Form 4506-C is the authorization a lender or other IRS Income Verification Express Service (IVES) participant uses to obtain your tax-return, account, record-of-account, or wage-and-income transcripts. You review the request, correct errors through the lender, and then approve it electronically or sign the paper form. The lender—not you—submits the completed authorization to IRS IVES.
The IRS says it provides tax-return or wage transcripts to a third party only with the taxpayer’s consent. A missing taxpayer signature, incorrect tax year or form, absent IVES participant information, or an invalid signature method can cause the request to be rejected.

What Form 4506-C does in a mortgage or loan application
Form 4506-C gives a lender permission to request income records directly from the IRS. Depending on what the lender selects, the request can cover tax-return transcripts, account or record-of-account transcripts, and wage-and-income transcripts. The lender uses those records to verify information supplied in a mortgage or other loan application.
The authorization is consent-based. The IRS taxpayer guidance states: “The IRS only provides transcripts of your tax return or wage transcripts to third parties with your consent.” The named recipient and the IVES participant information on the form identify who is allowed to receive the records.

You may receive the request in an IRS online or business tax account, or as a PDF supplied by the lender. In an online workflow, you review the displayed information and approve it in the account. In a paper workflow, you review the PDF, sign it using the method requested, and return it through the lender’s secure channel. Keep a copy and any submission confirmation.
The form revision is dated October 2022. IRS guidance says that, beginning March 1, 2023, the newly published version was the only form IVES participants could use. If your lender sends an older version, ask whether it should be replaced before you sign.
What to have ready before you open the form
- Your legal name, taxpayer identification number (usually your Social Security number), current address, and any previous address requested on the form.
- The names and taxpayer identification numbers of every taxpayer whose records are being requested.
- The lender’s exact recipient name, telephone and address information, IVES participant name and ID, and any client information shown on the request.
- The tax-return form type, wage-and-income form types, and tax years the lender wants.
- Access to the IRS account or the secure return method specified by the lender.
Do not guess at a tax year, tax form number, participant ID, or client number. Ask the lender to correct an error before you approve or sign. IRS processing guidance says an IVES participant may no longer modify the taxpayer’s request under the October 2022 revision, so a mistake generally requires a corrected request rather than an informal edit after signing.

1. Verify every taxpayer listed
Start with the taxpayer section. Compare each name, taxpayer identification number, current address, and previous address with the information used on your tax filings and the lender’s application. Small differences can prevent the IRS from matching the request to its records.
Only list a spouse or co-borrower when that person’s transcripts are being requested. Every taxpayer listed on the authorization must provide a signature. If a spouse appears on the form but will not sign, the request can be rejected; ask the lender to remove that taxpayer or issue a corrected form if the spouse’s records are not needed.
If the lender is requesting records for two taxpayers, check that the request actually covers both people and that each person has an appropriate signature field. Do not assume that one borrower’s signature authorizes release of the other borrower’s records.

2. Confirm the recipient, IVES participant, and client information
Review the lender or other designated recipient shown on the form. Confirm the recipient name, address, telephone number, and any client or file information. Also verify the IVES participant name and participant ID.
IRS guidance identifies IVES participant and client information as required. Missing information can cause rejection even when the taxpayer data and signatures are correct. If the recipient is unfamiliar, pause and contact the lender through a trusted telephone number or secure portal before sending tax information.
3. Review the transcript selections on lines 6 and 7
Line 6 is used for tax-return transcript form numbers. The form number should match the tax-return form you actually filed for the requested year. Only one tax-return form type may be requested per Form 4506-C.
Line 7 identifies wage-and-income forms and years. The updated process allows up to three wage-and-income form types on the request. For example, the lender may identify the particular wage or information-return forms it needs rather than asking for every possible form.
More than one transcript type can be requested on a single Form 4506-C, subject to the form’s limits. A request can therefore include a tax-return transcript selection and wage-and-income selections, but it cannot list multiple tax-return form types on the same form. If the lender needs different tax-return form types, ask whether separate authorizations are required.

|---|---|---|
| Line 6 | Tax-return transcript form number and the form actually filed | Choosing a different return form from the one filed |
| Line 7 | Wage-and-income form types and requested years; up to three form types are allowed under the updated process | Leaving out a form the lender specifically requested or entering an incorrect year |
| Transcript combination | Multiple transcript types may be requested, but only one tax-return form type may be requested per authorization | Trying to list several tax-return form types on one form |
4. Check the tax years and any date-of-return entries
Verify every year against the lender’s written request. A lender may need a particular set of years, and the correct year is the year of the return or income record being requested—not simply the year in which you submit the authorization.

Check that any date-of-return entries use the format shown on the form. If a year or date is wrong, do not cross it out or overwrite a signed form. Ask the lender to issue a corrected request before you sign, or to tell you how to restart the online approval.
Do not assume that a transcript exists for every year or that the IRS can provide a requested record immediately. Availability depends on the filing and the type of transcript. The lender can explain which alternative documentation it will accept if a transcript is unavailable.
5. Complete the correct signature method
Do not sign until all applicable lines are complete. The Form 4506-C instructions state: “Do not sign this form unless all applicable lines have been completed.”

Every taxpayer listed must sign. If an authorized representative signs for a taxpayer, the representative must use the authorized-representative designation and check the authorized-representative box where required. Follow the lender’s process for documenting that authority.
Electronic and wet signatures have different confirmations:
- Electronic signature: use the lender’s or IRS account workflow and check the electronic-signature confirmation box when the form requires it.
- Wet-ink signature: sign the printed form as instructed and leave the electronic-signature confirmation unchecked.
A wet-ink signature with the electronic-signature confirmation box checked is rejected. Conversely, do not mark an electronic confirmation if you are returning only a handwritten signature. If more than one taxpayer is listed, each person must use the method required for that request.

6. Return the authorization promptly and retain proof
Use the secure upload, portal, mail, or other channel specified by the lender. Do not email tax information to an address you have not independently verified. Save the completed form and the portal receipt, upload confirmation, or other proof of delivery.
IRS processing guidance requires the form to be received within 120 days of the signature date. The deadline is measured by when IRS receives the request, not simply when you sign or hand it to the lender. Because the lender submits the form to IVES, return it promptly and ask how it will be transmitted.
Spouse and co-borrower signatures
A spouse does not automatically have to sign every Form 4506-C. The deciding question is whether the spouse’s transcripts are being requested and the spouse is listed as a taxpayer on the authorization. If the spouse is listed, that taxpayer must sign. If the lender needs only your records, ask whether the spouse should be removed from the request rather than leaving an unsigned taxpayer on the form.
The same rule applies to a co-borrower or any other taxpayer named on the request: each listed taxpayer must provide the required signature. One borrower cannot substitute for another taxpayer’s consent.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Electronic approval, PDF signing, and online transcript delivery
The delivery method determines what you do. An IRS online or business tax account may display the request for review and electronic approval. A lender may instead send a PDF for signature and secure return. Follow the exact method provided; do not convert a paper signature into an electronic confirmation or vice versa.
IRS FAQs describe real-time delivery for some transcripts approved through WebUI or A2A portals. That operational description applies to those portals and should not be treated as a guaranteed turnaround for every lender submission. The lender controls when it submits the authorization and how it handles a response.

Why a Form 4506-C is rejected—and how to fix it
| Rejection or problem | Why it happens | What to do |
|---|---|---|
| Missing taxpayer signature | A person is listed as a taxpayer but did not sign. | Ask the lender for a corrected or replacement authorization and obtain every required signature. |
| Missing IVES participant ID or client information | Required participant or client fields are blank or incomplete. | Have the lender complete the fields; do not invent an ID or client number. |
| Signature placed before the form was complete | The form was signed while applicable lines were blank or later changed. | Request a new, complete form and sign only after reviewing all entries. |
| Electronic confirmation checked for a wet signature | The form indicates an electronic signature even though it carries a handwritten signature. | Use the correct signature workflow and obtain a replacement if the signed form has already been submitted. |
| Signature is more than 120 days old | IRS received the request outside the 120-day validity period. | Sign a new authorization and return it quickly enough for the lender to submit it within the period. |
| Name, address, TIN, form, or year does not match | The request differs from IRS records or from the lender’s intended request. | Ask the lender to correct the request; do not alter an already signed authorization yourself. |
When a request is rejected, ask the lender which field failed and whether it needs a new signature or a fully new Form 4506-C. Re-signing a photocopy with altered entries can create another mismatch and may not satisfy the IVES process.
Privacy, recipient checks, and recordkeeping
Tax transcripts contain sensitive financial information. Section 6103(c) limits the disclosure and use of return information obtained through consent. The instructions also warn that recipients can face penalties for unauthorized access, use, or redisclosure.
- Confirm that the named recipient is your intended lender or another authorized IVES participant.
- Use a secure lender portal or the return method supplied in your loan instructions.
- Keep the signed authorization, the exact version you submitted, and the submission confirmation.
- Record which taxpayers, tax years, and transcript types were authorized.
Retaining the copy helps you identify what information you consented to release and gives the lender a reference if a replacement form is needed.

Fannie Mae describes Form 4506-C as the borrower’s permission for a lender to request tax transcripts and discusses obtaining a signed form at or before loan closing. Individual lenders can set different timing, signature, transcript, and resubmission requirements. A lender may request the authorization during underwriting, again near closing, or after an earlier form expires.
When comparing two lender requests, compare the substance rather than just the document title:
| Comparison point | Question to ask |
|---|---|
| Transcript type | Is the lender requesting tax-return, account, record-of-account, wage-and-income, or more than one type? |
| Years and forms | Which tax years, tax-return form number, and wage-and-income forms are listed? |
| Taxpayers | Are all listed taxpayers actually needed, and will each one sign? |
| Signature method | Is the request electronic or wet-signed, and is the matching confirmation selected? |
| Submission deadline | When must you return it, and how will the lender ensure IRS receives it within 120 days? |
Is Form 4506-C the same as Form 4506-T?
No. Form 4506-C is the IVES authorization used by lenders and other IVES participants to request tax information after the taxpayer consents. The lender submits the completed authorization to IRS IVES. A lender may mention Form 4506-T or another transcript request in its instructions, but you should complete the exact form and workflow it provides rather than substituting one form for another.
