How to compare insurance policy exclusions, waiting periods and claim conditions

The safest way to compare insurance is to put the actual policy wording, schedule or certificate, and product disclosure document side by side. For each event or treatment you might claim, record what is covered, what is excluded or restricted, when cover starts, what you must do to claim, and how much of the bill you would still pay. Premium alone is not a reliable measure of value.

Rules differ by country and by insurance product. The method below is broadly useful, while the examples from Australia, Canada, the European Union and India are labelled so you do not mistake a local rule for a universal one.

Start with the documents that control a claim

Sales pages, comparison summaries and policy names are useful for finding options, but they are not the complete contract. Obtain the current policy wording and schedule or certificate, plus the applicable Product Disclosure Statement (PDS), Customer Information Sheet or equivalent document. Check the policy number, edition date, insured people, benefit level, territory and renewal period so that you are comparing the same version of each product.

The Australian Competition and Consumer Commission (ACCC) specifically tells consumers not to rely on an insurer’s website alone and to read the policy’s PDS. It also recommends asking the insurer to clarify terms and keeping a record of the conversation. India’s insurance regulator directs health-insurance consumers to read the terms and conditions and review the Customer Information Sheet; confirm the current documents directly because a search-result summary is not a substitute for the policy.

Collect the complete set for every option

  • Policy wording, including definitions, exclusions, conditions and endorsements.
  • Schedule or certificate showing the insured parties, limits, excess or deductible, dates and selected options.
  • PDS, customer information sheet or other mandated summary.
  • Renewal notice and any variation, if you are comparing an existing policy with a new one.
  • Any provider, hospital or network directory that affects eligibility or the amount payable.

Save the documents with the date you received them. If an adviser or call-centre representative explains a clause, ask for the answer in writing and keep the message with the policy. An informal explanation may help you understand the wording, but only the contract and any formally issued endorsement determine cover.

Compare the same event, not broad labels

Choose the losses, treatments or incidents that matter to you and test every policy against exactly the same scenario. An insurer pays for the specific losses described in its policy, as the Financial Consumer Agency of Canada explains. “Comprehensive” or “standard” labels do not tell you whether a particular event is covered, restricted or excluded.

For each scenario, identify the insured event and the eligibility definition first. Then read the exceptions, conditions and limits attached to that definition. A treatment may be listed as a benefit but limited to a certain provider, capped at a sub-limit, subject to a co-payment, or unavailable during a waiting period.

Comparison worksheet

Comparison point Questions to answer for each policy Evidence to record
Covered event or benefit What event, diagnosis, treatment or loss is insured, and who qualifies? Clause number, definition and eligibility wording
Exclusions and restrictions What is entirely excluded? What is covered only if a condition is met? Exclusion, exception and restricted-category text
Waiting period How long is the wait, when does it start, and which benefit or condition does it affect? Duration, start date and benefit-specific rule
Switching or increasing cover Does continuity apply, or does a new or longer wait apply to added benefits? Transfer, upgrade and continuity provisions
Notice and filing deadline How must you notify the insurer, and by what date must the claim be filed? Phone, portal or form route and contractual deadline
Proof and cooperation Which forms, records, receipts, reports or examinations are required? Document list and any cooperation obligation
Cost sharing What deductible or excess and co-payment apply? Amount, percentage and whether each applies per claim or period
Caps and sub-limits What annual, lifetime, per-claim or category limits reduce payment? Limit amount, counting method and reset date
Provider and territory Must you use a named network, hospital, territory or approved supplier? Network rules and out-of-network consequences
Premium and changes What is the premium, when can it change, and what happens at renewal? Quoted amount, payment frequency and change terms
Written clarification Has the insurer answered an unresolved question tied to a specific clause? dated response, staff name and referenced wording

Use one row per policy and one worksheet per scenario. This prevents a broad comparison from hiding a critical difference, such as one policy covering a treatment only at a particular hospital while another pays at any eligible provider.

Separate exclusions, restrictions and limits

Complete exclusions

An exclusion removes a loss, service, person or circumstance from cover. If the event falls within an exclusion, the insurer may pay nothing for that part of the claim. Look for exclusions in the general section and in the benefit-specific section; an exception elsewhere can alter the result.

Conditional or restricted cover

A restriction leaves some cover in place but attaches a condition. Examples include a requirement to use an approved provider, obtain pre-authorisation, meet a definition of medical necessity, or satisfy an eligibility rule. Missing the condition can reduce or eliminate payment even though the benefit appears in the schedule.

Caps, sub-limits and cost sharing

A cap limits the maximum payable over a year, policy period or claim. A sub-limit applies to a category inside a larger overall limit. Deductibles or excesses are amounts you bear before the insurer contributes; co-payments are amounts or percentages you share after the policy responds. Record whether each applies per claim, per person, per service or per period.

The ACCC warns that excluded or restricted treatment can leave a consumer responsible for most or all of the cost. Provider arrangements can also change the out-of-pocket amount, so a policy with a wide headline benefit may be less useful if your preferred hospital or practitioner is outside its arrangement.

Map every waiting period

Do not write down only “12 months” or “no waiting period.” A useful entry states the benefit or condition, the length of the wait, the date the clock starts, and the event that ends it. Check whether the period runs from application, acceptance, payment, policy commencement, a treatment date or another defined trigger.

Switching and increasing cover

When you switch or increase cover, ask four separate questions:

  1. Does the new policy recognise the time already served at the same level of cover?
  2. Does an added benefit have its own waiting period?
  3. Does increasing a limit restart or add a wait only for the additional amount?
  4. What happens if the old policy ends before the new one is active?

In Australian private health insurance, the ACCC says a wait may apply after starting a policy or increasing cover. Switching at a similar level usually does not require serving the waits again, while added benefits or better conditions may carry a new wait. These are Australian product-specific points, not a rule for every country or type of insurance.

Example of a jurisdiction-specific schedule

For Australian private health cover, the ACCC describes a 12-month wait for birth and obstetric services and a usual 12-month wait for pre-existing conditions. It also describes a two-month exception for psychiatric, rehabilitation and palliative care. Check the current policy and applicable Australian rules before relying on these periods; they should not be generalised to travel, property, life or health policies in another jurisdiction.

Check the claim trigger and the process before a loss

A policy can respond only when the facts fit its definition of the insured event. Write a plain-language scenario, then locate the contract’s definition and conditions. Note who is eligible, what date or occurrence starts the claim, and any requirement to obtain approval before treatment, repairs or other costs are incurred.

Notification and filing

Record the permitted reporting route and the deadline separately. The Financial Consumer Agency of Canada says claim-submission limits commonly vary from 90 days to 12 months in its Canadian consumer guidance, with the policy wording controlling the actual deadline. European Insurance and Occupational Pensions Authority (EIOPA) guidance tells consumers to notify the insurer as soon as possible, check restrictions and exclusions, submit supporting documents and avoid late claims.

A deadline can apply to initial notice, the completed claim form, or both. If a policy uses different deadlines for different benefits, copy each one into your worksheet. If circumstances make a deadline impossible, notify the insurer immediately and ask what extension or alternative process the contract permits; do not assume an extension has been granted without written confirmation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Evidence and cooperation

List every item the policy requires before an event occurs: claim forms, invoices, receipts, medical records, reports, proof of ownership, photographs, identification, police or incident reports, and any examination or cooperation obligation. The exact list is policy-specific. Keep originals and submit copies or secure uploads as instructed, retaining a dated record of what you sent.

Compare what you will actually pay

Put the premium beside the deductible or excess, co-payment, caps, sub-limits, excluded expenses and provider differences. A lower premium can come with lower cover or higher out-of-pocket costs; the ACCC highlights this trade-off in its private-health guidance.

Illustrative premium and excess trade-off

The ACCC gives an illustrative Australian example of a policy costing $250 per month with a $500 excess versus $350 per month with no excess. It is an example, not a market average or recommendation. To compare the options, calculate the annual premium difference, then consider how often a claim is plausible and whether you could comfortably pay the excess when it is due. Also check whether the excess applies once per year, per person or per claim.

Illustrative effect of a deductible

The Financial Consumer Agency of Canada illustrates a $600 claim with a $500 deductible, leaving $100 for reimbursement before other policy provisions. This is also an illustrative example, not a typical claim amount. A high deductible may be sensible for infrequent, large losses but poor value if you expect smaller, repeated claims that fall below it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not stop at the first reimbursement figure. Apply sub-limits, co-payments, excluded items, provider pricing and any remaining annual limit. The amount the insurer approves and the amount you ultimately owe can differ substantially.

Resolve ambiguous wording in writing

Mark every clause that could change a decision, especially definitions of pre-existing conditions, medical history, “reasonable and customary” charges, waiting-period start dates, notification deadlines and the meaning of the insured event. Ask a narrow question that quotes or identifies the clause and gives a concrete scenario.

For example, ask which date starts a named waiting period, whether a prior diagnosis falls within the policy’s definition, or whether an added benefit receives continuity when you switch at the same level. Request the applicable endorsement or current wording, not merely a verbal assurance. Save the response with the date, channel, representative and policy version. Treat the answer as clarification unless the insurer formally changes the contract.

Use a decision framework instead of counting exclusions

The policy with the fewest exclusions is not automatically the best choice. Rank each policy on five dimensions that reflect your likely use:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Coverage fit: Does it cover the events, people, providers and territory you actually need?
  2. Exclusion severity: Would a key exclusion remove protection for a financially serious or likely loss?
  3. Time to usable cover: When will the relevant benefits become available, including any new wait after a switch or upgrade?
  4. Claim burden: Are notice routes, evidence requirements, approvals and deadlines practical for you?
  5. Likely out-of-pocket cost: What remains after premium, deductible or excess, co-payment, caps, sub-limits and provider restrictions?

Give greatest weight to exclusions and waits connected to your actual circumstances. A minor restriction may matter less than a short waiting period for a benefit you need soon; a low premium may be unattractive if the deductible or network leaves you unable to use the cover.

Country and product differences to keep visible

Australia

The ACCC material cited here concerns Australian private health insurance. Its guidance covers PDSs, excluded and restricted treatment, provider arrangements, waiting periods, excesses and the effect of switching or increasing cover. Apply those examples only to the relevant Australian product and check the current policy terms.

Canada

The Financial Consumer Agency of Canada explains that insurers pay for the specific losses described in the policy, that claim deadlines vary, and that supporting documentation may be required. Its reference to limits commonly ranging from 90 days to 12 months is Canadian consumer guidance, not a universal deadline.

European Union

EIOPA’s consumer guidance advises reviewing restrictions and exclusions, notifying the insurer promptly, submitting supporting documents and avoiding late claims. Individual countries, products and contracts can add requirements, so the policy wording remains the operative source.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

India

Search-result guidance from India’s Insurance Regulatory and Development Authority points health-insurance consumers to exclusions, waiting periods, co-payments, limits, eligible providers and the Customer Information Sheet. Because the underlying page was not available for verification here, use the regulator’s current publication and your policy documents for definitive terms rather than relying on a search excerpt.

Common comparison mistakes and their fixes

  • Comparing only premiums: Add deductibles, co-payments, caps, exclusions and provider costs before deciding.
  • Reading a summary but not the wording: Use the summary to navigate, then verify each material point in the contract and schedule.
  • Assuming a switch preserves every benefit: Confirm continuity at the same level and identify waits for added or improved cover.
  • Recording one generic waiting period: Map each benefit or condition to its own duration and start date.
  • Waiting until a loss to find the deadline: Put notice routes, filing dates and evidence requirements in your records now.
  • Relying on an unrecorded phone answer: Ask a clause-specific question in writing and preserve the response.
  • Assuming a listed provider is always affordable: Check network status, hospital arrangements and the resulting out-of-pocket charge.
  • Treating comparison sites as complete: The ACCC cautions that comparison sites may omit insurers or policies and may receive payment from listed businesses. Verify the full market and commercial relationships before relying on a result.

A practical sequence before buying, renewing or switching

  1. Define the scenarios: List the losses or treatments that would create a serious financial problem for you.
  2. Collect current documents: Download the wording, schedule or certificate, PDS or equivalent, endorsements and provider information for each option.
  3. Mark the controlling clauses: Highlight definitions, exclusions, restrictions, waiting periods, deadlines, evidence rules, limits and cost sharing.
  4. Build the side-by-side worksheet: Enter one policy per row and use the same scenario and units for every option.
  5. Test a realistic claim: Apply the benefit limit, deductible or excess, co-payment, provider rule and excluded items to estimate your remaining bill.
  6. Ask written questions: Send precise clause-and-scenario questions for every unresolved issue and file the answers.
  7. Check transition risk: Confirm the old policy’s end date, the new policy’s start date and any waits or gaps caused by an upgrade.
  8. Set a compliance reminder: Store the reporting route, claim deadline and evidence checklist where you can find them quickly after an incident.
  9. Recheck at renewal: Compare the new wording and schedule with the prior version; do not assume the exclusions, limits or premium stayed the same.

What to do when a claim becomes possible

  1. Read the relevant definition, exclusion and condition before committing to discretionary costs where the policy requires approval.
  2. Notify the insurer through the specified channel as soon as possible, even if you are still collecting documents.
  3. Ask for a claim reference and confirm the filing deadline, required forms and preferred evidence format.
  4. Keep dated copies of photographs, invoices, receipts, reports, correspondence and upload confirmations.
  5. Follow cooperation, examination and mitigation requirements in the contract, and record any instruction that changes the process.
  6. Check the insurer’s calculation against the policy’s deductible or excess, co-payment, caps, sub-limits and exclusions.

If the insurer and policy wording seem inconsistent

First ask the insurer to identify the clause supporting its position and provide the current wording or endorsement. Reply in writing with the facts, dates and documents, and keep the complete file. If the issue remains unresolved, use the complaints process and then the relevant local regulator or ombudsman. The appropriate body depends on the country and insurance line; a general comparison website cannot replace that formal dispute route.

Your records should make it possible to show which version of the policy applied, what you asked, what the insurer answered, when you notified it and which evidence you supplied. That discipline will not guarantee payment, but it reduces avoidable disputes over deadlines, missing documents and misunderstandings about exclusions or waiting periods.
