How to Compare Medical-Device Stocks by Growth, Cash Flow, and Dividend Safety

Compare medical-device stocks across several years, not by one quarter or a single headline growth rate. Check whether revenue growth is reported or organic, measure cash left after capital spending, and weigh dividends against that recurring cash and the company’s other obligations. The framework below uses reported figures from Medtronic, Abbott, and Boston Scientific as examples—not as a ranking or a buy-or-sell recommendation.

Build a like-for-like comparison first

Before comparing companies, align the fiscal periods, business scope, currency, and accounting definitions. Medtronic’s fiscal year ends in April; the cited Abbott and Boston Scientific reports cover calendar years. Abbott also operates beyond medical devices, so its consolidated cash flow is not device-segment cash flow. A comparison that ignores these differences can make unlike results appear directly comparable.

For each company, collect the same measures for several fiscal years. Record the source period and units alongside every figure, and distinguish U.S. GAAP results from company-defined non-GAAP measures.

Measure What to record How to use it
Revenue and growth Revenue by fiscal year; reported and organic growth where available; segment results and material portfolio changes Look for a multi-year pattern and identify whether currency, acquisitions, or divestitures affected the comparison.
Cash from operations Net cash provided by operating activities for the same fiscal years Use the cash-flow statement and inspect working-capital changes that may temporarily lift or depress cash.
Capital spending Additions to property, plant, and equipment Subtract from operating cash flow to estimate cash remaining after capital expenditure; label this calculation.
Dividends Cash dividends paid or declared, per-share amounts, and the period covered Compare the cash commitment with cash remaining after capital needs; keep yield separate from the dividend amount.
Financial capacity and risks Debt, liquidity, maturities, other obligations, planned investment, and disclosed operating risks Assess whether cash generation appears resilient enough to support the company’s plans and any dividend.

How to compare revenue growth

Keep reported and organic growth distinct

Reported growth reflects revenue as recorded for the period. Organic growth is a company-defined measure intended to show performance apart from specified effects, which may include currency movements or portfolio changes. The exact calculation varies by issuer, so read the company’s definition before comparing it with another company’s figure.

Medtronic reported FY2026 revenue of $36.4 billion, with 8.4% growth as reported and 5.8% organic growth. Its earnings release separately discusses foreign-exchange and other adjustments in the organic-growth comparison. These figures illustrate why the growth label and calculation matter; they are not a direct peer ranking. See Medtronic’s FY2026 results release.

Find the drivers behind the rate

After checking the consolidated trend, examine segment results and the factors management identifies as contributors. Separate growth from existing operations from the effects of acquisitions, divestitures, and currency where the company provides that information. Also consider whether procedure volumes, product adoption, reimbursement, pricing, regulation, or supply constraints could change the trajectory.

How to compare cash generation

Start with GAAP operating cash flow

Use net cash provided by operating activities from the cash-flow statement and compare several years. Changes in working capital and tax payments can move operating cash independently of sales, so examine those lines before treating a cash-flow increase as a lasting improvement in operating performance.

For calendar years 2023–2025, Abbott reported net cash from operating activities of $7,261 million, $8,558 million, and $9,566 million, respectively. Its cash-flow statement separately lists property-and-equipment spending of $2,202 million, $2,207 million, and $2,171 million for those years. The figures are consolidated Abbott results, not device-segment cash flow. The 2025 Abbott Annual Report also provides the surrounding cash-flow statement and discussion.

Boston Scientific reported operating cash flow of $2,503 million in 2023, $3,435 million in 2024, and $4,534 million in 2025. Its annual report presents capital purchases and acquisitions separately; acquisition outlays varied materially between years. Do not treat acquisition spending as recurring operating expenditure when assessing operating cash generation. See the Boston Scientific FY2025 Form 10-K.

Estimate cash after capital expenditure carefully

A simple, transparent calculation is operating cash flow minus additions to property, plant, and equipment. Call the result cash after capital expenditure or a similarly explicit label. Do not assume it is identical to another issuer’s “free cash flow”: companies may define that non-GAAP measure differently, so check its reconciliation and definition before comparing it.

Medtronic defines free cash flow as operating cash flow less additions to property, plant, and equipment, and says the non-GAAP measure is supplemental to—not a substitute for—U.S. GAAP results. For FY2026, its Form 10-K reports $7,330 million of operating cash flow less $1,904 million of capital additions, resulting in $5,426 million of free cash flow. For FY2025, it reports $7,044 million less $1,859 million, resulting in $5,185 million. These are company-reported fiscal-year results, not forecasts. See the Medtronic FY2026 Form 10-K.

How to assess dividend safety

Test coverage, not just yield or history

Dividend yield changes with the share price; it does not show whether the company can sustain the cash payment. Compare dividends paid or declared with operating cash left after capital spending, then consider debt maturities, liquidity, pensions or other obligations, and planned investment. A dividend increase streak can provide context, but does not establish future safety.

Abbott declared common dividends of $2.08 per share in 2023, $2.24 in 2024, and $2.40 in 2025. Its 2025 annual report says management expects annual operating cash flow to continue to exceed capital expenditures and cash dividends. That is management’s expectation, not an assurance; readers can examine the reported cash-flow and capital-spending figures alongside the dividend record in the annual report.

Check whether the company pays a dividend at all

Dividend policies differ. Boston Scientific reported that it paid no cash dividend on common stock in 2023, 2024, or 2025 and said it did not then intend to pay one. It may be relevant to a growth and cash-generation comparison, but that disclosure does not make it a current common-dividend income example. Recheck later filings for any policy change.

Medtronic’s June 2026 results release said its board increased the first-quarter FY2027 dividend to $0.72 per share, or $2.88 annualized, and described the increase as the 49th consecutive year. Annualizing one quarterly declaration is not a promise of future payments; review later declarations and cash coverage. The release also reported FY2026 operating cash flow of $7.3 billion and free cash flow of $5.4 billion. Details are in Medtronic’s FY2026 results release.

Risks that can change growth and cash conversion

Past growth or cash flow is not a reliable stand-in for future results if operating conditions change. Risks disclosed by Boston Scientific include currency, competition, reimbursement and regulation, trade policy, supply-chain disruption, procedure volumes, new-product adoption, pricing, acquisitions and integration, and litigation. These factors can affect revenue, costs, and the timing or amount of cash available.

When comparing companies, connect disclosed risks to the revenue drivers and cash-flow trends you have identified. A fast-growing product may depend on adoption or reimbursement; a supply interruption or acquisition can alter cash conversion; and litigation or regulatory changes may affect future costs. Use each issuer’s current filings for the risks and financial periods relevant to your comparison.
