How to Choose a Medigap Plan When Switching From Medicare Advantage

Choose Medigap only after you confirm why you are leaving Medicare Advantage, the date that coverage ends, and whether a federal guaranteed-issue or trial right applies. Those facts determine whether an insurer must sell you a policy without health questions, which plan letters you can buy, and when your application must be filed. If no special right applies, an insurer may use medical underwriting, charge more, or decline your application.

Do not cancel Medicare Advantage until the replacement coverage, effective dates, and any separate prescription-drug coverage are arranged. Medicare, your insurer, your State Insurance Department, and a State Health Insurance Assistance Program (SHIP) can help verify the rules for your situation.

Can I switch from Medicare Advantage to Medigap?

Yes. You can leave Medicare Advantage and return to Original Medicare, then apply for a Medicare Supplement (Medigap) policy. The important qualification is that leaving voluntarily does not automatically create a federal right to buy Medigap without underwriting.

Federal guaranteed-issue rights apply in specific circumstances. Medicare’s buying guide identifies examples such as your Medicare Advantage plan leaving Medicare, stopping care in your area, or your move outside the plan’s service area. The plan letters you may select and the length of your protection depend on the exact event. Your state may also provide protections beyond the federal rules.

Two trial-right situations can be especially important:

  • You joined Medicare Advantage for the first time at age 65, when first eligible for Medicare, and return to Original Medicare within the first year.
  • You dropped a Medigap policy to join Medicare Advantage for the first time and return to Original Medicare within 12 months.

The policy you can recover, or the alternatives available if it is no longer sold, depend on the facts and state law. Ask Medicare or your State Insurance Department to identify the right before you disenroll.

When there is no special federal right

If you are outside the federal Medigap Open Enrollment Period and do not have a guaranteed-issue or trial right, the insurer generally controls whether it will accept your application. It may ask health questions, apply medical underwriting, charge a higher premium, or refuse coverage. State law can change that result, so a denial or expensive quote is a reason to check state protections rather than assume every insurer has the same rule.

What is the deadline to apply after leaving Medicare Advantage?

For the qualifying switching situations described by Medicare, the application window can begin 60 days before Medicare Advantage coverage ends and run no more than 63 days after it ends. Medigap cannot begin before the Medicare Advantage policy ends. This is not a universal deadline for every voluntary disenrollment; first establish that your event qualifies.

Medicare generally says Medigap begins on the first day of the month after you apply, unless you request another effective date that the insurer and Medicare rules allow. The requested date must coordinate with the end of Medicare Advantage so you do not create a gap or pay for overlapping coverage you did not intend to keep.

Build a date file

  • Save the Medicare Advantage termination notice, plan letters, emails, and any document showing the last day of coverage.
  • Write down when you first learned that the plan was leaving the area, ending its contract, or otherwise triggering a possible right.
  • Ask the plan, Medicare, and the insurer you are considering to state the last day of the application window in writing.
  • Keep copies of every application, quote, call reference number, and confirmation of the requested effective date.

Medicare says these notices and records can help prove a guaranteed-issue right. If dates conflict, resolve the conflict before cancelling existing coverage.

Which switching situation describes you?

Situation What it may provide What to verify
Medicare Advantage plan leaves Medicare or stops serving your area A guaranteed-issue right may apply when you return to Original Medicare. Which plan letters are available, the final coverage date, and the required application window.
You move outside the plan’s service area Medicare lists this as a circumstance that can create guaranteed-issue protections. Whether the move qualifies under the plan and state rules and which documents prove the move.
You joined Medicare Advantage for the first time at 65 and leave within the first year A trial right may let you return to Original Medicare and obtain Medigap under the applicable rules. Your original Medicare eligibility date, enrollment date, and the policy or alternatives available in your state.
You dropped Medigap to join Medicare Advantage for the first time and leave within 12 months A separate trial right may apply. Whether your former policy is still available and what happens if it is not.
You simply prefer Original Medicare and leave Medicare Advantage without one of these circumstances No automatic federal guaranteed-issue right is established by the choice alone. Whether you are in Medigap Open Enrollment and whether state law offers additional access.

This table is a screening tool, not a legal determination. Tell Medicare and the insurer exactly what happened; small differences in timing or enrollment history can change the protection.

Medigap Open Enrollment is different from Medicare Open Enrollment

The federal Medigap Open Enrollment Period is a one-time six-month period that starts when you are at least 65 and enrolled in Part B. During it, federal law generally gives you broader access to Medigap without medical underwriting. It is not the annual Medicare Open Enrollment period used for changing Medicare health or drug coverage.

If your six-month Medigap period has ended, do not assume that an annual Medicare enrollment period restores guaranteed access to a supplement. Outside the six-month period, you need a qualifying guaranteed-issue or trial right, or a state rule that provides another path.

Check your status before requesting quotes

  1. Confirm the date you became both 65 or older and enrolled in Part B.
  2. Mark the six-month Medigap period that began at that point and determine whether it has already ended.
  3. Match your Medicare Advantage exit to a federal qualifying event or trial right.
  4. Ask your State Insurance Department whether your state grants additional switching, birthday, anniversary, or other protections. Do not rely on a rule from another state.

How to compare Medigap plan letters

There is no universally best plan letter. First identify the letters you are legally allowed to buy in your situation and that insurers actually sell in your state. Then compare the standardized benefits of those letters with the cost-sharing you expect and the premium you can sustain.

Use this decision sequence

  1. List eligible letters. Ask Medicare, the insurer, or your State Insurance Department which letters your guaranteed-issue or trial right permits. A right may not make every letter available.
  2. Match benefits to likely expenses. Review the official standardized benefit descriptions for each eligible letter. Consider how often you use medical care, whether predictable cost-sharing is important, and how much risk you can afford.
  3. Compare the same letter across companies. For a particular letter, standardized benefits are the same from one company to another, but premiums are not. Request official quotes rather than comparing a low quote for one letter with a higher quote for a different letter.
  4. Ask how the premium is priced. Have each insurer explain its pricing method and what can cause premiums to change. The amount you receive is a quote for your location and circumstances, not a national price.
  5. Confirm availability where you live. Not every letter is sold in every state, and not every company sells every letter. Use your actual state and county or service area when requesting quotes.
  6. Verify the effective date. Have the insurer confirm in writing when the policy starts and how that date relates to the last day of Medicare Advantage.

What standardized benefits do—and do not—standardize

Standardization makes a letter’s covered benefits comparable across insurers. It does not make premiums identical, guarantee that the letter is offered in your state, or override a limited guaranteed-issue right. A cheaper quote can reflect a different letter, a different pricing method, or different eligibility assumptions, so check the letter and effective date on every quote.

How to switch without creating a coverage gap

  1. Ask Medicare Advantage about disenrollment. Request the permitted disenrollment method and the exact end date. A plan representative can explain its process, but Medicare or your State Insurance Department should confirm your Medigap rights.
  2. Classify your right. Describe your reason for leaving, move date if relevant, first Medicare Advantage enrollment date, prior Medigap history, age, and Part B status. Ask which federal rule or state protection applies.
  3. Collect proof. Keep termination notices, relocation records, plan communications, and enrollment confirmations. Send copies only through secure channels and retain the originals.
  4. Get multiple official quotes. Compare the same eligible plan letter and ask each insurer to confirm underwriting requirements, premium pricing, availability, and the earliest lawful effective date.
  5. Apply within the correct window. Submit the application early enough for processing, while preserving evidence of the qualifying event. Ask for a receipt or application number.
  6. Arrange drug coverage separately. If your Medicare Advantage plan included prescription drugs, review a stand-alone Part D option and its enrollment and effective-date rules before the old drug coverage ends.
  7. Wait for confirmation. Do not cancel or allow Medicare Advantage to end based only on a quote. Confirm acceptance, the policy number, the start date, and drug coverage before treating the transition as complete.
  8. Check the first month. Compare the new Medigap effective date with your Medicare claims and Part D enrollment information. Report any mismatch promptly to the insurer and Medicare.

Will I need a separate Part D plan?

Usually, you should assume prescription coverage is a separate decision. Medigap policies sold after 2005 generally do not include prescription-drug coverage. If your Medicare Advantage plan included drugs, leaving it can remove that benefit even when your Medigap application is accepted.

Check whether you qualify to enroll in a stand-alone Part D plan, when its coverage can start, and how that date coordinates with the end of Medicare Advantage. Do not wait for a Medigap policy packet to answer the drug question; the supplement is not a substitute for Part D.

Medicare’s annual Open Enrollment information also treats health and drug coverage as separate choices. Ask Medicare or a SHIP to review your enrollment timing so you do not assume that a Medigap application automatically enrolls you in Part D.

State rules and free help can change the answer

Federal rules are only the starting point. Medicare directs people to their State Insurance Department for protections beyond federal guaranteed-issue rights. A state may provide additional opportunities to buy Medigap, regulate underwriting, or specify which letters are available in a switching situation. The rule in the state where you live controls; a friend’s experience elsewhere is not reliable evidence.

SHIPs provide free, local, unbiased counseling. A counselor can help you identify the applicable enrollment period, organize proof of a qualifying event, compare official quotes, and coordinate Part D. Use a SHIP when an insurer gives an answer that conflicts with Medicare, when you receive a denial, or when you cannot tell whether your situation is a trial right.

Common mistakes and how to fix them

Assuming any voluntary exit guarantees Medigap

Why it fails: Federal guaranteed issue is tied to specified circumstances, not simply a preference for Original Medicare.

Fix: Ask Medicare to classify the event and obtain the answer in writing before applying or disenrolling.

Confusing the two open-enrollment periods

Why it fails: The one-time six-month Medigap period is separate from the annual Medicare period for health and drug plan changes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Fix: Locate your Part B and age-based Medigap start date and check whether a separate guaranteed-issue or state right applies.

Waiting until after coverage ends to start

Why it fails: A qualifying window can begin before Medicare Advantage ends, and processing takes time.

Fix: Start gathering documents and requesting quotes as soon as you receive an ending notice. Use the confirmed last day of coverage to calculate the applicable window.

Choosing by letter name alone

Why it fails: The same letter has standardized benefits, but premiums, availability, and eligibility differ by insurer and state.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Fix: Compare the same eligible letter across several insurers and ask each one to state the premium and effective date in the quote.

Letting a low quote replace written confirmation

Why it fails: A quote is not proof that the application will be accepted or that the requested date is permitted.

Fix: Obtain an application receipt, underwriting or guaranteed-issue determination, policy number, and start date before ending existing coverage.

Assuming Medigap includes prescriptions

Why it fails: Post-2005 Medigap policies generally exclude drug coverage.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Fix: Evaluate a separate Part D plan and verify its enrollment and effective dates.

Practical examples

Your plan stops serving your county

Keep the plan’s notice showing the final coverage date. Ask Medicare which guaranteed-issue letters are available, apply during the qualifying window, and set the Medigap start date no earlier than the Medicare Advantage end date. Obtain a separate Part D decision if the plan supplied drugs.

You joined Medicare Advantage at 65 and changed your mind within the first year

Tell Medicare that you are using the first-time enrollment trial right and provide your enrollment dates. Ask whether your circumstances let you buy the former Medigap policy or a permitted alternative. The answer can depend on whether the former policy is still sold and on state rules.

You have been in Medicare Advantage for years and simply prefer Original Medicare

Your preference alone may not create federal guaranteed issue. Check whether you are still in the six-month Medigap Open Enrollment Period, then ask the State Insurance Department about state protections. If neither applies, be prepared for underwriting, a higher premium, or a denial and compare the consequences before disenrolling.

A final pre-dis enrollment checklist

  • Reason for leaving and exact Medicare Advantage end date documented.
  • Federal guaranteed-issue or trial right identified, or state protection checked.
  • Eligible Medigap letters confirmed for your state.
  • Same-letter quotes compared from more than one insurer.
  • Premium pricing method and future-change explanation requested.
  • Medigap application submitted within the applicable window.
  • Acceptance, policy number, and effective date received.
  • Separate Part D eligibility, plan, and start date confirmed if needed.
  • Notices, applications, and call records saved.
  • Medicare Advantage not cancelled until the replacement transition is confirmed.

Medicare’s guide Choosing a Medigap Policy (2025) is a useful official reference; because editions can change, check Medicare publications for a newer version. For personalized help, contact Medicare, your State Insurance Department, or a SHIP rather than relying on a generic plan chart.
