Answer every question on the accident-insurance application accurately and completely, including questions about prior diagnoses, symptoms, treatment, occupation and higher-risk activities. Do not assume that an accident-only policy follows the disclosure rules or pre-existing-condition definitions used for comprehensive health insurance.
Read the policy’s definitions and exclusions, ask the insurer or a licensed local broker for written clarification when wording is unclear, and keep the final application and correspondence. The exact duty to disclose and the consequences of an omission depend on your jurisdiction, insurer, policy wording and application.

What you must disclose
Your starting point is the proposal form itself. Complete every question as written, without guessing what the insurer considers important and without silently leaving out information because it seems unrelated to an accident. A personal-accident policy wording from Allianz Malaysia says applicants should take reasonable care to answer proposal questions fully and accurately and disclose other matters they know are relevant. That wording also describes consequences for inaccurate information.
Disclosure is broader than naming a diagnosed disease. Depending on the form, relevant information may include:

- Your occupation, work environment and duties where requested.
- Sports, travel, recreation or other personal pursuits that create unusual injury risk.
- Prior insurance, declined applications, exclusions or cancellations when the form asks about them.
A Pacific Insurance Malaysia disclosure sheet expressly lists occupation and personal pursuits as examples of facts that can affect risk assessment. That is an example of how a particular form approaches lifestyle information, not a universal list for every policy. Use the actual questions and definitions in your application.
Accident-only cover is not comprehensive health insurance
Accident insurance generally pays specified benefits when a covered accident produces an outcome listed in the contract. Consumer material from the National Association of Insurance Commissioners describes benefits such as a lump sum for accidental death, dismemberment or disability, and possible hospital or medical-care benefits caused by an accident. It is not a promise to pay for every illness, treatment or health consequence.
That distinction matters when you have a pre-existing condition. A condition may be relevant to underwriting, excluded in specified circumstances, or treated differently if it contributes to an injury. The product label alone does not answer those questions. Read the contract’s definitions of “accident,” “injury,” “illness,” “pre-existing condition” and any language about conditions that cause, contribute to or worsen a loss.

A Washington insurance-regulator explanation likewise concerns when health insurers may ask health questions. Neither source establishes a universal rule for accident-only products. Your accident policy and local law control.
Prepare before you open the application
A short preparation exercise reduces omissions without encouraging you to disclose irrelevant material. Create a private timeline and then use it only to answer questions the form actually asks.
Build a health and injury timeline
- List diagnoses and significant symptoms, with approximate dates and whether they are resolved, recurring or ongoing.
- Note consultations, tests, emergency visits, hospital admissions, procedures and rehabilitation.
- Record current and recent medicines, including the reason for taking them and the prescriber if that information is requested.
- Identify prior injuries and whether they affected your ability to work, exercise or perform daily activities.
- Mark any treatment, investigation or referral that was pending when you applied.
Do not manufacture precision. If you do not know an exact date, use the best accurate description the form permits and explain the uncertainty when appropriate. Never replace a missing fact with “none” merely because you cannot remember the details.

Describe work and recreation accurately
Write down your actual job title, core duties, work locations and unusual hazards. A desk-based title may not describe regular work at height, heavy machinery, driving, offshore sites or manual handling. Similarly, list activities such as climbing, motor racing, diving, mountaineering, contact sports or aviation when the application asks about hazardous pursuits. Whether an activity is excluded or merely affects pricing is policy-specific.

Have dates, physician or clinic names, discharge summaries and medication information available if the insurer requests them. Keep copies of anything you send. The goal is consistent, verifiable answers—not a larger disclosure than the application requires.
Work through the form question by question
Health, diagnosis and symptom questions
Read the time period and scope in each question. “Have you ever,” “in the last five years” and “currently” seek different information. Answer the stated question, including conditions that are controlled or asymptomatic if they fall within its wording. If the form asks about symptoms, do not assume a symptom is irrelevant because no diagnosis was made.
Treatment and medication questions
Report treatment, medication, testing or follow-up when requested. A condition that required no ongoing treatment may still need to be reported if the form asks about its history. Conversely, do not volunteer unrelated medical details that the application does not seek unless another question makes them relevant.
Occupation and personal-pursuit questions
Use plain language and enough detail for the underwriter to understand the risk. “Construction” may be incomplete if you work on roofs; “diving” may be incomplete if it means commercial or deep technical diving. State frequency and role when the form requests them. Do not downgrade a risky activity to obtain a lower premium or avoid an exclusion.
Prior insurance and applications
If asked whether another insurer declined, postponed, rated or excluded cover, answer exactly. A previous decision does not automatically bar accident insurance, but concealing it can create a separate misrepresentation issue where the question is material.

When a question is unclear
Do not choose the most convenient interpretation. Ask the insurer or a licensed broker what information the question seeks and request the answer in writing. Quote the question exactly, explain the fact you are unsure about, and save the response with your application records.
For example, you might ask whether “back problems” includes a single resolved strain, whether “dangerous sports” includes recreational skiing, or whether a medication taken briefly years ago falls inside the stated period. The response may vary by product. Written clarification gives you a record of how you understood the form; it does not replace the insurer’s final policy wording.
Read the policy before relying on the label
Check the schedule, definitions, exclusions, benefit conditions and claim provisions before paying the premium. Pay particular attention to:
- Covered event: what counts as an accident and whether the event must be sudden, external or otherwise defined.
- Medical connection: how the policy treats an injury connected with illness or a pre-existing condition.
- Activity and occupation exclusions: sports, travel, work or equipment that are excluded or require special acceptance.
- Qualifying periods: any waiting or survival period before a benefit becomes payable.
- Benefit triggers and caps: the specific degree of disability, hospital stay or other outcome required, plus limits on payment.
- Renewal and changes: whether you must report a new occupation, activity, diagnosis or other changed fact at renewal or after a variation.
- Claim conditions: deadlines, medical evidence and notice requirements.
Where pre-existing-condition exclusions apply, model-regulation material from the National Association of Insurance Commissioners says advertising should explain the extent of the exclusion and should not imply that medical history can never affect issuance or payment. That is model-regulation guidance; the law adopted in your jurisdiction and your actual contract govern.
Submit an accurate application
- Read the entire form first. Note every health, injury, occupation, activity and prior-insurance question before entering answers.
- Use your timeline. Check dates, diagnoses, treatment and medication against the periods specified in each question.
- Describe risks precisely. Include the duties and pursuits the form asks about, rather than relying on a broad job title or casual activity label.
- Clarify uncertainty in writing. Ask the insurer or licensed broker before submission and retain the response.
- Review the completed form. Correct omissions, transcription errors and answers you cannot support.
- Attest personally. If an agent or another person entered the answers, read every response yourself before signing or approving it.
- Save the record. Keep the final application, declarations, attachments, written explanations and the policy schedule together.
Electronic applications still require the same care. A checked box or electronic signature generally represents your answer, so do not approve a form you have not reviewed.
What if your circumstances change?
Check the application and policy if a material fact changes before issue, variation or renewal. Examples include a new occupation, a newly diagnosed condition, a substantial change in duties, or beginning a hazardous pursuit. Ask the insurer promptly whether an update is required and keep its written response.

An Allianz Malaysia personal-accident wording states that the policyholder and insured person must tell the company immediately if information given for the contract becomes inaccurate or changes after the contract is entered into, varied or renewed. That is a term in that specific wording; other policies may define the timing and scope differently.

What an omission or misstatement can cost you
The remedy depends on the fact, the wording, the timing and local law. Possible outcomes can include a claim being denied or reduced, changed terms, termination, or avoidance of the contract. The Allianz Malaysia wording lists those possibilities for its policy.
Ontario provides a separate statutory example. Under the Ontario Insurance Act, the applicant and insured person must disclose facts within their knowledge that are material and not already disclosed by the other party, in the application, medical examination and written evidence of insurability. The statute says material nondisclosure or misrepresentation may make the contract voidable, subject to statutory qualifications. Ontario law should not be presented as the rule in another province, state or country.
“Material” does not mean every fact about your life. It means information the applicable legal or contractual test treats as relevant to the insurer’s decision. Because that test is local, do not assume that an omission is harmless or that an insurer can automatically cancel a policy.
If you discover an omission after applying
- Do not wait for a claim. Contact the insurer or broker promptly and explain what was omitted, why, and when you noticed it.
- Ask for a written correction process. The insurer may require an amended application, medical evidence, a new declaration or underwriting review.
- Keep the original and corrected records. Do not delete or overwrite the first submission.
- Ask how the change affects cover. Confirm in writing whether the policy will be issued, endorsed, repriced, excluded, cancelled or left unchanged.
- Escalate locally if necessary. A licensed adviser or qualified insurance lawyer can explain the jurisdiction-specific consequences.
Do not rewrite history to make an answer look cleaner. A transparent correction is more useful than a second inconsistent statement.

| Comparison point | What to look for | Why it matters |
|---|---|---|
| Medical-history questions | Look-back period, diagnoses, symptoms, treatment, medication and pending investigations | Determines what you must disclose and what underwriting can assess |
| Lifestyle questions | Occupation, duties, travel, sports and other personal pursuits | May change acceptance, price or exclusions |
| Definitions | Accident, injury, illness, pre-existing condition and hazardous activity | Controls whether an event falls within the insuring clause or an exclusion |
| Qualifying conditions | Waiting, survival or other qualifying periods | Sets when a benefit can become payable |
| Benefits and limits | Death, dismemberment, disability, hospital or medical benefits; caps and event requirements | Shows what is actually paid and under what proof standard |
| Change and renewal duties | When new facts must be reported and how renewal is handled | Prevents a later change from becoming an avoidable dispute |
| Correction process | How to amend an application and who must approve changes | Provides a path if you find an error before a claim |
If a comparison cell is not stated in the policy documents, ask the insurer rather than inferring that two similarly named products use the same term.
Worked examples
A controlled condition and an accidental fracture
Suppose an applicant has controlled asthma and the form asks about respiratory diagnoses or medication. The applicant should answer that question fully. Whether a later accidental fracture is covered depends on the accident definition, any pre-existing-condition or contribution exclusion, and the claim requirements. The diagnosis does not by itself prove that every accident is covered or excluded.
A job with changing duties
An office employee begins regular work at height after applying. If the policy or renewal documents ask about occupation or changed risk, the employee should ask whether the new duties must be reported before renewal or a variation. The answer should be obtained from the insurer for that contract.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
A risky hobby omitted because it is occasional
A person who occasionally participates in a high-risk pursuit should not decide unilaterally that “occasional” means irrelevant. If the form asks about that activity, disclose it and describe the frequency requested. The insurer can then state whether it is covered, excluded or subject to different terms.
Keep an evidence trail
Store the signed application, policy schedule, full wording, endorsements, medical or occupational explanations, payment confirmation and every written clarification. Record the date and name of anyone who answers a question by phone, then request confirmation in writing. At renewal, compare the new declaration with the prior version so changed answers are deliberate rather than accidental.
Protect medical information while retaining it. Send documents through the insurer’s approved channel, keep access limited and avoid emailing sensitive records to an unverified address.
When professional help is worthwhile
A licensed local broker or adviser can help interpret an ambiguous question, identify which policy documents apply and explain how an occupation or activity is usually presented. Seek jurisdiction-specific legal advice if the insurer alleges material misrepresentation, threatens avoidance or denies a claim because of nondisclosure. An adviser cannot guarantee acceptance or coverage; the insurer’s contract and applicable law remain decisive.
