If your employer offers health insurance, do not automatically waive it or accept it. First obtain the plan documents, then compare the job-based offer with every realistic alternative for each household member. The decisive questions are whether the employer offer blocks Marketplace subsidies, what you will pay in a normal and high-use year, whether your doctors and medicines are covered, and whether changing coverage could leave a gap.
This guide uses U.S. federal rules and HealthCare.gov terminology. Marketplace plans, deadlines, networks and prices vary by state. It is general information, not individualized insurance or tax advice.

Start with the employer plan, not the enrollment form
You cannot make a reliable comparison from a payroll deduction alone. Ask human resources or the plan administrator for the current Summary of Benefits and Coverage (SBC), the full plan document, and the enrollment deadline. Gather these items before looking at alternatives:

- Details of any health savings account (HSA), flexible spending account (FSA), health reimbursement arrangement (HRA) or individual coverage HRA (ICHRA), including employer deposits and deadlines.

The SBC is designed to put major benefits, cost-sharing, exclusions and standardized coverage examples in a common format. Use it to compare plans, but do not treat its examples as a forecast of your own bills. Your claims, providers, medicines and year of care will differ.

Check whether the employer offer affects Marketplace savings
An offer of job-based insurance can change your eligibility for Marketplace premium tax credits even if you decline the employer plan. Apply with accurate household, income and employer-offer information to get an eligibility determination instead of guessing.
The 2026 affordability test
For 2026 coverage, HealthCare.gov lists the job-based affordability threshold as 9.96% of household income. The employee test generally uses the employee’s required contribution for the lowest-cost plan that covers only the employee and provides minimum value. If that offer is affordable and meets minimum value, the employee generally cannot receive Marketplace premium tax credits.
The percentage is tied to the applicable plan year. HealthCare.gov’s glossary lists 10.22% for 2027, so do not carry the 2026 figure into a later enrollment year without checking the current guidance.
Why family members can get a different result
Affordability is not necessarily the same for everyone in the household. When a spouse or dependents are offered coverage through the employee’s job, the calculation can consider the price to cover the household. An employer plan may therefore be affordable for the employee while the dependent tier is unaffordable. In that situation, family members may qualify for Marketplace savings even though the employee does not.
Do not calculate this informally from a brochure. Enter the employer offer, household size and expected income in the Marketplace application. The application is the practical way to determine who can receive savings and which coverage choices are available.

Individual coverage HRAs require a separate check
An ICHRA is an employer reimbursement arrangement rather than a conventional group policy. For 2026, HealthCare.gov describes the offer as affordable when the employee’s monthly cost for the lowest-cost Silver Marketplace plan, after the HRA reimbursement, is below 9.96% of one-twelfth of annual household income. An affordable ICHRA offer generally prevents Marketplace premium tax credits for the employee and household.
If the ICHRA offer is unaffordable, an employee may choose the ICHRA or an eligible Marketplace premium tax credit, but not both. Read the employer’s ICHRA notice and confirm the current calculation with the Marketplace before accepting or declining the arrangement.

The cheapest payroll deduction is not automatically the cheapest coverage. Build a plan-year comparison that includes predictable premiums, likely cost sharing and the amount you could owe in a bad year.
| Comparison axis | What to record for each option | Why it matters |
|---|---|---|
| Premiums | Employee-only and household amounts per pay period and for the full year; employer contributions; whether deductions are pre-tax | A low employee premium can hide a much higher dependent cost or a large deductible. |
| Tax-credit eligibility | Marketplace determination based on household, income, employer offer, affordability, minimum value and any HRA | An employer offer can make a person ineligible for premium tax credits even when the person declines enrollment. |
| Annual exposure | Deductible, copayments, coinsurance, out-of-pocket limit and costs excluded from that limit | This shows both routine spending and the maximum covered in-network exposure in a high-use year. |
| Care access | Doctors, hospitals, pharmacies, referrals, network tiers and out-of-network rules | A plan that excludes a needed hospital or specialist may be impractical regardless of price. |
| Medicines | Formulary tier, copay or coinsurance, prior authorization, quantity limits and pharmacy network | One drug’s tier or specialty-pharmacy rule can outweigh a small premium difference. |
| Typical care scenarios | The SBC’s standardized examples, such as routine care, childbirth or diabetes treatment | Examples make plans easier to compare, but they are illustrations rather than personal predictions. |
| Employer accounts | HSA, FSA or HRA deposits, eligibility, tax treatment, eligible expenses, deadlines and rollover rules | Employer money can materially reduce net cost, while an HRA can also affect Marketplace savings. |
| Timing and continuity | Enrollment deadline, effective date, current coverage end date and qualifying event | A mathematically attractive plan is not useful if it starts after a prescription refill or scheduled procedure. |
Calculate the premium floor
Multiply each per-pay-period deduction by the number of deductions in the plan year. Include any separate premium for dental or vision only if you are comparing the same benefits across options. Record employer HSA or HRA contributions separately; they are not the same as a premium reduction and may have spending restrictions.
Estimate a routine-use year
List the care your household can reasonably anticipate: primary-care visits, specialist visits, therapy, prescriptions, laboratory work and planned procedures. Apply each plan’s copay, deductible and coinsurance rules to those services. Use in-network prices and the formulary tier for the specific medicine, not a generic estimate.

Stress-test a high-use year
Compare the annual premium plus the in-network out-of-pocket limit, then subtract employer account contributions only to the extent you can actually use them for eligible expenses. Check which charges do not count toward the limit, such as excluded services, some out-of-network bills or balance bills. This is a risk comparison, not a promise that every expense will be covered.
Keep the SBC’s coverage examples in a separate column. They are standardized illustrations intended to show how cost sharing works; they do not include every condition, provider charge or medication a real household may have.
Verify doctors, hospitals and medicines at the plan level
Network names are not enough. Confirm the exact plan and network identifier for each option. Call the insurer and each provider’s billing office, and ask the provider to verify participation for the coming plan year. Direct confirmation is important because directories can lag behind contract changes.
Provider checklist
- Primary-care doctor and every specialist currently treating a household member.
- Preferred hospital and any facility needed for planned surgery, maternity care or emergency follow-up.
- Urgent-care locations and laboratory or imaging facilities you commonly use.
- Referral or prior-authorization requirements for specialist and behavioral-health care.
- Separate tiers that charge more for certain hospitals or physicians.
- Out-of-network benefits, balance-billing exposure and the plan’s rules for emergencies.
Medication checklist
- Exact drug name, dose and formulation, including an equivalent or biosimilar if the prescriber uses one.
- Formulary tier and whether the cost is a copay or coinsurance.
- Prior authorization, step therapy and quantity limits.
- Required mail-order or specialty pharmacy.
- Whether the deductible applies before the prescription benefit.
- What happens if the drug is removed from the formulary or your prescriber changes.
Ask the insurer to confirm coverage for the exact plan year and keep the reference number. A provider’s general statement that it “takes the insurer” does not establish participation in your particular network.
Understand HSA, FSA, HRA and ICHRA trade-offs
HSA
An HSA can let you pay certain qualified cost-sharing expenses with untaxed dollars. Eligibility depends on the type of health plan and other coverage, so verify that the plan is HSA-compatible. HSA funds generally cannot be used to pay health-insurance premiums. Compare the employer deposit, your contribution opportunity and the plan’s deductible and out-of-pocket exposure together.

FSA
An FSA is an employer-sponsored account for eligible expenses. Check the election deadline, annual contribution limit shown in your benefits materials, claim-submission process, carryover or grace-period rule and what happens when employment ends. Do not count an FSA election as cash you can use for premiums unless the plan documents expressly allow it.
Traditional HRA
An HRA is funded by the employer, and reimbursement rules are controlled by the specific arrangement. Determine whether it pays deductibles, copayments, coinsurance or premiums; whether unused amounts carry forward; and whether you lose access when you leave the job.
ICHRA
An ICHRA may reimburse individual-market premiums and eligible expenses under its written terms. It also has a direct effect on Marketplace tax-credit eligibility, so compare the reimbursement and the Marketplace alternative only after completing the affordability determination. Accepting an affordable ICHRA generally means giving up premium tax credits for the employee and household; an unaffordable offer requires choosing one path rather than combining both.
Use this step-by-step decision process
- Collect documents. Download the SBC, plan documents, premium schedule, provider directory, formulary and account notices for every employer option.
- Separate household members. List who needs coverage, whose employer offers coverage, expected income and any current doctors, hospitals or medicines that cannot easily change.
- Enter the employer offer in the Marketplace application. Use the lowest-cost self-only premium and the requested family premium information. Let the application determine tax-credit and program eligibility.
- Build a side-by-side cost sheet. Record annual premiums, deductible, copayments, coinsurance, out-of-pocket limit, exclusions and employer account contributions.
- Verify care access. Check each doctor, hospital, pharmacy and medicine against the exact network and formulary. Resolve discrepancies with the insurer and provider before enrolling.
- Model two use levels. Estimate a routine-use year and a high-use year. Include predictable prescriptions and planned care, then compare the financial risk if substantial treatment occurs.
- Review account restrictions. Confirm HSA eligibility, FSA deadlines, HRA reimbursement rules and any ICHRA effect on Marketplace savings.
- Confirm timing. Write down the enrollment deadline, effective date, current coverage termination date and any required proof of a qualifying event.
- Enroll and retain evidence. Save the confirmation number, effective-date notice, SBC, formulary result and provider participation confirmations. Do not cancel existing coverage until the replacement is active.
When splitting coverage between employer and Marketplace plans can make sense
Coverage does not have to be all-or-nothing for the household. A common pattern is an employee remaining in an affordable employer plan while dependents use Marketplace coverage because the dependent premium is unaffordable. Another is an employee using an ICHRA while a spouse evaluates a separate eligibility result. Each person’s eligibility and cost must be tested using the actual household application.

Splitting coverage creates administrative work. Doctors may be in-network under one plan but not the other, deductibles do not combine across insurers, and claims or prior authorizations must be handled separately. Compare the savings with that additional complexity before choosing separate plans.
Protect continuity and enrollment rights
Do not terminate first
Keep current coverage until the replacement plan’s enrollment is accepted and its effective date is confirmed in writing. Verify that the first premium has been paid when required and that prescriptions and scheduled care will be covered on the transition date.

Loss of job-based coverage
Involuntary loss of employer coverage can create a Special Enrollment Period. The exact dates and documentation requirements depend on the event and Marketplace. Start the application as soon as you know the coverage end date so you can coordinate the effective date.
Voluntarily dropping dependent coverage
Simply choosing to drop dependent coverage generally does not, by itself, create the cited Special Enrollment Period. Check the Marketplace rules for your specific event before declining or terminating coverage.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesCoordinate overlapping plans
If two plans overlap, ask both insurers how coordination of benefits works and which plan is primary. Do not assume that paying two premiums makes every bill payable twice or that deductibles will merge.
Decision rules for common situations
Keep the employer plan when
- The offer is affordable and meets minimum value, so Marketplace premium tax credits are unavailable to the employee.
- The employer contribution makes the total annual cost competitive.
- Your essential doctors, hospital and medicines are reliably in-network and covered.
- The employer account contribution offsets the plan’s deductible or other eligible cost sharing.
- You need predictable continuity and the Marketplace alternative would disrupt ongoing treatment.
Investigate Marketplace coverage when
- The employer premium is unaffordable under the applicable test or the plan does not meet minimum value.
- Dependents face an unaffordable family premium even though the employee’s self-only offer is affordable.
- The Marketplace application shows premium tax credits or other savings that materially change net cost.
- The employer plan excludes a necessary provider, hospital or medicine and a Marketplace plan covers it under acceptable terms.
- An ICHRA notice offers a reimbursement that may be less valuable than an eligible Marketplace tax credit.
Pause before switching when
- You cannot confirm the new plan’s effective date or provider participation.
- A planned procedure, pregnancy, specialty drug or ongoing treatment would cross the transition date.
- The comparison ignores out-of-network exposure, non-counting expenses or account restrictions.
- You are relying on a directory, premium estimate or tax-credit assumption that has not been confirmed for the exact plan year.
Three examples of how the framework works
Affordable employee offer, expensive family tier
An employee’s lowest-cost self-only option passes the applicable affordability test, but adding dependents costs substantially more. The employee may remain in the job-based plan while dependents apply through the Marketplace. The household should use the application’s determination, then compare separate deductibles, networks and drug formularies rather than looking only at the combined premiums.
Low premium, high medication exposure
A plan with the smallest payroll deduction places a long-term specialty medicine on coinsurance, requires prior authorization and uses a specialty pharmacy. A higher-premium plan with a predictable copay may produce a lower routine-use cost and less cash-flow risk. The formulary and benefit design, not the premium alone, decide the result.
ICHRA versus Marketplace tax credit
An employer offers an ICHRA that reimburses part of an individual Silver plan. The employee calculates the remaining monthly premium under the applicable affordability rule and completes the Marketplace application. If the ICHRA is affordable, premium tax credits generally are not available; if it is unaffordable, the employee chooses either the ICHRA or the eligible tax credit and follows the selected arrangement’s rules.
Questions to answer before you enroll
- What is the full-year premium for each person, not just the paycheck deduction?
- Which doctors, hospitals, pharmacies and medicines must remain available?
- What will the household likely spend in a routine year, and what is the high-use financial exposure?
- Which expenses do not count toward the out-of-pocket limit?
- What employer account money is available, when does it become usable and what can it pay?
- Does an employer offer or HRA change Marketplace savings eligibility?
- When does current coverage end, when does replacement coverage begin and what event supports enrollment?
- Where are the confirmation numbers and documents you will need if a claim or eligibility decision is disputed?
HealthCare.gov summarizes the practical approach in a short sentence: “There’s no risk in applying.” Applying to check eligibility does not automatically enroll you in a Marketplace plan. It gives you an official determination so you can compare an actual offer with an actual alternative before making a coverage change.
