The right way to compare health insurance stocks is to normalize business mix, underwriting ratios, government-program exposure, cash generation, capital strength, and valuation over several reporting periods. A single medical loss ratio or one quarter’s earnings can be misleading because insurers sell different products, use different segment definitions, and recognize claims and reserves at different times.
Use the framework below to build an apples-to-apples comparison. It is an analytical process, not a ranking or personalized investment recommendation.

Why headline revenue and one loss ratio are not enough
Health insurers can look similar on an income statement while carrying very different risks. One company may earn most of its premium from Medicare Advantage, another from employer plans, and another from Medicaid. Some also own pharmacy, care-delivery, data, or administrative-services businesses whose economics are not insurance underwriting.
That mix changes how to interpret growth, claims costs, margins, cash flow, and regulation. Compare each company on the same period, segment level, accounting basis, and denominator. If those foundations differ, a precise-looking ranking can still be wrong.

Start with a consistent comparison sheet
For every insurer, record the latest annual filing and at least four to eight quarters of history. Keep these fields separate:
- Premium revenue and other service revenue.
- Membership by commercial, Medicare Advantage and Part D, Medicaid, and other government programs.
- Operating income by insurance and non-insurance segment.
- Medical, benefit, or medical-care ratio, with the issuer’s definition.
- Administrative or operating expense ratio.
- GAAP operating income, adjusted operating income, and the reconciliation between them.
- Operating cash flow, debt, interest expense, parent liquidity, and regulated-subsidiary capital.
- Share price and valuation inputs measured on one common date.
Do not combine a consolidated ratio from one issuer with an insurance-segment ratio from another unless you have verified that the populations and denominators match.
1. Map each company’s business mix before comparing performance
Business mix tells you what is actually being underwritten and where earnings come from. Separate revenue share from profit share: a large revenue segment may have a low margin, while a smaller services business may contribute disproportionate operating income.
Insurance products to identify
| Business area | What to inspect | Why it changes the comparison |
|---|---|---|
| Commercial and employer plans | Fully insured premium, self-funded administrative fees, membership, renewal pricing, and employer mix | Fully insured plans carry claims risk; administrative-services contracts generally earn fees instead of underwriting premium risk. |
| Medicare Advantage and Part D | Enrollment, bids, risk-adjustment revenue, star ratings, quality bonuses, medical trend, and benefit changes | Federal payment rules and member acuity can move revenue and claims differently from commercial insurance. |
| Medicaid | State contracts, eligibility changes, rate updates, acuity, redeterminations, and contract renewals | State budgets and membership churn can alter both volume and claim severity. |
| Non-insurance services | Pharmacy, care delivery, technology, consulting, and other service revenue and operating profit | These businesses may have different margins, capital needs, and cyclicality from underwriting. |
Use segment disclosures, not brand assumptions
UnitedHealth Group reported that 44% of its consolidated revenue in 2025 came from premiums paid by the Centers for Medicare & Medicaid Services, most of it generated by Medicare & Retirement. The group also operates Optum services, so consolidated results are not a pure insurance comparison.
Humana reported that 82.6% of its Insurance segment premiums and services revenue came from Medicare in 2025. That is a segment revenue share, not a percentage of consolidated group revenue, and it should not be compared directly with UnitedHealth’s consolidated CMS-premium share.
Humana’s concentration means Medicare policy, risk adjustment, star ratings, and utilization deserve more weight in its analysis than they would for a diversified insurer. Conversely, a diversified group may have more stable consolidated results while a particular insurance segment is under pressure. Always read the segment footnotes to determine what is included.
2. Understand MLR, MCR, and benefit ratios
A medical loss ratio (MLR), medical care ratio (MCR), or benefit ratio measures claims or medical costs relative to a defined premium base. In simplified form:

The exact numerator and denominator vary. Some issuers include prescription-drug costs, quality-improvement expenses, or other adjustments; some report a consolidated ratio, while others report ratios only for an insurance segment. Treat the label as a starting point, then use the filing’s definition.
A medical ratio is not a profit margin
If an insurer reports a 90% medical ratio, the remaining 10% is not automatically profit. Administrative costs, selling expenses, technology, taxes, interest, reserve changes, amortization, and non-insurance operations still affect earnings. Compare the medical ratio with an operating expense ratio and an operating margin.
| Measure | Question it answers | Common interpretation mistake |
|---|---|---|
| Medical or benefit ratio | How much of the defined premium base was consumed by medical benefits? | Calling 100% minus the ratio “profit.” |
| Administrative or operating expense ratio | How much was spent on running and selling the business? | Assuming the ratio is calculated on the same denominator at every company. |
| Underwriting margin | Did premiums cover medical and related insurance costs before investment income and other items? | Ignoring reserve development or premium-deficiency charges. |
| Operating margin | How much operating income was produced from total revenue? | Comparing adjusted margins with GAAP margins without reconciling adjustments. |
| Net margin | What remained after operating items, interest, taxes, and other non-operating effects? | Using it alone to judge underwriting quality. |
Illustrative reported ratios
| Company or source | Period | Reported ratio | Qualification |
|---|---|---|---|
| Humana | Full year 2025 | 90.2% consolidated benefit ratio | Included approximately 80 basis points of favorable prior-period medical-claims reserve development. |
| Humana | Second quarter 2026 | 91.1% consolidated benefit ratio | Quarterly result; member mix and reserve development affected year-over-year movement. |
| Humana | First half 2026 | 90.2% consolidated benefit ratio | Six-month measure, not directly interchangeable with a quarter. |
| Molina Healthcare | Full year 2025 | 91.7% MCR | Up from 89.1% in 2024, primarily because utilization was higher than expected and member acuity shifted. |
| CVS Health | Second quarter 2026 | 87.4% medical benefit ratio | Improvement partly reflected better underlying Government performance and the absence of a $471 million prior-year premium-deficiency reserve. |
| Oliver Wyman comparison | Fourth quarter 2025 | Aetna 94.8%; Elevance 93.5%; UnitedHealthcare 92.4%; Cigna 88.4% | Third-party quarterly comparison; seasonal variation and differing definitions mean it is context, not a buy ranking. |
The table demonstrates why a lower reported ratio is not automatically better. A ratio may improve because of favorable reserve development, a one-time reserve release, a change in member mix, or unusually low seasonality rather than better recurring pricing and claims management.

Plot each company’s medical ratio, premium growth, membership, operating margin, and operating cash flow for several quarters. Then explain each significant change using the filing’s reconciliation.
Separate current claims from prior-period reserve development
Insurers estimate claims before all bills arrive. When later information shows that an earlier estimate was too high, favorable development lowers the current period’s reported costs; unfavorable development raises them. A ratio that benefits from a reserve release can look strong even while current-year claims are deteriorating.
Humana’s 2025 consolidated benefit ratio included approximately 80 basis points of favorable prior-period development. Treat that contribution as a historical adjustment, not as a recurring improvement in underlying medical trend.
Account for seasonality
Utilization varies through the year. A fourth-quarter ratio can differ from a first-quarter ratio because of enrollment timing, elective-care patterns, flu and respiratory seasons, and the timing of claims submissions. Oliver Wyman specifically cautioned that its fourth-quarter 2025 cross-company figures reflect seasonal variation. Compare the same quarter across years or use a full-year view before drawing conclusions.

Investigate utilization, acuity, and pricing
Management commentary should identify whether claims changed because members used more services, the average member became sicker, provider prices rose, benefits changed, or an earlier pricing assumption was wrong. Molina said its 2025 MCR rose to 91.7% from 89.1% in 2024 primarily because utilization was higher than expected and member acuity shifted. Its 2025 operating income fell to $781 million from $1.707 billion in 2024, showing how a seemingly modest ratio change can have a large earnings effect on a high-volume premium base.

Check the bridge from reported to adjusted results
Read the reconciliation for adjusted operating income and adjusted earnings per share. Identify restructuring, acquisition integration, amortization, litigation, reserve changes, premium-deficiency reserves, and other exclusions. Use adjusted figures to understand management’s view of recurring operations, but keep GAAP results in the model because cash, capital, and valuation ultimately depend on reported obligations as well.
4. Evaluate government-program exposure
Government business can be attractive and durable, but its economics depend on rules outside an insurer’s control. Identify the products and subsidiaries exposed to each program before extrapolating growth.
Medicare Advantage and Part D
- Base payments and bids: Federal payment rates and submitted bids influence premium revenue and benefit design.
- Risk adjustment: Documented diagnoses affect payments; coding changes or audit results can alter revenue.
- Quality ratings: Star ratings can affect bonuses and member enrollment, while weaker ratings can pressure pricing and benefits.
- Utilization and acuity: Higher service use or a sicker-than-priced membership can raise the medical ratio.
- Part D rules: Prescription-drug benefit changes can shift both revenue and claims exposure.
UnitedHealth said continued funding pressure and higher care patterns affected its 2026 Medicare Advantage outlook. That statement is a reminder to distinguish membership growth from profitable growth: adding members at inadequate rates can reduce earnings.
Medicaid
Medicaid results depend on state contract rates, eligibility and redetermination activity, enrollment churn, acuity, and state procurement decisions. A post-redetermination membership decline may reduce revenue but leave fixed administrative costs in place. Conversely, rapid enrollment growth can strain claims operations if rates lag the risk of new cohorts.
Commercial insurance and ACA rules
Commercial fully insured plans are exposed to employer pricing, retention, provider contracts, and medical trend. The Affordable Care Act’s minimum-loss-ratio rules can require rebates when an issuer’s ratio falls below the applicable threshold after permitted adjustments. Rebates affect pricing and cash timing, so do not interpret a low ratio without checking whether a future rebate obligation exists.
5. Test membership growth for quality, not just size
Enrollment is a scale indicator, not a profitability measure. Break growth into retained members, new sales, acquisitions, product exits, and changes in government eligibility.
Questions to ask about enrollment
- Which products and geographies produced the growth?
- Did membership come from an acquisition, an automatic assignment, or organic sales?
- Are new members healthier or sicker than the existing book?
- Did the company exit unprofitable counties or products?
- Are premium rates keeping pace with medical trend for the incoming cohort?
- What happened to retention and disenrollment?
Elevance Health reported 44.9 million medical members on June 30, 2026. That scale provides context for operating leverage and negotiating reach, but it does not establish underwriting quality or future margins. Pair every enrollment figure with product mix, ratio, pricing, and operating-income data.

Cash-flow checks
- Compare operating cash flow with net income over several years.
- Investigate large differences caused by claims-payable timing, premium receipts, reserve changes, or one-time settlements.
- Separate unrestricted parent cash from cash trapped in regulated subsidiaries.
- Check capital spending and acquisitions before calculating a free-cash-flow yield.
NAIC reported adjusted operating cash flow of negative $3.4 billion for the aggregate U.S. health insurance industry in 2025. That sector figure is not a forecast for any individual company, but it illustrates why an earnings-only comparison can miss cash pressure.
Debt and interest
Record total debt, maturities, average interest cost, and interest coverage. A high-debt insurer may report acceptable operating margins while having less flexibility to absorb adverse claims development or regulatory capital needs. Include lease obligations and acquisition-related debt where material.
Risk-based capital and surplus
Insurance subsidiaries must maintain capital under state regulation. Parent-company cash cannot automatically be moved into a regulated entity without regulatory approval. Review statutory capital and surplus, risk-based-capital disclosures where available, dividend capacity, and any capital-management restrictions.
NAIC reported aggregate industry capital and surplus of $223.9 billion in 2025, up 4.9% from 2024 in its measure excluding affiliated amounts. NAIC’s Financial Data Repository contains ten years of annual and quarterly statutory data, covers more than 98% of U.S. premiums and more than 4,500 insurers, and supports financial-ratio and risk-based-capital analysis. Use those filings to validate company-reported trends.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.7. Put industry conditions in context
Industry aggregates can reveal the environment but cannot substitute for issuer analysis. NAIC reported that the U.S. health insurance industry’s aggregate combined ratio rose to 101% in 2025, with an aggregate loss ratio of 90.3%. The industry recorded an $8.1 billion net underwriting loss but remained profitable with help from investment income.
NAIC also reported a sector profit margin of 0.4% in 2025, down from 0.8% in 2024; hospital and medical expenses increased 14.8%, or $155 billion; and investment income reached $14.8 billion, up 6.6% year over year. These figures suggest a high-cost, low-underwriting-margin environment in which investment income can materially affect results. They should not be used as a company-level earnings forecast.
8. Normalize valuation before comparing stocks
Valuation is meaningful only when the price date, earnings period, and expectations are consistent. Build the comparison after normalizing unusual claims and reserve items.
Recommended Free Tools
Use several valuation lenses
| Measure | Useful for | Normalization required |
|---|---|---|
| Trailing P/E | Comparing price with recently reported earnings | Remove or separately show unusual reserve development, impairments, and nonrecurring charges. |
| Forward P/E | Comparing price with expected earnings | Use the same forecast date and assumptions for medical trend, membership, rates, taxes, and capital returns. |
| Free-cash-flow yield | Assessing cash generation relative to market value | Adjust for working-capital swings, acquisitions, capital spending, and regulated cash restrictions. |
| Enterprise-value measures | Comparing companies with different debt levels | Define enterprise value consistently and avoid mixing insurance and services earnings without segment detail. |
| Dividend yield and payout | Assessing shareholder distributions | Check statutory surplus, debt covenants, cash generation, and sustainability rather than yield alone. |
Use a shared market-price date and a common set of forward assumptions. No verified same-date valuation comparison is provided, so this comparison cannot support a claim that one named insurer is the cheapest or most attractive.

9. A repeatable comparison workflow
- Collect primary filings: Download each company’s latest annual report, quarterly filing, earnings release, segment footnotes, and non-GAAP reconciliation.
- Build the mix table: Record premium, membership, and operating income by commercial, Medicare, Medicaid, pharmacy, care delivery, and other services.
- Standardize ratios: Write down each issuer’s MLR, MCR, or benefit-ratio definition and denominator before entering the figure.
- Chart at least four quarters: Compare the same quarter year over year and calculate a full-year or trailing-period view.
- Explain every major movement: Tag changes as pricing, utilization, acuity, membership mix, seasonality, reserve development, benefit changes, or one-time items.
- Reconcile earnings: Keep GAAP operating income and adjusted operating income in separate columns.
- Stress government exposure: Identify sensitivity to Medicare rates, risk adjustment, ratings, Part D, Medicaid rates, eligibility, and ACA rebates.
- Check cash and capital: Compare operating cash flow, debt, interest, liquidity, statutory surplus, and risk-based-capital information.
- Normalize valuation: Use one price date, consistent forecasts, and normalized earnings; show the assumptions behind each multiple.
- Write the conclusion as a risk-adjusted judgment: State which assumptions must hold, what could invalidate them, and what evidence would change your view.
Common comparison mistakes and how to avoid them
Mistake: treating 100% minus MLR as profit
Medical costs are only one part of the income statement. Add administrative costs, taxes, interest, reserve movements, and non-insurance results before estimating profitability.
Mistake: comparing consolidated and segment ratios
A consolidated benefit ratio may include businesses or eliminations that are absent from another company’s insurance-segment ratio. Match the scope or label the comparison as approximate.
Mistake: rewarding the lowest quarterly ratio
Seasonality, reserve releases, and a favorable member mix can temporarily depress a ratio. Look for repeatable improvement across several periods and reconcile the bridge.

Membership growth can reduce margins when rates lag claims or new cohorts are riskier. Pair enrollment with pricing, acuity, utilization, and retention.
Mistake: counting parent cash as insurer capital
Regulated subsidiaries have separate capital requirements and dividend constraints. Review statutory filings rather than assuming all consolidated cash is available.
Mistake: comparing valuations from different dates
Share prices, analyst forecasts, interest rates, and policy expectations change. Record the date and use consistent forward estimates for every company.
What a well-supported investment comparison should say
A defensible comparison does not end with “Company A has the lowest MLR.” It explains which business mix produced the ratio, whether the result included reserve development, how membership and pricing are changing, what government rules could alter revenue, whether earnings convert to cash, and whether regulated capital can support growth and distributions.
