How to Compare Medicare Advantage Plans by Provider Network, Costs, and Coverage

By TheFinanceBase Team 11 min read

The most reliable way to compare Medicare Advantage plans is to use Medicare.gov Plan Compare for your ZIP code and coverage year, enter your prescriptions and pharmacies, then verify your doctors, hospitals, costs, coverage limits, and access rules in the plan’s current documents. Compare total likely spending—not just the advertised premium—and confirm important network and drug details with both the plan and your providers before enrolling.

Start with the same ZIP code and coverage year

Medicare Advantage options are local. A plan available in one county may not be offered in another, and premiums, provider networks, formularies, benefits, and authorization rules can change for each coverage year. Begin every comparison with the ZIP code where you will receive care and select the specific year in which coverage will start.

Use Medicare.gov Plan Compare rather than relying on a national ranking or an old directory. The tool can show plans available in your location and estimate prescription costs after you enter your medications and preferred pharmacies. Check the year shown in the results before recording any premium, copayment, benefit, or network information.

Build a personal comparison file before looking at plans

A plan that looks inexpensive in general may be impractical if it excludes a doctor you see regularly or imposes high cost sharing on a service you expect to use. Make a list of the facts that determine your own fit.

  • Current prescriptions, including dose and frequency, and the pharmacies you would realistically use.
  • Expected care during the year, such as specialist visits, procedures, therapy, diagnostic tests, mental-health care, or recurring supplies.
  • Travel or a second residence that could require routine care outside the plan’s service area.
  • Other insurance or assistance that could affect what you pay.
  • The maximum amount you could comfortably pay for covered care in a difficult year.

This list turns Plan Compare into a personal estimate instead of a generic plan search.

Use Plan Compare to create a short list

  1. Enter your location and year. Use the ZIP code in which you will be enrolled and confirm the coverage year displayed by the tool.
  2. Choose Medicare Advantage plans. Review the plan type, service area, premium information, and whether prescription coverage is included.
  3. Add every prescription. Enter the drug name, strength, quantity, and frequency as accurately as the tool allows. A missing or incorrectly entered drug can make the annual estimate misleading.
  4. Add the pharmacies you might use. Pharmacy choice can change the estimated drug cost and the terms that apply to your prescriptions.
  5. Save several candidates. Keep plans that appear workable on network, annual spending, access rules, and coverage—not merely those with the lowest monthly premium.
  6. Open each plan’s details. Record the plan’s current Summary of Benefits and Evidence of Coverage information, then verify details directly when a decision depends on them.

Plan Compare provides an estimate based on the inputs you supply. It does not replace the plan’s current documents or a confirmation from the plan when a provider, service, or prescription is especially important.

Verify the actual provider network

Network status is specific to the plan, product, location, and coverage year. Do not assume that a physician listed for one plan from an insurer is in-network for another plan from the same insurer. A provider’s own office may also have an outdated directory entry.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Check every high-priority provider

Search the candidate plan’s current provider directory for your primary-care doctor, specialists, preferred hospital, and facilities where you receive recurring care. Check the exact practice location and, when relevant, the individual clinician rather than only the medical group. Record whether each provider is in-network and whether the facility used for a procedure is separately listed.

Confirm with both sides

If keeping a provider matters, call the provider’s office and the plan. Ask whether the provider is in-network for the exact plan name and coverage year, whether the office is accepting new patients under that plan, and whether the facility and clinician are contracted separately. Keep the date, representative’s name or reference number, and the answer. Direct confirmation is particularly important when a directory and an office give different answers.

Check service-area and travel rules

Ask what happens when you are outside the service area. Emergency and urgent care generally have special protections, but routine out-of-area care follows the plan’s rules. Out-of-area dialysis is an exception commonly addressed in HMO rules. A person who travels frequently should compare the plan’s travel provisions, not infer them from the insurer’s national presence.

Understand how plan type affects access

Plan type is a useful starting clue, not a substitute for the plan’s own terms. The following descriptions are general Medicare.gov distinctions; exceptions and cost sharing must be checked in the candidate plan’s materials.

Plan type Provider access Referrals and out-of-network care Prescription coverage
HMO Generally requires network providers for covered care, with exceptions such as emergency or urgent care and out-of-area dialysis. Referrals are common. Some HMO Point-of-Service plans cover certain out-of-network services at a higher cost. Some plans include Part D; choose an HMO that includes drug coverage if you want it through the same plan.
PPO Allows use of in-network and, for covered services, out-of-network providers. Out-of-network care usually costs more. Referrals are generally not required. Some plans include Part D; verify the specific plan.
PFFS Permits use of Medicare-approved providers who accept the plan’s payment terms and agree to treat you. Out-of-network use can cost more, and a provider can decline the plan’s terms for a particular service. Drug coverage may or may not be included.
SNP Network rules depend on whether the SNP is structured as an HMO or PPO. Follow the underlying HMO or PPO rules and the SNP’s additional eligibility and care requirements. Every SNP includes Part D drug coverage.
MSA Generally has no network; members may use Medicare-approved providers who agree to treat them and have not opted out. Verify provider acceptance and the plan’s payment terms before care. MSA plans do not include drug coverage; separate Part D may be needed.

Compare the full cost of care

Medicare Advantage costs vary by plan. Compare the Part B premium you must continue paying, any additional plan premium, deductibles, copayments or coinsurance, and the annual out-of-pocket limit for covered medical services. A zero or low plan premium does not show what you will spend when you use care.

Use a total-cost worksheet

Cost category What to record for each plan
Part B premium The Part B premium you will owe while enrolled; it generally continues even when you join Medicare Advantage.
Plan premium Any additional monthly premium for the Medicare Advantage plan.
Deductibles Medical, hospital, drug, or service-specific deductibles and when they apply.
Routine care Copayments or coinsurance for primary care, specialists, urgent care, mental-health care, and other services you expect to use.
High-use services Cost sharing for hospital stays, outpatient procedures, therapy, imaging, durable equipment, and recurring supplies relevant to you.
Annual out-of-pocket limit The plan’s annual limit for covered medical services; note whether separate in-network and out-of-network limits apply.
Prescription spending The Plan Compare estimate based on your medications and pharmacies, plus the plan’s current formulary and pharmacy terms.

For a practical annual estimate, add the Part B premium, plan premiums, expected medical cost sharing, and estimated prescription spending. Then test a high-use scenario using the plan’s annual out-of-pocket limit for covered medical services. Keep routine and worst-case estimates separate: a plan can be inexpensive for occasional visits but expose you to a different level of spending during a serious illness.

Read copayments and coinsurance in context

A fixed copayment and a percentage coinsurance can produce very different bills for the same service. Check whether a listed amount applies before or after a deductible, whether it is per visit or per episode, and whether the service has a plan-specific limit. Also check whether the quoted amount is for in-network care; out-of-network rates may follow a separate schedule.

Check referrals and prior authorization before you need care

Access rules can delay or change the cost of treatment. Medicare.gov states: “You may need to get approval (prior authorization) from your plan before it covers certain services or supplies.” Review the plan’s current Evidence of Coverage for services that matter to you, including specialist visits, advanced imaging, inpatient admissions, rehabilitation, home health, durable medical equipment, and recurring supplies.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Primary-care and referral rules

Determine whether you must select a primary-care doctor and whether that doctor must refer you to a specialist. PPO plans generally do not require referrals, while HMO plans commonly do, but the exact plan controls. Ask whether a referral expires, applies to a particular specialist or service, or must be renewed for follow-up care.

Prior authorization and step requirements

Find out which services require approval, who submits the request, and how much notice is needed. For a recurring service, ask whether approval covers one visit, a defined period, or a set number of treatments. For supplies and prescriptions, check whether the plan requires a particular supplier, quantity limit, or trial of another option before coverage begins.

Compare medical coverage and extra benefits by their conditions

Medicare Advantage plans cover Medicare-covered medically necessary services and may add benefits beyond Original Medicare. A benefit label alone does not establish that a particular need will be covered. For each benefit you value, check four things:

  • Covered service: exactly what the plan pays for and what it excludes.
  • Eligibility: medical, age, diagnosis, or plan-specific conditions you must meet.
  • Limits: annual visit counts, dollar allowances, frequency limits, approved locations, or time periods.
  • Cost sharing: copayments, coinsurance, deductibles, or charges when you use an out-of-network provider.

Apply the same test to dental, vision, hearing, transportation, fitness, over-the-counter allowances, meal support, or other extras shown in plan materials. An advertised extra may be useful only if its eligibility and limits match your situation.

Review prescription coverage as a separate decision

Enter every medication and pharmacy into Plan Compare, then open the candidate plan’s current formulary and pharmacy information. Confirm that each drug is covered, identify any utilization rules, and check whether your preferred pharmacy receives the plan’s stated terms. The estimate should reflect the quantities and refill pattern you actually expect.

If you want drug coverage through Medicare Advantage, choose a plan that includes Part D. HMO and PPO members generally need to select an HMO or PPO plan with Part D rather than assuming a separate drug plan can be attached. Every SNP includes Part D. MSA plans do not, so a person choosing an MSA may need separate Part D coverage.

Recheck the formulary for the coverage year you are comparing. Drug tiers, pharmacy arrangements, covered quantities, and authorization rules can change, so an old list is not a reliable basis for enrollment.

Use a side-by-side decision worksheet

After narrowing the list, put the same fields in the same order for every plan. This prevents a prominent premium or benefit from overshadowing a less visible restriction.

Decision area Questions to answer
Provider network Are each of my important clinicians, hospitals, and facilities in-network for this plan and year? What are the service-area and out-of-network rules?
Routine and high-use care What will I pay for the visits and services I expect, and are there deductibles or plan-specific limits?
Catastrophic exposure What is the annual out-of-pocket limit for covered medical services, and are in-network and out-of-network limits different?
Premiums What Part B premium continues, and what additional plan premium applies?
Access rules Must I select a primary-care doctor, obtain referrals, or get prior authorization?
Prescriptions Is Part D included, are my drugs covered, which pharmacies can I use, and what annual cost does Plan Compare estimate?
Extra benefits Are the benefits I value included, and what eligibility rules, limits, and cost sharing apply?
Personal fit How does the plan handle my travel, regular providers, expected care, other insurance, and budget?

Use notes rather than memory. Record the plan name, plan type, coverage year, the date you checked the directory, and unresolved questions. This makes it easier to spot when two plans are being compared using different years or assumptions.

Common comparison mistakes and how to correct them

Choosing by premium alone

Problem: A low premium hides high visit costs, coinsurance, or a less favorable out-of-pocket limit.

Correction: Read the benefit’s conditions and limits in the current plan documents.

Entering incomplete drug information

Problem: Omitting a prescription or pharmacy can understate annual drug spending.

Match the plan structure to your likely use

If keeping specific doctors is your priority

Start with the network search, not the premium. Eliminate plans that do not include essential clinicians or facilities unless you have confirmed affordable, usable out-of-network benefits. Then compare referrals, prior authorization, and cost sharing among the plans that preserve access.

If you expect frequent or complex care

Focus on the services you expect to use and the annual out-of-pocket limit. Compare hospital, specialist, therapy, imaging, equipment, and supply costs, and identify which services require approval. A plan with a higher premium can still be the better fit if its cost sharing and access rules align with your expected care.

If your prescriptions drive your budget

Enter every medication and pharmacy first. Compare the estimated annual drug cost, formulary status, pharmacy terms, and utilization rules before weighing extra benefits.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If you travel or split time between locations

Review service-area, urgent-care, emergency, dialysis, and out-of-network provisions in detail. A plan type that permits out-of-network care does not mean every provider will be affordable, and an HMO’s routine out-of-area rules may not fit frequent travelers.

Complete the final verification before enrolling

  1. Reopen Plan Compare and confirm the ZIP code and coverage year.
  2. Check the current Summary of Benefits and Evidence of Coverage for each finalist.
  3. Verify every essential provider, facility, prescription, and pharmacy.
  4. Confirm premiums, deductibles, copayments or coinsurance, and the annual out-of-pocket limit.
  5. Ask about primary-care selection, referrals, prior authorization, supplier rules, and out-of-network costs.
  6. Review the exact eligibility and limits for extra benefits you intend to use.
  7. Contact the plan about any unresolved question and keep the response with your comparison notes.
  8. Enroll only after the plan’s current terms match your provider list, expected care, prescriptions, travel needs, and budget.
