How to calculate the out-of-pocket cost of health insurance with a deductible

Estimate a plan-year medical bill by adding the deductible you still owe for covered services, the copayments for applicable services, and coinsurance on each service’s allowed amount after the deductible. Track those qualifying payments against the plan’s out-of-pocket maximum. Keep premiums, uncovered care, out-of-network care, and charges above the allowed amount in separate lines because they may not be protected by that maximum.

What “out-of-pocket cost” means

Out-of-pocket costs are the medical expenses you pay instead of the insurer. For covered care, they normally include your deductible, fixed copayments, and coinsurance. You can also owe the full price of services the plan excludes. Your premium is different: it buys coverage whether or not you use care, so add it separately when you want a total annual household budget.

As an Amazon Associate I earn from qualifying purchases.

HealthCare.gov summarizes total coverage cost as premium plus deductible, out-of-pocket costs, and copayments or coinsurance. Actual spending depends on the care and prescriptions you use, the plan’s rules, and whether each provider is in network.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The information you need before calculating

Use the plan’s Summary of Benefits and Coverage (SBC), policy or certificate, member portal, and insurer cost estimator. The policy controls if a general definition differs from your plan language.

  • Deductibles: Record the individual and family amounts, plus any separate prescription, hospital, or other deductibles.
  • Service rules: For every expected visit, test, treatment, facility charge, and prescription, note whether it is covered, subject to the deductible, or covered before the deductible.
  • Network status: Verify that both the facility and the professional are in network. Network rules affect the allowed amount and whether payments count toward the maximum.
  • Allowed amount: Obtain the insurer’s negotiated or allowed amount, not just the provider’s billed charge.
  • Cost sharing after the deductible: Write down the copayment or coinsurance percentage for each category of care.
  • Out-of-pocket maximum: Capture the individual and family limits and exactly which expenses the plan counts.
  • Premium: Record the amount and payment frequency separately from medical cost sharing.

How to calculate your estimate step by step

1. Define the result you want

Choose between two outputs:

  • Expected medical spending: deductible payments, copayments, coinsurance, and any noncovered or otherwise unprotected charges.
  • Total annual health budget: expected medical spending plus the annual premium (monthly premium multiplied by 12, unless your payment schedule differs).

Do not call the second figure an out-of-pocket maximum. The maximum generally limits qualifying covered in-network cost sharing, not premiums or every possible health expense.

2. List the care you expect to use

Make one row for each service or prescription. Include the number of occurrences, such as six primary-care visits, one MRI, or twelve monthly prescriptions. For recurring care, multiply the per-service result by the expected number of uses.

3. Apply the remaining deductible

For a covered service that is subject to the deductible, pay the allowed amount until the deductible is satisfied. If your remaining deductible is smaller than the allowed amount, only that remaining balance is assigned to the deductible; the plan’s post-deductible copay or coinsurance then applies to the rest if the plan says so.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not automatically put every service under one deductible. A plan can cover certain benefits before the deductible, and it can have separate medical and prescription deductibles. Preventive services and other categories may follow different rules, so use the service-specific line in the SBC.

4. Calculate coinsurance on the allowed amount

Coinsurance is a percentage of the covered service’s allowed amount after the deductible rules are met. For example, 20% of a $100 allowed amount is $20. If a provider bills $160 but the insurer’s allowed amount is $100, the normal in-network coinsurance calculation uses $100, not $160. Any amount above the allowed amount can remain your responsibility and may not count toward the out-of-pocket maximum.

5. Add fixed copayments

A copayment is a fixed dollar amount, such as a stated office-visit charge. Apply the copay shown for that service and account for any rule that changes the copay before or after the deductible. A copay and coinsurance can both appear in one episode of care—for example, a facility copay plus coinsurance for a covered professional service—so check each line item.

6. Apply the out-of-pocket maximum

Track eligible deductible, copayment, and coinsurance payments as the plan processes claims. Once you reach the relevant plan-year limit, the plan generally pays 100% of covered in-network benefits for the rest of that plan year. The maximum does not automatically absorb premiums, uncovered services, out-of-network expenses, or charges above the allowed amount under HealthCare.gov’s Marketplace explanation.

Use the individual limit for one member’s covered spending when the plan has an individual threshold. On family coverage, also monitor the family threshold; the plan may require one person to reach an embedded individual deductible or maximum before that person’s benefits change, while the household has a separate aggregate limit.

7. Keep excluded costs in separate rows

Your worksheet should show at least four totals:

  1. Qualifying covered in-network cost sharing, capped at the applicable out-of-pocket maximum.
  2. Premiums for the period.
  3. Out-of-network or uncovered spending.
  4. Amounts above the allowed amount or other balance-billed charges.

This layout prevents the common error of treating the maximum as a cap on every bill you could receive.

Core formulas

For each covered service subject to the deductible:

deductible portion = minimum(allowed amount, remaining deductible)

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

After the deductible portion is satisfied, use the plan’s rule:

coinsurance = coinsurance rate × applicable allowed amount

Then add any fixed copay:

service cost sharing = deductible portion + copay + coinsurance

Worked calculations

One office visit after the deductible

Suppose the plan’s allowed amount for an office visit is $125, the deductible has already been met, and the plan charges 20% coinsurance for that service. Your share is 20% × $125 = $25. If the plan instead specifies a $30 office-visit copay, use $30 rather than calculating 20%. The plan’s service rule determines which method applies.

HealthCare.gov high-cost illustration

HealthCare.gov illustrates a plan with $12,000 in allowable costs, a $3,000 deductible, 20% coinsurance, and a $6,850 out-of-pocket maximum. The member pays the first $3,000. The remaining allowable costs are $9,000, and 20% of that is $1,800. Deductible plus coinsurance equals $4,800, which is below the $6,850 maximum. These figures demonstrate the arithmetic; they are not universal plan terms.

Plan-shopping illustration

Another HealthCare.gov example uses a $1,500 deductible, 20% coinsurance, and a $5,000 maximum. After the deductible, 20% of a $125 office visit is $25. After the member reaches the plan’s maximum during the year, the member pays $0 for another covered service in that plan scenario. The result applies only to the stated example’s rules.

A practical annual worksheet

Build a table before choosing a plan or budgeting for the year. Enter the insurer’s figures rather than estimates based on a provider’s sticker price.

After entering each row, total qualifying covered in-network cost sharing and stop counting it once the applicable maximum is reached. Continue listing premiums and unprotected charges because they remain part of your real budget.

How family coverage changes the calculation

Family plans often have both an individual deductible and a family deductible. A single member may reach the individual threshold while the household continues toward the family threshold. Repeat the same check for the out-of-pocket maximum: identify whether the plan has an embedded individual limit, a family aggregate limit, or both. Your SBC should state how payments are assigned.

For a family estimate, create a separate column for each member and a household total. Allocate each claim according to the insurer’s accumulator display rather than assuming every payment is shared equally.

Why the allowed amount and network status matter

The provider’s billed charge is not necessarily the amount used for coinsurance. In-network claims generally use the insurer’s negotiated allowed amount. If an out-of-network provider bills more than the plan allows, the difference can become your responsibility. Out-of-network spending can also be excluded from the plan’s out-of-pocket limit, and balance-billing rules vary by plan and circumstance.

Verify network status for the facility, physician, laboratory, anesthesiologist, and other professionals involved in a procedure. One out-of-network participant can create a separate bill even when the main facility is in network.

Comparing two health plans

Estimate the same expected care under each plan instead of choosing on premium alone. HealthCare.gov advises comparing estimated yearly costs, including premium and expected cost sharing.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Comparison factor Question to answer
Premium What is the annual premium at the payment frequency you actually use?
Deductible How much applies to an individual and family, and which services are subject to it?
Pre-deductible coverage Which visits, preventive services, drugs, or supplies are covered before the deductible?
Copayments What fixed amounts apply to primary care, specialists, urgent care, emergency care, prescriptions, and facility services?
Coinsurance What percentage applies after the deductible, and is it based on the allowed amount?
Network exposure Are your doctors and facilities in network, and what happens if they are not?
Out-of-pocket maximum What are the individual and family limits, and which payments count?
Separate accumulators Are pharmacy, medical, hospital, or other deductibles tracked separately?

Run at least three scenarios: low use, expected use, and a high-cost year. A plan with a higher premium may produce a lower total in a year with substantial covered care, while a lower-premium plan may cost less when little care is used. The arithmetic must use each plan’s actual terms.

Current Marketplace maximums for 2026

HealthCare.gov states that the Marketplace out-of-pocket maximum limits for plan year 2026 are $10,600 for an individual and $21,200 for a family. These are federal Marketplace figures for that plan year, not the limit on a specific policy unless the policy lists the same amount. They do not cap premiums, uncovered services, out-of-network care, or charges above the allowed amount.

Common calculation mistakes and fixes

Counting the premium toward the deductible

Premium payments buy coverage and normally do not reduce the deductible or out-of-pocket accumulator. Keep a separate premium total.

Using the billed charge for coinsurance

Ask for the allowed amount or use the insurer’s estimate. Coinsurance generally applies to that covered amount, while an excess charge may remain outside the maximum.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Assuming every service is subject to the deductible

Read the service-specific benefit line. Some benefits are covered before the deductible, and some plans use separate deductibles.

Treating the out-of-pocket maximum as an all-in cap

Continue budgeting for premiums, noncovered treatment, out-of-network care, and above-allowed charges. Only qualifying covered in-network cost sharing is generally protected.

Ignoring the plan year

A deductible or maximum can reset when the plan year renews. Date each claim and confirm when the accumulator resets.

Mixing individual and family thresholds

Track the member and household accumulators separately. A family plan can apply two thresholds at the same time.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Relying on a generic glossary

Educational definitions can differ from policy language. If the SBC and policy disagree with a general explanation, ask the insurer which term governs the claim.

How to keep the estimate accurate during the year

  1. Save the SBC, policy, provider estimates, and every explanation of benefits.
  2. After each claim, compare the allowed amount and member responsibility with your worksheet.
  3. Update remaining deductible and out-of-pocket balances in the insurer’s member portal.
  4. Check network status again before a scheduled procedure, including facility-based professionals.
  5. When a bill differs from the explanation of benefits, contact the insurer before paying and request an itemized explanation.
  6. At renewal, replace the old deductible, copay, coinsurance, maximum, and premium figures; do not carry last year’s assumptions forward.

What your final budget should show

A defensible estimate has a medical cost-sharing subtotal, an annual premium subtotal, and separate totals for uncovered, out-of-network, and above-allowed charges. The medical subtotal should reflect the plan’s deductible, copayments, coinsurance, allowed amounts, network rules, and applicable individual or family maximum. This format tells you both what the insurance protects and what it does not.
