How to Check a Hospital Bill for Errors and Dispute Incorrect Charges

Do not pay a hospital bill simply because it arrived. First obtain an itemized statement, your insurer’s Explanation of Benefits (EOB), relevant medical records and receipts, then reconcile every charge with the care you received and the insurer’s calculation. Challenge factual errors with the provider in writing; challenge a denial or underpayment through the insurer’s appeal process. Separate surprise-billing and collections issues because each has different eligibility rules, deadlines and escalation channels.

Why a hospital bill can be wrong

A hospital statement is a request for payment, not proof that every line is accurate or that the amount shown is your final legal responsibility. Errors can arise from data-entry mistakes, duplicate submissions, incorrect procedure codes, services attributed to the wrong patient, an insurer claim processed under the wrong network status, or a balance that fails to reflect contractual adjustments and insurance payments.

Reviewing a bill does not mean assuming the hospital acted improperly. It means testing the statement against independent records. The Centers for Medicare & Medicaid Services (CMS) advises patients to request a detailed bill and compare it with the Explanation of Benefits and medical records. CMS puts the principle plainly: “Bills can be complex and confusing, but it’s important to review your bill for accuracy.”

There is no reliable national percentage establishing how many hospital bills contain errors. Treat dramatic claims such as “80% of bills are wrong” as unsupported unless a source defines the sample, date, billing type and error standard.

Collect the documents before calling or paying

Put the documents in one folder—paper or digital—and record the account number and dates of service. Keep the original statement even if you later receive a corrected one.

  • Summary bill: The statement currently requesting payment, including its due date, balance and payment address.
  • Itemized statement: A line-by-line list of each service, drug, supply, room charge, quantity, code and price. Ask the hospital billing department for this if it was not included.
  • Explanation of Benefits: The insurer’s claim result showing billed charges, allowed amount, insurer payment, deductible, copayment, coinsurance, noncovered amounts and the stated patient responsibility. An EOB is generally not an invoice.
  • Medical records: Discharge summary, procedure notes, medication administration record, laboratory and imaging reports, and other records that identify what was actually provided.
  • Receipts and prior estimates: Proof of copayments or deposits, a written estimate, and—when uninsured or self-pay—a good-faith estimate.
  • Coverage information: Insurance member and group numbers, the plan’s customer-service number, and the provider’s network status for the date of care.

Request records through the provider’s medical-records process if the billing office cannot supply them. Note every conversation: date, representative’s name or identification number, promised action and any reference number.

Confirm the bill belongs to you and uses the right coverage

Before analyzing prices, verify identity and claim routing. Check:

  • Patient name, address and account number.
  • Every date of service, including admission, discharge and any separate professional-service dates.
  • Hospital, physician, laboratory, ambulance or facility name shown on each statement.
  • Insurance member ID, group number and subscriber information.
  • Whether the claim was processed as in-network or out-of-network under the plan in force on that date.

The Consumer Financial Protection Bureau (CFPB) recommends confirming that the bill is yours and that insurance payments and coverage are reflected. A wrong member number or an old insurance card can make a correctly provided service appear unpaid.

Audit every line of the itemized statement

Match services to your records

Mark each line as confirmed, unclear or disputed. You should be able to connect a treatment, test, medication, supply or room charge to your medical record or to a clearly documented part of the stay. CMS states: “You shouldn’t get a bill for anything that isn’t documented in your records.” An unfamiliar description may be an abbreviated code rather than an error, so ask for an explanation before concluding that it is invalid.

Look for duplicate charges

Compare descriptions, dates, units and codes. The same procedure can legitimately appear once on a facility bill and separately on a professional bill, but identical lines on the same account—or a quantity that exceeds what the record shows—deserve a written explanation. Check recurring daily room or monitoring charges against the actual admission and discharge dates.

Check quantities, dates and codes

Look for a medication, supply or laboratory test billed more times than documented; a service dated when you were not present; or a procedure code that does not describe the care received. Ask the billing office to identify the code’s plain-language description and the record supporting it. Do not change a code yourself; request that the provider and insurer correct the claim.

Test the arithmetic and adjustments

Recalculate line extensions and totals. Then compare the provider’s balance with the EOB’s allowed amount, insurer payment, deductible, copayment and coinsurance. A statement may initially show the full billed charge while a claim is pending, but the final patient balance should reflect the plan’s adjudication and any contractual adjustment. A balance that ignores an insurer payment, applies a deductible twice or bills you for a network write-off is a warning sign.

Reconcile the hospital statement with the EOB

Use the EOB as a claim-by-claim map rather than expecting its total to look identical to the hospital’s original charge total. For each date and service, identify:

Item What to compare What a mismatch may mean
Billed charge Provider’s submitted amount Different figures can reflect separate claim lines or corrected submissions.
Allowed amount Plan’s negotiated or otherwise recognized amount A network contract may require an adjustment that the patient does not owe.
Insurer payment Amount paid to the provider The bill may be stale if this payment has not posted.
Deductible Amount assigned to your annual deductible Check that the amount agrees with your plan’s remaining deductible.
Copayment and coinsurance Fixed copay or percentage of the allowed amount Incorrect network status or a coding error can change the calculation.
Patient responsibility EOB’s final amount for that claim The provider’s final statement should reconcile to this amount, subject to legitimate separate balances.

If the EOB shows “denied,” “not covered,” “needs information” or a different responsibility than the bill, call the insurer and ask for the denial reason, the exact claim line, the correction needed and the appeal deadline. CFPB guidance says that if something does not look right, ask for an itemized list of charges. Do not assume a provider can fix an insurance coverage decision; the insurer controls claim adjudication, while the provider controls its own coding and statement.

Dispute an incorrect provider charge in writing

Call first if you need the correct department or documents, but make the actual dispute dated and written through a portal, email process or letter. Send it to the billing address shown on the statement and retain a copy and delivery confirmation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Include these details

  • Patient name, account number and every disputed date of service.
  • The exact line description, procedure or revenue code, quantity and amount.
  • A factual explanation: duplicate line, undocumented service, wrong date, wrong patient, incorrect quantity, code mismatch or insurance payment not applied.
  • Supporting pages from the EOB, medical record, receipt, estimate or insurance correspondence.
  • The remedy requested: remove or correct the line, resubmit a corrected claim, apply the insurer payment or issue a new statement.
  • A request for a written response identifying the result for each disputed line.

Ask the provider to place the disputed amount on hold while it investigates, but confirm whether it will do so and for how long. There is no universal rule that stops every payment or collection activity merely because a patient has sent a dispute; state law, provider policy, debt status and the insurance plan can change the result. Continue monitoring due dates and request a written agreement for any payment arrangement.

Short dispute-letter structure

Subject: Dispute of account [account number] — dates of service [dates]

I dispute the following line(s): [description, code, date, quantity, amount].
The charge is incorrect because: [specific factual reason].
Attached are: [EOB, record excerpt, receipt or estimate].
Please correct the statement or resubmit the claim and send a written response
for each line. Please confirm whether the disputed amount is on hold while you investigate.

Name, address, phone, date

Keep proof of submission, attachments and every response. If a corrected claim is sent to the insurer, obtain the resubmission or reference number and watch for a new EOB.

Appeal an insurance denial or underpayment

An insurance appeal addresses coverage or claim processing, not merely a hospital’s price. HealthCare.gov states that consumers have a right to an internal appeal for a denied claim or canceled coverage. Your plan documents and denial notice set the submission method and deadline; use those dates rather than a generic calendar.

  1. Read the denial notice and identify the precise reason: coding, missing records, medical-necessity determination, network status, authorization or an exclusion.
  2. Ask the insurer for the claim file, clinical or policy provision used, and instructions for an internal appeal.
  3. Gather the EOB, itemized bill, medical records, clinician letter, authorization information and any corrected claim from the provider.
  4. Write a focused appeal that addresses each denial reason and states the requested outcome—reprocessing, payment at the correct network level or correction of patient responsibility.
  5. Submit it using the plan’s required channel before the stated deadline and keep confirmation.
  6. If the internal appeal fails and the plan offers or requires external review, follow the denial letter’s external-review procedure and deadline.

CFPB describes the right to appeal when you disagree with charges or want more information. A provider correction and an insurer appeal can proceed at the same time when the error involves both a wrong code and a resulting denial.

Check whether the No Surprises Act applies

Insured patients

Key federal No Surprises Act protections took effect January 1, 2022, according to CFPB. For insured patients, the law can restrict certain out-of-network emergency charges and out-of-network facility-based charges connected with care at an in-network facility. Whether protection applies depends on the service, setting, notice and consent requirements, plan and other facts. An out-of-network label alone does not answer the question.

Compare the bill with the EOB and ask the insurer or facility to explain why a protected service was processed as out-of-network or balance billed. CMS’s No Surprises Help Desk is listed at 1-800-985-3059 for questions about eligible surprise-billing problems.

Uninsured or self-pay patients

If you were uninsured or chose not to use insurance, a patient-provider dispute process may be available when the bill is at least $400 above a good-faith estimate. CFPB describes that threshold, and CMS describes an initial-bill timing condition generally requiring action within 120 calendar days. Preserve the estimate, bill, dates and proof of when the bill was received. Eligibility and procedure details should be confirmed through the federal process; do not assume every estimate or service qualifies.

Choose the right escalation path

No Surprises Act process Federal or designated dispute process, depending on issue Eligible protected out-of-network bill or qualifying uninsured/self-pay estimate dispute Bill, EOB or estimate, dates and consent/network facts; a $400 threshold and 120-calendar-day condition may apply to the self-pay route CMS No Surprises Help Desk at 1-800-985-3059
Patient advocate or state Consumer Assistance Program Advocate assists; agency scope varies Complex provider, insurance or surprise-bill problem Complete file; ask about credentials, fees, conflicts and privacy before sharing records Appropriate state or federal complaint channel
Collections or credit-reporting dispute Collector and, for reporting issues, credit-reporting system Debt is inaccurate, not yours, already paid or improperly reported Written debt dispute, account records and proof; collector and state deadlines can apply CFPB complaint or legal advice where appropriate

If the account is sent to collections

Do not ignore a collection notice. First verify the patient, provider, dates, amount and whether insurance processing or a written provider dispute is pending. Send the collector a written dispute of inaccurate information and keep proof of delivery. Explain the specific error rather than merely saying the amount is unfair.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

CFPB reported $88 billion in medical bills in consumer credit records as of June 2021. That figure describes reported medical debt at that time, not the amount of your bill and not a finding that the debt is valid. If a collector or credit report contains inaccurate information, CFPB is an avenue for a complaint. Collection rights and credit-reporting rules depend on the facts and applicable law, so obtain qualified legal help for a threatened lawsuit, garnishment or time-sensitive state issue.

A provider dispute does not automatically cancel the debt or stop collection activity everywhere. Ask the provider and collector in writing what review status applies, continue the insurance appeal if relevant and preserve every notice.

Use help without surrendering control of your records

CMS recommends considering a patient advocate or state Consumer Assistance Program. Before hiring or authorizing anyone, verify:

  • Credentials and experience with provider billing, insurance appeals, collections or the specific issue you have.
  • Fees, hourly rates, contingency arrangements and cancellation terms.
  • Conflicts of interest, including financial relationships with providers or insurers.
  • Privacy and record-retention practices, including how medical information is shared.
  • Exactly what the advocate will do and which decisions remain yours.

Give an advocate a copy of the file, not your only originals. Keep submitting documents yourself when a plan or dispute process requires the patient’s signature.

  5. Send provider corrections in writing and request a corrected statement or claim reference.
  6. Appeal an insurer denial under the plan’s stated internal and, if available, external-review deadlines.
