How to Compare Health Insurance Premiums, Deductibles, and Out-of-Pocket Costs

By TheFinanceBase Team 10 min read

The least expensive health plan is rarely the one with the lowest monthly premium alone. To compare plans, annualize each premium, estimate the cost sharing you expect to use, and then check your maximum possible spending for covered, in-network care. HealthCare.gov puts it plainly: "When picking a Marketplace health plan, it’s important to compare your estimated total yearly costs for each plan – not just the premium."

This method applies to plans offered through the U.S. Health Insurance Marketplace. Employer coverage and policies outside the United States may use different terms, limits, and rules. Your result is an estimate, not a guaranteed bill: actual spending depends on the services, medicines, providers, and allowed amounts you use.

Premium

The premium is the amount you pay to keep coverage active, usually every month. You owe it whether or not you visit a doctor. Calculate the annual premium as:

Monthly premium × 12 = annual premium

A premium may be reduced by an advance premium tax credit if you qualify. Use the amount you are actually expected to pay after any credit when comparing your household budget, but retain the unsubsidized amount when checking whether eligibility or income changes could alter your bill.

Deductible

The deductible is the amount you pay for certain covered services before the plan begins paying under its rules. It may not apply to every service. For example, a plan can cover specified preventive services before the deductible, while requiring you to meet the deductible for other care. A family plan can have both an individual deductible and a larger family deductible, and some plans use separate deductibles for medical and prescription benefits.

Copayment

A copayment, or copay, is a fixed amount for a covered service, such as a stated amount for a primary-care visit or a prescription tier. Check whether the copay applies before or after the deductible and whether different facilities, specialists, urgent-care centers, and emergency departments have different amounts.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Coinsurance

Coinsurance is a percentage of the allowed cost that you pay. HealthCare.gov uses 20% coinsurance in an illustrative example after a deductible; that percentage is not a universal Marketplace rate. If the allowed amount is $1,000 and your coinsurance is 20%, your share is $200, subject to the plan’s deductible and other rules.

Out-of-pocket maximum

The out-of-pocket maximum is a limit on qualifying cost sharing for covered, in-network care during a plan year. After you reach it, the plan pays 100% of covered in-network benefits for the rest of that period under the plan’s terms. Premiums do not count toward the maximum. Neither do uncovered services, most out-of-network spending, or charges above the plan’s allowed amount.

Use the maximum as a high-use exposure check, not as a prediction. A person who never reaches the deductible will not automatically spend the maximum, and a person using out-of-network care can face bills that do not receive the maximum’s protection.

A worksheet for estimating yearly cost

Use one worksheet for every plan and keep the household, doctors, prescriptions, and care assumptions identical. The comparison is useful only when the inputs are consistent.

  1. Record the net monthly premium. Multiply it by 12. If the plan is offered through an employer, include the employee share, not the employer’s contribution.
  2. List expected care. Separate routine primary-care visits, specialist visits, therapy, laboratory work, imaging, urgent care, emergency care, hospital treatment, durable medical equipment, and each regular prescription. Include planned procedures, pregnancy care, or recurring treatment when relevant.
  3. Apply the plan’s rule to each item. Identify whether the service has a copay, is subject to the deductible, uses coinsurance, or is excluded. Use the plan’s allowed amount rather than a provider’s undiscounted charge.
  4. Add prescription spending. Note whether medicines are subject to a separate pharmacy deductible, a fixed tier copay, coinsurance, prior authorization, or quantity limits. Confirm that the exact drug and pharmacy are in network.
  5. Calculate expected annual spending. Add the annual premium to estimated cost sharing. This is an expected total, not a promise that your bill will equal the result.
  6. Run a high-use check. Add the annual premium to the plan’s out-of-pocket maximum for a rough upper exposure for covered, in-network cost sharing. Keep excluded spending separate.
  7. Repeat at three utilization levels. HealthCare.gov describes low, medium, and high expected-care assumptions. Use all three so one unusually healthy or unusually expensive year does not determine the choice.

Low-use scenario

Include the premium and only the care you are reasonably likely to receive, such as preventive visits, a few generic prescriptions, or occasional primary-care appointments. Do not assume that every service is free; check the plan’s preventive-service and prescription provisions.

Medium-use scenario

Add the visits, tests, specialist appointments, and medicines the household commonly uses. A chronic condition, regular therapy, or predictable annual imaging belongs here even if the previous year was different.

High-use scenario

Model substantial covered care and compare the resulting cost sharing with the out-of-pocket maximum. Once qualifying in-network cost sharing reaches the limit, additional covered in-network services for that plan year are paid at 100% under the plan terms. Premiums still continue.

Compare plans on the same decision axes

A side-by-side table prevents a low premium from hiding a large deductible or an unsuitable network. Fill in the figures from each plan’s Summary of Benefits and Coverage and the full policy documents.

Measure Why it matters What to record
Annual premium Fixed spending even when you use no care Monthly premium × 12, after any applicable credit
Deductible structure Determines how much you may pay before the plan shares costs Individual and family amounts; medical and pharmacy deductibles; services exempt from the deductible
Office and facility charges Shows the price of routine and high-cost visits Primary care, specialist, urgent care, emergency room, inpatient and outpatient rules
Prescription cost sharing Regular medicines can dominate annual spending Drug tiers, copays or coinsurance, pharmacy network, deductible, authorization and quantity rules
Coinsurance A percentage can become large when allowed charges are high Percentage, the services it applies to, and whether the deductible comes first
Out-of-pocket maximum Limits qualifying in-network cost sharing in a high-use year Individual and family maximum, what counts, and the plan-year dates
Network and coverage A cheap cost-sharing design is not useful if needed care is out of network or excluded Doctors, hospitals, pharmacies, medicines, referrals, prior authorization and exclusions

Compare the amount you expect to spend, the cash you must have available before the deductible is met, and the worst-case covered in-network cost sharing. A plan can win on expected annual cost but be difficult to fund during the first few months of a major illness.

Worked example: how the trade-off changes with use

The following figures are hypothetical to demonstrate the arithmetic; they are not a recommendation or a typical plan. Suppose Plan A costs $250 per month, has a $6,000 deductible, 20% coinsurance after the deductible, and a $8,500 out-of-pocket maximum. Plan B costs $425 per month, has a $1,500 deductible, $40 primary-care copays, 20% specialist coinsurance after the deductible, and a $5,500 out-of-pocket maximum.

Item Plan A Plan B
Annual premium $250 × 12 = $3,000 $425 × 12 = $5,100
Low-use cost sharing (illustrative) $500 $350
Low-use estimated total $3,500 $5,450
High-use covered in-network exposure before premiums Up to $8,500 Up to $5,500
High-use estimated total including premiums Up to $11,500 Up to $10,600

In this illustration, Plan A has the lower cost in a light-use year because its premium is $2,100 lower annually. Plan B requires more premium but reduces the deductible and the maximum exposure. The right choice depends on the probability and timing of care, available cash, and whether both plans cover the providers and medicines needed. The arithmetic does not establish what either plan would charge for a real claim.

How the out-of-pocket maximum should influence your choice

Look at the maximum in two ways:

  • Annual exposure: add the maximum to the annual premium to see the approximate upper spending for covered, in-network cost sharing plus premiums.
  • Cash-flow exposure: determine how quickly you could be required to pay the deductible, coinsurance, or copays before reimbursements or payment plans.

The federal Marketplace maximum limits shown by HealthCare.gov are $10,600 for an individual and $21,200 for a family for the 2026 plan year, and $12,000 for an individual and $24,000 for a family for 2027. These are federal plan-year limits, not a statement that every plan uses those exact amounts. They do not cap premiums, uncovered care, out-of-network care, or charges above the allowed amount. Verify the applicable plan year because these figures can change.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Family plans can expose one member to an individual limit while the family limit applies to the household. Read the plan’s explanation of individual and family accumulators rather than assuming every dollar paid by one member is credited in the same way.

Marketplace premiums, metal categories, and financial help

What affects a Marketplace premium

Under the Marketplace rules described by HealthCare.gov, premiums can depend on location, age, tobacco use, plan category, and whether dependents are covered. State limits can affect how much those rating factors change the price. Health status, medical history, and sex cannot be used to set the premium under those rules.

A preview price is an estimate based on the information entered. HealthCare.gov says the exact price follows a completed application and eligibility determination. Check the plan year and final result before treating a displayed premium as final.

What metal categories do and do not tell you

Bronze plans usually have lower premiums and higher out-of-pocket costs. Platinum plans usually have higher premiums and lower out-of-pocket costs. The categories describe typical cost-sharing patterns; they do not guarantee a particular deductible, copay, coinsurance rate, network, or out-of-pocket maximum. Silver and Gold plans can also differ substantially from one another, so inspect the actual figures.

Cost-sharing reductions

Eligible consumers receive Marketplace cost-sharing reductions only by enrolling in a Silver plan. A reduction can lower the deductible, copayments, coinsurance, and out-of-pocket maximum. Compare the Silver plan’s reduced figures, not the standard figures shown before eligibility is applied.

Premium tax credits

Premium tax credits are a separate form of savings and may be used with plans in other metal categories, subject to eligibility and current rules. Do not assume that a plan with the smallest displayed premium remains the least expensive after your eligibility result, or that a lower premium compensates for a deductible you cannot comfortably fund.

Read the plan documents before enrolling

The Summary of Benefits and Coverage is a starting point, not the entire contract. Use it with the policy, evidence of coverage, provider directory, formulary, and any Marketplace eligibility notice.

  • Confirm that your preferred primary-care clinician, specialists, hospital, laboratory, and pharmacy are in network for the exact plan, not merely for the insurer generally.
  • Check whether referrals are required and whether prior authorization applies to imaging, specialist care, procedures, or expensive medicines.
  • Identify the allowed amount and your responsibility for out-of-network claims. A provider’s billed charge can exceed the allowed amount, and the excess may not count toward the maximum.
  • Check whether deductibles and out-of-pocket limits reset on the calendar year or another plan-year date.
  • Review separate medical and prescription accumulators, family rules, and exclusions.
  • Verify the coverage year. A deductible or federal maximum from one year cannot safely be carried into another.

Policy terms govern when a glossary definition and a plan document appear to differ. HealthCare.gov notes that insurance terms can vary by policy, so use the plan’s own documents for the final interpretation.

Common comparison mistakes and how to avoid them

Choosing by premium alone

A low premium can be offset by a large deductible, coinsurance on hospital care, or expensive prescriptions. Always add the annual premium to a realistic cost-sharing estimate.

Treating the deductible as the total you will pay

The deductible is only one threshold. After it, you may still owe copays or coinsurance until reaching the out-of-pocket maximum. Conversely, some services can have a copay before the deductible. Map each service to its actual rule.

Calling the out-of-pocket maximum a total-cost cap

Premiums, uncovered services, out-of-network spending, and amounts above allowed charges can remain payable after you reach the maximum. Keep those categories outside the maximum calculation.

Comparing different care assumptions

Do not compare one plan using last year’s visits with another plan using a planned surgery. Use the same low, medium, and high scenarios for every option, then change one assumption at a time to see which result moves.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ignoring network and formulary details

A mathematically attractive plan can become costly if a needed doctor, hospital, or medicine is out of network or excluded. Verify names and drug coverage before relying on the estimate.

Using a preview price as a final quote

Marketplace previews are estimates. Complete the application and check the final eligibility result, effective date, and plan-year documents.

A practical decision rule

Start with the plan that covers the care you actually need. Among plans with suitable networks and benefits, compare expected annual cost under low, medium, and high use. Then ask whether you could pay the deductible and early cost sharing from available cash. Favor a higher premium when the reduction in expected cost sharing or maximum exposure is worth it for your likely care and risk tolerance; favor a lower premium when use is likely to be light and you can accept the larger exposure.

Document the assumptions beside every estimate: household members, plan year, premium after or before credits, expected visits, prescriptions, allowed amounts, and whether each expense is in network. That record makes it easier to update the comparison when income, eligibility, providers, medicines, or plan documents change.
