How to Budget for Medicare Premiums on a Fixed Social Security Income in 2026

Start with the Social Security payment that actually reaches your bank account, then add every Medicare charge that will be paid separately. For 2026, the standard Part B premium is $202.90 a month, but your deduction may be higher because of income-related surcharges (IRMAA), a late-enrollment penalty, or a different coverage choice. Part D, Medicare Advantage, Medigap and non-premium medical costs must be budgeted separately.

A reliable budget therefore has two parts: a fixed monthly premium line and a reserve for deductibles, copayments, coinsurance and prescriptions. The steps below show how to calculate your own amount, plan for bills that are not withheld from Social Security, and check assistance before cutting essential living expenses.

Know which Medicare costs can reduce your Social Security income

Medicare does not remove one universal amount from every Social Security check. Your withholding depends on the parts of Medicare you use, your plan choices, your tax history and, in some cases, your income from two years earlier.

Coverage or charge 2026 amount or rule How it is usually paid
Part A (hospital insurance) $0, $311 or $565 per month, depending on how long you or your spouse paid Medicare taxes Premium billed or withheld when applicable; many people qualify for $0 premium Part A
Part B (medical insurance) Standard premium $202.90 per month; annual deductible $283 Usually deducted from Social Security; some people receive a bill
Part D prescription coverage Plan premium varies; IRMAA or a late-enrollment penalty can increase the amount Plan billing, Social Security withholding, or another selected method; Part D IRMAA is billed monthly
Medicare Advantage Premiums, deductibles, copayments, coinsurance and the annual out-of-pocket limit vary by plan and county Often a plan premium in addition to any Part B premium
Medigap or other supplemental coverage Premium varies by policy and insurer Separate premium payment

The $202.90 Part B figure is a 2026 standard baseline, not a promise that this is the amount on your notice. Use the exact deduction shown in your Social Security notice or the amount on your Medicare bill.

A five-step method for building your monthly budget

1. Record the net Social Security deposit

Use the amount deposited after federal tax withholding, Medicare deductions and any other withholding. Do not budget from the gross benefit shown on an award letter. If your income changes during the year, update the worksheet using a recent bank statement and the latest Social Security notice.

2. Identify the actual Part B deduction

Find the Medicare deduction line on your Social Security benefit statement. Enter that number in your budget. Use $202.90 only when you are estimating before receiving a 2026 notice and you have no known IRMAA or penalty. If your notice shows an amount above the standard premium, keep the higher figure until Social Security issues a revised determination.

3. Add premiums that are not included in the deduction

List the monthly Part D premium, any Medicare Advantage or Medigap premium, and a Part A premium if you do not qualify for premium-free Part A. A plan may collect its premium from a bank account or by invoice even when Part B is withheld from Social Security. Keep separate lines so a change in one plan does not hide the effect of another.

4. Create a medical-cost reserve

Premiums are only the predictable part of health spending. Set aside money for the $283 Part B deductible, plan deductibles, specialist and hospital copayments, coinsurance, dental or vision services not covered by Medicare, and prescriptions. The reserve can be a separate savings account or a clearly labeled category in a budgeting app. Base the amount on your prescriptions, appointments and recent bills rather than a generic percentage of your check; no authoritative rule sets one percentage for everyone.

5. Match the budget to payment dates

If Medicare sends you a CMS-500 bill, reserve the money for its stated due date. Medicare says payment must arrive by the 25th of the month. A bill that arrives quarterly requires you to save one-third of the bill each month, even though the cash leaves your account only once per quarter.

Worked examples using the method

Example: standard Part B with separate drug and supplement premiums

Suppose your net Social Security deposit is $1,800. Your 2026 Part B deduction is the standard $202.90, your Part D plan costs $35 a month, and your supplemental policy costs $120. Your recurring Medicare-related amount is $357.90 ($202.90 + $35 + $120), leaving $1,442.10 before setting aside money for deductibles, copayments and prescriptions. The calculation is illustrative; your actual plan premiums and withholding control your budget.

Example: a quarterly Part B bill

If a beneficiary is not having Part B withheld and receives a $600 quarterly bill, the monthly budget line should be $200. Save that amount in each of the three months before the bill is due, and verify that the full payment reaches Medicare by the 25th deadline. Do not assume a bill will be converted to withholding automatically.

Example: an income-related surcharge

A beneficiary whose Part B deduction is higher than $202.90 should enter the notice amount, not the standard figure, in the worksheet. If Part D IRMAA is billed separately, add that monthly invoice as another fixed line. Keep the notice with your budget records so you can explain the change if the amount is revised.

Why your premium can be higher: IRMAA

The income-related monthly adjustment amount (IRMAA) can raise Part B and Part D costs. Medicare generally uses modified adjusted gross income from the federal tax return filed two years earlier. For 2026, the handbook lists threshold income of $109,000 for individual filers and $218,000 for married couples filing jointly.

Because the calculation looks back two years, a current reduction in income may not appear immediately in your premium. Review the annual Social Security IRMAA notice rather than estimating from this year’s paycheck. If your income fell because of retirement or another qualifying life event, ask Social Security whether you can request an IRMAA reduction. Keep documentation such as a retirement date, a reduced-work statement or another record Social Security requests.

Budget the amount actually shown in the notice until Social Security approves a change. A tax return that is later amended does not by itself guarantee an immediate premium adjustment; Social Security makes the determination and sends a revised notice when applicable.

How late-enrollment penalties affect a fixed income

Part B penalty

Late Part B enrollment generally adds 10% for each full year you could have enrolled but did not, and the increase usually continues while you have Part B. Confirm whether employer coverage or another qualifying situation protected you before declining Part B; a penalty can permanently raise a fixed monthly expense.

Part D penalty

Late Part D enrollment generally adds 1% for each month without creditable prescription coverage. Medicare’s 2026 example uses a $38.99 national base beneficiary premium when illustrating the calculation. Your actual penalty is tied to the applicable base premium and is added to your plan cost, so keep evidence of prior creditable coverage if you are asked to prove it.

When you first become eligible, compare the cost of enrolling on time with the risk of a lifetime surcharge. A low-premium plan can still be valuable if it prevents a penalty and provides coverage for medicines you may need later.

Automatic deductions, bills and Medicare Easy Pay

Social Security withholding

Most people have Part B deducted automatically from their Social Security benefit. The deduction normally appears before the deposit reaches your bank account, so use the net deposit for day-to-day spending and the notice amount for your Medicare line item.

Direct billing

People who do not receive Social Security or who are not using withholding may receive a quarterly Part B bill. Part D IRMAA is billed monthly. Keep a calendar reminder several days before each due date and maintain enough cash in the payment account for the full amount.

Medicare Easy Pay

Medicare Easy Pay can schedule automatic withdrawals for eligible premiums, but deductions can take six to eight weeks to begin. Until the first withdrawal is confirmed, continue paying any bills that arrive. Check your bank statement and Medicare account before stopping manual payments to avoid a delinquency.

Use assistance programs before cutting necessities

Medicare Savings Programs

Medicare Savings Programs are administered through your state. Depending on the program, they can pay Part A and Part B premiums and may also cover certain deductibles, copayments and coinsurance. Applications go through the state Medicaid or benefits agency, not through a private plan.

Program 2026 federal monthly income limit for an individual 2026 federal resource limit for an individual
Qualified Medicare Beneficiary (QMB) $1,350 $9,950
Specified Low-Income Medicare Beneficiary (SLMB) $1,616 Resource limit varies by program rules
Qualifying Individual (QI) $1,816 Resource limit varies by program rules

These are federal 2026 figures for an individual. State limits may be higher, and counting rules can differ. Ask your state agency how it treats income, bank accounts, vehicles and other resources. Apply even if you think you are slightly over a federal number; state rules and allowable deductions can change the result.

Compare plans by total exposure, not just the premium

A low monthly premium can produce higher costs when you use care. Compare plans in your county using the same set of figures:

  • Total annual premium: multiply each monthly premium by 12 and include any Part B or Part D surcharge that is billed separately.
  • Deductible: note when it applies and whether medical and drug deductibles are separate.
  • Drug formulary and pharmacy cost: check every prescription, dosage and preferred pharmacy tier.
  • Medical copayments and coinsurance: price routine visits, specialists, imaging, emergency care and likely hospital services.
  • Provider access: verify that your doctors, hospitals and pharmacies participate in the plan’s network.
  • Annual out-of-pocket limit: for Medicare Advantage, compare the plan-specific limit because it is a major protection against a bad medical year.

Medicare Advantage costs differ by plan and county, so a national average cannot tell you what your household will spend. Use the plan comparison for your county and save the summary of benefits with your budget records.

An annual workflow for a fixed-income household

  1. When the annual notices arrive, record the new Part B deduction, any IRMAA, and the effective date.
  2. During open enrollment, compare total annual premiums, drug coverage, cost sharing, provider access and the out-of-pocket limit rather than choosing on premium alone.
  3. At the start of the year, fund a medical reserve for deductibles and prescriptions before discretionary spending.
  4. Each month, reconcile the Social Security deposit and every Medicare or plan charge against your worksheet.
  5. Each quarter, confirm that any CMS-500 bill is scheduled to arrive by the 25th deadline.
  6. After a major life change, such as retirement or a sharp income reduction, review IRMAA relief and assistance-program eligibility.
  7. Before reducing food, housing or utility spending, contact your state Medicare Savings Program office and check Extra Help.

Troubleshooting common budgeting problems

The deduction is higher than $202.90

Read the Social Security notice for an IRMAA amount or late-enrollment penalty. Enter the stated amount in your budget and review whether a qualifying life event allows an IRMAA reduction request.

A premium bill arrives after you enrolled in Easy Pay

Six to eight weeks may be needed for Easy Pay to start. Pay the current bill by its due date, then verify the first bank withdrawal before changing your payment routine.

You budgeted for Part B but forgot drug or supplemental coverage

Review bank statements, plan enrollment confirmations and insurer invoices. Add each recurring premium as a separate line and set a reminder for annual premium changes.

A quarterly bill causes a cash-flow shortfall

Divide the bill by three and transfer that amount monthly to a dedicated reserve. If the bill is already overdue, contact Medicare promptly and ask about the available payment status rather than ignoring the notice.

Your income dropped but IRMAA did not

Because 2026 IRMAA generally uses income from two years earlier, the old tax return may still control the charge. Contact Social Security, explain the qualifying life event and provide the requested evidence for a reduction review.

What a sustainable Medicare budget looks like

Your sustainable amount is the sum of the actual Part B deduction, Part A premium when applicable, Part D premium and surcharge, Medicare Advantage or Medigap premium, and a realistic reserve for cost sharing and prescriptions. Recalculate whenever a notice, plan change, tax-related IRMAA decision or assistance award changes one of those inputs. The result is individualized; a fixed percentage of every Social Security check would ignore the differences in coverage, income and health needs.
