Compare each option’s annual premium, likely cost sharing for the care you expect, and worst-case in-network exposure under the out-of-pocket limit. Then verify that the doctors, hospitals, medicines, and pharmacies you need are covered.
HealthCare.gov summarizes the decision this way: “When picking a Marketplace health plan, it’s important to compare your estimated total yearly costs for each plan – not just the premium.” Use the method below with the actual plan documents for your location and plan year; availability, subsidies, networks, and benefits vary.

What to compare first
For every plan, put these figures side by side:
- Annual premium: the monthly premium multiplied by 12.
- Expected cost sharing: the deductible, copayments, coinsurance, and prescription-drug charges you are likely to pay.
- In-network out-of-pocket maximum: the plan-year ceiling for eligible cost sharing on covered, in-network care.
- Coverage fit: whether your clinicians, facilities, services, medicines, and pharmacies are covered under the plan’s current network and formulary.
- Access rules: referrals, prior authorization, and the financial consequences of using out-of-network care.
A low projected total is not useful if a needed specialist is outside the network or a regular medicine is excluded. Plan details can change at renewal, so repeat the comparison each year.

Understand the four numbers that drive your bill
Premium
The premium is the recurring amount paid to keep coverage active, whether or not you use medical care. For a simple annual baseline, calculate monthly premium × 12. Use the premium shown for your own household, location, eligibility, and subsidy status rather than a national example. Premiums are separate from deductibles, copayments, and coinsurance.
Deductible
The deductible is what you pay for certain covered services and prescription drugs before the plan begins paying under its terms. Preventive services may be covered before the deductible, and some plans have separate medical and drug deductibles or copays that bypass the deductible. Read the SBC to see which services are subject to it.
Recommended Free Tools
Meeting the deductible does not usually make all care free. You may still owe a fixed copayment or a percentage coinsurance for covered services until you reach the out-of-pocket maximum.
Copayment and coinsurance
A copayment is a fixed amount for a covered service, such as a primary-care visit. Coinsurance is a percentage of the plan’s allowed cost. The percentage normally applies after any applicable deductible, although the plan document controls. Check whether office visits, emergency care, hospital services, laboratory work, imaging, and each drug tier use copays, coinsurance, or both.
Out-of-pocket maximum or limit
The out-of-pocket maximum is a ceiling on your cost sharing for covered benefits delivered in network during the plan year. After you reach it, the plan pays 100% of covered benefits within that defined scope for the remainder of the coverage period.
It is not a ceiling on every health-related expense. Monthly premiums, non-covered services, out-of-network care, and charges above a plan’s allowed amount do not count. A provider who bills more than the allowed amount can therefore create spending outside the limit, subject to applicable consumer protections.

For Marketplace plans in plan year 2026, HealthCare.gov states that the permitted out-of-pocket limit can be no more than $10,600 for an individual or $21,200 for a family. Those are maximum permitted limits, not the amount every plan charges and not a cap on premiums or excluded expenses. The rule is specific to Marketplace coverage; do not apply it automatically to employer, Medicare, short-term, or other coverage.
See the official definitions for the out-of-pocket maximum and total health-care costs.
Calculate an expected annual cost
Use a consistent estimate for each plan instead of comparing isolated line items.
- Annualize the premium. Multiply the quoted monthly premium by 12. If a subsidy or employer contribution applies, use the amount you actually pay.
- List expected care. Include routine visits, specialist appointments, therapy, recurring prescriptions, laboratory tests, imaging, and planned procedures. Separate predictable care from an unlikely emergency.
- Apply each plan’s cost-sharing rules. For every item, check whether the deductible applies, then use the stated copay or coinsurance and the plan’s allowed amount. Use in-network rates for a comparable calculation.
- Add the member share. Sum the estimated copays, coinsurance, and deductible payments for the year. Keep prescription costs separate when the plan has a distinct drug deductible or formulary tiers.
- Add the annual premium. This produces an estimated normal-use total, not a guarantee.
- Run a high-use scenario. Model substantial covered care and compare the result with the in-network out-of-pocket maximum. Add premiums separately, because they never become part of that maximum.
A useful worksheet is:
| Comparison item | Plan A | Plan B | What to verify |
|---|---|---|---|
| Monthly premium | Enter quote | Enter quote | Your household, location, subsidy, and plan year |
| Annual premium | Monthly amount × 12 | Monthly amount × 12 | Whether the quoted amount recurs for all 12 months |
| Medical deductible | Enter SBC amount | Enter SBC amount | Services subject to it and preventive-care exceptions |
| Drug deductible | Enter amount or “none stated” | Enter amount or “none stated” | Drug tiers and whether the medical deductible applies |
| Primary/specialist visit | Copay or coinsurance | Copay or coinsurance | Before or after deductible; referral requirement |

| Out-of-network rules | HMO/PPO/POS/EPO terms | HMO/PPO/POS/EPO terms | Whether non-emergency out-of-network care is covered |
For a more formal calculation, write estimated yearly total = annual premium + estimated member cost sharing. Treat the output as a planning estimate. It cannot predict an unknown diagnosis, an unavailable provider, a changed formulary, or a bill based on a different allowed amount.
Worked example: how the deductible and coinsurance interact
HealthCare.gov gives an illustrative plan with a $1,500 deductible, 20% coinsurance, and a $5,000 out-of-pocket maximum. Suppose the member has already met the deductible and receives a covered visit with an allowed cost of $125. At 20% coinsurance, the member pays $25 and the plan pays $100.
If the member later reaches $5,000 in included cost sharing for covered health services, the plan pays the full cost of covered health services for the rest of that coverage period, within the plan’s defined scope. The illustration is not a current plan quote, and prescription-drug cost sharing can follow different rules.
The example shows why a premium-only comparison can mislead: one plan may charge more each month but expose you to less cost sharing when you use specialists, prescriptions, or hospital services.

Should you choose a lower premium or a lower deductible?
Neither feature wins in every household. Use your expected utilization and ability to absorb a large bill.

- You expect little care beyond preventive services and occasional primary-care visits.
- You have enough savings to handle the plan’s deductible and coinsurance if an unexpected event occurs.
- The plan’s drug tiers and network include the care you need.
- Its out-of-pocket maximum is affordable in a bad year, even if it is higher than another option.
A higher-premium, lower-deductible plan may fit when
- You regularly see specialists, use expensive medicines, receive therapy, or have planned procedures.
- You prefer more predictable copays instead of percentage-based bills.
- A large deductible would create a cash-flow problem.
- The lower cost-sharing design also provides a meaningfully lower in-network out-of-pocket maximum.
Use a break-even check
Calculate the annual premium difference between two plans. Then estimate how much additional cost sharing the lower-premium plan would impose for your expected services. If expected extra cost sharing exceeds the premium savings, the higher-premium plan may be cheaper for that usage pattern. Run the same comparison with a high-use scenario; do not assume the deductible alone determines the result, because copays, coinsurance, drug rules, and the out-of-pocket limit also matter.
Network design can outweigh a cost estimate
Metal categories describe how costs are shared, not the quality of care. Bronze, Silver, Gold, and Platinum plans can have different premiums, deductibles, networks, and referral rules. A higher metal category is not a quality rating.
Network type affects both access and financial risk:
| Network label | Questions to ask |
|---|---|
| HMO | Is a primary-care clinician required? Do you need referrals for specialists? Is non-emergency out-of-network care generally excluded? |
| PPO | Are out-of-network services covered at a higher member cost, and do they have a separate deductible or limit? |
| POS | Which services require referrals, and how does the plan price in-network versus out-of-network care? |
| EPO | Is the network effectively limited to in-network care except for emergencies? |
These are general design patterns, not promises for a particular plan. Confirm the actual rules in the SBC and evidence of coverage. Search the directory for the individual doctor, the exact hospital campus, outpatient facility, and any anesthesiology, radiology, or laboratory group involved in planned care.
HealthCare.gov’s plan-comparison checklist recommends checking doctors, providers, services, drugs, and pharmacies before enrolling.
Check prescription coverage in detail
Do not stop at a plan’s statement that prescription drugs are covered. Search the formulary for each medicine and record its tier, copay or coinsurance, deductible treatment, prior-authorization requirement, quantity limit, step-therapy rule, and preferred pharmacies. A drug covered at a non-preferred pharmacy can cost more than the same drug at a preferred location.

Ask the prescriber whether an equivalent covered medicine is clinically appropriate, but do not switch treatment solely to make a spreadsheet look cheaper. If a medicine is missing from the formulary, find out whether an exception or appeal process exists and what documentation it requires.

Emergency and consumer-protection rules can affect how certain out-of-network bills are handled, but do not assume every out-of-network invoice is protected. Contact the plan before planned care and ask which clinicians and facilities will bill in network.
Compare plans at renewal, not just at first enrollment
Coverage and networks can change from one plan year to the next. The Centers for Medicare & Medicaid Services’ September 2025 checklist advises Marketplace enrollees to review affordability of the premium and deductible, the out-of-pocket limit, doctors and providers, covered services, prescription drugs, and pharmacies. Use the current documents rather than last year’s notes.
- Download the new SBC and evidence of coverage.
- Recalculate annual premiums using the new household price and any updated financial assistance.
- Repeat your expected-care and high-use estimates.
- Recheck every regular doctor, facility, medicine, and pharmacy in the new directory and formulary.
- Confirm referral, prior-authorization, and out-of-network rules for planned treatment.
- Save screenshots or PDFs of the documents and the date you checked them.

Common comparison mistakes and fixes
Choosing by premium alone
Fix: add annual premiums to expected cost sharing and test a high-use year.
Treating the deductible as the total you could owe
Fix: include post-deductible copays or coinsurance and identify the out-of-pocket maximum.

Counting premiums toward the out-of-pocket maximum
Fix: keep premium payments in a separate column; they are not cost sharing.
Assuming the maximum covers every bill
Fix: exclude premiums, non-covered care, out-of-network charges, and amounts above allowed charges from the protected total.
Assuming a metal category measures quality
Fix: compare actual benefits, network access, quality information available in the plan materials, and your clinicians’ participation.
Trusting a directory without confirming
Fix: call the plan and the provider before enrollment or a planned procedure, especially when multiple clinicians or facilities will bill separately.
Using a generic national price
Fix: use the quote for your household, location, eligibility, and plan year. Marketplace estimates are not guarantees.
A practical decision framework
After filling in the worksheet, narrow the choice in this order:
- Eliminate coverage mismatches. Remove plans that do not cover a required doctor, facility, medicine, or service under acceptable terms.
- Protect against an unaffordable bad year. Compare annual premium plus the in-network out-of-pocket maximum. This is a planning ceiling for eligible covered care, not a universal spending cap.
- Price your ordinary year. Apply your expected visits, prescriptions, and planned care to each plan’s actual rules.
- Check cash flow. Ask whether you could pay the deductible or a large coinsurance bill when care occurs, even if the annual estimate looks favorable.
- Confirm operational rules. Verify referrals, prior authorization, pharmacy requirements, and the process for finding in-network providers.
- Keep the documents. Save the SBC, formulary, directory results, and insurer answers that support your decision.
Because location, eligibility, utilization, and benefit terms determine the result, no article can calculate your exact annual spending without your plan quotes and household details. The HealthCare.gov total cost estimate definition likewise treats the figure as an estimate rather than a promise.
