To change a beneficiary, update the record for each retirement account or life insurance policy separately. Contact the plan administrator, account custodian, retirement system or insurer, use its current form or online workflow, submit it exactly as instructed, and keep proof that the provider received and accepted the change. A will, divorce, marriage or an update to one account does not reliably change designations on other accounts.
Start by identifying every benefit and who controls its record
Beneficiary designations are maintained by the organization that administers the particular benefit. Make a complete inventory before changing anything:
As an Amazon Associate I earn from qualifying purchases.
- Individual retirement accounts and other accounts held by a brokerage, bank or trust company.
- Employer retirement plans, such as a 401(k), whose plan administrator or recordkeeper controls the designation.
- Pensions and retirement systems, including any separate survivor-annuity election.
- Employer-provided group life insurance.
- Individually purchased life insurance policies.
Do not assume that a single form updates an entire household or all benefits from one employer. The U.S. Office of Personnel Management (OPM), for example, uses separate processes for Federal Employees’ Group Life Insurance (FEGLI), CSRS/FERS retirement lump-sum benefits and the Thrift Savings Plan (TSP). North Carolina Retirement Systems likewise explains that benefits are paid according to the designation held by the provider for each listed plan.

When should you review a beneficiary?
Review designations whenever your family, finances or intentions change. Important triggers include:
- Marriage or divorce.
- Birth, adoption or death of a beneficiary.
- A change in the relationship with a person previously named.
- Creation, amendment or termination of a trust.
- A change in the way you want assets divided among primary and contingent beneficiaries.
- Retirement, a job change or replacement of employer life insurance.
- A move that leaves an address or contact detail stale.
OPM specifically tells federal employees to complete a new FEGLI form after marriage or divorce. Its guidance also says an address change should be handled by filing a new designation rather than writing over the old form. A divorce decree or will may have legal effects in some circumstances, but you should not rely on either document to update the provider’s record. OPM warns that an unchanged federal lump-sum designation can leave a former spouse named.
How to change the designation: a provider-by-provider procedure
1. Make an account and policy inventory
List the institution, account or policy number, employer or retirement system, current primary beneficiaries and any contingent beneficiaries. Include coverage provided through work even if you never selected it yourself. Note whether each benefit is still active and whether payments from a pension have begun.

2. Contact the correct administrator
Call the number on a recent statement, use the secure message function or sign in to the official account portal. Ask these questions:
- What is the current beneficiary-change form or online path?
- Does this benefit require a spouse’s consent, a witness, notarization or additional documentation?
- Where must a paper form be sent?
- When is the change effective: when you sign, when you submit, when the provider receives it or when the provider approves it?
- Can the beneficiary be irrevocable, and if so, what consent is required to change it later?
Requirements vary. Federal examples illustrate particular rules for federal programs; they are not universal requirements for private retirement plans or insurance policies.
3. Gather accurate beneficiary information
Use the exact legal name and identifying information requested by the provider. The Department of Veterans Affairs (VA), in its life-insurance guidance, asks for a beneficiary’s name, Social Security number, birth date, relationship and contact information. Other providers may request different data. For multiple beneficiaries, determine the percentage or fractional share for each and check the provider’s arithmetic requirement. The VA instructs that shares for multiple beneficiaries total 100 percent.
Decide whether you need primary beneficiaries, contingent beneficiaries or both. A contingent beneficiary receives the benefit only if the relevant primary beneficiary cannot receive it under the plan’s rules. Do not assume that every provider handles per-stirpes distributions, surviving descendants or charities in the same way; ask for the plan’s permitted choices.

4. Complete the current form or electronic workflow
Enter the requested information exactly as instructed. Do not cross out, overwrite or modify a paper form if the provider says alterations invalidate it. Follow every direction about signatures, dates, witnesses, notarization, attachments and spouse consent. If an online portal displays a confirmation screen, save or print it before leaving the session.
5. Submit it to the specified office
Send a paper form to the address or delivery channel named in the instructions. A form sent to an employer’s human-resources office may not update an insurer or a separate retirement account unless that office is the designated recipient. For an electronic submission, retain the confirmation number, timestamp and downloaded copy.
6. Confirm receipt and acceptance
Do not stop at mailing or uploading the form. Check the account, call the administrator or request written confirmation that the designation is on file. Keep a dated copy of the completed form, delivery evidence and any acceptance notice with your estate documents. If the provider reports a rejection, correct the issue and resubmit rather than assuming the old designation has been replaced.
7. Repeat the process for every benefit
Use the same checklist for each account and policy. A successful update at one institution has no effect on an unrelated account, pension or insurance policy. Set a calendar reminder for a periodic review and revisit the records after any major family change.

Some plans make receipt by a particular office a condition of effectiveness. OPM’s FEGLI guidance states that if the insured dies before the employing office receives the new designation, benefits are paid under the next prior designation on file or, if none exists, under the applicable order of precedence. In other words, signing a form or placing it in the mail may not be enough.
OPM’s federal retirement form instructions similarly describe a written, signed and witnessed designation that becomes effective when received by the agency or OPM, depending on the person’s status. Use the current instructions for the specific federal benefit and retain evidence of delivery. Private plans and insurers may use different rules, so ask the administrator when receipt, processing or approval controls.
Federal employee examples: separate forms for separate benefits
For FEGLI, OPM identifies Standard Form 2823, Designation of Beneficiary. The insured signs the form in front of two witnesses, and a witness cannot also be named as a beneficiary. The employing office must receive the completed form before death for the new designation to control.

CSRS and FERS retirement lump-sum benefits use a different designation process, including Standard Form 3102, Designation of Beneficiary: Civil Service and Federal Employees Retirement Systems. The form’s instructions describe a written, signed and witnessed designation and explain when receipt by the agency or OPM makes it effective. The TSP has its own designation process. These examples show why copying a form from one federal benefit to another, or from a federal program to a private plan, can produce an invalid update.
For certain federal lump-sum benefits, OPM describes an order of precedence when no designation is on file. That federal order should not be treated as the rule for a private retirement plan or life insurance policy.
A beneficiary designation is not the same as a retirement payment election
Changing a beneficiary while you are alive is an administrative update. The tax and distribution rules that apply after death are a separate question. The Internal Revenue Service explains that inherited retirement-account distributions depend on the beneficiary category, timing and plan terms. For a qualified plan such as a 401(k), the plan document establishes the available distribution options, and the beneficiary should contact the plan administrator. The designation itself does not determine every tax result.
Pensions create an additional distinction. When you retire, you may select a payment option that provides an annuity to a survivor after your death. That survivor-annuity election is part of the retirement payment choice, not merely a name in a beneficiary field. North Carolina Retirement Systems says certain listed survivor options generally cannot be changed after the first regular payday or first recorded payment, except under specified conditions. Other pension systems have their own rules. Before trying to replace a pension beneficiary, ask whether you are changing a revocable designation or a survivor option that is already fixed.
Choosing a person, minor, trust, estate or organization
Individual beneficiaries
Use the legal name and identifying details requested by the provider, and specify the intended share. Check that the total allocation satisfies the provider’s requirements. If a named person dies, update the form rather than assuming the provider will automatically redirect that share as you intended.
The Tool Desk
Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Minor beneficiaries
Naming a minor can complicate payment. VA life-insurance guidance says payment for a minor may require a court-appointed guardian or a VA-appointed fiduciary and may be delayed. State law and provider procedures differ. Ask the insurer or plan administrator how a minor’s benefit would be held and paid before naming one directly.
Trusts
The VA advises that a trust intended as a life-insurance beneficiary must already exist before it is named. Obtain the exact trust name, date and trustee information required by the provider. A trust can have legal and tax consequences, so discuss the choice with a qualified estate-planning professional when the arrangement is more than a straightforward beneficiary update.

Estates, charities and other organizations
Confirm that the plan permits the organization or estate and ask for its legal name, tax identification number and address. A designation to an estate can subject proceeds to probate or creditor claims in some circumstances; the provider and a qualified lawyer can explain the consequences under the governing law. Do not select an estate, trust or organization merely because its name appears convenient on a form.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Common mistakes and how to correct them
Updating only one account
Problem: You change a workplace plan but leave an IRA, pension or life policy untouched.
Fix: Work from a complete inventory and obtain confirmation for every record.
Assuming a will or divorce changes the provider’s record
Problem: You expect a will or divorce decree to replace the named beneficiary automatically.
Fix: Submit the provider’s current designation form and obtain acceptance. Federal guidance specifically warns that a former spouse can remain named when a federal lump-sum designation is not changed.
Using an old or unrelated form
Problem: You download a form for a different benefit or send it to the wrong office.
Fix: Request the current form directly from the administrator and follow its delivery instructions. FEGLI, CSRS/FERS lump-sum benefits and TSP use distinct processes.

Fix: Keep tracking information, electronic confirmation and a dated copy; call if no acknowledgement arrives. Receipt can determine which designation controls.
Naming a minor or an uncreated trust without planning
Problem: Payment is delayed or the designation cannot be administered as intended.
Recommended Free Tools
Fix: Ask about guardian, fiduciary and trust requirements first. Create the trust before naming it, if that is the chosen structure, and obtain professional advice for legal or tax issues.
Treating a pension survivor option like an ordinary beneficiary entry
Problem: You try to change a survivor benefit after pension payments have begun.
Fix: Ask the retirement system whether the election is still changeable and which exceptions, if any, apply.
A practical records checklist
- Inventory of every retirement account, pension and life insurance policy.
- Provider name, account or policy number and contact channel.
- Current form or portal instructions for that specific benefit.
- Legal names, identifying information and contact details for proposed beneficiaries.
- Primary and contingent percentages that meet the provider’s rules.
- Required spouse consent, signatures, witnesses, notarization or attachments.
- Submission date, destination, tracking number or electronic confirmation.
- Written acknowledgement that the designation was accepted and is on file.
- Calendar reminder to review after marriage, divorce, birth, adoption, death or another change in plans.
If a dispute involves a divorce decree, an irrevocable beneficiary, a trust, a pension election or competing claims, contact the administrator promptly and consult a qualified attorney or tax professional as appropriate. The provider’s governing plan document and applicable law control the outcome.
