How to Choose a Long-Term Care Insurance Policy and Avoid Common Mistakes

By TheFinanceBase Team 11 min read

The right long-term care (LTC) policy starts with a care-and-finance plan, not a quote. Decide whether you would want care at home, in assisted living or in a nursing facility; estimate costs where you expect to live; identify the assets you want to protect; and set a premium that could remain affordable through retirement. Then obtain proposals from several licensed insurers or agents and compare the contract—not just the initial price.

Focus on the policy’s covered settings, benefit triggers, daily or monthly benefit, total benefit pool, benefit period, elimination period, inflation protection, exclusions, nonforfeiture provisions, underwriting and the insurer’s history of rate increases. Read the complete policy before signing. No single design is best for every age, health profile, state or financial plan.

Why long-term care insurance matters

Long-term care is assistance with everyday activities or supervision for people who cannot live safely without help. It can be delivered in a private home, an adult day-care setting, an assisted-living residence, a nursing facility or another qualifying setting. The need may follow frailty, a chronic illness, an accident or cognitive impairment.

Seventy percent of adults who survive to age 65 develop severe long-term-services-and-supports needs before they die, and 48% receive some paid care over their lifetime, according to the U.S. Department of Health and Human Services Assistant Secretary for Planning and Evaluation, as cited by the National Association of Insurance Commissioners (NAIC) in 2026. Those figures describe risk, not a prediction for any one person, but they explain why a retirement plan should address care costs explicitly.

What ordinary health coverage usually does not pay

Traditional health insurance generally pays for medical treatment, not prolonged custodial help with bathing, dressing, eating, toileting, transferring or supervision. Medicare may cover a limited nursing-home stay only after specific qualifying circumstances and for a limited duration; it is not a general source of long-term custodial-care funding. Medicaid (called Medi-Cal in California) is means-tested, administered under state rules and generally requires financial eligibility.

Medicare does not endorse or sell LTC policies. NAIC warns that official-looking mailers and celebrity advertising can mislead consumers. Verify the sender and the seller before sharing personal or financial information.

Start with a care-and-finance plan

1. Choose the care settings you would actually use

Write down your preferred order of care. You might want to remain at home as long as practical, move to assisted living if household tasks become unsafe, or use a nursing facility for intensive care. A policy that pays generously in a nursing facility but has a small home-care limit may not match your plan.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Home care: Check whether the policy covers licensed home-health agencies, independent caregivers, homemaker services, adult day care, care coordination and respite. Confirm visit, hour, daily or monthly limits.
  • Assisted living: Verify that the residence meets the contract’s licensing definition and that room, board and services are treated as eligible expenses.
  • Nursing facility: Confirm the facility qualifications, skilled versus custodial coverage and any per-day or per-month cap.
  • Hospice and respite: Look for separate provisions, waiting periods and dollar limits rather than assuming they are included.

2. Estimate local costs and your protection target

Use current prices in the state and metropolitan area where care is most likely. Separate the amount insurance would need to cover from the amount you could pay from income, savings or a planned home sale. Your target may be to protect a spouse’s income, preserve an inheritance, avoid selling a business under pressure or retain a reserve for medical and housing costs.

The official sources reviewed for this guide do not establish one nationwide premium that applies across ages, states, health classes and benefit designs. Premiums must therefore be obtained through state-licensed channels and compared on identical assumptions.

3. Set a sustainable premium ceiling

Test the premium against retirement income, inflation, taxes, a market downturn and a possible insurer rate increase. A policy that is affordable at application but unaffordable at 75 can fail at the point it is needed. Include premiums for other insurance, property taxes, health coverage, prescriptions and an emergency reserve in the same cash-flow test.

Understand the policy mechanics before comparing prices

Benefit triggers

Most policies begin paying when the insured meets the contract’s benefit trigger. Read the exact definitions of activities of daily living (ADLs) and cognitive impairment. Contracts commonly use inability to perform a specified number of ADLs or a qualifying cognitive impairment, but the number, wording, certification process and supervision requirement are policy terms—not assumptions to make from a sales illustration.

Ask who must certify the impairment, what records are required, whether a plan of care is needed and when benefits can begin. Confirm whether the trigger is the same for home care, assisted living and nursing-facility care.

Daily or monthly benefit

The daily or monthly benefit is the maximum the policy will pay for eligible care during a covered period. A monthly benefit can be easier to use when home-care bills vary by week; a daily benefit may align with facility billing. Ask whether unused daily amounts can be pooled within a month and whether home-care benefits are a percentage of the facility benefit.

Total benefit pool and benefit period

The total pool is the maximum amount payable over the life of the policy. It can be expressed as a dollar pool, a daily benefit multiplied by a stated number of years, or a lifetime benefit. For example, a $200 daily benefit with a five-year period produces a nominal $365,000 pool before any inflation increases or reimbursement rules. The contract controls how the pool is reduced and whether different care settings draw from the same pool.

Do not choose a benefit period solely because it is a familiar number. Compare your assets, family support, health history and willingness to self-fund a longer claim. A shorter period with a larger daily amount can be preferable to a longer period that pays too little for the care you would use.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Elimination period

The elimination period is the time you pay eligible expenses before the insurer starts reimbursement. Confirm all of the following in writing:

  • Whether the period is measured in service days (days on which you receive care) or calendar days.
  • Whether it applies once in a lifetime, once per claim or again after a break in care.
  • Whether days in different care settings count together.
  • Whether a physician’s certification or an approved plan of care is required before the count begins.
  • Which expenses you must pay during the waiting period.

California’s Department of Health Care Services states that the policyholder is personally responsible for long-term-care expenses during the elimination period. That is a practical cash-reserve requirement, not merely a contract detail.

Price future care with inflation protection

Inflation protection raises the premium but preserves the purchasing power of a benefit that may not be used for decades. NAIC’s shopper guide calls it one of the most important features you can add to an LTC policy. Compare the increase method, not just the stated percentage:

  • Simple increases add the same original-dollar amount each year.
  • Compound increases apply the percentage to the growing benefit, producing much more protection over long periods.
  • Future-purchase options let you buy additional coverage later, but the future price and underwriting rules may differ.

NAIC’s 2021 guide illustrates the effect: a $150-per-day nursing-home cost in 2018 would reach $398 per day after 20 years at 5% inflation. The same guide shows a $200 daily benefit growing to $531 by 2050 with 3% compound increases. These are illustrations, not forecasts of your local cost or a promise that a policy will use either assumption.

Ask whether inflation increases continue after a claim starts, whether they apply to the daily amount and the remaining pool, and whether you can decline an increase. Compare the premium with and without protection and document the resulting benefit at ages 75, 80 and 85.

Compare policy types on the same decision axes

Traditional stand-alone LTC insurance is only one form of coverage. Employer or group coverage and life-insurance or annuity policies that include or add an LTC benefit may also be available. Their suitability depends on your finances, health, state and goals.

Decision axis Stand-alone LTC policy Employer or group coverage Life-insurance or annuity with LTC benefit
Underwriting Usually health underwriting; contract terms vary May offer simplified or guaranteed-issue enrollment during specified windows; verify eligibility Life or annuity underwriting and contract requirements apply
Premium pattern and rate risk Premiums can be subject to approved rate increases; review history Group rates and continuation rules depend on the employer or association Costs and guarantees depend on the life or annuity contract; ask how premiums affect cash value
Benefit pool and duration Specified daily/monthly amount and lifetime pool or period Specified by the group certificate; portability may be limited LTC benefit may accelerate a death benefit or use an annuity value; read the calculation
Inflation protection Available options and increase method vary Group options vary May be included, optional or absent; compare the resulting future benefit
Other value Primarily LTC coverage May be convenient while employed Can provide cash value, an income stream or a death benefit if LTC is not used
Lapse consequences Coverage and any nonforfeiture benefit depend on the contract Leaving employment can change cost or portability Lapse can affect cash value, death benefit and LTC access
Medicaid-partnership eligibility May qualify if the policy meets state requirements Depends on the certificate and state approval Depends on the specific product and state rules

Do not infer that a group policy is automatically cheaper for life, that a hybrid policy is automatically safer, or that a stand-alone policy is automatically more comprehensive. Obtain the certificate or full contract and compare benefits dollar for dollar.

Investigate exclusions, limitations and nonforfeiture options

Read the definitions and exclusions section before relying on an illustration. Pay particular attention to:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Pre-existing-condition look-back periods and any post-issue limitations.
  • Exclusions for particular diagnoses, substance use or self-inflicted injuries, if present in the contract.
  • Facility licensing and certification requirements.
  • Home-care reimbursement limits, caregiver eligibility and informal-care exclusions.
  • Requirements for a written plan of care, physician certification or prior approval.
  • Respite, hospice, adult day-care and care-management provisions.
  • Nonforfeiture benefits, such as reduced paid-up coverage or a shortened benefit period if you stop paying.

Ask an agent to show the exact page for every answer. If an explanation differs from the contract, rely on the contract and request clarification before applying.

Stress-test premiums and future rate increases

Premiums are not necessarily fixed. NAIC identifies an insurer’s historical rate increases as a comparison item. Request the company’s increase history for the policy series and ask how a future increase would affect your premium at ages 70, 80 and 90.

Also ask what reduced-benefit choices are available if a premium becomes unaffordable. Options might include reducing the daily benefit, shortening the benefit period, dropping an inflation rider or taking a nonforfeiture benefit; the available choices and deadlines are contract-specific. Determine whether a change preserves an existing pool, requires new underwriting or affects partnership status.

Check licensing, financial strength and tax treatment

Verify the seller and insurer

  1. Confirm that the agent and insurer are licensed in your state through the state insurance department.
  2. Check the insurer’s financial-strength ratings from established rating services and understand that ratings are opinions, not guarantees.
  3. Request the policy form, outline of coverage, rate-history information and sample benefit calculations.
  4. Contact the state insurance department or a senior insurance counseling program when an answer remains unclear.

Separate tax claims from sales claims

Benefits from a tax-qualified policy are generally not taxable, while payments from a non-qualified policy may be taxable. The outcome depends on the policy and the payment circumstances, so consult a tax professional before relying on a tax statement. Keep the tax qualification language with your records.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Understand Medicaid-partnership rules

A qualifying partnership policy may allow an asset disregard when you later apply for Medicaid, but rules differ by state. California describes dollar-for-dollar asset disregard for qualifying Medi-Cal partnership benefits. Confirm whether the policy is partnership-qualified in your state, whether a move changes treatment and how benefits paid are documented.

Common mistakes and how to prevent them

Buying one quote

Contact several companies and agents. Policies with similar headline benefits can have different premiums, exclusions, inflation provisions and rate histories. Compare proposals using the same benefit amount, period, elimination period and inflation assumption.

Choosing a benefit that ignores inflation

Place the projected benefit beside the projected local care cost at the ages when a claim is most plausible. If the gap is large, adjust the inflation option, daily amount or self-funding reserve rather than assuming today’s benefit will be adequate.

Misunderstanding the waiting period

Ask whether the count uses service or calendar days, whether it resets and who pays. Hold enough liquid money to cover the stated period and any expenses the policy excludes.

Overlooking triggers and exclusions

Read ADL and cognitive definitions, look-back language, facility rules and home-care limits. Do not rely on a brochure’s phrase such as “comprehensive coverage” without the contract definitions.

Assuming premiums cannot rise

Obtain rate history and identify your fallback option before buying. A premium plan without a realistic fallback can force a lapse after years of payments.

Buying from an unverified seller

Verify licensing and financial strength independently. Be especially cautious with mailers that appear to be government notices; NAIC advises checking with the named government agency before trusting them.

Confusing tax or Medicaid promises

Ask for the statutory or contract basis of any tax or partnership statement and obtain state-specific professional advice.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Buying coverage you cannot sustain

Run a retirement cash-flow test that includes inflation, a rate increase, investment losses and other insurance premiums. If the policy fails that test, reduce the design or reconsider the amount of risk you will self-fund.

A practical purchase process

  1. Write your care plan. Rank home care, assisted living and nursing-facility care; note who could provide unpaid help and where you expect to live.
  2. Set the protection target. Estimate local costs, identify assets to protect and choose the reserve you would still retain after paying premiums.
  3. Request comparable proposals. Use several licensed insurers or agents and specify the same daily or monthly benefit, pool, benefit period, elimination period and inflation option.
  4. Build a comparison sheet. Record triggers, covered settings, exclusions, home-care limits, rate history, nonforfeiture choices, tax qualification and partnership status.
  5. Model future years. Show benefits and premiums at multiple ages under the actual inflation method and at least one plausible rate increase.
  6. Review underwriting. Answer health questions completely. Ask how medical records, prescriptions and recent treatment affect approval or exclusions.
  7. Verify the contract. Read the outline of coverage and full policy, not just an illustration. Resolve every discrepancy in writing.
  8. Use the review period. Keep the policy, applications and correspondence together. During the state-required free-look period, reread the contract and cancel if it does not match the agreed design.
  9. Revisit the plan. After retirement, a move, marriage, divorce, inheritance or major health change, review whether premiums, assets and care preferences still align. Do not cancel before understanding reinstatement, nonforfeiture and Medicaid consequences.

How to make the final decision

Choose the policy that fits your intended care settings, pays a meaningful share of projected costs, includes inflation protection you can afford, has an elimination period you can fund and remains sustainable under a rate-increase scenario. A lower premium is not a bargain if the policy excludes the care you would use or exhausts before a long claim ends.

Document why you selected the benefit amount, pool, inflation option and waiting period. Keep the policy and beneficiary, premium and claims contacts where a family member or trusted adviser can find them. The decision is ultimately a trade-off between transferring more care-cost risk and preserving flexibility in your retirement budget; the contract and your state rules determine the result.
