How to check whether your doctors and hospitals are in a Medicare Advantage plan’s network

To verify network participation, use the provider directory for your exact Medicare Advantage plan, then confirm the listing with the plan and the doctor’s office or hospital when you schedule care. Check the specific clinician, billing group and location—not merely whether the facility or professional accepts Medicare—because network status is tied to the plan, product, service area and effective date.

What “in network” means in Medicare Advantage

A Medicare Advantage network is the set of doctors, hospitals and other providers that have contracted with a particular Medicare Advantage product. A carrier can sell several products in one county, and each can have different participating providers, referral rules and cost-sharing. A doctor who participates in one product is not automatically participating in another from the same carrier.

Network status can also depend on the exact practice location and the legal entity that bills for care. A hospital may be listed while a physician group, anesthesiology practice, radiology group or other ancillary provider is not. For that reason, verify the people and facilities involved in the service, not just the hospital’s name.

Medicare’s official fact sheet explains how to use a plan directory and why you should confirm with the provider when scheduling: Understanding Your Medicare Advantage Plan’s Provider Network.

Follow this five-step network check

1. Identify the exact plan before searching

Read the member ID card and enrollment paperwork and record:

  • The complete plan name, including any HMO, PPO, PFFS or SNP designation.
  • The plan ID or contract identifier shown on the card.
  • The plan year and your county or service area.
  • The effective date for the coverage you will use.

Use these details every time you ask about network status. Searching by a carrier’s name alone can return a directory for a different product or county.

2. Search the plan’s own provider directory

Open the directory through the plan’s member website, or call member services and request the directory. Medicare says plans make directories available online and can provide them on request. Search each doctor and each facility separately, then open the result for the exact office or campus where care will occur. The directory guidance appears in the CMS fact sheet linked above.

Check the provider’s specialty, street address, phone number and whether the listing identifies a medical group. For a hospital stay, search the hospital and ask which physician or ancillary groups will bill you. Save or print the result with the date you found it; a directory entry is evidence of the plan’s listing, not a permanent guarantee.

3. Call the plan’s member-services number

Use the phone number printed on your ID card rather than a general number found elsewhere. Give the representative the provider’s full name, practice or facility address, your plan name and plan ID, and the date you expect to receive care. Ask the representative to confirm all of the following:

  • Whether that clinician, facility or billing group is in network for your exact product.
  • Whether the provider is accepting members in that plan at that location.
  • Whether a primary-care referral, prior authorization or site-of-care rule applies.
  • What the plan covers if the provider is out of network and what cost-sharing would apply.
  • Whether the answer is valid for the date on which you expect treatment.

Ask for the representative’s name or identification number and a call reference number if one is available. A verbal confirmation does not replace the plan’s written terms, but it creates a useful record if the directory and an office’s answer conflict.

4. Confirm with the doctor, hospital or facility

When making the appointment, provide the exact plan name, product and the member information the office requests. Ask, “Is this clinician in network for my exact plan at this street address for the date of my appointment?” A hospital question should be equally specific: “Is this hospital in network, and which physician and ancillary groups must I verify separately?”

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ask whether the provider is accepting new patients under your plan. Some offices accept a carrier’s commercial insurance but not its Medicare Advantage products, or accept one Medicare Advantage product but not another. The office should check the insurance information used for the actual visit, not rely on a general statement such as “we take Medicare.” Medicare recommends confirming network status with the provider when scheduling.

5. Record the answer and check again near the appointment

Keep a note or electronic record containing the date and time of each call, the names or identification numbers of the people you spoke with, the exact provider and location, and the answer about network participation, referrals and authorization. Reconfirm shortly before a planned procedure, admission or specialist visit. Medicare says plans may add or remove providers during the year and recommends checking with the provider when you schedule.

If a regularly used primary-care provider or other regularly used provider leaves the network, CMS describes a plan’s good-faith effort to give members at least 30 days’ notice in its 2025 network fact sheet. That notice does not make an old directory result reliable for every future appointment, so perform the appointment-date check yourself.

How your plan type changes the result

Out-of-network rules are not interchangeable. Use your plan’s Evidence of Coverage and member-services representative for the exact dollar amounts and conditions. Medicare’s comparison page summarizes the structural differences: Compare types of Medicare Advantage Plans.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Plan type Typical network rule What to verify before non-emergency care
HMO Routine care generally must use network providers. Medicare identifies emergency care, out-of-area urgent care and temporary out-of-area dialysis as exceptions. Confirm the provider is in network, whether your primary-care doctor must issue a referral, and whether prior authorization is required. An HMO-POS version may cover selected out-of-network services, usually at higher cost.
PPO Covered services can generally be obtained from out-of-network doctors or hospitals, but your share is usually higher. Ask for the out-of-network deductible, coinsurance or copayment, any separate annual limit, and whether authorization or a referral still applies.
PFFS Some PFFS plans have networks; others allow care from providers that accept the plan’s payment terms for the specific service. Out-of-network care can cost more. Ask whether the plan has a network and whether the provider has agreed to the plan’s terms for this visit. Do not assume a provider will accept those terms simply because the provider accepts Medicare.
SNP Rules follow the plan’s underlying HMO or PPO structure and the SNP’s own terms. Confirm the network, referral process and any care-coordination requirements for the particular SNP.

Medicare’s detailed explanation of plan choices and provider access is also available in Compare Original Medicare & Medicare Advantage.

Emergency and urgent care when the provider is out of network

Medicare says you can obtain emergency and urgent care even when the provider is out of network. Follow your plan’s instructions when circumstances allow, and ask the plan how emergency and urgent-care cost-sharing works. The general Medicare explanation is at How to get Medicare services.

Emergency access is different from choosing an out-of-network provider for a scheduled office visit, elective procedure or planned admission. For non-emergency care, use the network and authorization rules that apply to your plan type. If you are traveling, ask how the plan defines urgent care, what locations qualify and whether you must notify the plan after treatment.

Why “accepts Medicare” is not enough

Original Medicare and Medicare Advantage use different provider-choice systems. Medicare says a person with Original Medicare can generally see any doctor or hospital that accepts Medicare. A Medicare Advantage member may instead need to use the plan’s network and service area for non-emergency care. Therefore, a provider’s participation in Medicare—or a listing in a general Medicare search—does not prove participation in your Medicare Advantage product.

Checking a hospital admission or procedure

Verify every participant in the episode of care

A planned hospital service can generate claims from several entities. Ask the hospital which organization will bill for the facility, and ask your surgeon or admitting clinician about the professional claim. For anesthesia, pathology, radiology, emergency physicians and other ancillary services, ask which groups may participate and whether each is in network for your plan.

Call the plan with the procedure name or code if the representative requests it. Confirm the hospital campus, department and expected date, then ask about prior authorization, referral requirements and any required in-network site of care. Keep the authorization number and the name of the plan representative with your appointment records.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When the directory and office disagree

Do not rely on the most convenient answer. Take screenshots or save the directory result, note the office’s response, and call member services again with the discrepancy. Ask the plan to explain which source controls and to provide written confirmation if possible. Before treatment, ask the provider’s billing office to verify the same plan and location in its eligibility system.

If you receive a bill that treats scheduled care as out of network despite these confirmations, contact the plan’s member services and follow its appeal or grievance instructions. Include your call notes, directory record, authorization information and appointment date. The plan can explain whether the claim was submitted under the wrong provider entity, lacked authorization or was processed under a different benefit rule.

Use network checks when comparing plans

During the annual Medicare Open Enrollment Period—October 15 through December 7—check the doctors and hospitals you actually use before selecting a plan. Medicare specifically advises checking providers while comparing plans. Start with your primary-care doctor, specialists, preferred hospital, regular laboratory, pharmacy-related clinicians and any provider involved in a planned procedure.

Comparison question Why it matters
Are my specific doctors and hospitals listed for the plan and location? A carrier’s name or a provider’s general Medicare participation does not establish product-specific network status.
What happens if I must go out of network? PPOs may cover it at a higher rate; HMOs usually restrict routine care except for stated exceptions; PFFS and SNP terms vary.
What referrals and prior authorizations are required? A provider can be in network yet still require a referral or authorization before the plan pays as expected.
Does the network cover my service area and travel needs? Network access can change outside the plan’s county or service area, especially for routine care.
Does the directory identify the exact office, campus and billing group? Different locations or entities under a similar name may have different contracts.

The enrollment dates and plan-comparison context are described in Medicare’s official network fact sheet: Understanding Your Medicare Advantage Plan’s Provider Network.

A practical call script

Reading a prepared question reduces the chance that a representative gives a general answer. Adapt these prompts to your situation:

  • “Is Dr. [name], at [street address], in network for my exact [plan name and product] for [year]?”
  • “Is this hospital in network for my plan, and do I need to check the physicians or other groups involved separately?”
  • “Is the provider accepting new patients under this plan?”
  • “Do I need a primary-care referral or prior authorization for this service?”
  • “If this provider is out of network, does my plan cover the service, and what would I pay?”
  • “Can you confirm this for the date I expect to receive care?”

Ask both the plan and the provider’s office the location-specific question. Their systems serve different purposes: the plan determines the benefit and network contract, while the office verifies the provider entity and billing information it intends to use.

What to keep in your records

Create one folder—paper or electronic—for each planned course of care. Include:

  • A copy or screenshot of the directory listing, showing the date.
  • The exact plan name, plan ID, plan year and service area.
  • Provider and facility names, addresses, specialties and billing groups.
  • Dates, times, names or IDs of plan and provider representatives, and call-reference numbers.
  • Referral, prior-authorization or service-site instructions and authorization numbers.
  • Appointment date, expected service and any written estimate of cost-sharing.

This record helps you identify a change before care and gives the plan the information needed to investigate a claim that was processed unexpectedly.

Common mistakes that create surprise bills

  • Searching only by carrier: The result may belong to another product, county or plan year.
  • Checking only the hospital: Separate physician and ancillary groups can submit their own claims.
  • Trusting an old directory: Plans may add or remove providers during the year.
  • Confusing Medicare participation with plan participation: Accepting Medicare does not establish a Medicare Advantage contract.
  • Skipping authorization: In-network status alone does not guarantee payment when a referral or prior authorization is required.
  • Assuming out-of-network means either “never covered” or “same price”: PPO, HMO-POS, PFFS and SNP terms produce different outcomes.
  • Failing to state the location: A provider can have multiple offices with different network arrangements.

The decision rule

For a scheduled service, treat a provider as confirmed only after the plan and the provider’s office have matched the exact clinician or facility, address, product, plan year and expected date. Then check the referral, authorization and out-of-network rules that apply to your plan type. Repeating that verification when you schedule is the most reliable way to account for network changes.
