If your job-based health insurance is ending, compare three paths for the same household and coverage dates: a Marketplace plan, COBRA continuation, and Medicaid or CHIP. Losing employer coverage can trigger a Marketplace Special Enrollment Period (SEP). HealthCare.gov says to apply within 60 days after coverage is lost, and a Marketplace plan can start the first day of the month after the employer plan ends. The application also screens for premium tax credits, other savings, and Medicaid or CHIP.
Do not choose by premium alone. Check the actual effective date, total expected cost, doctors and hospitals, prescriptions, and whether you can document the loss of coverage. Prices, networks and eligibility depend on your location, household, income and plan documents.

Your main options when employer coverage ends
Marketplace coverage through a Special Enrollment Period
Loss of job-based coverage generally qualifies you for a Marketplace SEP, including when you quit or were fired, provided the event is the loss of qualifying coverage. HealthCare.gov says to apply within 60 days after the coverage ends. The application uses your household information to determine whether you qualify for premium tax credits, other savings, Medicaid or CHIP.
You may be asked for documents showing that the employer plan ended. If you do not submit required proof, the Marketplace may decide that you do not qualify for the SEP. Keep the employer termination notice, insurance termination letter, or other acceptable evidence identified in your application instructions.

COBRA continuation of the employer plan
COBRA may let the former employee and covered family members continue the same employer-sponsored group plan temporarily. HealthCare.gov describes COBRA as usually lasting 18 months. You generally pay the entire premium, including the amount the employer previously contributed, plus a small administrative fee.
Keeping the existing plan can be valuable when you are in active treatment, want to keep established doctors, or have already met much of the plan deductible. It is not automatically the least expensive choice: compare the full COBRA bill with Marketplace prices after any available savings and with the cost sharing you expect to use.
HealthCare.gov states, “You don’t have to enroll in COBRA.” Treat the election notice as a deadline-driven decision, not as an instruction to take COBRA automatically.

Submit the Marketplace application even if you think you will not qualify. It can screen household members for Medicaid or CHIP. Eligible applicants can enroll in those programs at any time, but eligibility follows household and state program rules. Unemployment by itself does not establish eligibility.
Wait for a final eligibility decision before cancelling existing coverage. A pending application is not the same as an approval, and cancelling too soon can create an avoidable gap.

Start with dates and the people who need coverage
Write down the last day each person is covered by the employer plan and the date a replacement plan could begin. Compare the same people over the same period; otherwise a plan can look cheaper simply because it covers fewer family members or starts later.
- List everyone currently on the employer plan, including a spouse and children.
- Record the employer plan’s termination date and any COBRA election deadline in the notices you received.
- Mark the 60-day Marketplace SEP period beginning after job-based coverage is lost, as described by HealthCare.gov.
- Check the proposed effective date. HealthCare.gov says Marketplace coverage can start the first day of the month after the job-based plan ends; confirm the exact date shown for your application.
- Identify appointments, procedures, prescriptions and refills that must be covered during a transition month.
Do not cancel COBRA or other coverage until you know when replacement coverage is active and have received the relevant eligibility decision or enrollment confirmation.
Compare the options on the same five measures
| Measure | Marketplace plan | COBRA continuation | Medicaid or CHIP |
|---|---|---|---|
| Eligibility and timing | Loss of job-based coverage can create an SEP; HealthCare.gov says to apply within 60 days after the loss. Documents may be required. | Available only if the employer plan and your situation meet COBRA rules; the election notice controls the deadlines. Coverage is usually temporary, generally 18 months according to HealthCare.gov. | Determined by household and state program rules; eligible people can enroll at any time through the application process. |
| Premium | Varies by location, household details and plan. The application displays any premium tax credit or other savings for which you qualify. | Usually the full group-plan premium, including the former employer’s share, plus a small administrative fee. | Depends on the applicable state program and eligibility determination; do not assume a zero-cost result before approval. |
| Doctors, hospitals and prescriptions | Check the plan’s provider directory, pharmacy network and covered drugs before enrolling. | Usually preserves the employer plan’s network and benefits, subject to the plan’s terms. | Check the state program’s participating providers, pharmacies and covered services. |
| Deductible and other cost sharing | Compare deductible, copayments, coinsurance and out-of-pocket limits with the care you expect to use. | The group plan’s existing deductible and cost-sharing rules continue; verify how the plan treats amounts already paid. | Cost sharing follows the applicable program rules and your eligibility category. |
| Coverage continuity | Potentially begins the first day of the month after employer coverage ends, but verify your specific effective date. | Can continue the employer plan if elected and paid on time under the notice terms. | Do not end other insurance until the program sends a final decision and you know the start date. |
No route is universally cheapest or best. Exact premiums, networks and benefits require your state, household, income, doctors, prescriptions and the plan documents or Marketplace results.
Calculate the cost you are most likely to pay
Separate fixed premiums from care-related spending
For each option, estimate the premium for the entire comparison period. Then add the deductible, copayments and coinsurance you are likely to incur for the care your household expects. Include regular visits, specialist care, therapy, laboratory work, hospital services and prescriptions rather than comparing only a healthy-month scenario.

For COBRA, use the complete bill shown in the election materials. That amount normally includes the employer contribution you no longer receive and a small administrative fee. For a Marketplace plan, use the premium after the application shows any tax credit or other savings; do not base the calculation on an advertised full-price premium when your application produces a different amount.
Use two planning scenarios
- Routine-care scenario: add the annual premiums, expected office-visit copayments, prescriptions and ordinary tests.
- High-care scenario: add the annual premiums, deductible exposure, coinsurance for likely treatment and the plan’s out-of-pocket limit if the household could reach it.
This is a decision aid, not a guarantee. A plan’s treatment rules, prior authorization requirements and covered-service limits can change the actual bill. Use the Summary of Benefits and Coverage and current plan documents where available.

Doctors and hospitals
Search the exact plan network for every doctor, hospital and facility you expect to use. A familiar insurer name does not prove that a particular Marketplace product uses the same network as your employer plan. Call the provider and the plan if the directory is unclear, and record the date and answer.
Prescriptions
Check each medication against the plan’s formulary, tier, quantity limits and pharmacy network. Confirm whether your usual pharmacy is in network and whether a prescription requires prior authorization or step therapy. A lower premium can be outweighed by a drug that is excluded or placed on a high cost-sharing tier.

Ongoing treatment
If you are pregnant, receiving cancer treatment, managing a chronic condition, or scheduled for surgery, ask which option preserves access to the treating team and avoids a mid-course authorization problem. COBRA may offer continuity because it keeps the group plan, but compare its full price with the Marketplace alternatives rather than assuming continuity justifies any premium.
How to apply for a Marketplace plan after losing coverage
- Gather household information. Have the names of everyone seeking coverage, your address, expected household income and the date employer coverage ends.
- Submit the application promptly. HealthCare.gov says the SEP application deadline is within 60 days after job-based coverage is lost. Do not wait for a COBRA decision if doing so could push you past that window.
- Report other coverage offers. Include any qualifying offer from a spouse’s or another family member’s employer. Declining that offer can still affect savings eligibility.
- Upload requested proof. Provide acceptable documents confirming the loss of coverage by the deadline in your account. Missing documents can cause the SEP determination to fail.
- Review the eligibility results. The application may show Marketplace savings, Medicaid or CHIP eligibility, or a request for more information.
- Compare available plans. For the same household, review premium, deductible, copayments, coinsurance, out-of-pocket limit, network, prescriptions and effective date.
- Enroll and save confirmation. Keep the enrollment record, first premium instructions and effective-date notice. Coverage is not active merely because you viewed a plan.
Use HealthCare.gov or your state’s Marketplace when making the decision; rules, plan availability and prices can change.
How COBRA timing affects a later switch
You can elect COBRA and later move to a Marketplace plan in some circumstances, but the event that ends COBRA matters. COBRA expiring can qualify you for a Marketplace SEP. Involuntary loss of COBRA eligibility and certain changes in an employer’s contribution may also qualify under current rules.
Voluntarily ending COBRA early generally does not create a Marketplace SEP outside Open Enrollment. Do not cancel COBRA on the assumption that cancellation itself opens a new enrollment window. Before ending it, verify that another qualifying event applies, confirm the Marketplace deadline and establish the replacement plan’s effective date.

If you initially decline COBRA, keep the election notice and deadline information. You may have limited rights to elect it later within the applicable election period, but the notice—not a general assumption—controls your situation.
Account for a spouse’s employer offer
A spouse’s or family member’s employer may offer qualifying coverage after your job ends. Report that offer on the Marketplace application even if you decline it. Whether the offer blocks premium tax credits or other Marketplace savings depends on affordability and minimum-standards rules that apply to the offer and the household.
Recommended Free Tools
Ask the employer for the employee and family premium amounts, the coverage effective date and documents describing the plan. Compare those figures with Marketplace results before rejecting or accepting the offer.
Common mistakes and how to avoid them
- Choosing the lowest monthly premium. Add expected deductibles, copayments, coinsurance and prescriptions before deciding.
- Assuming COBRA is always better. Continuity can matter, but COBRA usually shifts the full premium to you.
- Assuming Marketplace coverage is always cheaper. Savings depend on household information and application results.
- Missing the SEP deadline. Apply within the 60-day period described by HealthCare.gov and respond to document requests.
- Skipping network verification. Check each doctor, hospital, pharmacy and medication in the exact plan you are considering.
- Letting coverage lapse while waiting for Medicaid or CHIP. Keep current coverage until a final eligibility decision and start date are confirmed.
- Cancelling COBRA voluntarily to force enrollment. Early voluntary termination generally does not qualify for an SEP outside Open Enrollment.
- Ignoring a spouse’s offer. An offer can affect savings eligibility even when declined, depending on affordability and minimum-standards rules.
- Confusing an application with active insurance. Save the enrollment confirmation and verify when the first premium is due and when coverage begins.
A practical decision sequence
- Write down the employer plan’s end date, the people who need coverage and any immediate medical needs.
- Apply through the Marketplace within the stated 60-day SEP period and provide proof of the coverage loss if requested.
- Review Marketplace savings and the Medicaid or CHIP screening result for every household member.
- Obtain the COBRA premium, administrative fee, election deadline and continuation period from the notice.
- Build a side-by-side comparison using the same coverage dates and the same care scenarios.
- Verify networks, prescriptions and effective dates directly in the plan materials.
- Choose only after confirming how and when the replacement coverage becomes active; keep existing coverage until then.
When to get help
Contact the Marketplace or your state Medicaid agency when the application requests documents, reports conflicting eligibility results or shows an effective date you do not understand. Contact the employer’s plan administrator for the COBRA premium, election deadline and continuation terms. A licensed insurance counselor or navigator can help explain plan documents, but no adviser can identify the best option without your location, household, income, coverage dates, providers, prescriptions and expected care.
Rules and prices change. Recheck current HealthCare.gov or state Marketplace and Medicaid guidance when you apply, especially if your COBRA coverage is ending, an employer contribution changes, or you receive another qualifying coverage offer.
The bottom line
Compare COBRA, Marketplace coverage and Medicaid or CHIP for the same people and dates. Marketplace enrollment may be available for 60 days after job-based coverage is lost, COBRA usually preserves the employer plan for about 18 months at the full premium, and Medicaid or CHIP can enroll eligible people at any time. The right choice is the one whose verified effective date, total expected cost, provider and prescription access, and eligibility fit your household—not the option with the lowest advertised premium.
