How to Check Whether Your Medicare Advantage Plan Covers Your Doctors and Prescriptions

To verify coverage, use the exact Medicare Advantage plan name and plan year—not just the insurer’s name—then check every doctor, facility, prescription, and pharmacy. Start with Medicare’s plan-comparison tool, review the plan’s provider directory and formulary, and confirm uncertain entries with both the plan and the provider’s office. Finally, compare the total medical and drug costs, network rules, referrals, and prior-authorization requirements.

A provider directory or online search is a useful starting point, but it is not a guarantee that a doctor will remain in network or that a prescription will stay covered on identical terms for the entire year. Networks, formularies, prices, and benefits can change by plan, service area, and coverage year.

What “covered” means in a Medicare Advantage plan

Medicare Advantage plans are offered by private companies and combine Medicare-covered medical services under plan-specific rules. “Covered” has two separate questions:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Provider access: Is the doctor, specialist, hospital, clinic, laboratory, or other facility participating in this exact plan, at the location where you will receive care?
  • Prescription access: Is each medication on this plan’s current Part D formulary, and what tier, restrictions, and pharmacy cost apply?

A doctor who accepts Original Medicare is not automatically part of a Medicare Advantage network. Medicare’s comparison guidance explains that people in Medicare Advantage may need network providers for non-emergency care, while some plans permit out-of-network care at a higher cost. The exact plan’s Evidence of Coverage controls the details.

Coverage also depends on the plan type. An HMO generally requires network care except for specified situations; a PPO generally allows Medicare-covered care outside the network at a higher cost; and a private fee-for-service (PFFS) plan depends on whether a provider accepts the plan’s payment terms for that visit. These distinctions make checking the exact plan and location essential.

Gather the details before you search

Having complete information prevents a result for the wrong plan, year, drug, or practice location. Assemble:

  • The plan’s full name, plan identification number if shown on your card or plan materials, and coverage year.
  • Your ZIP code and county or service area.
  • Every primary-care doctor, specialist, hospital, outpatient facility, laboratory, and other provider you expect to use.
  • The street address of each practice location where you receive care. A physician can be in network at one location and not another.
  • Each prescription’s exact name, strength, dosage form, and frequency as printed on the label.
  • The pharmacies you are willing to use, including the specific location where you normally fill prescriptions.
  • Your expected travel or split-state living pattern, if you spend part of the year outside your home area.

This is an insurance-coverage check, not a reason to change a medication. If a formulary search suggests an alternative drug, discuss any change with the prescribing clinician.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 1: Identify the exact plan and service area

Go to Medicare.gov Plan Compare and enter your ZIP code. The tool displays plans available in that area and lets you enter prescriptions to estimate monthly and yearly drug costs. Select the exact plan name and coverage year before relying on any result.

Do not stop at a company name. The same company can offer multiple HMOs, PPOs, SNPs, or other plans with different networks, formularies, premiums, and cost sharing. If you are reviewing an existing plan, use the plan information on your member ID card and the current annual plan materials rather than an older brochure.

Request or download the current versions of these documents from the plan:

  • Summary of Benefits: a concise overview of premiums, deductibles, copayments, coinsurance, and major rules.
  • Evidence of Coverage: the binding explanation of covered services, referrals, authorization, network, and appeal rules.

Check that every document applies to the same plan and effective year. A general Medicare provider listing cannot settle whether a provider participates in your Medicare Advantage network.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 2: Verify every doctor and facility

Search the plan directory

Use the plan’s provider search or Medicare’s provider tools to look up each doctor, specialist, hospital, and facility. Search by the provider’s full name and confirm the practice address, specialty, and location. Include the places where care actually occurs: a specialist’s office, hospital, outpatient surgery center, imaging facility, and laboratory may all be separate network entries.

Ask the provider’s office a plan-specific question

Call the billing or insurance office and ask whether it participates in the exact plan name and plan year at the specific address you intend to use. The question “Do you accept Medicare?” is not enough. Ask whether the office is in network for your plan, whether the doctor is accepting new patients under that plan, and whether the facility used for procedures or testing is also in network.

Have the insurer confirm the entry

Call the member-services number on the plan’s official materials and give the representative the provider’s name, address, specialty, and your plan identification. Ask the representative to verify network status for the coming coverage year and to explain any referral or prior-authorization requirement that applies to the service. If the directory and the provider’s office disagree, ask the plan to research the discrepancy before you enroll or schedule non-emergency care.

Medicare’s joining guidance summarizes the practical check this way: “Ask your doctors and pharmacies if they’re in the plan’s network.” Treat a directory result as a lead to confirm, not as a promise that the provider will remain available throughout the year.

Step 3: Check each prescription in the formulary

Search the exact drug entry

Every Part D plan has its own formulary. Open the selected plan’s current formulary and search each prescription separately. Match the exact strength and dosage form; a tablet, capsule, liquid, extended-release product, or different strength may appear as a separate entry.

For each drug, record:

  • Whether the medication appears on the formulary.
  • The listed tier or cost category.
  • Any coverage rule shown, such as prior authorization or step therapy.
  • The estimated cost at the pharmacy you expect to use.

Use Medicare Plan Compare to enter the complete medication list and estimate monthly and yearly drug costs. Compare the result with the plan’s formulary and cost-sharing documents; an estimate is not a guarantee of the final claim amount.

Check pharmacy participation and preference

Ask whether your chosen pharmacy is in the plan’s network and whether it is a preferred pharmacy for the plan. Drug costs can vary by plan and pharmacy, so run the estimate for realistic pharmacy options rather than assuming every in-network location has the same price. Confirm the pharmacy’s participation for the same coverage year as the formulary.

Discuss missing or restricted drugs with the prescriber

If a medication is absent or subject to a restriction, do not substitute another drug on your own. Ask the prescriber whether an appropriate covered option exists and whether the plan’s process allows an exception or authorization. The plan can explain its requirements; the prescriber must decide whether an alternative is medically suitable.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 4: Compare the complete cost and access picture

A low monthly premium does not necessarily produce the lowest annual cost. Compare the following for every plan you are considering:

Item What to check Why it matters
Monthly premium The plan premium, including whether a separate Part B premium still applies Shows recurring spending but not your likely total cost
Medical deductible Whether you pay a deductible before certain services Changes early-year spending
Medical copayments and coinsurance Your share for primary care, specialists, hospital care, tests, and other services Costs depend on the services you actually use
Prescription deductible and cost sharing Drug deductible, tier copayments, or coinsurance for each prescription Formulary placement can materially change drug spending
Annual maximum out-of-pocket limit The plan’s medical annual limit and which expenses count toward it Provides a ceiling for covered in-network medical services under the plan’s rules
Pharmacy access In-network and preferred-pharmacy availability where you live A pharmacy that is convenient but outside the preferred network may cost more
Referrals and prior authorization Whether specialist referrals or approval before a service or supply are required Missing a required step can delay care or affect payment
Out-of-network rules Whether non-emergency Medicare-covered care can be obtained outside the network and at what cost Important for travel, split-state living, and limited local networks

Medicare states that costs and out-of-pocket limits vary by plan, and drug costs can vary by both plan and pharmacy. Build an annual estimate using your actual doctors, prescriptions, expected services, and likely pharmacy—not a generic premium comparison.

How network rules differ by Medicare Advantage plan type

Plan type Typical network and referral rule Prescription coverage note
HMO Generally uses network doctors and facilities for routine care. Emergency care, urgent care, and out-of-area dialysis are exceptions. Some HMO Point-of-Service plans permit selected out-of-network services at a higher cost. Referrals to specialists are generally required. Check the exact plan’s Part D formulary and pharmacy network.
PPO Has a network, but Medicare-covered services may generally be received out of network at a higher cost. Referrals are generally not required. Part D coverage generally requires choosing a PPO that offers it.
PFFS A provider must accept the plan’s payment terms and agree to treat you for the service. Participation is not the same as simply accepting Medicare. Drug coverage may or may not be included; verify the plan’s formulary.
Special Needs Plan (SNP) Network and referral rules vary according to whether the SNP is structured as an HMO or PPO. SNPs must offer Part D, but the formulary and restrictions remain plan-specific.
Medical Savings Account (MSA) Generally has no network. You can use Medicare-approved providers who agree to treat you and have not opted out, subject to the plan’s rules. MSA plans do not provide Part D, so separate drug coverage is needed if you want prescription insurance.

These are general characteristics, not a substitute for the plan’s Evidence of Coverage. A plan can require prior authorization before covering certain services or supplies, and its directory and cost rules govern your particular enrollment.

Step 5: Confirm discrepancies before enrolling or switching

Do a final two-source confirmation for every item that matters to your care:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Call the provider’s office and ask for network participation under the exact plan name, plan year, and location.
  2. Call the plan using the number in its official materials. Give the representative the provider’s full details, each drug’s strength and form, the pharmacy location, and the coverage year.
  3. Ask whether referrals, prior authorization, step therapy, or other listed restrictions apply.
  4. Ask how the answer changes for emergency care, urgent care, planned care outside the service area, or care while you live in another state for part of the year.
  5. Write down the date, representative’s name or identification, confirmation details, and any reference number. Keep the notes with your plan documents.

If a provider says it is in network but the plan says it is not, do not assume one answer is correct. Request clarification from the plan and ask the provider’s billing office to verify its contract for your specific product. Resolve the discrepancy before arranging non-emergency treatment whenever possible.

What to do when a doctor or prescription is not covered

If your doctor is out of network

Ask whether the plan has an in-network clinician with the same specialty and whether your records can be transferred. If continuity with the current doctor is important, compare a plan that includes that provider or a plan type with usable out-of-network benefits. A PPO may allow Medicare-covered out-of-network care at a higher cost, but the specific deductible, coinsurance, and provider billing rules still apply. An HMO generally will not cover routine out-of-network care except under its stated exceptions.

If a hospital or facility is missing

Verify the complete care team, not just the admitting physician. A hospital, surgeon, anesthesiologist, imaging center, or laboratory can have separate participation status. Ask the plan which facilities are available for the service you need and whether prior authorization is required.

If a prescription is absent or restricted

Ask the prescriber about a clinically appropriate formulary alternative and ask the plan how its authorization or exception process works. Check the exact drug, strength, form, tier, restriction, and pharmacy again after any change. Never stop or switch a prescription solely because an online formulary result is inconvenient.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If the plan’s cost is higher than expected

Recheck the pharmacy selection and formulary tier, then compare the annual total rather than only the premium. Include deductibles, copayments or coinsurance, medical spending, and the annual medical out-of-pocket limit. A plan with a higher premium can still be less expensive for someone who uses particular specialists or several high-cost prescriptions.

Official tools and free help

Medicare Plan Compare

Medicare Plan Compare uses your ZIP code to show available plans and lets you enter prescriptions to estimate monthly and yearly drug costs. Use it to narrow choices, then verify providers, pharmacies, and formulary rules with the plan and the relevant offices.

Medicare provider tools

Medicare’s Helpful Tools area links to provider searches. These searches can help identify doctors and facilities, but a general listing does not establish participation in your particular Medicare Advantage plan. The insurer and provider office must confirm plan-specific network status.

State Health Insurance Assistance Program (SHIP)

Medicare says your local State Health Insurance Assistance Program provides free, personalized Medicare counseling and is not connected to an insurance company or health plan. SHIP counselors can help you compare plan documents and prepare questions without representing a particular insurer.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Medicare telephone help

Medicare’s joining guidance lists 1-800-MEDICARE (1-800-633-4227); TTY users can call 1-877-486-2048. Use these numbers for Medicare assistance and enrollment questions. For a plan-specific network or formulary answer, the plan’s own member-services contact remains necessary.

2026 Medicare handbook

The official 2026 Medicare & You handbook directs readers to Medicare Plan Compare to check drugs and advises contacting the plan for its current formulary. Always use the version for the coverage year you are evaluating.

When to repeat the check

Repeat this process whenever you consider enrolling in a new plan, switching plans, moving, adding or changing a prescription, or changing a regular doctor or pharmacy. At minimum, recheck for the next coverage year using the new provider directory, formulary, Summary of Benefits, and Evidence of Coverage. Medicare Advantage availability, networks, formularies, prices, and benefit rules can change by year and service area.

For people who live in more than one state, ask specifically whether the plan supports routine care in both locations. Emergency and urgent-care rules are not the same as regular planned care, and a plan that works well in one service area may not provide convenient routine access elsewhere.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A one-page verification checklist

  • Exact plan name, plan identification, coverage year, ZIP code, and service area recorded.
  • Primary-care doctor, every specialist, hospital, facility, laboratory, and practice location checked.
  • Each provider’s network status confirmed by the provider office and the plan.
  • Every prescription searched using the exact name, strength, and dosage form.
  • Drug tier, estimated cost, prior authorization, step therapy, and other listed restrictions recorded.
  • Preferred and in-network pharmacy status confirmed for the pharmacy you expect to use.
  • Monthly premium, medical and prescription deductibles, copayments, coinsurance, and annual medical out-of-pocket limit compared.
  • Referral, authorization, out-of-network, travel, and split-state rules reviewed.
  • Current provider directory, formulary, Summary of Benefits, and Evidence of Coverage saved.
  • Plan and provider confirmation dates, representative names, and reference details documented.

No single Medicare Advantage plan is best for everyone. The defensible choice is the plan that covers your actual providers and prescriptions under rules and total costs that fit your care needs, location, and budget.
