median
Home care $34 per hour A Place for Mom, 2026 report, national median starting rate based on January 9, 2026 data
Assisted living $6,200 per month CareScout, 2025 Cost of Care Survey, national median service rate
Semi-private nursing-home room $9,581 per month CareScout, 2025 Cost of Care Survey, national median
Private nursing-home room $10,798 per month CareScout, 2025 Cost of Care Survey, national median
Non-medical caregiver service $80,080 per year CareScout, 2025; $35 per hour, 44 hours per week, 52 weeks

For another view of annual spending, the Genworth and CareScout 2024 survey reported $70,800 per year for assisted living, $75,504 for homemaker services, $111,325 for semi-private nursing-home care and $127,750 for private nursing-home care. It also reported a $26,000 annual median for adult day care. The survey contacted more than 140,000 providers and completed more than 15,000 surveys from July through December 2024. Its reported year-over-year changes were 10% for assisted living, 10% for homemaker services, 7% for semi-private nursing-home care and 9% for private nursing-home care. Those are survey results, not a guaranteed increase for any particular community.

CareScout cautions that the cost for a particular provider may differ significantly from the median for similar providers in the same location. Treat every national number as a screening tool: it tells you whether a quote is in the general range, not whether the quote is complete or affordable.

A step-by-step method for building a local estimate

1. Describe the care scenario in writing

Create a one-page care profile before calling communities. Record the resident’s mobility, continence, cognition, medication routine, ability to eat and dress independently, fall history, need for nighttime checks and expected transportation. Distinguish current needs from a possible future need. A community can price a profile consistently only when every provider receives the same facts.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

2. Set a local base rate

Use CareScout’s calculator or state-level data for an initial local baseline, then contact at least three communities or agencies. Ask each provider to quote the same room size and care profile. Keep the national medians in a separate column so you can see whether a local quote differs because of geography, services or a different definition of “starting price.”

3. Demand an all-in written quote

Request a fee schedule, residency agreement and a sample monthly bill. Ask the sales representative to identify every recurring, one-time and usage-based charge. A verbal “starting at” number is not a budget. Have the provider write the monthly total for the specified resident, the amount due at move-in and the formula used when care changes.

4. Separate fixed and variable charges

Charge category Examples How to model it
Fixed monthly Base rent, meal plan, utilities, standard housekeeping, activities and basic transportation Enter the contractual amount and the stated increase date
Care-related ADL tier, care points, medication administration, nurse visits, escorts and transfers Price the current tier and at least one higher tier
Consumable or usage-based Incontinence supplies, laundry beyond an allowance, therapy, salon services and extra transportation Use a realistic monthly quantity, then test a higher-use month
Optional or household Pet fees, internet upgrades, larger floor plans, guest meals and parking Include only if the household will actually use them, but record the one-time setup cost
One-time Application, community fee, deposit, assessment, move-in coordination and furniture removal Place in the first-year cash-flow column rather than hiding it in monthly rent

5. Model time, not just the first invoice

For a five-year baseline, multiply the all-in monthly amount by 60 and add any entrance fee or deposit. For example, an assisted-living quote equal to A Place for Mom’s $5,419 monthly median would total $325,140 over 60 months before increases, care changes or one-time fees. The $6,690 memory-care median would total $401,400 on the same basis. These calculations are not forecasts; they show how quickly a recurring charge accumulates.

Build three cases in a spreadsheet: a low case with the current care level, an expected case that uses the provider’s stated annual increase and a likely care transition, and a high case with a higher care tier, more paid services or a move to nursing care. Use your own documented assumptions rather than presenting an unsupported inflation rate as fact. Show the assumption beside each year so another family member can audit the model.

6. Price the likely transition

Ask the community to quote independent living to assisted living, assisted living to memory care and any nursing arrangement it uses. A contract may guarantee access to a higher-care unit without locking in its price. Ask whether the resident keeps the same apartment, pays a second move-in charge, waits for an opening or must leave the campus if the required care is unavailable.

7. Check funding before choosing a setting

Medicare is not a general source of payment for ongoing custodial room and board. Medicaid rules depend on state eligibility, the person’s finances and the services covered. The U.S. Government Accountability Office reported that, as of March 2025, 44 states covered some assisted-living services through Medicaid programs and 29 states used home- and community-based-services waivers for assisted-living services. Coverage may apply to care services while excluding rent, meals or other charges, so request a written determination from the state program rather than assuming the entire bill is covered.

What an advertised monthly price may leave out

A Place for Mom reports that its published starting prices generally exclude additional care, pet and optional charges, and that larger floor plans carry premiums. That limitation is why two communities with the same “starting at” rent can produce very different monthly bills.

Questions to ask about inclusions

  • Are meals, snacks, utilities, internet, housekeeping and laundry included, and is there a limit?
  • Is medication administration included in the base rate, or charged per medication, pass or visit?
  • What supplies are excluded, including incontinence products, gloves, wound supplies and mobility equipment?
  • Are nurse assessments, wellness checks, escorts to meals and transportation billed separately?
  • How are care tiers or points assigned, who reassesses them and how much notice precedes a price change?
  • Is there an annual increase, a periodic market adjustment or a right to raise fees when costs change?
  • What deposit, community fee, application fee or end-of-lease charges are due, and when are they refundable?

Compare the provider’s sample bill with your own household budget. If a line is described as “as needed,” obtain the unit price and a realistic usage estimate. If the provider will not disclose the formula, treat the uncertainty as a financial risk and keep a larger reserve.

Comparing home care with facility care

Home care can look cheaper when quoted hourly, but the weekly schedule determines the total. CareScout’s 2025 figure of $80,080 per year assumes a $35 hourly non-medical caregiver rate for 44 hours each week over 52 weeks. That is a specific utilization example, not a universal home-care budget.

Care arrangement Illustrative calculation using published median What the calculation does not include
Home care, 20 hours per week $34 × 20 × 52 = $35,360 per year Housing, meals, agency minimums, payroll taxes, overtime, backup coverage and medical care
Home care, 44 hours per week $35 × 44 × 52 = $80,080 per year Non-medical rate only; housing and uncovered services remain the household’s responsibility
Assisted living $5,419 × 12 = $65,028 per year before additions Extra care, supplies, larger unit, medication and annual increases may be separate
Memory care $6,690 × 12 = $80,280 per year before additions Higher supervision, incontinence supplies, one-to-one care and future increases may be separate

Home care also places operational work on the family: recruiting or using an agency, covering cancellations, adapting the home and coordinating meals and transportation. A facility bill may consolidate those services but can rise sharply when care needs cross a tier. Compare equivalent hours and services, not the hourly rate against a monthly rent.

How to evaluate a CCRC entrance-fee contract

CCRCs combine housing with a promised or available path to higher care, but the entrance fee and contract language determine the financial exposure. The National Investment Center reported that average CCRC entrance fees across 99 markets rose from about $400,000 in 2020 to more than $480,000 in 2025, a 22.3% cumulative increase.

NIC’s 2025 analysis also reported average independent-living asking rent of $4,325 per month at entrance-fee CCRCs. Rental CCRCs reported asking rates of $6,459 per month for assisted living and $8,238 per month for memory care. These are asking rates and may exclude discounts; they are not substitutes for an individual contract proposal.

Compare these contract terms side by side

Contract feature What to verify Why it matters
Entrance fee Amount, due date, payment source and whether it changes by unit It can consume liquid assets before monthly care begins
Refund formula Refund percentage, beneficiary rights, timing and deductions “Refundable” can mean a limited amount payable only after resale or death
Contract type Type of life-care, modified, fee-for-service or rental arrangement Determines how much future care is prepaid versus charged later
Monthly fee What is included, the increase history and the authority to raise fees A low entry fee can be offset by higher recurring charges
Future-care pricing Rates for assisted living, memory care and nursing, including medication and therapy Shows whether the promised continuum is financially usable
Transfer rights Priority, wait-list rules, apartment changes and move costs A guarantee of access is not necessarily a guarantee of an immediate unit or price
Health underwriting Initial qualification, later reassessment and consequences of a change in health Eligibility can affect both admission and future options
Financial reserves Required income, assets, guarantors and rules if assets are depleted Reveals the risk of losing access when the household can no longer pay

Have an elder-law attorney or qualified financial professional review the contract before paying an entrance fee. Ask the CCRC for audited financial statements, reserve-fund information, fee-increase history and its written policy for residents who outlive assets.

Build a five-year cash-flow worksheet

Use one row per month or year and keep the following fields separate:

  1. Record entrance fees, deposits and move-in costs in the month they are due.
  2. Enter base rent or the home-care schedule.
  3. Add meals, utilities, housekeeping, transportation, medication and supplies.
  4. Add the current care tier and a second row for a higher tier.
  5. Apply the provider’s contractual increase on the date stated in the agreement.
  6. Insert a transition month for a move to memory care or nursing care.
  7. Subtract documented public benefits or insurance payments only after confirming eligibility and covered services.
  8. Calculate the remaining cash requirement and compare it with income, liquid assets and a protected emergency reserve.

For a quick check, a $6,200 CareScout assisted-living median equals $372,000 over 60 months before increases. A semi-private nursing-home median of $9,581 equals $574,860 over 60 months, while a private-room median of $10,798 equals $647,880. Those totals demonstrate why a care transition deserves its own scenario rather than being treated as a small monthly adjustment.

Stress-test the plan

  • What if the resident needs a higher care tier one year earlier than expected?
  • What if the entrance fee is not refundable when the resident leaves?
  • What if investment income falls while the monthly fee rises?
  • What if a spouse remains at home and needs separate support?
  • What if Medicaid covers care services but not rent and meals?
  • What if the preferred community has no higher-care opening when needed?

Document the answer to each question in dollars, months of remaining liquidity and the person responsible for taking action. A plan that works only if every assumption holds is not a durable plan.

Financial and operational due diligence before signing

Review the agreement, not just the brochure

Look for definitions of care levels, notice periods, reassessment rights, discharge rules, arbitration clauses, late fees, refund timing, damage charges and responsibility for outside providers. Confirm whether the quoted room is guaranteed, whether a roommate can be assigned and whether a transfer changes the monthly rate or contract type.

Verify staffing and safety capacity

Ask how the community handles overnight coverage, falls, medication errors, hospital returns, wandering, emergencies and staffing shortages. Request the licensing and inspection records available from the state. These operational details affect whether a resident can remain safely at the quoted level of care.

Get every promise in writing

If a representative says a service is included, ask for the exact contract section or fee-schedule line. Keep dated versions of the quote, rate sheet and correspondence. A promise that exists only in a tour conversation is difficult to budget or enforce.

A decision framework for comparing options

Score each option on the same axes so a low monthly number does not hide a costly limitation:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Total five-year cost, including entrance fees and likely transitions
  • Monthly base fee and the documented annual increase
  • Care-level, medication and supply charges
  • What meals, utilities, transportation, internet, laundry and activities include
  • Staffing, safety features and overnight response
  • Availability and price of memory care or nursing care
  • Contract protections, refundability and financial-reserve requirements
  • Distance from family and the practical cost of visits
  • Eligibility for Medicaid, veterans’ benefits or other public programs

Use a weighted score only after calculating dollars. A community that is farther away may cost more in travel but offer a contract that protects access to higher care; a cheaper community may require a disruptive move later. Make the trade-off explicit instead of treating it as an unmeasured preference.

Common estimating mistakes and how to correct them

Using a national median as a quote

Problem: A national median ignores local wages, unit size and support level.
Correction: Use it to screen, then obtain three comparable local quotes.

Comparing a base rate with an all-in bill

Problem: Care, medication and supplies may be excluded from the advertised rate.
Correction: Request a sample bill for the resident’s actual care profile.

Ignoring the next level of care

Problem: A plan works only while the resident remains independent.
Correction: Price at least one likely transition and ask about access, timing and move charges.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Assuming “continuum of care” means one locked price

Problem: A CCRC may guarantee access but charge market rates for later care.
Correction: Read the contract type, future-care schedule and refund rules.

Counting on Medicare for long-term room and board

Problem: Medicare is not general custodial funding.
Correction: Confirm Medicaid eligibility and covered services with the state, and budget rent and meals separately when they are excluded.

Your next actions

  1. Write the current and likely future care profile.
  2. Gather a local baseline from CareScout or state data.
  3. Send identical information to at least three providers.
  4. Collect fee schedules, sample bills and contracts.
  5. Build low, expected and high five-year cases.
  6. Verify public-program eligibility before relying on it.
  7. Have an attorney or qualified adviser review any entrance-fee agreement.
  8. Choose only after the remaining cash requirement fits the household’s income, assets and reserve plan.

The most useful output is not a single “average” price. It is a written range showing what the household pays now, what changes when care needs rise, which fees are optional, and how long available resources last under realistic scenarios.

Product prices and availability are accurate as of the date/time indicated and are subject to change. Any price and availability information displayed on Amazon at the time of purchase will apply.
