A low payroll deduction or a “high-deductible” label cannot answer either question by itself.
Use the current Summary of Benefits and Coverage (SBC), full plan rules, provider directory, formulary and your family’s expected care to compare plans. Calculate an expected-use year and a high-cost year, then check whether Marketplace coverage could be cheaper for some relatives and whether an HSA changes the after-tax trade-off.

Start with the two tests every family plan must pass
Judge a plan on two separate dimensions. Combining them prevents a common mistake: choosing a plan that is inexpensive to keep but unusable for a child’s specialist, or choosing broad access while exposing the household to a bill it cannot absorb.
| Test | Question to answer | Evidence to use |
|---|---|---|
| Access and coverage | Can every covered person use the doctors, hospitals, pharmacies, medicines and services they are likely to need? | Current provider directory, hospital listings, prescription formulary, SBC and complete plan document |
| Financial protection | After premiums and cost sharing, can the household fund both ordinary care and a severe medical year? | Payroll premium rates, deductible rules, copayments, coinsurance, in-network out-of-pocket maximum and out-of-network terms |
A plan fails if either answer is no. “Enough” is therefore a household judgment based on actual plan terms and your family’s finances, not a universal rating for a plan design.

Collect the documents before comparing plans
Ask your benefits office or insurer for the same set of documents for every option available to you. Job-based plans must provide a plain-language SBC. HealthCare.gov describes the SBC as information that helps people make “apples-to-apples” comparisons when looking at plans.
- Summary of Benefits and Coverage: record the deductible, copayments, coinsurance, out-of-pocket limit, exclusions and example service costs.
- Payroll premium schedule: capture the employee-only and family-tier amounts for each pay period, and how many pay periods occur each year.
- Full plan document: use it for definitions, deductible sequencing, exclusions, authorization rules, claim procedures and out-of-network treatment that the SBC summarizes only briefly.
- Current provider directory: check each family member’s doctors, specialists, hospitals, urgent-care locations and behavioral-health providers.
- Prescription formulary: verify every recurring medication, its tier, quantity limits, prior authorization and specialty-pharmacy requirements.
- HSA information: obtain the employer contribution schedule, eligibility conditions and the account’s contribution and investment rules if the plan is HSA-qualified.
Save the plan year and the date you checked each directory. Networks and formularies can change, so a result from an old benefits booklet is not confirmation for the current enrollment year.
Build one comparison sheet for every plan
Use identical family assumptions for each option. Separating fixed premiums, expected use and a severe year makes trade-offs visible.
| Item | Plan A | Plan B | What to record |
|---|---|---|---|
| Annual employee premium | Enter current payroll rate × pay periods | Enter current payroll rate × pay periods | Employee-only tier, if relevant to an alternative calculation |
| Annual family premium | Enter current payroll rate × pay periods | Enter current payroll rate × pay periods | Employee-plus-children, employee-plus-spouse or family tier as offered |
| Deductible design | Record individual and family amounts | Record individual and family amounts | State whether the family deductible is embedded or aggregate |
| Routine-use cost | Estimate cost sharing for expected services | Estimate cost sharing for expected services | Visits, prescriptions, therapy, tests and planned care |
| In-network out-of-pocket maximum | Record individual and family limits | Record individual and family limits | Confirm what counts and what is excluded |
| High-cost-year total | Premiums plus applicable in-network maximum | Premiums plus applicable in-network maximum | Model family exposure, then examine out-of-network exposure separately |
| Employer HSA contribution | Record amount and deposit timing | Record amount and deposit timing | Include only if the plan and family are HSA-eligible |
| Access and drug fit | Pass/fail for each person | Pass/fail for each person | Doctors, hospitals, pharmacies, medications and authorizations |
For a simple annual premium calculation, multiply the per-paycheck deduction by the number of paychecks. Do not subtract premiums from the out-of-pocket maximum unless the plan document explicitly says they count; premiums generally remain an additional cost.
Calculate what an ordinary year could cost
List care that is reasonably foreseeable for the next plan year rather than relying on a generic “healthy family” assumption. Include each person separately:
- Primary-care, pediatric and specialist visits.
- Recurring laboratory work, imaging, monitoring or durable medical equipment.
- Behavioral-health visits, physical therapy, occupational therapy or speech therapy.
- Recurring generic, brand or specialty prescriptions and any expected dosage changes.
- Planned surgery, delivery, fertility treatment or another known episode of care, if applicable.
- Emergency or urgent-care use that your family has experienced repeatedly.
For each service, identify whether the plan uses a copayment, deductible-first pricing or coinsurance. Apply the plan’s network rules and drug tier, then add the annual premium. If the SBC gives a cost-sharing example, use it as a standardized input but do not treat it as a promise of your claim result; the actual bill depends on the provider, coding, negotiated rate and services delivered.
Keep a separate cash-flow column. A plan can have a reasonable annual total while requiring a large payment early in the year before the deductible is met. Ask whether your household can pay that amount without borrowing or delaying care.

Model a high-cost year separately
A catastrophic-year calculation answers a different question: what amount might the family need to fund if one or more people have a serious illness, an admission or a major procedure?
- Record the individual and family in-network deductibles.
- Apply the plan’s copayments and coinsurance rules until the in-network out-of-pocket maximum is reached.
- Add the annual premiums, because they generally are not part of the out-of-pocket maximum.
- Check whether excluded services, balance bills and out-of-network care sit outside the maximum.
- Repeat the exercise for a family event that affects more than one person; the family maximum, not merely one person’s limit, may control.
This produces a planning ceiling for covered, in-network care under the plan’s terms, not a guarantee that every medical bill will stop there. Noncovered services, out-of-network claims and charges that the plan excludes may create additional exposure. Read the full plan document for those boundaries.
Understand the family deductible before judging a “low” deductible
Family deductibles do not all operate the same way. Confirm the mechanics in both the SBC and full plan document.
Embedded individual deductible
With an embedded design, each person has an individual deductible inside the family deductible. Once one member reaches that individual amount, the plan may begin paying that person’s deductible-based benefits even though the family total has not been met. Record both figures and the services to which each applies.
Recommended Free Tools
Aggregate family deductible
With an aggregate design, the family may have to meet the full family deductible before the plan begins paying for services subject to the deductible. A family with one high user can therefore face a very different early-year cash requirement than a family with an embedded deductible.
Questions to ask the benefits administrator
- Which services bypass the deductible and use a copayment from the first visit?
- Does one member’s spending trigger individual coverage before the family deductible is met?
- Are prescription and medical deductibles combined or separate?
- Do deductible amounts reset on January 1 or on another plan-year date?
- Which payments count toward the deductible and out-of-pocket maximum?
Verify the network for every person, not just the employee
A network label is not enough. Search the current, plan-specific directory for every doctor, facility and pharmacy your family expects to use.
Doctors and hospitals
Check the exact practice location and the hospital where each physician admits patients. A doctor can appear in a directory while a particular facility, anesthesiology group, laboratory or radiology provider is outside the network. Confirm the facility and ancillary providers for planned procedures and childbirth.

Prescriptions and pharmacies
Look up each medication by name, strength and quantity. Note the formulary tier, deductible treatment, quantity limits, step therapy, prior authorization, specialty-pharmacy requirement and preferred pharmacies. Ask the prescriber whether an approved alternative exists only if your clinician considers it appropriate; a lower tier is not automatically an equivalent treatment.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsReferrals and prior authorization
Record whether the plan requires a primary-care referral for specialists, authorization for imaging or therapy, or advance approval for inpatient care. Missing an authorization can change the payment level or leave the household responsible for a noncovered charge.
Behavioral health and recurring services
Confirm network availability for therapists, psychiatrists, developmental services and physical or occupational therapy. A plan that looks inexpensive can be impractical if appointments are unavailable near home or a required provider is out of network.
Compare exclusions, limits and anticipated care
Read beyond the deductible and out-of-pocket number. For each expected service, ask:
- Is the service covered at all, and under what medical-necessity definition?
- Is there a visit, dollar, annual or lifetime limit?
- Does the plan require a referral, prior authorization or designated facility?
- Does coverage change when care is obtained outside the network?
- Are equipment, therapy, home health, infertility, maternity, mental-health or pediatric services subject to special rules?
Write the answer beside the service in your worksheet. “Covered” does not mean “free,” and “not subject to the deductible” does not mean “no cost”; the applicable copayment or coinsurance still matters.

Use the 2026 affordability rule correctly
For 2026, HealthCare.gov gives the employer-coverage affordability threshold as 9.96% of household income. This is a premium-tax-credit eligibility test, not a verdict that a plan is adequate, comfortable or affordable for your particular budget.
When the employee is offered coverage through their own job
The affordability calculation uses the employee’s required premium for the lowest-cost qualifying self-only coverage that meets minimum value. It does not use the family-tier premium for the employee test.

When coverage is offered through another household member’s job
For the household affordability test based on that offer, the family premium is used. The applicable calculation depends on whose job provides the offer and on the household’s tax and coverage facts.
Why family members can reach different Marketplace results
An employee’s coverage can be affordable under the employee test while the spouse or children face a different result for Marketplace savings. Check eligibility and net cost for each person rather than applying the employee’s result to the whole household. Use current Marketplace and IRS guidance for your tax filing status, income, employer offer and other coverage circumstances.
Evaluate a Marketplace alternative person by person
If some relatives might enroll through the Marketplace, compare the complete household outcome, not just a quoted premium.
- List which family members would remain on the employer plan and which would seek Marketplace coverage.
- Determine the applicable affordability test for each person under the current rules.
- Compare the net Marketplace premium after any savings with the incremental employer premium for covering that person.
- Repeat the expected-use and high-cost-year calculations for the split arrangement.
- Verify each option’s doctors, hospitals, medicines, deductibles, out-of-pocket maximums and authorization rules.
- Check how the choice interacts with the household’s tax return and eligibility for premium tax credits.
A lower monthly premium can be offset by a narrower network, a higher deductible or loss of access to a family doctor. Conversely, placing only some members on a Marketplace plan may reduce the employer-tier premium enough to change the result. The correct answer is household-specific.
Price an HSA-qualified high-deductible plan honestly
An HSA can improve the after-tax value of a high-deductible plan, but it is not insurance and it does not eliminate the need for cash reserves.
2026 HSA and HDHP figures
Internal Revenue Service Revenue Procedure 2025-25 sets these calendar-year 2026 figures for HSA-qualified high-deductible health plans and HSA contributions:
| Measure | Self-only | Family | What it means |
|---|---|---|---|
| Minimum annual deductible | $1,700 | $3,400 | Qualification threshold for an HSA-qualified HDHP |
| Maximum annual out-of-pocket expenses | $8,500 | $17,000 | HDHP qualification limit; premiums are excluded |
| HSA contribution limit | $4,400 | $8,750 | Calendar-year 2026 contribution limit |
These are year-specific qualification and contribution figures, not universal caps or recommended spending levels for every employer plan. Verify that your particular plan is HSA-qualified and that your family is eligible to contribute.

Include the employer deposit and tax effect
Add the employer’s HSA contribution to the plan’s value, noting when the money is deposited and whether you must remain enrolled for the full year to receive it. Consider the household’s expected tax treatment of contributions and withdrawals. HealthCare.gov explains that HSA funds can generally pay deductibles, copayments, coinsurance and certain other qualified expenses, but generally cannot pay premiums.
Test the cash-flow risk
Subtract the employer contribution only after checking whether your family can pay bills before the deductible is met. Money in an HSA may soften an after-tax cost, but it does not make a high deductible affordable if the account is unfunded or the household cannot access cash when care occurs.
Set a household affordability limit before choosing
Use your own budget to define a tolerable exposure. Consider three separate amounts:
Free tools Windows power users keep installed
One-click scans. No signup required.
- Monthly commitment: the payroll premium plus any regular prescription or visit costs.
- Early-year liquidity: the amount available for deductibles, coinsurance and deposits before reimbursement or HSA funding.
- Worst covered year: annual premiums plus the applicable in-network out-of-pocket maximum, with a separate contingency for excluded and out-of-network charges.
A plan is financially unsuitable if its high-cost-year exposure would force you to miss rent, mortgage, utilities, debt payments or essential savings. Do not treat an employer contribution or tax benefit as cash you can spend before it is actually available.
Red flags that require a closer review
- The directory lists a doctor but not the hospital or facility where care occurs.
- A recurring medication appears on the formulary but has a quantity limit or prior-authorization requirement you have not planned for.
- The family deductible is shown without identifying whether it is embedded or aggregate.
- The out-of-pocket maximum is quoted without explaining whether out-of-network, excluded or balance-billed charges count.
- The plan’s lowest premium is paired with an HSA contribution that arrives late, leaving an early-year cash gap.
- A benefits summary is from a prior plan year or does not match the plan identification number in your enrollment system.
- A Marketplace comparison uses the employee-only affordability result for a spouse or child without a person-by-person calculation.
A practical enrollment checklist
- Download the SBC, full plan document, premium schedule, directory, formulary and HSA materials for every available plan.
- Write down each family member’s doctors, facilities, pharmacies, medications and expected services.
- Mark network and formulary status using the current plan-specific tools, then call the insurer for any ambiguous entry.
- Record individual and family deductibles, deductible sequencing, copayments, coinsurance and in-network out-of-pocket limits.
- Calculate annual premiums and expected-use cost for each plan.
- Calculate a high-cost in-network year and list exposures that sit outside the maximum.
- Check authorization, referral, exclusion and service-limit rules for anticipated care.
- For an HSA plan, record eligibility, employer deposits, contribution limits and the cash needed before the deductible.
- If considering Marketplace coverage, calculate affordability and net cost separately for each household member under current rules.
- Recheck directories, formularies, premiums and plan-year dates immediately before submitting enrollment.
What the comparison can and cannot tell you
This method can show which plan better fits your documented doctors, medicines, expected services, budget and tolerance for a severe year. It cannot predict the exact claim outcome: treatment may change, providers may bill differently, and networks and formularies can be updated. Keep the plan documents and confirmation numbers so you can challenge a claim using the plan’s stated rules.
The strongest choice is the one that passes both tests for your household: dependable access for every covered person and a worst-case cost that your finances can withstand. The 2026 affordability threshold, HSA limits and any employer contribution are inputs to that decision, not substitutes for it.
